1.A study on normal value of eyeball protrusion in Korean adult using CT

CT may be valuably used indetermination of proptosis. Normal value of eyeball protrusion was measured innormal Koreans using CT. Among 86 Koreans, 26 have normal both eyes, but 60 have one normal eye with one abnormaleye (chiefly, proptosis and and eyeball tumor) The results of 112 eyes are as follows; Absolute measurement of“a”, that is, the distance between cornea & IZL (interzygomatic line) is 16.0±3.4mm in male (for 63 eye),15.7±4.0 mm in female for 49 eyes. Ratio of “a” over “A” (A-P diameter of eyeball) is 57±12% is male (for 63eye), 57±14% in female (for 49 eye) Discrepancy of eyeball protrusion between both eyes is 0.67±1.5mm (for 26adults) Practically, normal range of eyeball prototrusion(“a”) is 12.6mm< male< 19.4mm, 11.7mm

2.Clinicopathological Analysis on the 104 Cases of Malignant Melanoma.

The cliniopathological analysis was done on the 104 cases of malignant melanoma diagnosed at the Seoul National University Hospital (SNUH) from 1984 to 1993. The basic clinical data and the pathological items were based on the New Mexico Melanoma Registry Worksheet. The results were as follows. The male to female ratio was 1 : 0.79. Primary cutaneous melanoma was more common in the male (M : F=1 : 0.56) but primary extracutaneous melanoma with slight female dominancy (M : F=1 : 1.25). The peak age was the 6th decade in both cutaneous and extracutaneous malignant melanoma. In 66% (35 cases) of primary cutaneous malignant melanoma, the primary site was located in the acral area (including cases of acral lentiginous and nodular type), of which 63% (41% of total cutaneous melanoma) was acral lentiginous type. Major components of tumor cells were epithelioid. Clark's level of tumor was III or more at the time of the first visit in the majority of the cases (85%). The incidence rate of extracutaneous melanoma was 34.6% (36 cases) among the primary melanoma, and the eyeball (17.3%) was the most prevalent organ. All these features suggest that the racial difference between the Korean and the Caucasian is evident and also that etiologic role of sun damage is not quite marked in the Korean. We also suggest that an early detection program is very important to cure this malignant tumor.

3.Clinical Evaluation of Radionuclide Esophageal Transit Study in Patients with Nonspecific Esophageal Motility Disorder.

OBJECTIVES: Nonspecific esophageal motility disorder(NEMD) is a vague category used to group poorly defined contraction abnormalities. We evaluated the clinical characteristics and esophageal transit time (ETT) in patients with NEMD. METHODS: Total 205 patients with NEMD were compared with 20healthy controls and 99patients with other motility disorders of the esophagus. Esophageal manometry was performed with a lowcompliance pneumohydraulic capillary infusion system and esophageal scintigraphy was performed for the liquid and solid swallow after manometric study. RESULTS: 1) Among the total 258abnormal contractions in 205patients with NEMD, non-transmitted contractions were 125(45.5%), low amplitude 110(42.6%), triple peaked 3(1.2%), prolonged duration contractions 12(4.7%) and isolated incomplete LES relaxation 8(3.1%). 2) NEMD patients have significantly delayed ETT similar to that seen in patients with diffuse esophageal spasm, nutcracker esophagus, hypertensive lower esophageal sphincter for liquid, and diffuse esophageal spasm, hypertensive lower esophageal sphincter for solid. 3) ETT for liquid and solid according to abnormal contractions were 39.0 and 55.6seconds in non-transmitted, 38.3, 68.4 seconds in low amplitude, 17.0, 30.0 seconds in triple peaked, 29.4, 25.8 seconds in prolonged-duration contractions and 13.7, 15.5 seconds in isolated incomplete LES relaxation, respectively. CONCLUSION: Patients with NEMD have significantly delayed ETT for liquid and solid compare to normal control. The main abnormal contractions of NEMD were non-transmitted and low amplitude contractions. And low amplitude contractions were the main cause of delayed solid transit in patients with NEMD(p<0.01).

4.Epidermolysis Bullosa Simplex Generalisata.

We report a case of epidermolysis bullosa simplex occurred in a 12-year-old girl, who presented with intractable bullous lesions of the hands and feet after minor traumas ever since her early neonatal period. The bullous lesions were noted on the 2nd and 4th webs of the fingers and dorsum of the hands as well as on the skin of the ankle. The lesions were healed without scar formation. Family history was not contributary and seasonal pattern was not noted. Histologic sections revealed intraepidermal bullae just above the well preserved basement membrane. Electron microscopic findings revealed cytolysis of basal cells with preservation of the basement membrane, indicating the epidermal type of epidermolysis bullosa simplex.

5.Brain Abscess Formation Accompanied by Improvement of Orbital Cellulitis.

PURPOSE: To present a case of brain abscess formation accompanied by improvement of orbital cellulitis. CASE SUMMARY: A 54-year-old male came to our clinic complaining of swelling and pain of the left periorbital area and decreased visual acuity (VA) of the left eye. Initial best-corrected visual acuity (BCVA) was 0.3 and intraocular pressure was 27 mmHg in the left eye. Eye movement in all directions was restricted and 4 mm of proptosis was observed in the left eye. An orbital CT scan demonstrated pansinusitis and orbital cellulitis of the left eye. The patient underwent endoscopic sinus surgery and was treated with systemic antibiotics. However, periorbital swelling was aggravated and another orbital CT scan was performed and analyzed. The CT scan showed localized periorbital abscess of the left eye, and the authors performed an incision and drainage (I&D) of abscess procedure. After the operation, BCVA of the left eye was recovered to 0.8 and eye movement improved and periorbital swelling decreased. However, 15 days after the I&D, the patient complained of a severe headache. Brain magnetic resonance imaging (MRI) was performed and showed an abscess of the left frontal lobe of the brain. The authors consulted with a neurosurgeon, and the patient received intravenous antibiotics and mannitol. The headache steadily decreased, and three months after the first visit, a follow-up brain MRI was performed. The MRI showed almost complete disappearance of the abscess and six months after the first visit, BCVA was recovered to 1.0 and eye movement was full in all directions. CONCLUSIONS: Although orbital cellulitis is improved by treatment of antibiotics and surgery, if the patient complains of neurologic symptoms such as headache, other complications such as brain abscess formation should be considered.

