2.Effect of cystic fibrosis transmembrane conductance regulator gene mutation on male fertility

Large number of cystic fibrosis transmembrane conductance regulator (CFTR) gene mutations are responsible for a wide spectrum of disease expressions in CF (cystic fibrosis). They includs not only the malfunction of the lung and pancreas, intestinal obstruction, and incapable of salt reabsorption, but also some kinds of male infertility, such as congenital bilateral absence of the vas deferens (CBAVD),poor sperm quality, the obstruction and agenesis of the epididymis. More recent studies have indicated that CFTR gene mutations have a far-reaching effect on human reproduction.
3.The clinical effect of Qingfei-Jiedu decoction for the female patients with acne of lung and stomach heat type

Objective To evaluate the efficacy of Qingfei-Jiedu decoction in the treatment of Acne. Methods A total of 93 female patients with acne were randomly divided into 3 groups according to random number table, 31 in each group. The observation group took the oral Qingfei-Jiedu decoction, and the Chinese medicine control group took the oral tanshinone capsule, and the western medicine control group took the oral acetic acid ring propyl alcohol progesterone. All 3 groups were treated for 4 weeks. The levels of serum dihydro (DHT), IL-6 and IL-8 were measured by ELISA. The changes of the lesions before and after treatment were observed and the adverse events were recorded during the treatment. The clinical effect was evaluated. Results After treatment, the serum DHT (56.94 ± 15.74 pg/ml vs. 74.48 ± 18.53 pg/ml, t=2.089), IL-6 (22.84 ± 11.77 pg/ml vs. 30.37 ± 14.50 pg/ml, t=2.135), IL-8 (22.64 ± 7.38 pg/ml vs. 29.54 ± 9.65 pg/ml, t=2.057) in the observation group were significantly lower than those in the Chinese medicine control group (P<0.05). After treatment, the disappearance rate of skin lesions (65.4% ± 15.0% vs. 44.7% ± 12.9%, 42.7% ± 13.6%, F=6.862) and comprehensive syndrome effective rate (57.2% ± 12.6% vs. 46.3% ± 12.8%, 44.8% ± 11.7%, F=5.322) in the observation group were significantly higher than those in the other two control groups (P<0.05). The observation group and the traditional Chinese medicine control group did not have adverse events, but the western medicine control group showed one case nausea. Conclusions The Qingfei-Jiedu decoction can reduce the level of serum DHT, IL-6 and IL-8 in female patients with acne vulgaris, and improve the skin lesion disappearance rates and comprehensive syndrome rates.
4.Optimized design of mini-implant diameter and length in IV osteoid through three- dimensional finite element analysis

BACKGROUND:Diameter and length of mini-implant have effects on its stability, which has been reported mostly in I and II osteoid, but less in IV osteoid. OBJECTIVE:To optimize the design of mini-implant diameter and length in IV osteoid by a three-dimensional finite element analysis. METHODS:Implant-mandible solid model was established. A 2 N orthodontic force that was perpendicular to the long axis of the implant and at a 30° angle with the distal central axis was applied onto the top of the implant. The implant was designed for different diameters (1.2-2.0 mm) and lengths (6-10 mm). Peak stress and peak displacement of the mandible were mechanicaly assessed, and stress sensitivity variables were analyzed. RESULTS AND CONCLUSION:The stress and displacement of the implant were mainly concentrated in the neck of the implant. The stress of implant-bone interface mainly focused on the contact area of the implant-cortical bone interface, and the stress of the cancelous bone was relatively smal, but the stress of the cortical bone was weakened faster. When the implant length was constant, the implant diameter had a great effect on stress changes, and the stress of bone tissue was reduced with the increase of implant diameter. When the implant diameter was constant, the implant length had no significant effect on the stress of bone tissue. To sum up, the stress of bone tissue and displacement were sensitive to the change of implant diameter rather than the change of implant length. These findings indicate that implant diameter has a greater effect on stress distribution of bone tissue than the implant length, and the implants with > 1.5 mm in diameter are suitable for IV osteoid.
5.Surgical treatment for pulmonary carcinoid tumors: a single-center analysis of 62 patients

