1.Clinical results of video-assisted thoracoscopic surgery for pulmonary pure ground-glass opacity

ObjectiveTo retrospectively investigate the clinical results of video-assisted thoracoscopic surgery for pulmonary pure ground-glass opacity.MethodsThe clinical data of 45 patients with pGGO underwent video-assisted thoracoscopic surgery were collected.The gender constitution,age,smoking status,surgical procedures,lesion size,histological types of tumor,lymph node metastasis and prognosis were retrospectively analyzed.ResultsThe numbers of females and non-smokers were significantly larger than those of males and smokers.All 45 patients underwent video-assisted thoracoscopic surgery.37 patients of solitary pGGO were all operated with cure intention.27 of the 37 patients received local resection ( wedge resection in 18 or segment resection in 9),and the other 10 patients underwent lobectomy.There are 8 patients of multiple nodes.Amongthese,6 were operated with cure intention,while 2 received VATS biopsy.Pathological examinations after operation revealed that there were 31 cases of bronchioloalveolar carcinoma and 14 cases of atypical adenomatous hyperplasia.And there was no lymph node metastasis.The diameters of bronchioloalveolar carcinoma lesions were significantly longer than those of atypical adenomatous hyperplasia lesions( P < 0.05 ).Patients were followed up for 4 months to 6 years,and all survived without local recurrence and distant metastasis.ConclusionThe diameters of bronchioloalveolar carcinoma lesions are longer than those of atypical adenomatous hyperplasia lesions.Video-assisted thoracoscopic sublobectomy for pulmonary pure ground-glass opacity may achive excellent clinical results.Computed tomography-guided localization is a useful,safe procedure.
2.Bone mineral density in adult males: multi-factors analysis in the low-to-moderate fluoride exposure areas of Henan Province

Objective:To explore the factors affecting bone mineral density (BMD) in adult males with low-to-moderate fluoride exposure in Henan Province.Methods:Adult male villagers from low-to-moderate fluoride exposure areas in Tongxu County, Kaifeng City, Henan Province were recruited from April to May 2017 based on cluster random sampling. Questionnaire survey, physical measurements and urinary samples collection were conducted respectively. Urinary fluoride (UF) was determined by fluoride ion-selective electrode. Ultrasound bone densitometer was used to measure BMD (T-score). Partial correlation analysis and multiple linear regression were used to analyze the influence factors of BMD.Results:A total of 439 adult males were included in this study. Age, body mass index (BMI), UF content, and T-score of the participants were (47.99 ± 8.49) years, (25.77 ± 3.23) kg/m 2, (1.34 ± 0.74) mg/L, and-1.79 ± 0.79, respectively. Partial correlation analysis showed a significantly positive correlation between BMI and T-score after age adjustment ( r = 0.194, P < 0.05). Multiple linear regression showed that T-score decreased by 0.015 (95% CI:-0.024 -- 0.005, P < 0.05) for each 1-year increase in age and T-score increased by 0.034 (95% CI: 0.009-0.059, P < 0.05) for each 1.0 kg/m 2 increase in BMI. Interaction analysis showed that T-score was closely related to the interaction between overweight (≥24.0 kg/m 2), non-smoking, tea drinking and UF [ β (95% CI): 0.134 (0.001-0.269), 0.163 (- 0.015-0.337), 0.215 (- 0.006-0.436), P < 0.10]. Conclusions:Our findings reveal a negative correlation between age and BMD, and a positive correlation between BMI and BMD in adult males with low-to-moderate fluoride exposure in Henan Province. In addition, low-to-moderate fluoride exposure is more likely to damage the BMD of smokers.

A temporary demand pacemaker with electrocardiosignal display is introduced in this paper. Double way low-noise electrocardiosignal preamplifier, amplitude limiter, high and low pass filter, 50 Hz notch filter, TTL level generator and stimulating pulse formation circuit are components of the hardware electrocircuit. The demand pacing and the electrocardiosignal display are separately controlled by the software in which the double microcontrollers communications technique is used. In this study, liquid crystal display is firstly used in body surface electrocardiosignal display or intracardial electrophysiologic signal display when the temporary demand pacemaker is installed and put into use. The machine has proven clinically useful and can be of wide appliation.

