2.A cephalometric study on the soft tissue profile changes by orthodontic treatment in female patients.

This study was undertaken to investigate soft tissue profile changes by orthodontic treatment in female patients. Traditional cephalometric appraisal yields data of dubious scientific value, the soft tissue profile forms were evaluated by finite element method. The subject was divided into three groups according to Angle' s classification and each group was composed of 25 female patients averaged aged 12-14 years at the start of treatment. The changes in soft tissue form were evaluated by computing the degree of distortion in each triangle after treatment compared with the triangle before treatment. The conclusions were as follows; 1. The soft tissue profile forms were evaluated by finite element method and independent evaluation of each element by local changes was possible. 2. Maximum and minimum principal strains showed marked variability depending on the particular finite element and each group and Class II, III sample was greater than Class I sample. 3. Soft tissue size changes as a result of orthodontic treatment was not related to those of shape. 4. Soft tissue changes by orthodontic treatment were variable in individual patient, and were not related to Angle' s classification.

3.Analysis of skeletal facial asymmetry with submentovertex cephalometric radiograph.

This study was performed to investigate the midline having the least difference between the right and left structure among the lines that had been used in the submentovertex analysis and secondarily to know the distribution of asymmetry and it's degree existed normal persons and asymmetric patients. The subjects consisted of 40 normal adult patients and 40 asymmetric adult patients. The computerized anaylses from submentovertex cepholometric radiograph were carried out. The results were as follows; 1. The right and left difference of the perpendicular bisecting line between right and left foramen spinosum was larger than the other midlines in the anterior area and it was decreased gradually as it progressed posteriorly. Specially the difference of this line was the smallest in the area where there was foramen spinosum. 2. The right and left difference of the perpendicular line through crista gali to the line between right and left foramen spinosum was smaller than the other midlines in the anterior area and it was increased gradually as it progressed posteriorly. 3. The right and left difference of the line between crista gali and anterior process of atlas was constant and smaller than the other midlines. 4. Asymmetry was a common finding in both normal and asymmetry group and left or right dominance of asymmetry was not statistically significant. 5. When the analyses were undertaken after submentovertex radiogram was divided into cranial base, upper face and lower face, the more inferior part showed relative asymmetry than the more superior part.

4.Study of craniocervical posture and craniofacial morphology in Korean young adults.

The purpose of this study was to estimate correlations of craniocervical posture and craniofacial morphology in Korean young adults. The sample consisted of 50 young adults (25 males and 25 females) who had good profile and Class I molar relationship. The analysis of craniocervical posture and craniofacial morphology was performed on lateral cephalograms taken in natural head position. The results were as follows; 1. The mean and the standard deviation of postural and morphologic variables were obtained. 2. Korean young adult had cervical lordosis of which degree between OPT and CVT shows 3.55+/-2.58degrees 3. Craniocervical posture and each of vertical ratio, facial prognathism, mandibular rotation showed high correlation. 4. Correlation coefficients between postural variables and each of intermaxillary relation, anteroposterior ratio were low. 5. The head positioning error of natural head position was smaller than the inter-individual variability of postural variables.

5.The pubertal growth spurt and skeletal maturity stages of the hand-and-wrist in normal occlusion.

To investigate the relationship between the pubertal spurt in body height and bone maturity of the hand-and-wrist in normal occlusion, the author X-rayed the hand-and-wrists of 1, 141 students (male 614, female 527) and assessed their bone maturity. In this study, eleven skeletal stages were selected, The bones used to determine skeletal maturity were the ulnar sesamoid of the metacapophalangeal joint of the first finger, the epiphyses of the proximal, middle, distal phalanges of the third finger, and middle phalanx of the fifth finger, and distal epiphysis of the radius. From the longitudinal data for height, an assessment was made of the change in growth velocity. The pubertal growth stage was divided into onset and peak height velocity phases. The results were as follows; 1. The onset of the pubertal growth was between the pp3=and MP3=stage for boys, and between the MP3=and MP5= stage for girls; the mean age of onset was 10.6 years for boys and 9.0 years for girls. 2. The peak height velocity was between the S and MP3cap stage for boys, and between the MP3cap and MPMP5cap stage for girls; the mean age of peak height velocity was 12.5 years for boys and 10.9 years for girls. 3. As the stages of bone meturity progressed from DP3u, to PP3u, MP3u, Ru, the peak height velocity had been reached, and the growth rate retarded, therefore the approach to full physical maturity was attained. 4. The evidence for the period of onset, peak height velocity and bone maturation sugggested that girls were in advance of boys. During the latte part of pubertal growth, the rate of boys' bone maturation was faster than that of grils'.

6.A Comparison of the Effectiveness of Before and After the Trauma Team's Establishment: Treatment Outcomes and Lengths of Stay in the Emergency Department.

