Objective To evaluate the clinical application value of Xpert detection system of Clostridium difficile (C.difficile).Methods A total of 43 stool specimens from the patients with diarrhea were collected, and C.difficile in stool specimens were detected by the Xpert detection system, the toxigenic culture method, and the toxin detection method which detected the toxin of C.difficile by VⅥDAS automatic analyzer after anaerobic culture, respectively. The analytic performance of Xpert detection system was evaluted based on the toxigenic culture method as the gold standard.Meanwhile, the consistency of the results from different detection methods was compared. The ribotype 027 strain (ATCC BAA-1870) simulating the stool specimen was further used to verify the Xpert detection system. Results Based on the gold standard of the toxigenic culture method, the sensitivity, specificity, positive predictive value and negative predictive value of the Xpert detection system were 90.9%,93.8%,83.3% and 96.8%,respectively.The Kappa values for the consistency between the Xpert detection system and the toxigenic culture method or the toxin detection method were 0.822 (P < 0.05) and 0.419 (P < 0.05),respectively.Moreover,the ribotype 027 strain simulating the stool specimen was verified by the Xpert detection system successfully. Conclusion The Xpert detection system may rapidly and accurately detect the C.difficile in stool specimens, especially the ribotype 027 strain with high toxicity.

Objective To observe the clinical curative effect of tripterygium polyglycoside combined with cozaar in elderly patients with IgA nephropathy (IgAN) and its influence on expressions of TGF-β1,PAI-1 and VEGF.Methods Eighty elderly patients with IgA nephropathy in Jiaxing Hospital of Armed Police Zhejiang Corps from April 2011 to January 2016 were selected and randomly divided into the control group and observation group,40 cases in each group. The two groups received the routine treatment of chronic basic diseases and took oral cozaar. On this basis the observation group took tripterygium polyglycoside for continuous 12 months.Urine protein, serum creatinine (Scr),blood urea nitrogen (BUN),plasma Alb,urine TGF-β1,PAI-1 and VEGF levels before treatment and at 3,6,12 months after treatment were compared between the two groups. The occurrence situation of adverse reactions was recorded. Results The 24 h urinary protein levels at 3,6,12 months after treatment in the two groups were decreased, moreover the 24 h urine protein level at each time point in the observation group was lower than that in the control group, the difference was statistically significant (P<0.05).The levels of Scr,BUN,urine TGF-β1,PAI-1 and VEGF after treatment in the two groups were decreased compared with before treatment, while the Alb level was increased compared with before treatment, moreover the levels of Scr,BUN,urine TGF-β1,PAI-1 and VEGF in the observation group were lower than those in the control group, while the Alb level was higher than that in the control group, the difference was statistically significant (P<0.05).The observation group appeared 2 cases of leukopenia,1 case of liver dysfunction and no serious adverse reactions occurred. Conclusion Tripterygium polyglycoside combined with cozaar has a significant therapeutic effect on elderly patients with IgAN,can significantly reduce urinary protein and protect renal function, moreover its clinical use is safer. Its efficacy may be related to reducing the expression levels of TGF-β1,PAI-1 and VEGF.

In recent years, the research on gold nanoparticles has made great progress. Gold nanoparticles with different morphologies have good application prospects in drug delivery and tumor treatment. Some gold nanoparticles have entered the stage of clinical trials. Gold nanorods have become important research objects due to their special optical properties and photothermal treatment potential. In this paper, the optical properties and main applications of gold nanorods were reviewed. Gold nanorods have good surface modifiable properties and can be modified through surface ligand exchange to improve their biocompatibility. The photothermal properties of gold nanorods can be improved by adjusting the aspect ratio to adjust the surface plasmon resonance (SPR) peak to achieve near-infrared light excitation. These characteristics make gold nanorods show good application prospects in the detection of biological macromolecules, real-time imaging in vivo, and early diagnosis and treatment of tumors. Using gold nanorods as a carrier and modified with different targeting molecules can improve the targeting of its drug delivery system and reduce damage to normal cells, so as to realize the combined application of chemotherapy and photothermal therapy, and finally achieve a better therapeutic effect. Combining gold nanorods with stem cells or certain specific biomolecules can form a hybrid gold nanorod system which provides new ideas for further improving the efficiency of tumor treatment.