6.A study on correlation between CT findings and clinical course of meningitis in children

63 cases of meningitis in children were reviewed to study correlation between brain CT findings and clinical course. We divided 63 cases into 3 groups according to clinical course, that is , Group I :Healed without significant sequelae. Group II: Discharged with sequelae such as neurologic deficit or complicated clinical course. Group III : Expired or considered to be expired after hopeless discharge. The CT finding were retrospectively analyzed and compared with each clinical group. We drawed several conclusions as follows; 1. The wrost prognostic CT finding is dirty basal cisternal enhancement. (Group I only 5%, Group II 50%, Group III 45%).2. Focal brain parenchymal lesion, especially multiple, such as granuloma and infarct shows unfavorable clinical outcome, that is, high rate of Group III and evident neurologic deficit, in contrast to only 7% of Group I. 3. In 7 cases of which CT finding is only hydrocephalus, the prognosis is rather favorable, that is, 57% were Group I, 43% were improved after V-P shunt (Group II) and no Group III. But hydrocephalus with dirty disternal enhancement results in grave prognosis, that is, Group I only 8%, Grouop II 54%, Group III 38%, With regard to overall hydrocephalus, predilectron for good or bad prognosis can't be mentioned. 4. No prognostic difference were noted between presence and absence of periventricular low desnity in hydrocephalus. 5. CSF pressure of hydrocephalus ismostly high (over 20cm H20). but normal pressure hydrocephalus were noted in 24%. CSF pressure of normal ventriclesize is mostly under 18cm H2O) but high pressure were noted in 18% of the nomral sized venticle (most of them shows intracranial space occupying lesion such as granuloma, acute infarct, subdural effusion, etc). 6. Most of diffuse braine swelling, diffuse brain atrophy and subdural effusion result in Group I, that is, favorable clinical outcome. 7. Normal CT findings ar found in 29%, of which 61% belong to Group I and 31% to Group II.

9.The Effects of 5% Oxygen Condition and Superoxide Dismutase ( SOD ) on the in - vitro Development of Preimplantation Mouse Embryos.

OBJECTIVE: In the human body the embryo initially gmws in the fallopian tube which is maintained in an 3-8% O2 concentration environment, and various substances such as growth factors and antioxidants present in tbe tubal fluid assists in maintaining a healthy environment for embryo development. But in IVF programs embryo cultures are conducted in incubators with 21.9% O2 and 5% CO2 condition, and such high oxygen concentrations have been reported to increase the production of oxygen free radicals within the embryo and is detrimental to the growth and development of the embryo. The objective of this study, therefore, is to determine the culture conditions which will decrease oxygen free radical production and thereby minimize the injury to the embryo. METHODS: Six to eight week old ICR strain mice embryos were cultured in 5% or 21.9% O2 conditions and in culture media to which inaement concentrations of superoxide dismutase (SOD) had been and the H2O2 concentration within the embryo, embryo developmental rate, and degree of fragmentation of the embryos was investigated. RESULTS: The control gmup embryos which were cultured in 21.9% O2 condition without addition of SOD showed developmental arrest at the 2-cell stage or fragmentation, while those cultured in 21.9% O2 condition with addition of SOD showed development to the blastocyst stage with deaeased fragmentation. In particular, the blastulation and fragmentation rates were the lowest in the group to which 500 IU/ml of SOD was added, but in the 5% O2 enviranment group many embryos reached the blastocyst stage and with no difference in frapnentation with or without addition of SOD. The HO relative intensity (120.5+/-20.2) within the embryos cultured in 21.9% O2 environment without SOD was significantly higher than that (56.8+/-10.8) of group with SOD (p<0.05). As showing that in the 5% O2 environment group without SOD it was 43.8+/-7.8 and in the group with SOD it was 37.3+/-5.4, the H2O2 concentration within embryos cultured in 5% 02 condition was significantly lower those that of 21,9% 02 environment regardless of SOD addition (p<0.05). CONCLUSION: The optimal oxygen concentration in incubator for mice embryo cultures is that which is similar to the 5% 0 concentration in vivo. When 20% 02 incubators are routinely used, the addition of SOD to the culture media will decrease the H2O2 concentration within the embryos with subsequent improvement in development. The optimal concentration which should be used is thought to be 500 IU/ml. It is suggested that the use of the above method in human IVF-ET programs will lead to improved embryo quality and enhanced pregnancy rates.

10.A Case of Radiation Induced Rectal Adenocarcinoma.

The development of colorectai cancer following pelvic irradiation has been a much less common event. A 58-years-old woman presented with tenesmus for 5 months. She had a stage IIb, carcinoma of the cervix for which she underwent radiation therapy in 27 years before. Colonscopic findings revealed polypoid mass and irregular ulceration on the rectum and proctitis associated with stricture that was compatible with her previous history of radiation. Histoyathological examination of the polypectomy specimen disclosed adenocarcinoma. So we reported a case of radiation induced carcinoma of reotum which might be the first case in Korean literature. It therefore, behooves the clinician who follows patients whose carcinoma of the cervix or corpus uteri has been cured by radiation therapy to be vigilant for the subsequent development of colon and rectal cancer.
