1.Minimum Five-Year Results of Arthroscopic Management with Labral Preservation in Borderline Hip Dysplasia

Purpose:
This study examined the patient-reported outcome scores at mid-term follow-up after arthroscopic labral preservation in patients with borderline hip dysplasia.
Materials and Methods:
Data were collected retrospectively from patients who underwent arthroscopic labral preservation. Among the 61 patients classified with hip dysplasia, patients with arthritis beyond Tönnis grade II were excluded, Patients with subchondral bony exposure were excluded even if they had arthritis of Tönnis grade I. Forty-two patients with borderline hip dysplasia were enrolled among patients whose lateral center edge angle (LCEA) was between 18° and 25°. The PRO scores including the modified Harris hip score (mHHS), hip outcome score activities of daily living (HOS-ADL), hip outcome score sport-specific subscale (HOS-SSS) and visual analog scale (VAS), were collected and compared preoperatively, and then at one year and a minimum of five years after surgery.
Results:
The mean preoperative LCEA and Tönnis angle were 20.9° (range, 18° to 25°) and 6.3° (range, 0° to 18°), respectively. The mean follow-up was 64.8 (range, 60 to 84) months. The mean mHHS increased from 73.5 before surgery to 86.2 in the fifth year after surgery (p<0.01), the mean HOS-ADL from 70.5% before surgery to 84.9% in the fifth year after surgery (p<0.01), and the mean HOS-SSS from 51.4 before surgery to 69.4 in the fifth year after surgery (p<0.01). The mean VAS score improved from 6.3 before surgery to 1.5 in the fifth year after surgery (p<0.01).
Conclusion
Arthroscopic labral preservation performed in appropriately selected patients with borderline dysplasia showed good results at mid-term follow-up. In borderline hip dysplasia without the progress of osteoarthritis and subchondral bone exposure, arthroscopic labral preservation may be considered a good and less invasive option.

3.Cytoskeletal Changes in Cortical Dysplasia.

Cortical dysplasia is a cause of intractable epilepsy and a candidate for surgical resection to control epileptic attacks. The neuronal cytomegaly and balloon cell change are the diagnostic hallmarks of cortical dysplasia. Little research has been performed about the normal-sized dysplastic neuron which has complex arborizing dendrites and lacks in its polarity. The aim of this study was to define the histopathologic characteristics of the neurons in cortical dysplasia. Twelve cases of cortical dysplasia who underwent partial lobectomy for intractable seizures were selected and immunohistochemical staining for NF-M/H, MAP2, tau, and ubiquitin was performed. The perikarya and dendrite of dysplastic neurons were more intensely labeled with antibodies for the high and medium molecular weight neurofilament proteins (NF-M/H) than normal neurons. Immunoreactivity with the MAP2 antibody expressed mainly within the somatodendritic regions was present in the dysplastic or normal neurons without any significant difference in intensity. The complex arborizing dendrites of dysplastic neurons were easily identified due to pronounced immunoreactivity within the somatodendritic regions. Immunoreactivity with the primary antibody against tau and ubiquitin was present in the normal-looking neurons as well as the dysplastic neurons. This study suggests that the dysplastic neurons in cortical dysplasia are accompanied by changes of cytoskeletal neurofilaments, and the immunohistochemical stains for NF-M/H, MAP2, tau, and ubiquigin are useful to detect them.

4.Lipid Peroxidation in Vivo Monitored as Ethane Exhalation in Hyperoxia.

In vivo ethane production in rats was used as an index of oxygen toxicity. The rats were allocated to four exposure conditions; hyperbaric oxygenation (HBO=5 ATA, 100% O2), normobaric oxygenation (NBO=1 ATA, 100% O2), hyperbaric aeration (HBA=5 ATA, 21% O2) and normobaric aeration (NBA=1 ATA, 21% O2). After 120 minutes of exposure, the rats exposed to high concentration and/or high pressure oxygen exhaled significantly larger amounts of ethane than those exposed to NBA, and the differences in ethane production between any two groups were statistically significant (p<0.01). This finding supports the hypothesis that hypothesis that hyperoxia increase oxygen free-radicals and the radicals produce ethane as a result of lipid peroxidation. It is notable that the ethane exhalation level of the HBA group was significantly higher than that of the NBO group. This difference could not be accounted for by the alveolar oxygen partial pressure difference between the two groups.

