2.An Extensive Cutaneous Erythema associated with Vasculitis in a Patient with Systemic Lupus Erythematosus.

A 42-year-old woman with systemic lupus erythematosus (SLE) has had wide-spread erythemas with some purpuric patches on her trunk and upper extremities during exacerbation of her disease. Biopsy findings from erythematous lesions and purpuric areas revealed the pathology of lupus erythematosus and leukocytoclastic vasculitis (LCV) with immune deposits, respectively. A feature of wide-spread symmetrical erythemas with foci of LCV occurring in this patient is considered to be an unusual presentation.

4.Clinical Study of Intra-articular Fracture of Distal Radius

The treatment of intra-articular fractures of distal radius is difficult and has various problems. Maximum recovery of wrist function is dependent on accurate and stable reduction of the radial articular surface. We analysed one hundred and five cases of intra-articular fracture of distal radius followed for more than 6 months at Yonsei University from January 1980 to September 1988 and studied the result according to the type of treatment. The results of this study were as follows; 1. The incidence was high in female who aged over 6th decade, and in male who is active aged. 2. The main cause of this injury was fall down(34.3%). 3. In a group of closed reduction and cast immobilization, the functional result was satisfactory in 45.4%. 4. In a group of closed reduction and percutaneous pinning, the functional result was satisfactory in 50%. 5. In a group of open reduction and internal fixation, the functional result was satisfactory in 83. 3%. 6. In a group of external fixation, the functional result was satisfactory in 33.3%. 7. Overall result was mainly correlated with initial severity of the injury and anatomical reduction of the intra-articular surface.

Paget's disease of bone (osteitis deformans), a chronic osseous disease of unknown etiology, is extremely rarein Asia although well known in western countries. This disease is characterized by the simultaneous absorption ofbone and the new bone formation of abnormal character affecting middle and later age groups. The authorsexperienced a case of monostotic Paget's disease involving left pelvic bone in 56-years old female. To ourknowledge, this is the first cas report of monostotic Paget's in Korea.

7.Prognostic Value of CD44v6 Isoform in Infiltrating Ductal Carcinoma of Breast.

CD44 is a family of transmembrane glycoproteins involved in cell-cell and cell-matrix interactions. Expression of CD44 isofonns (splice variants) has been shown to be associated with poor prognosis in several human cancers. We evaluated the expression patterns of the CD44 isofortn (CD 44 splice variant v6) in infiltrating ductal carcinoma of the breast by immunohistochemical and RT-PCR method. Paraffin embedded blocks from seventy-five cases of mastectomized samples were analyzed immunohistochemically using monoclonoal antibody against CD44v6. CD44v6 was detected in fifty-seven cases (76%) of the tumor samples. Adjacent normal myoepithelial cells and ductal epithelial cells revealed focal positive reaction to CD44v6. Thirtytwo cases (80.0%) with lymph nodal metastasis revealed overexpression of CD44v6 monoclonal antibody, but twenty-five cases (71.4%) without nodal metastasis also showed positive reaction to CD44v6 monoclonal antibody, and there is no statistically significant value. Other prognostic factors of infiltrating ductal carcinoma, such as tumor size, histologic grade and hormonal receptors did not show any significant correlation with CD44v6 expression. The RT-PCR studies for 9 cases of infiltrating ductal carcinoma showed the same band patterns both in the normal and tumor tissues. From the above results, it is concluded that the expression of CD44v6 is not a valuable prognostic marker of infiltrating ductal carcinoma of breast.

9.A Case of Bowen's Disease Partially Responded to Photodynamic Therapy.

Photodynamic therapy(PDT) is a treatment modality by highly reactive oxygen intermediates generated through the interaction of light with a photosensiziter. It has been shown to be an effective treatment for various cutaneous and noncutaneous malignancies. It is efficient for the curative and palliative treatment of epithelial skin tumor in situ or early invasive lesions. In effect, it is a useful alternative treatment for the lesions located on anatomically difficult areas or the large-sized lesions. We treated a case of Bowen's disease arising on the plantar area and 3rd and 4th toewebs of left forefoot in a 61-year-old man with PDT using the hematoporphyrin derivative, porfirmer sodium(Photofrin, Russia) as a photosensitizer and gold vapor laser as a visible light source. The outcome showed partial clinical improvement after about 2 months' follow-up.

10.Asymptomatic Primary Hematuria in Children.

Localized or solitary fibrous tumor (SFT) of the pleura has been classified as a type of mesothelioma, arising from the submesothelial connective tissue cells. The preoperative diagnosis of the tumor at the cytologic or histologic level is very important for the proper handling of the lesion. This preoperative diagnosis is now possible by means of the advance in the transthoracic fine needle aspiration biopsy (FNA) techniques and in the very experience of the cytopathologists. We describe FNA cytologic feature of two cases of SFT arising from the pleura. Cytologic, histologic, immunohistochemical, and electron microscopic characteristics of pleural SFT are discussed. The tumor cells of SFT are spindle or oval in shape with a variable amount of cytoplasm. They are arranged in irregular trabeculae intimately ass- ociated with capillaries. A unique cytologic feature observed in this tumor is that thick, eosinophilic, amorphous collagen bundles are scattered between tumor cells
