1.Cementless Bipolar Hemiarthroplasty for the Management of Fracture of the Femoral Neck.

Various methods for the treatment of fracture of the femur neck including total hip arthroplasty have been developed and performed, but it is still called the ""unsolved fracture"" in certain situations. The role of bipolar hemiarthroplasty as primary treatment of fracture of the femur neck, however, continues to be controversial. The authors reviewed and analysed 38 cases of cementless bipolar hemiarthroplasty in patients between the ages of 51 and 69 at the Department of Orthopaedic Surgery, Chosun University, during the period from January, 1989 to December, 1994. The purposes of the study were to analyse the clinical and radiological results and to detect the motions of bipolar cup. At the average 38 months follow up, 31 (81.6%) of the patients were satisfied by Lunceford methods. But, 24 (63.2%) of the patients suffered from inguinal area pain and discomfort while they resumed their normal Korean life-styles. As time elapsed, the amount of the outer bearing motion was more than that of the inner bearing motion. the acetabular erosion were present in 2 patients.

2.Atypical Pseudomonas Keratitis.

Pseudomonas aeruginosa is the most common and the most severe Gram-negative corneal pathogen. Pseudomonas keratitis often arises from the use of a contaminated fluorescein solution or from contaminated medications used either to examine or to treat a corneal abrasion or injury. Pseudomonas keratitis is an acute rapidly progressive corneal ulcer with greenish pus and hypopyon. A 49 year old Korean woman was found to have pseudomonas keratitis whose clinical features. were different from the classic pattern of pseudomonas keratitis.

Bronchial carcinoid tumor was a low grade malignant and it was regarded as predictable clinical course and good survivality after surgical resection. But despite of its low grade malignant potentiality, bronchial carcinoid tumor was clearly capable of metastasizing and causing death. We present 2 cases of bronchial carcinoid tumors. One of them was typical carcinoid tumor in 44 year-old female and another was atypical carcinoid tumor in 53 year-old male patient. Currative therapeutic procedure was performed by lobectomy and wedge resection.

6.The Effect of Calcium Chloride on Hemodynamic Responses to Protamine Sulfate in the Dog.

BACKGROUND: Protamine sulfate (PS), used to neutralize the effect of heparin, is often associated with systemic hypotension. The present study was aimed to investigate the protective effects of CaCl2 on adverse hemodynamic reaction to PS. METHODS: Forty-six dogs of three groups were studied. Animals were randomly allocated to receive either; saline 10 ml (controls, n = 26), CaCl2 5 mg/kg (n = 10) or 10 mg/kg (n = 10), added to PS 3 mg/kg given intravenously 5 min after heparin (300 IU/kg, iv). Mean aortic pressure (MAP), mean pulmonary arterial pressure (MPAP), left ventricular end-diastolic pressure (LVEDP), LVdP/dtmax, and -LVdP/dtmin, and pulmonary artery (cardiac output) and left circumflex blood flow (LCX flow) were recorded up to 15 min after PS. RESULTS: PS caused significant reductions in MAP (-49%), cardiac index (CI, -66%), systemic vascular resistance index (SVRI, -22%), LVEDP (-67%), LVdP/dtmax (-36%), and LVdP/dtmin (-55%), but increased MPAP (39%) and PVRI (3.8 fold), which all increases and reductions peaked 1-3 min after PS injection in the control group. The addition of CaCl2 to PS significantly attenuated reductions of MAP (-35 and -26% in CaCl2 5 and 10 mg/kg treated dogs, respectively), CI (-34 and -37%), LVdP/dtmax (-14, -11% ), and -LVdP/dtmin (-34, -21%), and increases in PVRI (1.8 and 2.4 fold). However, both doses of CaCl2 increased MPAP (48, 82%), but not SVRI (-28, -21%) or LVEDP (-73, -75%). LCX flow increased similarly in all groups (75-137%). CaCl2 5 and 10 mg/kg iv increased plasma Ca2+ by 0.23 and 0.36 mM, respectively. CONCLUSIONS: These results suggest that the simultaneous administration of CaCl2 attenuates the adverse hemodynamic effects of PS, used to reverse heparin anticoagulation in the dog.

