2.Study of Cancer Cases by Industry in Kwangju-Chonnam Area: Based on Industrial Medical Insurance Record.

In order to investigate the occurrence characteristics of cancer in terms of industry in Kwangju-Chonnam area, medical utilization records of industrial medical insurance corporations during the period of 1987 to 1988 were reviewed for the identification of neoplastic disease. The cases obtained from the medical records were followed up for the verification and to get additional information. Standardized incidence data were compared by occupational characteristics. Multiple logistic regression analysis was applied to analyze the difference of incidence or distribution of cancer as a whole or of some selected cancer. Total cases of cancer identified were 242 during the study period. Annual incidence rate was calculated as 123.1 per 100,000 person. The frequent types of cancer were cancers of stomach, liver, lung, colon and rectum, bladder and lymphoma in descending order. Employees of mine and other sand handling industries showed significantly higher risks for cancer of stomach and cancer as a whole. Employees of the transportation industry showed the higher risk for cancer of liver. Workers in small-sized industry (< or = 100) had a higher risk for cancer than who in large-sized industry (>100). These findings suggested the effect of occupational environmental exposure to cancer development.

In carpal injury, a good functional end result seems to correlate with final anatomic alignment of carpus after treatment. Some cases are often misdiagnosed and result in inadequate treatment. Forty six cases of carpal injuries were analysed which were treated for 9 years from 1975 to 1984 at the Department of Orthopedic Surgery, Seoul National University Hospital. The observations can be summarized as follows. 1. The male to female ratio was 3.5: 1 and 82% of total cases were in the third and fourth decades. 2. The most frequent cause of injury was fall from height with outstretched hand. 3. Trans-scaphoid perilunar dislocation and its variant were the most common diagnosis in this analysis. 4. All of the scapho-lunate dissociation showed volar flexion intercalated segmental instability with scapho-lunate gap more than 2 mm in our series. 5. In 11 cases of trans-scaphoid perilunar dislocation and its variant, carpal collapse was found in 10 cases. In 5 cases dorsiflexion intercalated segmental instability pattern and in one case volar flexion intercalated segmental instability pattern were found to be present. 6, There was an increase in scapho-lunate angle in all cases of dorsiflexion intercalated segmental instability, but a decrease in scapho-lunate angle is not a consistent finding in volar flexion intercalated segmental instability. 7. In 24 cases more than 6 months follow-up, excellent and good results were obtained in 20 cases. 8. As a result of above findings, authors conclude that scapho-lunate gap more than 2mm has diagnostic value in scapho-lunate dissociation but that dorsiflexion intercalated segmental instability or volar flexion intercalated segmental instability only means the changes in the relationship between carpal bones and does not have definite or specific diagnostic value for a specific carpal injury.

4.Effect of Topical Bromfenac as a Treatment of Cystoid Macular Edema Following Pars Plana Vitrectomy

Purpose:
To investigate the functional and anatomical effectiveness of bromfenac sodium hydrate ophthalmic solution for the treatment of cystoid macular edema (CME) after pars plana vitrectomy (PPV).
Methods:
A retrospective chart review of 53 patients was performed. Thirty-seven eyes received topical 0.1% bromfenac sodium hydrate ophthalmic solution (nonsteroidal anti-inflammatory drug [NSAID] group), and 16 eyes were treated with subtenon triamcinolone acetonide injection (injection group). Of the NSAID group, 27 eyes underwent PPV with cataract surgery, and 10 eyes underwent PPV without cataract surgery. Best-corrected visual acuity (BCVA), central macular thickness (CMT), and intraocular pressure (IOP) were measured at baseline and monthly after treatment initiation over a 6-month period.
Results:
No significant difference in mean BCVA from baseline was found for either group over the 6-month follow-up. The injection group showed significantly greater improvement in CMT than the NSAID group at 4 months; however, there was no significant difference between the two groups at the 5-month follow-up visit. IOP was significantly higher in the injection group (p = 0.035 at 2 months), but not in the NSAID group. In the NSAID group, neither PPV group, with and without cataract surgery, showed any significant difference in mean BCVA from baseline over the 6 months of follow-up. The baseline CMT was significantly greater in the PPV with cataract surgery group than in the PPV without cataract surgery group (p = 0.023). However, at the 1-month follow-up, no significant difference in mean CMT from baseline values was observed in either group over the remaining follow-up examinations, up to 6 months.
Conclusions
Topical bromfenac resulted in an improvement in the BCVA and a reduction in the CMT, with a less adverse effect in the treatment of CME after PPV, regardless of whether cataract surgery was performed at the same time.

