1.Measurement of optic disc parameter of Chinese normal people with Heideberg retina tomography Ⅱ

Background Heideberg retina tomography Ⅱ(HRTⅡ) can offer the quantitative description of optic disc topography and optic nerve fiber layer thickness.The normal optic disc parameter by HRT Ⅱ from Chinese has been reported,but the comparisons between males and females or the left and right eyes are lack.ObjectiveThe goal of this study is to measure the optic disc topography of healthy people with HRT-Ⅱ.Methods Four hundreds eyes from 108 healthy male subjects and 92 healthy female subjects aged 10-72 years were measured with HRT Ⅱ in this study.The parameters of optic disc including disc area,cup area,rim area,cup/disc area ratio,rim/disc area ratio,cup volume,rim volume,cup depth,maximal cup depth,height variation,mean RNFL thickness,and RNFL cross-sectional area were analyzed and compared between the gender or left and right eyes.Oral informed consent was obtained from all of the subjects before this procedure.Results There were no significant differences in measuring parameters by HRTⅡ between female and male subjects (P>0.05) or between right eye and left eye (P>0.05).Conclusion The results can be used as a normal reference of optic disc parameters of HRT Ⅱ.
2.Diagnosis experience of sarcomatoid renal cell carcinoma

Objective Explore the diagnosis experience of sarcomatoid renal cell carcinoma (SRCC).Method There were 5 cases of SRCC from April 2011 to June 2013,3 of them were females and 2 were males,with a mean age of 49 (27-71).All the patients were presented with pain accompanied with other symptoms such as hematuria,fever and polyuria.CT urography suggested renal occupied lesions,withuneven density and enhancement,and malignancy was highly suspected.The mean diameter of tumors was 9.2 cm(4-18 cm).Radical nephrectomy was performed on all patients,tumors were located in renal parenchymal,and with adherence to other tissues.One patient with lung metastasis was found before surgery.Result Pathologic findings of the resected tissues after operation noted SRCC.Under the microscope,a large number of spindle shaped cells were seen,and Vimentin and PCK were all positive in immunohistochemical.Three patients with metastases were found after surgery (adrenal gland,liver,pancreas).Postoperative pathological stages were one T2N0M1,three T4N0M1 and one T3N0M0.One patient received IL-2 immunotherapy.All patients died within 9 months after surgery with a median survival of 3.8 months.Immunohistochemistry in the postoperative,there were four patients with the level of Ki-67 LI > 50%,with distant metastasis and survival time was 1-4 moths.Conclusion The disease develops rapidly,and the prognosis of the patients is very poor.The Ki-67 LI may be considered as a prognosis marker and the patients with sarcomatoid differentiation can benefit little from surgery alone.
3.Insulin induces PKC-dependent proliferation of mesenteric vascular smooth muscle cells from hypertensive patients

Background and objectives Proliferation of human vascular smooth muscle cells (VSMCs) induced by hyperinsulinemia is a very common clinical pathology. Extensive research has focused on PKC (Protein kinase C)-MAPK (mitogen-activated protein kinase)intracellular signal transduction and the phenotypic modulation accompanied by reorganization of intracellular F-actins in VSMCs.Methods DNA synthesis, signaling of ERK1/2 MAPKs, and changes in α-smooth muscle (SM) actin and F-actin were studied in hypertensive and normotensive human arterial VSMCs exposed to insulin and PMA with and without the PKC inhibitor, GF109203X.Results Differences among cell types in MAPK signaling, α-SM actin, and F-actin isoforms in VSMCs harvested from the arteries of patients with essential hypertension (EH) and normotension (NT) were identified in response to insulin treatment. Proliferation and activation of MAPK were more pronounced in EH VSMCs than in NEH VSMCs. Insulin exposure decreased expression of α-SM actin and was accompanied by rearrangement of intracellular F-actins in VSMCs, especially in the EH group. These effects were reversed by treatment with the PKC inhibitor. Conclusions Human mesenteric VSMCs of EH and NT patients differed in proliferation, MAPK signaling, and degree of changes in α-SM actin and F-actin isoforms immediately following insulin exposure in vitro.
4.Effect of Mindfulness-based Stress Reduction on Perceived Stress and Mental State of Patients with in Vitro Fertilization

