1.The radiological study of optic canal in Korean

Various methods of taking of optic foramen view and avalubilities have been studied by many foreigninvistigators, but those in Korea have not been reported yet. The authors have measured and examined the size andshapes of optic foramina of optic foramen views in 111 Korean checked at Inje Medical College Hospital duringabout 3 years form June 1979 to March 1982. The cases were divided into gwo groups, A and B, for the convenienceof study. Group A consisted of 50 normal Korean 5 to 55 years of age with no visual symptoms and signs. Group Bincluded 69 patients 3 to 75 yrs of age with visual symptoms and signs. The results were as follows; 1. Mean valueof actual diameter of optic foramen meassured by Goalwin's method in Group A; Right; A diameter; 4.21mm, Bdiameter; 4.32mm, Left; A diameter; 4.29mm, B diameter; 4.30mm. 2. Absolute symmetry of the greatest diameters ofboth optic foramina in Group A; 36%, difference of 0.5 or lessl 28%, from 0.5 to 1.0mm; 18%, from 1.0 to 1.5mm;8%, uncomparable cases; 10%. The greatest difference was 1.4mm. 3. The shapes of optic foramen in Group A; ovalshape; 65%, circular ; 29%, keyhole or double foramen; 6%. 4. In Goup B, the peak age group was second decade(31%), the most comon clinical cause of examiniation of optic foramen view was optic nerve atrophy(30%), and thepathologic findings were only 4 cases(7%). 5. The technique of optic foramen view by modified Pfeiffer's methoddelineated the outline of optic foramen more clearly than ordinary Rhese's method in our experience.

2.Comparison between Myocardial Infarction and Congestive Heart Failure Using by Heart Rate Variability Analysis of 24 hours Holter Monitoring.

OBJECTIVES: Power spectrum analysis decomposes the heart rate signal into its frequency components and facilitates separation of sympathetic (low frequency) and parasympathetic (high frequency) activity. In congestive heart failure, augmented sympathetic tone and decreased parasympathetic tone were found. Autonomic nervous system was normalized 6 months after myocardial infarction. So we compared the autonomic nervous system activity by the heart rate variability in congestive heart failure and old myocardial infarction. METHODS: The protocol involved 20 healthy subjects (Group 1), 5 congestive heart failure patients not caused by myocardial infarction (Group 2), 4 congestive heart failure patients due to myocardial infarction and 11 old myocardial infarction patients without heart failure. We took 24 hour Holter monitoring by Del Mar Avionic tape recorder. All Holter tapes were analyzed with use of Model 563 Stratascan Holter Analysis System. We computed power spectra on each 256 sec segment of each hour during 24 hour recording. So, RR interval, SD of RR interval by time domain, and LF, HF, LF/HF ratio, Total PSD by frequency domain were measured. RESULTS: In congestive heart failure, nocturnal HF peak and diurnal variation of LF/HF ratio was decreased relative to healthy subjects. Nocturnal HF peak in old myocardial infarction was not visualized. All of LF, HF and Total PSD in congestive heart failure and old myocardial infarction patients relative to healthy subjects. CONCLUSION: On heart rate variability analysis using by 24 hour Holter monitoring, abnormal autonomic nervous activity was demonstrated in congestive heart failure and old myocardial infarction patients relative to healthy subjects.

3.Magnesium Sulfate Attenuate Opioid Tolerance in Patients undergoing Major Abdominal Surgery.

BACKGROUND: Magnesium is a noncompetitive antagonist of the N-methyl-D-aspartate (NMDA) receptor. Magnesium is thought to be involved in opioid tolerance by way of inhibiting calcium entry into cells. METHODS: The patients were randomly assigned to three groups according to the anesthetic regimens: Group M received magnesium sulfate and Group C received saline intravenously under remifentanil-based anesthesia. Group S received saline intravenously under sevoflurane based anesthesia in place of remifentanil. The patients in the group M received 25% magnesium sulfate 50 mg/kg in 100 ml of saline, and those patients in groups C and S received an equal volume of saline before induction of anesthesia; this was followed by 10 mg/kg/h infusion of either magnesium sulfate (group M) or an equal volume of saline (groups C and S) until the end of surgery. Pain was assessed on a visual analog scale at 1, 6, 12, 24, and 36 hours after the operation. The time to the first postoperative analgesic requirement and the cumulative analgesic consumption were evaluated in the three groups. RESULTS: The visual analog scales for pain and the cumulative analgesic consumption were significantly greater in group C than in other groups. The time to first postoperative analgesic requirement was significantly shorter in group C than that in the other groups. There were no differences between group M and S for side effects. CONCLUSIONS: A relatively high dose and continuous remifentanil infusion is associated with clinically relevant evidence of acute opioid tolerance. NMDA-receptor antagonist, magnesium sulfate as an adjuvant analgesic prevents opioid tolerance in patients who are undergoing major abdominal surgery under high dose and continuous remifentanil infusion-based anesthesia.