6.Delayed Onset Abducens Nerve Palsy and Horner Syndrome after Treatment of a Traumatic Carotid-cavernous Fistula

PURPOSE: We report a patient with delayed-onset abducens nerve palsy and Horner syndrome after endovascular treatment of traumatic carotid-cavernous fistula (CCF). CASE SUMMARY: A 68-year-female visited our ophthalmic department complaining of gradual-onset ptosis of the left eye and horizontal diplopia. She had undergone endovascular treatment to treat left-sided traumatic CCF after a car accident 10 years before; she had been told at that time that the treatment outcome was favorable. The left-sided ptosis gradually developed 6 years after the procedure, accompanied by diplopia. The left eye exhibited miosis and the extent of anisocoria increased in dim light. An extraocular examination revealed 30 prism diopters of left esotropia in the primary gaze and a −4 abduction limitation of the left eye. CCF recurrence was suspected; however, magnetic resonance imaging with magnetic resonance angiography of brain did not support this. The esotropia did not improve during the 6-month follow-up and strabismus surgery was performed. CONCLUSIONS: Delayed-onset abducens nerve palsy and Horner syndrome can develop even after successful endovascular treatment of CCF. Strabismus surgery should be considered in patients whose diplopia does not spontaneously improve.

8.Cushing's Disease: The Diagnostic Value of Radiologic Studies.

PURPOSE: To evaluate the diagnostic value of radiologic studies in the Cushing's disease MATERIALS AND METHODS: We analyzed retrospectively the CT(n=19, dynamic CT was not perfomed), MR (n=23, dynamic MR in 2 cases), and the results of inferior petrosal sinus sampling(IPSS)(n=13) in 25 patients(19 women, 6men, age range 16-58, mean 32.2) with pathologically-confirmed ACTH secreting pituitary adenoma. We analyzed the radiologic findings and determined how accurately each method could detect and lateralize the lesion within the pituitary gland. The results were compared with the surgical findings and with each other. RESULTS: Seven patients(28%) out of 25 had macroadenomas and the lesions were detected and correctly diagnosed by CT and MR in all. Eighteen patients(72%) had microadenoma. In patients with microadenoma the sensitivity was 38.5%(5/13) in CT, 64.7%(11/17) in MR, 90.9%(10/11) in IPSS. In two patients with dynamic MR, in whom the conventional Gd-enhanced MR showed false negative results, the lesions were detected as low signal intensity mass and correctly lateralized. Considering the results of both MR and IPSS, the sensitivity increased up to 94.1%(17/18). CONCLUSION: Considering the invasiveness of IPSS, Gd-enhanced MR, preferably dynamic, seems to be an imaging modality of choice in Cushing's disease. Combined use of IPSS in selected patients further increases the accuracy of diagnosis and lateralization.

9.A study for the development of the Korean orthodontic bracket.

The aim of this study was development of the Straight-Wire Appliance(SWA) suitable for the treatment of Korean. To accomplish the object of this study, Korean adult with normal occlusion were selected with following criteria: 1) no functional abnormality in the craniofacial area, 2) good dental arch form and posterior occlusal relationship, 3) Angle Class I occlusal relationship, 4) no experience of orthodontic, nor prosthodontic treatment, especially, no dental treatment on labial and buccal surfaces of teeth, 5) good facial profile. Impression were taken for upper and lower dental arches of the selected normal occlusion samples and the orthodontic dental stone models were fabricated. 5 well-trained orthodontists had examined the acquired dental stone models to select study samples which satisfy the Six keys to optimal occlusion of Andrews. 155 pairs of dental stone models (92 pairs of Male, 63 of Female) were finally selected. 3 dimensional digitization were performed with the Coordinate Measuring Machine(CMM, MPC802, WEGU-Messtechnik, Germany) and measuring of Angulation, Inclination, In-and-Out, Molar offset angle and Arch form were accomplished with a measuring software to achieve data for the development of SWA. Before the measurement, error study was performed on the 3 dimensional digitization with CMM, and the analysis of reliability of computerized measuring method adapted in this study and conventional manual method presented by Andrews was performed. Results of this study were as follows: 1. Equi-distance digitization with mesh size 0.25 mm, 0.5 mm and 1.0 mm were acceptable in 3 dimensional digitization of dental stone model with the CMM, and the digitization with 1.0 mm mesh size was recommendable in terms of efficiency. 2. Computerized measuring method with 3 dimensional digitization was more reliable than manual measuring method of Andrews. 3. Data were collected for the development of SWA suitable for the morphological characteristics of Korean with the computerized measuring method with 3 dimensional digitization.

10.Treatment of Class II malocclusions with upper second molar extraction.

The purpose of this report is to present the successful improvement of occlusal relationship and facial esthetics in class II division 1 malocclusion with severe labioversion of upper anterior teeth and severe overjet, and in class II malocclusion with infraversion of bilateral maxillary canines by MEAW mechanics, which enables us to get effective distal en mass movement of maxillary dentition, with upper second molar extraction. After treatment, there were natural contact points at canine and premolar regions, normal occlusal relationship and treatment results, satisfied the gnathologic concept, in this 2 cases. Compared with the routine treatment with premolar extraction, the treatment time and patients' discomfort were reduced. And the MEAW mechanics, which enables us to get effective distal en mass movement of maxillary dentition, resulted in reduction of the treatment time and getting the good treatment results. Consequently, the major concerns in orthodontic treatment are appropriate diagnosis and treatment plan, so, the application of second molar extraction with appropriate case analysis and diagnosis is very helpful to orthodontic treatment.