PURPOSE: The aim of this study was to analyze the influence of a trauma team's management. METHODS: A total of 181 patients with severe trauma were retrospectively divided into two groups. Of these 181 patients, 81 patients without a trauma team admitted between April and October 2008 were assigned to Group 1, and 100 patients with a Trauma team admitted between April and October 2009 were assigned to Group II. We compared general characteristics, the length of stay in the emergency department (ED) and treatment outcomes (24-h packed RBC transfusion, length of intensive care unit (ICU) stay, length of hospital stay, in-hospital mortality, 24-h mortality) between these two groups. RESULTS: The length of stay in the ED was significantly reduced in Group II compared to Group I (p=0.025). No significant differences were found in mean arterial pressure, Glasgow Coma Scale, Revised Trauma Score, Injury Severity Score, in-hospital mortality and 24-h mortality between the two groups. However, Group II had a lower amount of 24-h packed RBC transfusion and a shorter length of ICU and hospital stay than Group I, although these differences were not statistically significant. CONCLUSION: Through the establishment of a trauma team, the length of stay in the ED can be reduced remarkably. Furthermore, the need for 24-h packed RBC transfusions and the length of stay in the ICU and hospital were found to be decreased in patients managed by a trauma team.

7.p53 Gene Mutation in Gastric Cancer Tissue.

PURPOSE: p53 is one of the most commonly mutated genes in human tumors. The aim of this study was to analyze p53 mutation in gastric cancer and its correlations with the clinicopathologic variables to clarify the usefulness of p53 mutation as a prognostic factor. MATERIALS AND METHODS: Specimens from 331 patients with gastric cancer who underwent a gastrectomy between March 1999 and April 2001 at the Kyungpook National University Hospital were used. p53 gene mutations were assessed by using a polymerase chain-reaction single-strand conformation polymorphism (PCR-SSCP) analysis. The correlations between p53 gene mutation and clinocopathologic parameters were analyzed. RESULTS: p53 mutations were found in 66 (19.9%) tumors. Among those 66 cases, mutations were seen in 23 tumors at exon 5, in 8 at exon 6, in 21 at exon 7, and in 17 at exon 8. Two mutations were shown in 3 tumors. Thirty-six (23.1%) of 156 intestinal-type tumors and 19 (13.1%) of 145 diffuse-type tumors showed p53 gene mutation (P=0.007). The frequency of p53 gene mutation didn't show any significant differences according to age, sex, stage, location, or gross type. Exon 5 mutations showed more frequently in intestinal-type tumors than in diffuse-type tumors (9.7% vs. 2.8%, P=0.024), and p53 mutation were more frequent in lymph nodes metastasis group than lymph nodes non-metastasis group with statistical significance (25.0% vs 15.6%, P=0.034). The five-year survival rate showed no statistically significant difference with p53 mutation (P=0.704). CONCLUSION: p53 mutations assessed by PCR-SSCP had little value as a prognostic factor after gastrectomy in patients with gastric cancer.

Virtual Hospital, which is comprehensive medical information system on the web, has medical counseling program with various health information such as self-diagnosis, first-aid treatment information, drug information, Q and A, and so on. The aim of this study is to examine the content and purpose of medical counseling on the web. The subjects consisted of 277 counsels which visited 'Medical Counseling' in the Virtual Hospital for December 1996 through May 1997. Many questioners wanted to consult about their symptoms, to know prevention and treatment strategies, and to know specific medical knowledge. The most common questions are as follows: general and unspecified, digestive, musculoskeletal, and skin problems. These findings suggest medical counseling may be used to supplement the lack of direct medical Interviews with doctors.

9.Anatomical Basis of Pronator Teres for Electromyography Needle Placement Using Ultrasonography.

OBJECTIVE: To find the optimal needle insertion site for needle electromyography of the pronator teres (PT) muscle among commonly used sites. METHODS: Fifty forearms of 25 healthy subjects were evaluated. Four expected needle insertion points were designated as follows. Point 0 was positioned at the midpoint between the medial epicondyle and medial border of biceps tendon in the elbow crease. Points 1, 2, and 3 were located 2 cm, 3.5 cm and 5 cm distal to point 0, respectively. We assumed that the thickness of PT and the distances between a vertical line from each point to the medial margin of the PT were significant parameters for finding the optimal site. Thus, we measured these parameters through ultrasonographic examination. RESULTS: In men, the PT was thickest at point 2, and in women, at point 1. The distance between the expected needle insertion line and medial margin of PT was longest at point 1 in both men and women, and was statistically significant compared to points 2 and 3. Both men and women had neurovascular bundles located lateral to the expected needle insertion line. CONCLUSION: The most appropriate and safe needle electromyographic insertional site for the PT is 2-3.5 cm distal to the mid-point between the biceps tendon and medial epicondyle in the elbow crease and the needle should be inserted upward and medial.

10.A Case Report of Coronary Arteriovenous Fistula Diagnosed by Two-Dimensional and Transesophageal Echocardiography.

Since coronary arteriovenous fistula(CAVF) was first reported by Krause in 1865, more than 400 cases have been reported. It is relatively rare disease and originates more commonly in the right than in the left coronary artery. We report a case of CAVF between right coronary artery and right ventricular inflow tract with significant left to right shunt in a 34-year old female who was admitted for the cardiomegaly on routine chest X-Ray. It was detected by transthoracic and transesophageal echocardiography, and confirmed by cardiac catheterization and coronary angiography. The opening of the fistula draining into the right ventricle was obliterated with sutures. There was no significant shunt in postoperative cardiac catheterization and coronary angiography.