4.Effects of the Chronic Use of Anticonvulsants on Bone Mineral Density in Children with Epilepsy.

PURPOSE: Chronic administration of anticonvulsants for children with epilepsy may cause multiple abnormalities in bone metabolism. The aim of this study was to evaluate the effects of anticonvulsants on bone mineral density(BMD) in children with epilepsy. METHODS: Seventy patients with epilepsy were enrolled in this study. All subjects had taken anticonvulsants for at least 1 year or more. Subjects were classified into three groups according to age(5-7 yrs, 8-11 yrs, >12 yrs). BMD was measured by Dual-energy X-ray Absorptiometry(DXA) on the lumbar spine and whole body. Statistical analysis for BMD of anticonvulsants-treated subjects were compared to results with the published data. RESULTS: BMD was significantly decreased in the anticonvulsants-treated group as compared with the control group of all age groups(P<0.05). But BMD among groups according to anticonvulsants was not significantly different(P>0.05). CONCLUSION: Because chronic administration of anticonvulsants for growing children leads to a decrease in BMD, early detection and management of abnormal BMD is important in monitoring of children with epilepsy.

5.Kostmann's Disease: A Case Report.

Kostmann's disease is a rare hematologic disease entity which is characterized by persistent neutropenia and recurrent bacterial infections since early in infancy. The authors experienced a 2-month-old female infant with disseminated bacterial skin infection since 3 weeks of age and persistent neutropenia. Diagnosis was made by neutropenia in the peripheral blood, maturation arrest of myeloid hemopoiesis at myelocyte stage in the finding of bone marrow aspiration and improving response of neutropenia with G-CSF administration. She had experienced 4 episodes of sepsis and one episode of acute otitis externa during follow-up until 14 months of age.

6.Influence of Phenilamine on Pressor Responses of Norepinephrine and Tyramine.

The effect of Pheniramine(Avil), a histaminergic-1 receptor blocking agent presently employed in treating various allergic diseases on pressor actions of norepinephring(NE) and tyramine (TR) was studied in the rabbit. Pheniramine, when given into a femoral vein with a dose(3mg/kg) enough to block H1-receptor, potentiated markedly the pressor responses of NE and TR. The pressor action of NE augmented by pheniramine was not affected by additional adminstration of debrisoquin (Drenergic neuron blocker) or phenelzine(monoamine oxidase inhibitor) or desipramine(U1-uptake blocker), or while potentiated by additional treatment with chlorisondamine(ganglionic blocker)or reserpine(catecholamine depleter). The hypertensive response of NE to phenelzine or desipramine was reinforced significantly by addition of pheniramine, but the response of NE in rabbits treated with reserpine or chlorisondamine or debrisoquin was not influenced by pheniramine-addition. Elevation of blood pressure to TR potentiated by pheniramine was attenuated significantly by reserpine treatment with chlorisondamine made the significant augmentation of pressor action to TR after pheniramine. Tyramine-induced response of blood pressure after pheniramine, but the response of blood pressure to TR caused by phenelzine or desipramine was enhanced markedly by pheniramine-treatment. From the above experimental results, it is thought that the pressor effect of NE and TR potentiated by pheniramine is similar to that of debrisoquin, i.e. the sensitization of effector cell, and that central action of pheniramine can not ruled out.

7.The Effects of Fructose Polymer Levan on the Body Fat Accumulation and Serum Lipid Profiles of Korean Women.

