Pulmonary metastasis occurs frequently in patients with gestational choriocarcinoma and most of these patients achieve remission with chemotherapy alone. Thus, the indications for surgical intervention are limited, but in appropriately selected patients, resection of a chemotherapy-resistant lung lesion can be curative. We have experienced that pulmonary metastatic choriocarcinoma in a drug-resistant patient was cured by pulmonary resection. So we report this case with a brief review of literatures.

3.Treatment of the Ovarian Germ Cell Tumors.

Significant improvements in the management of ovarian germ-cell tumors have been achieved during the past two decades. The development of more effective chemotherapeutic regimens is clearly the leading cause for improved outcome for these patients. In addition, advancements in other disciplines led to the development of a more precise surgical staging system, improved radiographic imaging, more sophisticated pathology techniques, as well as improved supportive care and symptom control. A substantial majority of patients with ovarian germ-cell tumors are long term survivals and suffer minimal morbidity from treatment. Fertility-sparing surgical procedures enable a large proportion of young women with ovarian germ-cell tumors to preserve their reproductive potential.

5.A case of uterine tumor resembling ovarian sex-cord tumor.

Uterine tumor resembling ovarian sex cord tumors (UTROSCTs) are uncommon neoplasms that histologically resemble ovarian sex cord tumor. Over 50 cases have been reported up to recently. Clement and Scully reported 14 cases of UTROSCT, which they devided into two groups by clinicopathological features. Group I tumors are endometrial stromal sarcomas with partially sex-cord like elements (less than 40% of total tumor volume). On the other hand, group II tumors are mostly or entirely composed of sex-cord like elements. We have experienced a very rare case of type II UTROSCT at the uterine fundus in a 52-year-old woman and report it with a brief review of the literatures.

6.Pulmonary Edema Druing Hysteroscopic Myomectomy with Sorbitol-Mannitol Distention Medium.

A 45-Year-old para 1-0-1-1 had submucosal myoma diagnosed by ultrasonogram and hysteroscopic examination. During 60 minutes hysteroscopic myomectomy, 8,000 ml of sorbitol-mannitol distention medium was used and 6,500 ml collected, resulting in a deficit of 1,500 ml. The patient received Ringer`s lactate 1,000 ml intravenously. Her urine output was 400 ml. Blood loss was 40 ml. After operation, hyponatremia(115 mEq/L) was developed and pulmonary edema was also diagnosed clinically at that time and later confirmed by chest radiograph. The judicious administration of diuretics and normal saline fluid replacement to prevent overcorrection to hypernatremia resulted in return of serum electrolytes to normal level and resolution of the pulmonary edema by postoperative day 1. We have experienced a case of pulmonary edema during hysteroscopic myomectomy with sorbitol-mannitol distention medium, which is presented with a brief review of literatures.

7.Telomerase Activity of Endometrium Related to the Effects of the Sex Steroid Hormone and Endometrial Cancer.

During the reproductive period, human endometrium undergoes a pattern of cyclic change. Human endometrium undergoes a complex pattern of proliferation, secretory activity, and menstruation over an approximately 28 days period. Proliferative activity is highest during late proliferative phase under influence of estrogen, and minimal activity in the late secretory and menstrual phase. To study a possible change of telomerase activity during menstrual cycle, telomerase activities in normal and hormone treated endometrium were tested using telomerase repeat amplification protocol(TRAP) assay. Telomerase activities were detected in 9 of 10 proliferative endometrium(90%), and maximal activity was shown in late proliferative phase. Only 3 of 10 secretory endometrium(30%) revealed weak activity. However telomerase activity was not detected in menstrual phase endometrium(N 2) and senile endometrium(N=3). Four of tamoxifen treated endometrium(N 4) and 1 of provera treated endometrium(N 3) Levels of telomerase activity of treated endometrium(N 4) and late proliferative endometrium(N 6) were as high as them of detected in endometrial cancer and hyperplasia. Above findings reveal that telomerase activity of endometrium is changed according to menstrual cycle, And the level of telomerase activity is related to proliferative activity of endometrium that is dependent on the status of female sex steroid hormone and tamoxifen treatment.

8.B-lymphocyte, T-lymphocyte and Subsets in Unexplained Recurrent Spontaneous Abortion.

The depressed cell-mediated immunity in pregnant women may play an important role inthe protection of the fetus from rejection by the mother. In this of view, many studieshave been performed to evaluated of lymphocyte subsets in recurrent spontaneous abortionwomen. However, because of the discordance in published data, auther have reevaluatedperipheral blood T-lymphocyte, B-lymphocyte and subsets defined by monoclonal antibodieswith immunobead method for 37 recurrent spontaneous abortion women and 30 normal healthywomen.The results were as follows. No significant difference was observed in the componentratio of the B cell. The component ratio of T lymphocyte in study group(70.5+/-10.1%) wassignificantly decreased in control group(79.7+/-3.1%)(p < 0.001). The component ratio of Nullcell in study group(19.0+/-10.1%) was higher than control group(9.4+/-2.6%)(p < 0.001). In thecomponent ratio of T4 cell, no statistically difference were observed in both groups. In thecomponent ratio of T8 cell, study group(28.4+/-9.5%) was significantly lower than controlgroup(38.4+/-3.2%)(p < 0.001). T4/ T8 ratio in the study group(1.58+/-0.70%)was significantly higher than control group(1.09+/-0.14%)(p < 0.001).In the component ratio of T/B cell, no statistically differences were observed.In conclusion, decreased T8 cell component ratio and increased T4/T8 component ratio may be play an important role in recurrent spontaneous abortion.

9.Spontaneous Rupture of Iliac Vein: A case report.

Spontaneous rupture of iliac vein is extremely rare condition. Until now, only 15 cases were reported worldwide. Most of the reported cases were developed in previously healthy middle to older aged women. No definite pathophysiology was proved but relation of inflammation and temporary increase of intraluminal pressure related with straining during posture change were proposed. We experienced one case developed in 65 year old otherwise healthy woman.
