2.Comparison of the clinical results of fixed-bearing and mobile-bearing total knee arthroplasty

0.05) at preoperatively and postoperatively the total knee score,preoperatively and postoperatively pain score,preoperatively and postoperatively patient function score,and preoperatively and postoperatively Patellar score.The postoperative maximum flexion of the fixed-bearing group and the mobile-bearing group was 108??9.3? and 99.5??10.1? respectively.There was significant difference between the 2 groups for the postoperative maximum flexion (P0.05) at the femoral valgus angle,the tibial angle,the tibial slope angle,and preoperatively and postoperatively the patellar height (Install-Salvati ratio).[Conclusion]The short-term results of the fixed-bearing posterior stabilized prosthesis and the mobile-bearing prosthesis TKA were successful.There was no significance difference between the two groups for the total knee score,function score,pain score,patellar score and radiographic results.The postoperative maximum flexion in fixed-bearing group was higher than that in mobile-bearing group.
3.Effect of auricular-plaster therapy combined with aromatherapy on anxiety and quality of life in patients with colorectal surgery

Objective To explore the effect of auricular-plaster therapy combined with aromatherapy on anxiety and quality of life in patients with colorectal surgery.Methods A total of 135 patients with colorectal surgery were randomly divided into group A (auricular-plaster therapy combined with aromatherapy),group B(auricular-plaster therapy),group C (aromatherapy),group D(blank).Anxiety and life quality levels of four groups were determined by using self-rating anxiety scale(SAS),quality of life instruments for cancer patients (QLICP-CR)and were compared between four groups before and after intervention.Results The SAS and QLICP-CR scores of group A were lower than those of group B,group C and group D after intervention.The SAS and QLICP-CR scores of group B and group C were lower than that of group D after intervention.There were no statistical significant differences in SAS and QLICP-CR scores between group B and group C after intervention.Conclusions Auricular-plaster therapy combined with aromatherapy can effectively alleviate anxiety of patients with colorectal surgery and improve their quality of life,and the effect is better than that of auricular-plaster therapy and aromatherapy.
4.Comparison of stress responses to the intubation with lightwand and direct laryngoscope in elderly patients

Objective To compare the effects oflightwand(LW) and direct laryngoscope(DL) in elderly patients undergoing surgery on hemodynamics and stress responses.Methods Forty elderly patients with ASA grade Ⅰ-Ⅱ grade who selective surgery were divided into LW group and DL group by random digits table method,each group with 20 cases.Systolic blood pressure (SBP),diastolic blood pressure (DBP),heart rate(HR) were recorded respectively before intubation,30 seconds and 5 minutes after intubation.Extracted patients with peripheral arterial 3 ml at the same time,measured by radioimmunoassay epinephrine and norepinephrine levels.Results The SBP,DBP,HR,epinephrine,norepinephrine 30 seconds after intubation in LW group were significantly lower than those in DL group [(140.50 ± 21.91) mm Hg(1 mm Hg =0.133 kPa) vs.(167.95 ± 17.85) mm Hg,(67.80 ± 6.76) mm Hg vs.(84.15 ±9.89) mm Hg,(85.10 ± 11.76) times/min vs.(96.30 ± 13.72) times/min,(49.47 ± 8.09) ng/L vs.(61.25 ± 9.43) ng/L,(171.27 ± 17.11) ng/L vs.(187.40 ± 16.13) ng/L,P < 0.01].The SBP,epinephrine,norepinephrine 5 minutes after intubation in LW group were significantly lower than those in DL group [(120.75 ± 17.12) mm Hg vs.(136.65 ± 15.43) mm Hg,(39.80 ±7.38) ng/L vs.(47.63 ±8.48) ng/L,(155.93 ± 17.11) ng/L vs.(172.76 ± 13.45) ng/L,P < 0.01].Conclusion Both LW and DL can be well done in elderly patients intubation,but LW can reduce stress responses during the intubation.
5.The effect of the different swallowing training techniques on the pre-swallow peak pressure of upper esophageal sphinctor