5.Malignant Granular Cell Tumor of the Shoulder: A case report.

A malignant granular cell tumor (MGCT) occurred in the left shoulder of a 62-year-old man. The patient underwent wide marginal excision followed by chemotherapy and radiotherapy. A metastatic tumor was identified in the axillary lymph node 22 months after the excision of the shoulder mass. The primary tumor was a poorly circumscribed mass measuring 5 5 4 cm. On cut section, it was a solid mass with yellowish tan color. Histologically, both primary and metastatic tumor consisted of polygonal cells with abundant granular cytoplasm and a vesicular nucleus with a prominent nucleolus. Two to three mitotic figures per ten high power fields at 200 were counted. Tumor cells were weakly stained with periodic acid-Schiff (PAS) preparation both before and after diastase digestion, and were positive for S-100 protein, neuron-specific enolase (NSE), and vimentin. By electron microscopy, the cytoplasm was filled with numerous autophagolysosomes containing myelin figures, mitochondria, and fragmented rough endoplasmic reticula. Basal laminae and angulated bodies were also noted. These findings suggest schwannian differentiation of this tumor.

6.Effect of Allopurinol Pretreatment on Superoxide Dismutase Activity in Ischemia Reperfusion Injury to Skeletal Muscles of the Hindlimbs of the Rats

It has been well known that ischemia reperfusion injury to skeletal muscle following an acute arterial occulusion causes significant morbidity and mortality. The skeletal muscle, which contains high energy phosphate compounds, has ischemic tolerance. During the ischemia, the ATP is catalyzed to hypoxanthine anaerobically and hypoxanthine dehydrogenase is converted to xanthine oxidase. During reperfusion, the hypoxanthine is catalyzed to xanthine by xanthine oxidase under O2 presence and that results in production of cytotoxic oxygen free radicals. The authors perform the present study to investigate the effects of allopurinol, the inhibitor of xanthine oxidase, on reperfused ischemic skeletal muscles by measuring of the immunoreactivities and exzyme activities of superoxide dismutase(SOD) and the formation of malondialdehyde(MDA). A total of 104 healthy Sprague-Dawley rats weighting from 200 gm to 250 gm were used as experimental animals. Under urethane(3.0mg/kg., IP) anesthesia with 3.0mg/kg of urethane, lower abdominal incision was made and the right and left common iliac artery were ligated by using vascular clamp for 2 hours. Both the quandriceps femoris muscles were obtained at 0 hour, half hour, 1 hour, 3 hours, 6 hours and 12 hours after the removal of vascular clamp. In the allopurinol pretreated group, 50mg/kg of allopurinol was administered once a day for 2 days and before 2 hours of ischemia. The specimens were sectioned in 14µm thickness with cryostat and homogenated ischemia. The specimens were sectioned in 14 µm thickness with cryostat and homegenated in the phosphate buffer. The immunoreactivities and enzyme activities of SOD were observed. The results were as follows: 1. The immunoreactivitiy and enzyme activity of SOD are decreased and the MDA level is increased in the 2 hours inchemic quadriceps femoris muscle of rats. 2. During the reperfusion of ischemic quadriceps femoris muscle of rats, the immunoreactivities of SOD in the half hour reperfused ischemic group and the enzyme activities of SOD in the 1 hour reperfused group are the highest and the immunoreactivities and enzyme activities of 6 hours reperfused ischemic group are the lowest. 3. Pertreatment of allopurionl decreased the immunoreactivities and enzyme activities of SOD during the ischemia and reperfusion of the quadriceps femoris muscles of rat. This results suggest that the allopurinol decreases the damages of skeletal muscles of rate during ischemia and reperfusion.

7.A New Method in the Measurement of Tibiofemoral Angle

The tibiofemoral angle is widely used for the diagnosis and treatment of knee problems, especially in degenerative joint disease of the knee. Among the various methods of measurement of tibiofemoral angle, Bauers method is being used most frequently. But measurement by Bauers method has seemed to show wide individual differences. Therefore we designed a ruler which can be used very easily for the measurement of the tibiofemoral angle with little differences between persons who measure the angle. To testify the advantages of our method, we selected 4 orthopedic surgeons and let them measure the tibiofemoral angles of 60 knees of 30 patients by Bauers method and by our method using the ruler. And 20 orthopedic surgeons measured tibiofemoval angles of 2 left knees by Bauers method and by our method using the ruler. The values of the tibiofemoral angle by two methods were compared and analyzed statistically. There was no significant differences between mean values. But, the tibiofemoral angles by our new method showed little differences among measurers. (P < 0.05)

8.Clinical Trial on the Antihypertensive Effect of Perdipine.