Objective:To investigate the effect of Mindfulness-based stress reduction (MBSR) on perceived stress and mental state of patients with in vitro fertilization.Methods:83 cases women who were assisted reproduction with in vitro fertilization and embryo transfer (IVF-ET) in our hospital from May 2014 to December 2016 were selected as research objects.All the subjects were divided into control group and observation group according to the random number table method,with 41 cases in the control group and 42 cases in the observation group.Patients in the control group were given routine comprehensive nursing intervention,while the observation group was given MBSR intervention on the basis of the control group.The scores of self rating depression scale (SDS),self rating anxiety scale (SAS) and Chinese perceived stress scale (CPSS) before and after the intervention in the two groups were compared.Results:There was no significant difference in SDS score,SAS score and CPSS score between the two groups before intervention (P>0.05).The scores of SDS,SAS and CPSS in observation group after intervention were (43.53± 4.89),(42.38± 4.34) and (27.53± 4.89),which were lower than (57.96± 7.30),(56.42± 5.49),(37.96± 7.30) before intervention,and the differences were statistically significant (P<0.05).The scores ofSDS,SAS and CPSS in control group after intervention were (51.13± 5.52),(51.13± 5.52) and (31.13± 5.52),which were lower than (58.83± 7.28),(56.45± 5.51),(38.83± 7.28) before intervention,and the differences were statistically significant(P<0.05).And those scores in were lower in observation group than in control group,the differences were statistically significant (P<0.05).Conclusion:MBSR intervention can effectively improve the anxiety,depression and perceived stress of patients with in vitro fertilization,which is helpful in improving the success rate of IVF-ET,and it is worthy of clinical application.

6.Determination of national norm of Chinese questionnaire of quality of life in Chinese patients with cardiovascular diseases

Objective: To determine the national norm of Chinese questionnaire of quality of life in Chinese patients with cardiovascular diseases (CQQC) according patients with hypertension, coronary artery disease(CAD), and congestive heart failure(CHF)and to analyze the influence factors. Methods: The quality of life was estimated by CQQC to evaluate 7937 persons normal and with cardiovascular diseases(contain with hypertension, CAD, and CHF ) in 28 hospitals of 19 cities across China. The data were compared in gender, age, exercise, psychologic condition, education, number of family member and medical cost. Results: 1、The mean scores(national norms)of normal persons were (95.74±23.21); 2、The scores of hypertension patients were (73.75±23.62), the scores of hypertension stage 3 group (69.06±21.78)were least compared with other stages(P<0.01); 3、The scores of CAD patients were(64.67±20.59), the scores of unstable angina group(55.37±18.62)were least compared with other types in CAD patients(P<0.01); 4、The scores of CHF patients were (40.36±17.24), it was least compared with those of hypertension and CAD patients (P<0.01); 5、Influence factor: (1)Sex, age: The mean scores were (71.41±27.8)for 7937 persons, the scores of male persons were significantly higher than those of female (P=0.000). In normal persons, along with age increased, the scores decreased (P<0.05) ; except ≥70 ages group, the scores of male persons were higher than those of female(P<0.05) ; (2)Exercise: The scores of patients participating exercise were significantly higher than those of no participating exercise [(72.64±21.87)vs.(66.91±23.13), P=0.000]; (3)Psychological condition: The scores of patients with healthy psychological condition were significantly higher than those of with psychologic disorder [(70.13±25.66)vs.(68.91±20.44), P=0.001]; (4)Other: patients with better education, more than 1 family members and medical insurance or public expense had more scores (P<0.05~0.001). Conclusions: The sample size is large and the datum is reliable for Chinese questionnaire of quality of life in Chinese patients with cardiovascular diseases. It can reflect influence of age, sex, exercise, psychologic condition, family, education background, medical condition and disease, its item is brief, clear, and operation is easy, so it may well be spread.
7.Study of polymerizing hemoglobin on cation exchange chromatography.

