2.Delayed Radionecrosis of the Brain: A case simulating recurrent glioma.

An appreciation of the radiation changes in the brain adjacent to radiated glioma is of considerable importance from both clinical and pathological standpoint because the alteration in the brain parenchyma may result in a picture that mimics recurrence of the neoplasm. A 35 years old woman was admitted because of deterioration of consciousness which has started about 20 days ago. Past history revealed removal of left frontal brain tumor 4 years earlier and the diagnosis was grade II astroma. Postoperative radiation was done at that time. Sine then, she has been doing well. Computerized tomogram showed an ill defined huge low density mass at right frontal and left posterior parietal areas, which was irregularly enhanced in postcontrast study. Unlikely to the usual brain tumors, there was no mass effect on adjacent structures. Histologically acellular necrotic zone was alternating with hypercellular zone and most striking changes were vascular and glial reaction. Most of the vessels showed acellular hyliane thickening of their wall and some vessels were almost completely occluded. There was marked glial proliferation with considerable cellular and nuclear irregularities. Gemistocytic astrocytes were frequently seen and some were multinucleated.

5.A Case of Cervical Far Lateral Disc Herniation-Case Report-.

Compared to the lumbar region, it is very rare to encounter far lateral disc herniation in the cervical spine, and because of this, correct diagnosis before surgery is difficult: the condition can, however, be identified through the use of advanced MRI imaging techniques. In this case, far lateral disc herniation at C7-T1 was effectivery removed through posterior laminoforaminotomy, and soon after surgery, the patient's symptoms showed complete remission.

6.Effects of Social Support and Ego-resilience on Nursing Performance of Hospital Nurses.

PURPOSE: The purpose of this study was to identify the effects of the social support and ego-resilience on the nursing performance among hospital nurses. METHODS: The subjects of this study were 369 nurses in a senior general hospital. We used the self-reported questionnaire to assess the level of ego-resilience, social support and nursing performance of hospital nurses. The data were analyzed using descriptive statistics, frequency, t-test, ANOVA, Pearson's correlation coefficient and stepwise multiple regression. RESULTS: This study showed the positive relationship between the social support and ego-resilience on tne the nursing performance of hospital nurses. The higher group of the social support and ego-resilience showed the more performance of hospital nurses. And, the influencing factors on the performance are age, ego-resilience, social support, night working days a month in the order. CONCLUSION: Further studies will be needed to promote the performance of nurse in hospital and policies to be develop to elevate the social support and ego-resilience of nurses.

8.Anesthesia Experienee of Open Heart Surgery - Report of 77 cases.

To evaluate the results of anesthesia during open heart surgery, the cases of 77 patients in the Department of Anesthesiology, Ewha Womans University Hospital were weviewed from November, 1982 to February, 1985 and from March, 1986 to February 1987. The Results were as follows: 1) Thirty-nine percent of the 77 cases were acyanotic congenital heart anomalies , 18.2% were cyanotic anomalies and 42.8% were valvular heart diseases. The patients' ages ranged from 13 months to 63 years old, and the study included 30 male and 47 female patients. 2) Pethidine given as a premedication in mot cases, also atropine, glycopyrrolate, hydroxyzine, lorazepam and diazepam were utilized. 3) The induction agents included thiopental, ketamine diazepam, and morphine, and were injected singly or in combination. 4) Anesthesia was maintained with diazgepam, pancuronium, ketamine, morphine, and pethidine, N20 inhalation was used in the acyanotic and valvular cases. 5) Serum Na+ and K+ concentrations were lower during bypass than before and after bypass. 6) During bypass, body temperatuer was maintained below 30 degrees C, and the mean values for arterial pressure ranged from 50-80 mmHg. 7) The duration of andsthesia was 481+/- 88.7 minutes in the valvular cases and the duration of bypass was 118+/-18.3 minutes in the cyanotic cases. 8) In the analysis of arterial blood gases, the cyanotic cases displayed a low pH and base excess. In the cyanotic cases, the PaCO2, and PaO2, differed from the other cases before bypass, but not after bypass. 9) Overall mortality was 14.3%, The mortality rate with respect to each disease was 10 10/0% in the acyanotic cases, 28.6% in the cyanotic cases and 12.1% in the valvular cases.

10.Candida Albicans Shunt Infection: Case Report.

Two patients of shunt infection caused by Candida albicans are presented. They were low birth weight premature babies with prolonged antibiotic therapy for the neonatal pneumonia. They were diagnosed as posthemorrhagic and congenital hydrocephalus and underwent ventriculoperitoneal shunt(VPS). Postoperatively fever and setting sun eyes developed and anterior fontanel became tense and bulging within two months after VPS. One patient was documented as C. albicans shunt infection by facteriological study and scanning electron microscopy(SEM) and the other patient by bacteriological study only. The authors controlled the C. albicans shunt infection by removal of shunt device and intravenous Amphotericin B. In C. albicans shunt infection, it is important to remove the infected shunt device immediately after the diagnosis and bacteriological study is diagnostic, but SEM plays a role in the demonstration of the fungal elements in the shunt device.
