3.SMAD7 prevents heterotopic ossification by regulating endothelial-mesenchymal transition after Achilles tendon imjury
Chinese Journal of Tissue Engineering Research 2017;21(8):1178-1185
BACKGROUND: The endothelial-mesenchymal transition is known to play a central role in the pathological process of heterotopic ossification. OBJECTIVE: To verify the inhibitory effect of SMAD7 on endothelial-mesenchymal transition such as myofibroblast transformation and to explore whether it is a potential target for heterotopic ossification.METHODS: A lentivirus overexpressing SMAD7 was contructed and the optimal titre and transfection efficiency in rat aortic endothelial cells were determined. The lentivirus was then injected into a rat model of Achilles tendon injury, while the controls were given the injection of normal saline. Expressions of endothelial and mesenchymal markers at the injured site were analyzed by quantitative PCR and western blot assay. The heterotopic ossification was observed radiologically and histologically.RESULTS AND CONCLUSION: Local injection of SMAD7-delivering lentivirus resulted in an upregulation of CD31 and VE-cadherin, and a downregulation of N-cadherin and vimentin, suggesting that the endothelial-mesenchymal transition is blocked due to local SMAD7 overexpression. The inhibitory effect became more evident at 10 weeks than at 6 weeks after modeling. Radiology and histological staining further confirmed that the ossified structures in the tendon tissue disappeared after injection of SMAD7-delivering lentivirus, as opposed to the control group. These data suggest that local overexpression of SMAD7 can prevent postoperative heterotopic ossification with no effect on wound healing.

Objective To explore the relationship of T2DM with xerophthalmia after cataract surgery. Methods Retrospective analysis was made on the clinical data of 137 cataract patients (176 eyes) from April,2016 to October,2016.The DM patients were chosen as the DM group (58 cases,71 eyes),and single cataract ones were chosen as the control group (79 cases,105 eyes). The clinical data and xerophthalmia index were compared between two groups. Results The age and sex constituent ratio of DM group and control group had no significant difference (all P > 0.05).The postoperative MQ score,BUT,FL,SⅠt of DM group at 1 week,1 month and 3 months were significantly worse than those at preoperative 2 days (all P < 0.01),but the indexes relieved with time prolong. The postoperative MQ score,FL,SⅠt of control group at 1 week,1 month were significantly worse than those at preoperative 2 days (P < 0.01),the postoperative BUT and preoperative BUT had no significant difference (P > 0.05),all of the postoperative index and preoperative index had no significant difference(all P > 0.05).The postoperative MQ score,BUT,FL,SⅠt of DM group at 1 week,1 month and 3 months were significantly worse than those of the control group (all P < 0.01).Conclusion The xerophthalmia after cataract surgery of T2DM patients are more seriously than normal cataract patients.

Objective To elucidate the relationship between liver mitochondria calcium content and respi- ratory function in obstructive jaundice and biliary drainage. Methods The model of obstructive jaundice and bil- iary drainage was produced in dogs by common bile duct ligation and choledochoduodenostomy. The liver mitochon- dria were isolated and the calcium content and respiratory control rate were monitored. Results Mitochondria res- piratory control rate and calcium content significantly decreased and elevated respectively after bile duct ligation and there were varying degrees of recovery after biliary drainage, and the degree of recovery was evidently correlated with the duration of obstruction before drainage and the time after drainage. There was a strong negative correlation between the mitochondria calcium content and change of respiratory function. Conclusion The mitochondria cal- cium overload may be one of important factors for suppressed mitochondrial respiration in obstructive jaundice and early relief of the obstruction could favour the recovery of damaged liver mitochondria.

Objective To compare the curative effect of the different palm dorsal approach of open reduction and internal fixation for the treatment of distal radius fractures. Methods According to the stochastic indicator method,84 patients with distal radius fractures were divided into control group and observation group,42 cases in each group. The control group was treated with dorsal approach open reduction and internal fixation, while the observation group was treated by volar approach open reduction and internal fixation. The clinical curative effect was assessed, and indicators related to the operation and the fracture healing was recorded in detail.Postoperative complications of patients were analyzed during postoperative follow-up.Results The total effective rate of the observation group was 88.10%(37/42),which was significantly higher than 66.67%(30/42) of the control group(x2=5.509,P=0.019).Compared with the control group, the operation and hospital stay time were significantly shorter, intraoperative and postoperative blood loss of the observation group decreased obviously, and the differences were statistically significant(t=2.092,2.137,2.256,2.199;P=0.040,0.036,0.027,0.031).The Gartland-Weriey score and ulnar inclination of the observation group were (9.18±2.96)points and (10.52±2.64)°,which were significantly lower than those of the control group[(12.56±3.24)points,(16.22±2.87)°],while radial height[(11.61±2.87)mm]was higher than the control group[(10.92±2.65)mm],the differences were statistically significant(t=2.206,2.316,2.185;P=0.030,0.023,0.032).The incidence rate of complications of control group was 26.19%(11/42),which was higher than 9.52%(4/42) of the observation group(x2=3.977,P=0.046).Conclusion The curative effect of volar approach of open reduction and internal fixation in the treatment of distal radius fractures was better compared with dorsal approach. At the same time,it can effectively improve the operation indicators and fracture healing effect,in addition,it can effectively reduce the incidence of postoperative complications.