Objective To explore the change of the pre-swallow peak pressure of upper esophageal sphincter (UES) in patients with post-stroke cricopharyngeal achalasia,and investigate the effect of pre-swallowing peak UES pressure on swallowing function by quantitative analysis.Methods Fifty-seven stroke patients with cricopharyngeal achalasia were recruited and divided into balloon dilation group,combined training group and routine swallowing training group with 19 patients in eachp.All the three groups accepted routine swallowing training.In addtion,the routine swallowing training group and balloon dilation group accepted larynx elevation training and balloon dilation training,respectively,while the combined training group accepted larynx elevation training and balloon dilation training simultaneously.The pre-swallow peak UES pressure was measured by using PC polygraph high rate gastrointestinal dynamical detection system (PC Polygraf HR,CTD-synectics,Sweden) before and after 8 weeks of treatment.The swallowing function was assessed using swallowing function classification and water swallowing test.Results Before treatment,there was no significant difference among the 3 groups in terms of the pre-swallow peak UES pressure,swallowing function classification,water swallowing test and VFSS (P > 0.05).After treatment,pre-swallow peak UES pressure,swallowing function classification,water swallowing test and VFSS of the balloon dilation group and combined training group improved significantly compared with those before treatment (P < 0.05),and the improvement in the combined training group was to a significantly better extent than in the balloon dilation group(P <O.05).Conclusion Balloon dilation and larynx elevation training plus routine swallowing training can increase pre-swallow peak UES pressure,decrease the UES resting pressure of stroke patients with cricopharyngeal achalasia,which is of great importance for their recovery.
6.The effect of neural stem cell transplantation on angiogenesis after spinal cord injury

Objective To observe the influence of transplanting neural stem cells (NSCs) on angiogenesis in rats with spinal cord injury (SCI).Methods The Allen's method was used to create SCI models in sixty adult Sprague-Dawley (SD) rats.They were then randomly classified into a control group which received injections of phosphate buffered solution (PBS) and an NSC group which received injections of NSCs via the tail vein,with 30 rats in each group.Another group of 30 similar rats without SCI received injections of NSCs via the tail vein as the normal group.Each rat was evaluated before transplantation and at days 7 and 14 post-transplantation using the Basso,Beattie and Bresnahan (BBB) scale for testing hindlimb function.After sacrifice,the distribution of yon Willebrand factor (vWF) in both groups was determined by immunofluorescence,and Western blotting was used to detect vascular endothelial growth factor (VEGF) protein.Results The average BBB score of the normal group was 21 at every time point.Before transplantation,the BBB scoresof the control and NSC groups were both 0,however they increased over time.At day 7 post-transplantation,the BBB scores showed no significant difference between the control group and the NSC group.At day 14 post-transplantation,the average BBB score of the NSC group was significantly higher than that in the control group.At days 7 and 14,the counts of vWF-positive cells in the normal group were significantly higher than in the control and NSC groups.VEGF protein expression in the normal group was significantly lower than in the NSC and control groups.Conclusions NSC transplantation may promote angiogenesis after spinal cord injury and improve motor function by inducing the expression of VEGF.
7.Distribution and Antibiotic Resistance of Pathogenic Bacteria in Patients with Urinary Tract Infections

OBJECTIVE:To analysis the distribution and antibiotic resistance of common isolated pathogenic bacteria in urinary-tract infection cases in our hospital so as to provide scientific basis for the rational clinical use of antibacterials.MET-HODS:625 urine samples were collected from urinary-tract infection cases in our hospital in 2006,from which,344 strains of bacteria were isolated,which were cultured and subjected to drug sensitivity test.RESULTS:Of the 344 strains of bacteria,Gram-negative bacilli took the lead,accounting for 60.2% of the total,followed by Gram-positive cocci(33.4%) and fungi(6.4%).The detection rates of ESBLs-producing Escherichia coli.and Klebsiella pneumoniae were 49.2% and 27.3%,respectively.The bacteria had different resistant rate to different antibiotics and were multidrug resistant.CONCLUSION:Bacterial culture of urine and drug-sensitivity test should be performed promptly and drugs should be used with prudence for urinary infection cases to reduce drug-resistance bacteria.
8.The effects of stromal cel1-derived factor 1 on the migration of neural stem cells and the recovery of lower limb function after spinal cord injury