7.A Case of Nonfunctioning Paraganglioma of the Posterior Mediastinum.

Paraganglioma is a tumor from the extra adrenal paraganglion system and is rarely observed in the mediastinum. The authors experienced a case of nonfunctioning paraganglioma of the posterior mediastinum. The patient was 34-years-old male in whom abnormal mass lesion was nites in chest radiograph with hemoptysis. His blood pressure and serologic examination were within normal range upon admission to our hospital. Chest CT revealed a tumor in the left lower lobe. Diagnostic thoracoscopy was performed and diagnosed a posterior mediastinal mass. Surgical resection was them performed. Posterior mediastinal mass was removed successfully and histological examination of the surgical specimen diagnosed paraganglioma. He received radiotherapy after surgery and was followed up. Related literature are reviewed.

8.Clinical Observations of the Solitary Pulmonary Nodules.

The authors conducted a clinical observation of 55 cases of solitary pulmonary nodules at Yeungnam University Hospital from June 1986 to October 1990, and the following results were obtained: 1. The age distribution was ranged from 18 to 77 years, and the male-to female ratio was 1.8:1. 2. Among 55 cases of nodules, 28 cases were benign and 27 cases were malignant nodules, and of malignant nodules, the primary lung cancer was 23 cases and of benign nodules, 18 cases were tuberculoma. 3. 23 cases (41.8%) was asymptomatic and the other 32 cases were symptomatic; chest pain 12 cases, hemoptysis; 8 cases, cough; 8 cases and dyspnea; 4 cases. 4. The non-smoker-to-smoker ratio was 1:1.04, but among 23 smoker over 20 pack years, 14 cases were malignant nodules. 5. According to nodular size, there is no striking differences between benign and malignant nodules except 3-4 cm sized nodules. 6. The lobar distribution of nodules, 35 cases were in the right lung (upper lobe; 14 cases, middle lobe; 11 cases, and lower lobe; 10 cases) and 23 cases were in the left lung (upper lobe; 9 cases, lower lobe; 11 cases), and the malignant nodules were most commonly observed in the right upper lung.

9.Limited Open Reduction and Intramedullary Nailing of Proximal Femoral Shaft Fracture.

PURPOSE: To evaluate the result of treatment of proximal femoral shaft fracture with limited open reduction and intramedullary nailing. MATERIALS AND METHODS: Fifteen patients who had limited open reduction and intramedullary nailing due to proximal femoral shaft fracture for follow-up for more than 12 months were selected between March 2001 and December 2005. The clinical and radiologic results were analyzed. Winquist-Hansen classification and OTA/AO classification were used. RESULTS: Thirteen cases achieved bone union and 2 cases showed delayed union. The mean bone union period was 21.3 weeks (14~32). There was no postoperative infection. Nonunion was observed in 2 cases of which bone union was acquired with the exchange of intramedullary nail and bone graft in one case and with the additional plate fixation and bone graft in the other case. CONCLUSION: Treating proximal femoral shaft fracture with limited open reduction and intramedullary nailing seems to be a technique to manage proximal femoral shaft fracture that has combined fracture or ipsilateral femoral fracture or is unable to acquire acceptable reduction with closed reduction.

10.A case of small cell lung cancer developed in patients with dermatomyositis during therapy.

Dermatomyostis is an idiopathic inflammatory myopathy characterized by progressive symmetric proximal muscle weakness and typical cutaneous lesions. The association of adult dermatomyositis and malignancy in particular has generated much attention recently. A 66 year-old male biopsy proven dermatomyositis had bloody hemoptysis 1 week ago and skin manifestation aggravated during steroid therapy in outpatients rheumatology clinic. During routine evaluation mediastinal mass was found, and following bronchoscopic biopsy confirmed small cell lung carcinoma. In Korea a few cases were reported about stomach cancer, breast cancer and acute lymphoblastic leukemia but small cell lung cancer was not reported yet. So we reported a case of small cell lung cancer developed in patients with dermatomyosits during therapy with brief review of literatures.
