3.Balloon Catheter Dilatation in Esophageal Achalasia: Long Term Follow-Up.

PURPOSE: To evaluate the clinical efficacy of balloon catheter dilatation in the treatment of esophageal achalasia. MATERIALS AND METHODS: Seven patients(three males and four females) with esopha-geal achalasia were treated with balloon catheter dilatation. Balloon catheters of variable sizes were used depending on patient's conditions. The patients were followed up over a period of 12-39months. RESULTS: Balloon catheter dilatation in esophageal achalasia was successful in all patients without esophageal perforation. All patients were relieved from dysphagia. Recurrence was not found in 5 patients on long term follow-up study, but was seen in 2 patients after 18 and 21 months, respectively. CONCLUSION: Balloon catheter dilatation was a safe and effective method in the treatment of esophageal achalasia with low recurrence rate of 29% on follow-up study.

4.Dyspeptic Symptom Severity Related to Helicobacter pylori and Stress in Functional Dyspepsia.

BACKGROUND: It is well-known that dyspeptic symptoms in functional dyspepsia are related to psychological factors such as stress and mood disorder. But it is still controversial whether H. pylori infection is a important factor for functional dyspepsia. We conducted this study to investigate whether H. pylori infection or stress are related to dyspeptic symptom severity in functional dyspepsia. METHODS: From Mar. 1 1999 to Apr. 30 1999, attendees to a Health Screening Center and a Family Practice Center were eligible study subjects. The subjects were responded to questionnaire including dyspeptic symptom severity items and BEPSI-K(stress scale). And among eligible subjects, one hundred and thirty two diagnosed with functional dyspepsia by gastrofibroscopy were final study subjects. H. pylori infection was confirmed by antral biopsies & smear (Giemsa stain). RESULTS: In functional dyspepsia, high BEPSI-K score group(upper tertile) complained more severe dyspeptic symptom severity than low BEPSI-K score group(lower tertile) after adjusted for other variables(adjusted OR=8.389, 95% C.I.=2.232-31.231). But the dyspeptic symptom severity was not associated with H. pylori infection status (adjusted OR=1.514, 95% C.I.=0.567-4.046). CONCLUSIONS: The dyspeptic symptom severity in functional dyspepsia was associated with stress but not with H. pylori infection status.

BACKGROUND: The relationship between abnormal body fat distribution and increased risks of morbidity and mortality is well established. Less is known about the impact on functional health status and subjective well-being. This study quantified the association between body fatness and health-related quality of life. METHODS: Health-related quality of life was measured by Catholic Medical Center Health Survey(CMCHS) V1.0. Tertiles of body mass index(BMI) and waist circumference in 484 adult persons visiting a university hospital-based Health Screening Center were compared. To estimate the relative risks of poor health in those with high BMI and large waist circumference, odds ratio and 95% confidence interval(CI) were determined by logistic regression analysis. RESULTS: Compared with the reference group (subjects with waist circumference in the lowest tertile), the odds ratios for men with waist circumference in tertile 3 were 2.24 (95% CI:1.15-4.36) for social function, and 2.5(95% CI:1.02-6.11) for change in health. Women in tertile 3 had odds ratio of 3.23(95% CI:1.05-9.97) for bodily pain. Statistical significance was not shown in other domains. The odds ratios of subjects with the largest BMI, compared with those in the lowest tertile, were not statistically significant in 10 domains. CONCLUSION: Large waist circumferences affects quality of life in the domains of social function and change in health for men, and bodily pain for women.

8.Stent Assisted Coil Embolization of Unruptured Aneurysm Associated with Basilar Artery Fenestration: A Case Report.

We report a case of unruptured aneurysm associated with basilar fenestration treated with coil-assisted coil embolization. A 46-year-old man had suffered from intermittent, progressive occipital headache for 2 months. Magnetic resonance (MR) angiography showed a large aneurysm at the proximal basilar artery. Both three-dimensional CT angiography and angiography demonstrated a saccular, 20x15-mm aneurysm arising at the proximal end of associated fenestration at the basilar artery. The aneurysm was successfully occluded with stent-assisted coil embolization. The patient was discharged home after 1 week and received 75 mg clopidogrel once daily for 4 weeks and 100 mg aspirin once daily for life.

9.Repair Integrity and Functional Outcomes after Arthroscopic Repair of Transtendinous Full-thickness Rotator Cuff Tears Minimum Two-year Follow-up.

BACKGROUND: To evaluate the clinical outcomes and associated repair integrity in patients treated with arthroscopic repair for a transtendinous rotator cuff tear followed by resection of the remnant rotator cuff tendon. METHODS: Between July 2007 and July 2011, we retrospectively reviewed patients who were treated for transtendinous full-thickness tears in the tendinous portion of the rotator cuff by arthroscopic repair. Clinical outcomes were evaluated using the American Shoulder and Elbow Surgeons (ASES) score, the Shoulder Rating Scale of the University of California at Los Angeles (UCLA), the Constant-Murley score, a visual analogue scale (VAS) pain score, and range of motion (ROM). The repair integrity was determined by magnetic resonance imaging or ultrasonography. RESULTS: There were 19 shoulders with transtendinous full-thickness tears in the tendinous portion of the rotator cuff. The ASES, UCLA, Constant-Murley, and VAS pain scores showed significant improvements in function and symptoms (all p<0.001). The active ROM for forward flexion and abduction was also significantly improved (p=0.002 and p<0.001, respectively). The postoperative radiological examination showed cuff integrity without a re-tear in 68.4% of patients. However, the UCLA, ASES, and Constant-Murley scores were not significantly different between healed and re-torn group (p=0.530, p=0.885, and p=0.262, respectively). CONCLUSIONS: Although repair of transtendinous rotator cuff tears followed by resection of the remnant rotator cuff tendon in the footprint has a relatively high re-tear rate, no significant difference was observed in the short-term clinical results between the re-tear and healed groups.

10.The factors relates to the screening tests of colorectal cancer in men.

BACKGROUND: Cancer is the second leading cause of death in Korea. Colorectal cancers rank fourth in terms of total cancer mortality. Colorectal cancers have been the focus of screening for early detection. Few current data are available regarding factors associated with participation in cancer screening examinatians in the general population. This study investigated associatians of age, income, educatian, marital status, smoking, drinking, sleeping, presence of a hemorrhoid, and attitudes toward cancer prevention with participation in three cancer screening tests for colorectal cancer. METHODS: To identify the factors associated with participation in cancer screening examinations, 2372 men in a certain campany were surveyed in 1992. Cancer related questians were included to determine the attitudes toward cancer prevention and the history of tests performed that are used for screening for colorectal cancer. RESULTS: Statistically significant (P<0.05) strang predictors of ever having had a specific cancer screening test were as follows : (1) digital rectal examination:abave 40 years old(OR=3.4, 95% CI 2.1-5.4), having a hemorrhoid (OR=2.9, 95% CI 1.7-5.0), and regular exercise (OR=2.1, 95% CI 1.2-3.5) ; (2) fecal occult blood:above 40 years old (OR=4.2, 95% CI 2.5-7.2) and (3) colonoscopy or sigmoidoscopy : above 40 years old (OR=3.5, 95% Cl 2.4-5.2), married (OR=3.5, 95% CI 1.6-7.7), and heavy drinker (OR=1.8, 95% CI 1.2-2.7). CONCLUSIONS: The largest differences among the population for participation in cancer screening examinations involved age. Education and encouragement may persuade men to participate more in screening.
