1.Two Cases of Nasopharyngeal Carcinoma Treated with Co-60 HDR ICR.

The primary treatment modality of malignant tumors of the nasopharynx is radiation therapy owing to its inaccessibility to surgical intervention. Over the last two decades there were many changes in techniques of delivery, which include the use of higher doses of radiotherapy, the use of wide radiation field, including the elective radiation of the whole neck, the combined use of brachy- and teletherapy, and the use of split-course therapy. In spite of these advances local and regional recurrences remain the major cause of death. As a boost therapy after external irradiation, high-dose-rate intracavitary irradiation using remote control afterloading system (RALS) was used in two patients. Our results were satisfactory, however, this procedure should only be performed by those who have developed enough expertise in the use of intracavitary techniques for the treatment of nasopharyngeal cancer and have a supportive team including a physicist, dosimetrist, nurse, and trained technologist.

3.Etiology and Radiologic Findings of Anoxia Occured at Dan-IVlu-Ji(Salted Radish in Rice Bran) Manufacture: A Case Report and Results of Gas Analysis.

PURPOSE:To identify the main toxic gas released from salted radish in rice bran(Dan-Mu-Ji) and to introduce the radiological findings of the patient who was exposed to the gas. MATERIALS AND METHODS: Chest radiographs and CT scans of one survivor among three men who were exposed to the gas from Dan-Mu-Ji were reviewed. Gas obtained from the closed bottle containing Dan-Mu-Ji was analized by using the gas chromatography. RESULTS: The radiographic examinations of the survivor were suggestive of pulmonary edema with it's rapidly improving consolidations in both lung. The headspace gas within the bottle containing Dan-Mu-Ji was mainly composed with carbon dioxide, ethyl alcohol and hydrogen sulfide, of which hydrogen sulfide was considered the main toxic gas released. CONCLUSION: Under the anaerobic condition, Dan-Mu-Ji released toxic hydrogen sulfide. Inhalation of hydrogen sulfide might produce non-cardiogenic pulmonary edema.

4.A Case of Intracranial Mycotic Aneurysm due to Aspergillus species.

Intracranial mycotic aneurysms due to Aspergillus species are extremely uncommon but fatal. A medium-sized ruptured intracranial aneurysm at the middle cerebral artery bifurcation was identified in a 50-year-old female patient. Proper microsurgical clipping was not feasible due to the aneurysm's friable nature. Microsuture and wrapping were done instead. Histological findings confirmed a mycotic aneurysm caused by Aspergillus. Herein, we report on the clinical course and histopathological findings with a relevant literature review.

5.A Case of Vesical Dome Transitional Cell Carcinoma Resembling Urachal Carcinoma.

Vesical dome tumors are classified, according to their origin, as either vesical or urachal. A case of vesical dome tumor was diagnosed as an urachal carcinoma preoperatively because CT scan and cystoscopic findings were suitable to the diagnostic criteria of urachal carcinoma, but the transurethral biopsy result was transitional cell carcinoma (TCC). The final histopathologic report after extended partial cystectomy revealed that the tumor was originated from vesical mucosa and an intact urachal remnant was found around the resected mass. In a tumor situated on the midline of bladder dome, a transurethral biopsy before curative operation is essential.

7.Results of Hip Arthroplasty Using a COREN Stem at a Minimum of Ten Years

Purpose:
We report on the 10-year clinical hip function and radiologic outcomes of patients who underwent hip arthroplasty using a COREN stem.
Materials and Methods:
A consecutive series of 224 primary cementless hip arthroplasty implantations were performed using a COREN stem between 2009 and 2011; among these, evaluation of 128 hips was performed during a minimum follow-up period of 10 years. The mean age of patients was 65.4 years (range, 40-82 years) and the mean duration of follow-up was 10.8 years (range, 10-12 years). Evaluation of clinical hip function and radiologic implant outcomes was performed according to clinical score, thigh pain, and radiologic analysis.
Results:
Dramatic improvement of the mean Harris hip score (HHS) from 59.4 preoperatively to 93.5 was observed at the final follow-up (P≤0.01). Stable fixation was demonstrated for all implants with no change in position except for one case of Vancouver type B2 periprosthetic femur fracture. A radiolucent line (RLL) was observed in 16 hips (12.5%). Thigh pain was observed in only two hips (1.6%) at the final follow-up. There were no cases of osteolysis around the stem. The survival rate for the COREN stem was 97.7%.
Conclusion
Good long-term survival with excellent clinical and radiological outcomes can be achieved using the COREN femoral stem regardless of Dorr type.
8.A Case of Nephrogenic Adenoma of the Ureter.

Nephrogenic adenoma is rare, especially in the ureter. It is thought to represent a metaplastic phenomenon forming the tubules which resemble renal tubules. The cause is unknown but may be associated with chronic irritations such as surgical trauma, incarcerated calculi and infection. We report a case of nephrogenic adenoma of the ureter in a 44-year-old woman who bad been suffered from the left flank pain due to left ureteral stone with giant hydronephrosis.

10.A Case of Generalized Putular Psoriasis after Varicella Infection.

Generalized pustular psoriasis is the most servere form of psoriasis. This disorder is characterized by pustular skin lesions general symptoms such as high fever, weakness and peripheral blood leukocytosis. We have experienced a case of generalized pustular psoriasis after suffering from chickenpox which was diagnosed by clinical symptoms and pathologic features from a skin biopsy. This 4-year-old male patient was managed by local and oral corticosteroid therapy with excellent outcome. A brief review of the related literature is also included.