The antihypertensive effect and side reactions of perdipine were evaluated in 30 patients with essential hypertension. The results were as follows; 1) Before medication and after 2, 4, 5 and 8 weeks of medication, the over all average systolic and diastolic blood pressure were 170+/-13/104+/-7, 152+/-13/93+/-6, 146+/-11/91+/-6, 143+/-9/90+/-7, and 141+/-10/89+/-7mmHg, respectively. In 89% of all cases, marked or moderate degree of antihypertensive effect was observed. 2) There were no significant changes in heart rates before and after treatment. 3) In 83% of all cases, improvement of symptoms were observed. 4) The side reactions of oral perdipine were mild constipation, anorexia, facial flushing, dizziness, and headache, respectively one case. but there were no side reactions which required discontinuing the treatment, except 1 case which discontinued the medication because of severe bradycardia and dizziness.

9.A STUDY OF THE MICROLEAKAGE AT THE POST AND CORE AND TOOTH INTERFACE.

Post and core is used to restore endodontically treated teeth, and it is a very important part which supplies retention and support to the prosthesis. But occasionally, caries occurred due to the microleakage at the post and core and tooth interface, the failure of prosthesis has happened. In this study, the microleakage of cast gold post and core, amalgam core, and composite resin core was investigated and compared. The coronal part of the extracted upper anterior teeth were removed and endodontically treated with conventional method. The teeth were divided into three groups. In group 1, cast gold post and core was cemented with resin cement and in group 2, ready-made post was cemented with resin cement and amalgam core was built. In group 3, ready-made post was cemented with resin cement and composite resin core was built. All specimens were thermocycled between 5degrees C and 55degrees C with dwell time of 15 seconds, and immersed in 0.5degrees C aqueous solution of basic fuchsin dye for 24 hours. After embedded in the epoxy resin, the specimens were sectioned longitudinally and observed with stereomicroscope with the magnification of 25. From the findings of this study, the following conclusions were obtained. 1. There was microleakage at the core/tooth interface of all specimens. 2. The microleakage of gold post and core was significantly less than those of the other two groups. 3. There was no significant difference between the microleakage of amalgam core and that of composite resin core.

10.Results of Nuvita Phakic Anterior Chamber Intraocular Lens for High Myopia.

PURPOSE: The study was designed to evaluate the clinical results of an anterior chamber intraocular lens implantation (Nuvita MA 20 implantation) on high myopic patients. METHODS: We reviewed the clinical results of Nuvita MA 20 implantation surgery in 6 high myopic patients (11 eyes) with more than -7.0 diopters (D) for a year after the surgery. One patient was male,five patients were female and the mean age was 31.5 years old (25~41 years old). RESULTS: Mean preoperative spherical equivalent was -16.61 4.9 D and mean postoperative spherical equivalent was -0.9+/-0.5 D at 1 week, -1.275+/-1.14 D at 1 month, -1.14+/-1.22 D at 3 months, -1.18+/-1.18 D at 6 months and -1.05+/-1.2 D at 12 months. Mean uncorrected visual acuity preoperatively was 0.03+/-0.06 and postoperatively was 0.6+/-0.25 at 1 week, 0.57+/-0.25 at 1 month, 0.67+/-0.4 at 6 months, 0.8+/-0.32 at 12 months. Postoperative complications included 6 eyes of IOL rotation (55%), 4 eyes of pupil distortion (36%), 1 eye of intraocular pressure elevation (9%), 1 eye of pupillary block (9%), 1 eye of IOL decentration (9%) and 1 eye of nuclear cataract (9%), but serious postoperative complications such as retinal detachment and bullous keratopathy were not found. Mean endothelial cell density was 2677+/-148 (cells/mm2) at preoperative period, 2576+/-167 (cells/mm2) at 1 month, 2546+/-127 (cells/mm2) at 3 months, 2490+/-171 (cells/mm2) at 6 months and 2408+/-133 (cells/mm2) at 12 months. CONCLUSIONS: The implantation of Nuvita MA 20 into anterior chamber is considered to be a safe method of treatment for patients with high myopia.