ObjectiveTo evaluate prognostic factors and the surgical results of pulmonary carcinoid tumors.Methods We retrospectively reviewed the medical records of 62 patients who were diagnosed as pulmonary carcinoid tumors between January 2000 and October 2010 at Department of Thoracic Surgery,Shanghai Chest Hospital.The following information was available for each of the 62 patients:age,sex,pathological type,and TNM stage.ResultsThere were no operative death.The 3-year and 5-year survival rates after surgery were 92.1% and 77.8%,respectively.Of the 62 patients,42 were diagnosed as typical carcinoid tumor,and among them,4 patients (8.3%) had lymph node metastases.Their 3-year and 5-year survival rates were 97.8% and 94.7%,respectively.The remaining 20 patients were diagnosed as atypical carcinoid tumor,and among them,6 patients (37.5%) had lymph node metastases.Their 3-year and 5-year survival rates were 84.4% and 58.8%,which were statistically significant compared with typical carcinoid tumor( P =0.0047 ).There was significant difference in survival rate between the patients with lymph node metastases and the patients without lymph node metastases (P =0.0048).CondusionThe main risk factors affecting survival rate of those patients who were diagnosed as pulmonary carcinoid tumors were pathological types and lymph node metastases.
6.Investigation and study of depression status and influencing factors of rural widowed elderly with different genders

Objective To understand the depression status and its influencing factors of Chenzhou rural widowed elderly with different genders.Methods A random sample of 342 widowed elderly from 30 villages was investigated using the Geriatric Depression Scale and a self-designed questionnaire.Results Widowed elderly depression detection rate was 29.8%,depressive symptoms among elderly women detection rate was 34.7%,higher than men's depressive symptoms detection rate,which was 22.9%.Binary Logistic regression analysis showed:Alone,decreased self-care ability was the common risk factors of depressive symptoms of widowed elderly.The impact on men was bigger than on female.Age,chronic diseases were the main factors affecting depression status of elderly men,and economic security was the main factors affecting the depression status of elderly women.Conclusions The incidence of depressive symptoms in rural widowed elderly is high,but there are differences between widowed elderly with different genders in depression status and influencing factors.Therefore the community workers need to take targeted interventions to promote mental health for the elderly.
7.Detection of epileptic waves in EEG based on wavelet transform.

Detection of epileptic waves in EEG is particularly helpful in the interpretation of the underlying process in seizures. This study is aimed at providing a new method for automatic detection of epileptic waves through the wavelet analysis of EEGs. It mainly deals with the detection of spikes or spike-waves based on wavelet transform. Since spikes and spike-waves contain high frequency energy, they will be represented in a particular scale localized in a small time window. According to these feature waveforms of epileptic waves, a continuous processing system for epileptic waveforms detection is constructed. We apply discrete wavelet transform on EEGs. Because of the time-frequency domain localization of wavelet transforms, we can get the local maximal positions across several successive dyadic scales of wavelet transform. And these positions indicate the points of sharp transitions in EEGs. Then we calculate the distance between every two successive maximal positions in each scale. This distance stands for the period of subwave. Furthermore, the distribution of subwave periods of each scale can be worked out. Then, comparing the distribution of normal EEG's and epileptic EEG's. The difference between these two waveforms provides us the criteria for automatic detection and classification. In order to reduce the detection workload, we also compare the detection efficiency of each scale. The scale that provides highest accuracy is selected for our automatic detection system. The results presented in this study show that scale 3 provides the best detection accuracy. So, scale 3 is deemed to be the proper scale for automatic detection. This system has the following advantages: (1) Reduced the workload significantly by selecting proper scale(s) for automatic selection; (2) Enhanced the detection accuracy by selecting proper criteria and threshold; (3) Capable of continuous detection; (4) It is also fit for the detection of other biomedical signals. This system showed good performance, and the initial clinical results obtained are also encouraging.

8.Oral health knowledge, attitudes and practice among teachers of primary and secondary schools in Gongshu District of Hangzhou City

Objective To explore the current status and influencing factors of oral health knowledge,attitudes and practice (KAP) among the teachers of primary and secondary schools in Gongshu District of Hangzhou City.Methods A total of 652 teachers of primary and secondary schools were selected by using multi-stage stratified cluster random sampling method and completed a self-designed questionnaire,which included demographic characteristics and oral health KAP.The status and influencing factors of oral health KAP were analyzed by single and multiple-factor analysis methods.Results The questionnaire was completed by all the 652 teachers,627 of whom were qualified (96.2%).The total ratios of oral health knowledge awareness,attitudes correctness and behaviors formation were 77.1%,68.7% and 46.3%,respectively.There was significant difference of knowledge awareness rate by age,gender,marriage status,monthly income,life status and current cigarette smoking (x2 values were 11.48,8.57,8.12,8.46,23.97 and 4.64,respectively; all P<0.05).There was significant difference in oral health attitudes correctness rate by gender (x2=10.65,P<0.05) and behaviors formation rate by gender,education level and life status (x2 values were 3.95,10.88 and 10.41,respectively; all P<0.05).In logistic regression model,impact factors of oral health knowledge awareness were age,gender,life status and physical exercises; the impact factors of oral health attitudes correctness were gender and monthly income; the impact factors of oral health behaviors formation were gender,level of education and physical exercises.Conclusion The oral health awareness and attitudes correctness are acceptible among the teachers of primary and secondary schools in Gongshu District of Hangzhou City,although the formation rate of oral health behaviors is low.Oral health education should be enforced among the teachers of primary and secondary schools.
9.Current status and influencing factors of oral health behavior among the elderly living in Gongshu District of Hangzhou City