This study was performed to investigate the effects of a levan diet on the body fat accumulation and serum lipid composition of 29 Korean women over a period of up to 12 weeks (n = 13 for the control group, n = 16 for the levan group). The subjects ate an uncooked diet (6 g) with 400 ml of tap water twice a day. The mean body weight and height measurements of the subjects (levan group) at the outset were 66.0 +/- 8.8 kg and 156.7 +/- 5.3 cm, respectively. The subjects showed a significant reduction in weight, body fat mass, anthropometric measurements and skinfold thickness during the experimental period. The waist hip ratio (WHR) was 0.88 +/- 0.03 at the outset, and fell to 0.82 +/- 0.05 after 4 weeks. The intake of levan was also influenced on the levels of serum Fe, leptin, lipoprotein lipase, HDL-cholesterol, LDL-cholesterol, and triglyceride. The serum glucose levels were within the normal range during the experimental period. The initial serum triglyceride level was 121 mg/dl, but fell to 103 mg/dl after 4 weeks of levan supplementation. The current study demonstrates that a levan diet is effective in controlling weight, body fat, HDL-cholesterol, LDL-cholesterol, and triglyceride levels.

8.Mutational Analysis of FOXL2 Gene in Blepharophimosis-Ptosis-Epicanthus Inversus Syndrome (BPES) Patients.

PURPOSE: The purpose of this paper is to identify the forkhead transcription factor gene (FOXL2) mutations in Korean patients with blepharophimosis-ptosis-epicanthus inversus syndrome (BPES). METHODS: We have analyzed the mutations of FOXL2 gene in genomic DNAs extracted from 16 BPES patients and their families by PCR, PCR-SSCP, and sequencing. RESULTS: No deletion in exon 1 to 3 of the FOXL2 gene was observed by PCR. The PCR products were subjected to SSCP analysis and 9 patients showed SSCP shifts. The PCR products showing SSCP shifts were subcloned into plasmid vectors and sequenced to confirm the FOXL2 mutation. In total, 7 mutations (1 nonsense mutation, 1 deletion, and 5 duplications) in exon 2 were identified. CONCLUSIONS: The FOXL2 gene mutations were identified in the Korean BPES patients. Some of the mutations were previously reported and some were new mutations. This study will contribute to the molecular analysis and clinical counseling of BPES patients.

9.Effect of 2% chlorhexidine application on microtensile bond strength of resin composite to dentin using one-step self-etch adhesives.

OBJECTIVES: This study examined the effect of 2% chlorhexidine on the microTBS of a direct composite restoration using one-step self-etch adhesives on human dentin. MATERIALS AND METHODS: Twenty-four extracted permanent molars were used. The teeth were assigned randomly to six groups (n = 10), according to the adhesive system and application of chlorhexidine. With or without the application of chlorhexidine, each adhesive system was applied to the dentin surface. After the bonding procedure, light-cure composite resin buildups were produced. The restored teeth were stored in distilled water at room temperature for 24 hours, and then cut and glued to the jig of the microtensile testing machine. A tensile load was applied until the specimen failed. The failure mode was examined using an operating microscope. The data was analyzed statistically using one-way ANOVA, Student's t-test (p < 0.05) and Scheffe's test. RESULTS: Regardless of the application of chlorhexidine, the Clearfil S3 Bond showed the highest microTBS, followed by G-Bond and Xeno V. Adhesive failure was the main failure mode of the dentin bonding agents tested with some samples showing cohesive failure. CONCLUSIONS: The application of 2% chlorhexidine did not affect the microTBS of the resin composite to the dentin using a one-step self-etch adhesive.

10.A Case of X-linked Agammaglobulinemia.

X-linked agammaglobulinemia is attributed to the genetic defect for Bruton's tyrosine kinase at Xq22 region and the developmental arrest of pre-B lymphocytes. The characteristics of the disease are as follows; 1) male sex, 2) onset in infancy or early childhood, 3) severe panhypogammaglobulinemia, 4) normal cell mediated immunity, 5) recurrent, hardly controlled infection. The most common sites of infection are the respiratory tract and the gastrointestinal tract. The disease must be suspected when the recurrent, hardly controlled infection or the unusual, multiple sites of infection are present. Regular intravenous immune globulin approved the preventive effect against severe infection and fatal complication. But the final outcome remains grave in spite of intensive care. We report an adult case, 20 years old male patient, of X-linked agammaglobulinemia. He has been suffered from recurrent pneumonia and other sites of infection including meningitis and cellulitis. Pseudomonas aeruginosa was cultured from blood. X-linked agammaglobulinemia was diagnosed based on clinical history, severe panhypogammagloblinemia, lack of the gamma-fraction on the serum protein electrophoresis and absence of B-lymphocytes in peripheral blood. The clinical course waxed and waned until intravenous infusion of immune globulin, which dramatically improved pneumonia.