Poly-hemoglobin is the active component of hemoglobin-based blood substitutes. The excess reactivity of glutaraldehyde with hemoglobin in solution leads to poly-hemoglobin of a wide molecular weight distribution and a high average molecular weight. A new polymerization method has been tested to decrease the molecular weight distribution and the average molecular weight. The poly-hemoglobin with lower degree of modification (polymerization) was found enriched on the cation exchange columns and further polymerized with glutaraldehyde. The poly-hemoglobin of narrower molecular weight distribution has been prepared in this way.

8.Study on real-time wearable monitoring system for human heat and cold stresses.

In order to study the way of evaluating human performance under heat and cold stresses, we developed a wearable physiological monitoring system-intelligent belt system, capable of providing real-time, continuous and dynamic monitoring of multiple physiological parameters. The system has following features: multiuser communication, high integration, strong environment adaptability, dynamic features and real time physiological monitoring ability. The system uses sensing belts and elastic belts to acquire physiological parameters, uses WIFI to build wireless network monitoring for multiuser, and uses Delphi to develop data processing software capable of real-time viewing, storagng, processing, and alerting. With four different intensity-activity trials on six subjects and compared with standard laboratory human physiological acquisition instruments, the system was proved to be able to acquire accu-rate physiological parameters such as ECG, respiration, multi-point body temperatures, and body movement. The system worked steadily and reliably. This wearable real-time monitoring system for human heat and cold stresses can solve the problem facing our country that human heat stress and cold stress monitoring technology is insufficient, provide new methods and new ways for monitoring and evaluation of human heat and cold stresses under real task or stress environment, and provide technical platform for the study on human ergonomics.

Objective To analyze the status of Leishmania infantum asymptomatic infection in human population of a Kala-azar endemic area in Wenxian County,Gansu Province,and to evaluate the tests used.Methods Blood samples were tested by PCR using two pairs of primers,RV1-RV2 and K13A-K13B,for detecting Leishmania-specific DNA.ELISA and rK39-dipstick were used to detect Leishmania-specific antibodies.Results The positive rate of PCR,ELISA and rK39-dipstick was 30.9%(83/269),24.2%(65/269) and 0(0/269) respectively.Conclusion The prevalence of asymptomatic infection of L.infantum in humans is high in the area.PCR test based on RV1-RV2 and K13A-K13B primer pairs is a sensitive and specific method for detecting the asymptomatic infection.
10.Recent progress in the pathogenesis of idiopathic epiretinal membrane

Idiopathic epiretinal membrane(IERM)is an age-related retinal disease of the macula. In early stages, symptoms of IERM are usually characterized by visual impairment. As the disease progresses, it can lead to metamorphopsia and lower visual acuity in severe cases, the pathogenesis of IERM remains a mystery. Researchers have found that posterior vitreous detachment(PVD), anomalous PVD, activation of the renin-angiotensin system(RAS), and glial to mesenchymal transition(GMT)of Müller cells play important roles in the pathogenesis of IERM. In pathological research of IERM, astrocytes, microglia, Müller cells, hyalocytes, fibroblasts, myofibroblasts, and retinal pigment epithelium(RPE)cells were found to be the major cellular components of IERM, and the extracellular matrix of IERM was mainly composed of extracellular collagen fibers. In this article, we summarize the mechanisms of IERM formation in the light of some of the recent advances in research on the pathogeny, cells, cytokines, and extracellular matrix of IERM, in order to provide directions for its prevention and treatment.