Objective To explore the current status and influencing factors of oral health knowledge and behavior among the elderly living in Gongshu District of Hangzhou City.Methods A total of 600 elderly individuals were selected by multi-stage stratified cluster random sampling and interviewed with a self-designed questionnaire which included demographic characteristics and oral health knowledge and behavior.The status and influencing factors of oral health behavior were analyzed by single and multiplefactor analysis methods.Results A questionnaire survey was conducted among 600 elderly residents,with a response rate of 99.3% (596/600) and an effective response rate of 94.1% (561/600).The rate of good oral hygiene was 50.4%.Those elderly with different age,education level,medical insurance and oral hygiene showed significant difference in qualified rate of oral health behavior (x2 values were 10.79,21.32,5.72 and 16.33,respectively; all P<0.05).In Logistic regression model,education level was positively correlated with oral health behavior among the elderly,and the qualified rate of oral health behavior of the elderly with education level of junior school or above was 2.69 times higher than that of illiterate elderly (x2=10.53,P=0.001).Conclusion The awareness rate of oral health knowledge among the elderly living in Gongshu District of Hangzhou City is at a relatively higher level,though oral health behavior is moderate.Age,education level,medical insurance and oral hygiene could be impacting factors of oral health behavior.
10.Effects of ginger-partitioned moxibustion on the expression levels of PGF2α, E2, P, and mRNAs of PGF2αR and E2R in rats with primary dysmenorrhea due to cold-dampness stagnation

Objective: To observe the effects of ginger-partitioned moxibustion at Shenque (CV8) and Guanyuan (CV4) on the expression levels of endocrine-related molecules and their receptors in rats with primary dysmenorrhea (PD) due to cold-dampness stagnation, thus to explore their analgesic mechanisms. Methods: Thirty-two female Wistar rats were divided into a normal group, a model group, a ginger-partitioned moxibustion group, and a Western medicine group according to the random number table method, with 8 rats in each group. Except for rats in the normal group, all other rats were treated with oxytocin combined with ice-water bath to establish the rat models of PD due to cold-dampness stagnation. After successful modeling, rats in the normal group and the model group did not receive treatment; rats in the ginger-partitioned moxibustion group received treatments with ginger-partitioned moxibustion at Shenque (CV8) and Guanyuan (CV4); rats in the Western medicine group received ibuprofen by intragastric administration. The writhing response of rats was compared among groups, and the serum levels of prostaglandin F2α (PGF2α), estrogen (estradiol, E2), progesterone (P), and the mRNA expression of PGF2α and E2 receptors in the uterine tissues were detected. Results: No writhing behavior was observed in the normal group; compared with the normal group, the serum PGF2α and E2 levels in the model group were increased (P<0.01), while the P level was decreased (P<0.01), and the mRNA expression levels of the uterine PGF2α and E2 receptors were increased (P<0.01, P<0.05). Compared with the model group, the writhing behavior latency was prolonged, and the writhing response score was decreased in the ginger-partitioned moxibustion group and the Western medicine group (P<0.01); the serum PGF2α and E2 levels in the ginger-partitioned moxibustion group and the Western medicine group were decreased, while the P level was increased (P<0.05 or P<0.01); the mRNA expression levels of the uterine PGF2α and E2 receptors in the ginger-partitioned moxibustion group and the Western medicine group were decreased (P<0.05). Compared with the Western medicine group, the ginger-partitioned moxibustion group showed a prolonged writhing behavior latency, reduced writhing response score (P<0.05), and decreased serum E2 level (P<0.05), while no statistical differences in the serum PGF2α and P levels, or the mRNA expression levels of uterine PGF2α and E2 receptors (P>0.05).Conclusion: The analgesic effect of ginger-partitioned moxibustion on PD due to cold-dampness stagnation may be related to regulating the mRNA expression levels of PGF2α and E2 receptors in the uterine tissues.