4.Lobectomy in octogenarians with clinical stage Ⅰ non-small cell lung cancer-ten years of experience in a single center

Objective As the population ages,clinicians are increasingly confronted with octogenarians with early-staged non-small cell lung cancer(NSCLC).We reviewed the outcomes of octogenarians who underwent lobectomy for clinical stagc Ⅰ NSCLC,to determine whether there was a benefit to the VATS approach in this group,and to analysis the risk factors of complication and survival.Methods We conducted a retrospective single-institution review of patients age 80 years or greater who underwent lobectomy for NSCLC from January 2006 to December 2016.Clinical outcomes including complication rate and survival rate were analyzed.Results 162 octogenarians underwent lobectomy:98 VATS and 64 through open thoracotomy.Compared with thoracotomy,VATS patients had fewer complications (14.3 % vs.28.1%,P =0.03),shorter length of tube duration [(3.5 ± 1.5) days vs.(4.9 ± 2.0) days,P =0.04],and shorter length of stay [(5.5 ± 2.1) days vs.(7.8 ± 3.5) days,P =0.04].For patients with pathologic stage Ⅰ disease,the 5-year overall survival was 64.5%,for stage Ⅱ was 38.1%,and for stage Ⅲ was 20.1%.The 5-year overall survival rates of pathological stage Ⅰ and stage Ⅱ,Ⅲ are of significant differences(P =0.001).In a multivariate logistic regression analysis,the approach of thoracotomy emerged as an independent predictor of complication (OR =1.94,95% CI 1.214-5.135,P =0.03).In a multivariate COX regression analysis,pathological stage(OR =2.01,95%C1 1.453-5.865,P=0.03) and ASA(OR =1.81,95%CI 1.188-4.015,P =0.04) are independent predictors of over survival.Conclusion Octogenarians with NSCLC can undergo resection with low mortality and survival among stage Ⅰ patients,which is comparable with the general lung cancer population.The VATS approach reduces morbidity in this age demographic,resulting in shorter length of tube duration and shorter stay,while the approach of thoracotomy is an independent predictor of complication.Our study also demonstrated that pathological stage and ASA are independent predictors of overall survival rate.
5.Value of enhanced recovery after surgery in percutaneous endoscopic interlaminar discectomy based on three-dimensional visualized virtual surgical system

Objective To observe the application value of enhanced recovery after surgery(ERAS)in perioperative nursing for patients undergoing percutaneous endoscopic interlaminar discec-tomy based on three-dimensional visualized virtual surgical system.Methods Eighty-six patients with lumbar spinal stenosis were randomly divided into experimental group(receiving perioperative nursing based on ERAS)and control group(receiving conventional perioperative nursing)using a random number table method,with 43 patients in each group.The postoperative recovery,pain intensity[Vis-ual Analogue Scale(VAS)score],lumbar function[Japanese Orthopaedic Association(JOA)sco-ring system for lumbar disorders],rehabilitation motivation[Pittsburgh Rehabilitation Participation Scale(PRPS)score],compliance[Adherence Rating Scale(ARS)score],quality of life[World Health Organization Quality of Life BREF(WHOQOL-BREF)score],complications,and satisfaction were compared between the two groups.Results The first ambulation time,first meal time,and hos-pital stay after surgery in the experimental group were shorter than those in the control group(P<0.05).The total incidence of complications in the experimental group was 9.30%,which was lower than 27.91%in the control group(P<0.05).Three days after surgery,the VAS score of the experimental group was(1.77±0.58),which was lower than(2.54±0.72)in the control group(P<0.05).After intervention,the JOA scoring system for lumbar disorders,PRPS score,WHOQOL-BREF score,and satisfaction score in the experimental group were higher than those in the control group,while the ARS score was lower(P<0.05).Conclusion The application of ERAS in perioperative nursing for patients undergoing percutaneous endoscopic interlaminar discectomy based on three-di-mensional visualized virtual surgical system can not only shorten the first ambulation time,first meal time,and hospital stay after surgery for patients with lumbar spinal stenosis,improve rehabilitation motivation and compliance,significantly relieve pain,but also promote the recovery of lumbar func-tion,reduce occurrence of complications,and effectively enhance quality of life and patients'satis-faction.