Objective To assess the effect of stromal cell-derived factor 1 (SDF-1) on the migration of neural stem cells and recovery of lower limb function after spinal cord injury (SCI).Methods A modified version of Allen's method was used to establish a SCI model in each of 96 adult male Sprague-Dawley rats.They were then randomly divided into group A which received an injection of phosphate buffer solution,group B which received an injection of neural stem cells,group C which received a combination of SDF-1 and neural stem cells,and group D which received AMD3100 and neural stem cells 7 days after the modeling.The functioning of the hind limbs of all of the rats was assessed on the 7th,14th,21st and 28th day after the modeling.The rats were then sacrificed and frozen sections of their spinal cords were stained with hematoxylin-eosin (HE) and marked with CM-Dil under fluorescent light.Results An accumulation of fluorescing cells were observed in spine cords from both group B and C,with the counts of group B [(23.6 ±3.7),(18.9 ±5.6)and(15.2 ±4.3) respectively] at the day 14,21 and 28 after modeling significantly smaller than those of group C [(27.4 ± 4.7),(20.4 ± 5.2) and (18.3 ± 3.9) respectively].During the same period of time,the average BBB scores of groups B and C were significantly better than those of groups A and D.Moreover,the average score of group C was significantly higher than group B's at all time points.Conclusions Transplanted neural stem cells can migrate to the injured site,surviving and differentiating to promote the recovery of limb function in rats with spinal cord injury.SDF-1 can promote that migration and proliferation.Moreover,CXCR4 receptor's antagonist AMD3100 can significantly hinder neural stem cells' migration to the injured site on the spinal cord.
9.Effect of Heat-clearing and Detoxifying Herbs and Tonifying Herbs on Mice Hepatoma Cells Line H_(22) and Their Immunologic Mechanism

[Objective] To compare the effects of heat-clearing and detoxifying herbs with that of tonifying herbs on mice hepatoma cells line H22, and to explore their immunologic mechanisms. [Methods] Herba Rabdosiae Rubescentis (HRR) and Herba Scutellariae Barbatae (HSB) were selected as the heat-clearing and detoxifying herbs, and Radix Astragali (RA) as the representative of tonifying herbs. Fifty KM mice were inoculated with hepatoma cells line H22 and then were randomized into five groups: HRR group (30 g?kg-1?d-1 by gavage for 12 days), HSB group (15 g?kg-1?d-1 by gavage for 12 days), RA group (15 g?kg-1?d-1 by gavage for 12 days) , heat-shock group (treated 20 min at 43℃, once every 4 days and three times in all) and model group. Twelve days later, the mice were executed and the ascites smears were prepared. The cell feature in ascites was observed under light microscope and the expression of CD4+ and CD8+ on the cellular membrane of the T lymphocyte in ascites was detected with immunohistochemical method. [Results] A large amount of damaged and apoptotic H22 cells were found in HRR, HSB and heat-shock groups, and the changes of cell feature in RA group were not obvious. The expressions of CD4+ and CD8+ on the membrane of T cells in ascites was increased in HRR and HRB groups, but the expression of CD4+ was increased in RA group, and CD8+ expression was increased in heat-shock group.[Conclusion] Heat-clearing and detoxifying herbs can induce the necrosis and apoptosis of mice hepatoma cells line H22 and the mechanism may be related to the increase of cellular immunity, thus enhancing the body immunity to kill the tumor cells. RA can not kill the tumor cells directly and its antitumor action may be related to the increase of CD4+ expression.
10.Effect of Herba Hedyotis Diffusae on Living Murine Ascites Hepatoma H22 Cells and T Lymphocytes

【Objective】To observe the effect of Herba Hedyotis Diffusae(HHD)on the histological features of living murine ascites hepatoma H22 cells as well as CD+4 and CD+8 expression in T lymphocytes.【Methods】Special pathogen free(SPF)Kunming mice were randomized into 4 groups:model group,HHD(25 g/kg)group,Radix Codonopsis(RC,25 g/kg)group,heat-shock group.The murine ascites hepatoma H22 model was established by abdominal transplantation of H22 cells.The histological features of H22 cells from living murine ascites were examined through living mice ascites smear and giemsa staining.The CD+4 and CD+8 expression in ascites T lymphocytes was detected by immunohistochemical method.【Results】H22 cell deformation such as cell membrane incomplete and vesicant,some nucleus being migration and pycnosis was obvious in HHD and heat-shock groups.The expression of both CD+4 and CD+8 expression in T lymphocytes of HHD groups was increased,but only CD+4 expression in RC group and only CD+8 expression in heat-shock group was increased.【Conclusion】HHD exerts some inhibitory-killing effect on living murine H22 cells,and its mechanism may be related to the increase of immune function and to the cytotoxic effect.