7.Qualitative study on the working experience of new mobile nurses

Objective To probe into the real-life experiences of new mobile nurses, so as to provide a factual basis for the new mobile nurses management, make managers understand and focus on new mobile nurses, provide humane and scientific management, and improve the new mobile nurses' work adaptability. Methods Qualitative study methods were used to investigate 42 new mobile nurses for 42 times with semi-structured interviews and the contents of review were transcripted and summarized.Results The experiences of mobile nurses were divided into positive experiences and negative experiences. Positive experiences included: rich professional knowledge, enhanced professional skills; communication competence, and making friends; improving stress tolerance, and inspiring their own potential; the strain capacity of clinical commandoes was strengthened. Negative experiences included: bad adaptation for clinical nursing role; interpersonal relationship should be improved; confusion and concern of career planning.Conclusions Both positive and negative experiences are existed in the new nurses' working experiences, so managers should pay more attention to new recruited nurses' working experiences. Managers can establish a complete system which can provide multi-angle humane support and management on professional skills and psychological decompression, so that the new recruited nurses can adapt the motorized rotary positions, and give full play and confidence to their potential.
8.Development of multi-channels cardiac electrophysiological polygraph with LabVIEW as software platform and its clinical application.

Polygraph has become a necessary instrument in interventional cardiology and fundamental research of medicine up to the present. In this study, a LabView development system (DS) (developed by NI in U.S.) used as software platform, a DAQ data acquisition module and universal computer used as hardware platform, were creatively coupled with our self-made low noise multi-channels preamplifier to develop Multi-channels electrocardiograph. The device possessed the functions such as real time display of physiological process, digit highpass and lowpass, 50Hz filtered and gain adjustment, instant storing, random playback and printing, and process control stimulation. Besides, it was small-sized, economically practical and easy to operate. It could advance the spread of cardiac intervention treatment in hospitals.

9.Spiral CT evaluation of the alveolar cortical bone mineral density and bone cortex thickness of teenagers and adults

Objective:To compare the alveolar cortical bone density and thickness of jaw bones between teenagers (12-18 years of age) and adults(19-48 years of age).Methods:Spiral computed tomographic images of 60 cases were obtained from the subjects with the ages of 12 to 48 years(27 Males and 33 females,35 teenagers and 25 adults).Buccal cortical bone thickness and density in Hounsfield units were measured at 12 interradicular sites and to a depth of 14 mm.Results:The alveolar cortical bone thicknesses and densities of the jaw bones significantly increased from the crest to base of alveolar crest.The average cortical bone thicknesses rangedfrom 1.0 to 1.5 mm in the anterior part of the jaw bones,1.1 to 1.8 mm in maxillary posterior area and 1.5 to 2.8 mm in the mandible posterior areas.The cortical bone thickness and density were greater in the mandible than in the maxilla(P < 0.05).The thickness values did not show difference between males and females,between adolescents and adults.Higher values of bone density were found in the adults than in the teenagers(P <0.05).The highest bone density in the mandible posterior area of adults was observed,and the lowest bone density in the maxillary posterior area of adolescents.Conclusion:Alveolar cortical bone density of adolescents is lower than that of adults,especially in the maxilla posterior region.
10.Transcranial direct current stimulation can improve the effectiveness of robot-assisted rehabilitation of a hemiplegic upper limb

Objective:To observe any improvement in hemiplegic upper limb functioning when transcranial direct current stimulation (tDCS) is combined with robot-assisted upper limb treatment, and analyze the potential mechanism of neural plasticity through diffusion tensor imaging (DTI).Methods:Twenty stroke survivors with hemiplegia were randomly divided into a treatment group and a control group, each of 10, according to a random number table. Both groups were treated with conventional medication and rehabilitation training using an upper limb robot, while the treatment group also received tDCS daily, with the current increasing from 0 to 1mA over 10 seconds, and then decreasing to 0 over twenty minutes. The experiment lasted for 15 days. The upper extremity portion of the Fugl-Meyer rating scale (UE-FMA) and the Wolf Motor Function Rating Scale (WMFT) were used to evaluate motor functioning before and after the treatment. DTI was also conducted for both groups.Results:After the treatment, the average UE-FMA and WMFT scores of the two groups were significantly higher than before the treatment, with the average UE-FMA score of the treatment group (35.32±13.25), significantly higher than that of the control group (21.80±13.93). After the treatment there were significant differences between the groups in their average FA, rFA and FAasy of the posterior limb of the internal capsule, as well as in FA and the CST length of the central anterior gyrus.Conclusion:tDCS combined with robot-assisted upper limb rehabilitation training can significantly improve the motor functioning of hemiplegic upper limbs, possibly due to neuroplasticity mechanisms that promote CST integrity and symmetry changes. tDCS can be an important adjunct therapy in clinical neurorehabilitation.
