1.The application of seminar teaching on postgraduate education of ophthalmology

To investigate the application of seminar teaching on postgraduate education of ophthalmology,postgraduate from Grade 2008 to Grade 2010 were divided into experiment groups and control groups.Seminar teaching and conventional teaching were applied to two groups respectively.The performances of seminar teaching groups at each grade were better than the control groups ( P < 0.05 ).Thus,seminar teaching can enhance theory and operation ability of postgraduate.
2.Analysis of the factors affecting activities of daily life of patients with cerebral infarction

Objective To study the factors affecting the activities of daily life (AOL) of patients after the first stroke of cerebral infarction in order to formulate the intervention strategy for improving the capability of ADL of patients. Method A total of 149 patients with the first stroke of cerebral infarction admitted from Oct. 2008 to Dec.2008 were enrolled in this study. The demographics of patients, cerebral infarction risk factors, apopletic score as per National Institute of Health stroke scale (NIHSS), white blood cell count (WBC) and plasma glucose (PG) were recorded on the first day of admission, and many other laboratory examinations were done on the next morning. The occurrences of infection in lung and urinary tract, and atrial fibrillation were recorded during hospi-talization. The NIHSS score and score of Glasgow Coma Scale (GCS) were taken within 24 hours after admission, on the 14th day and at the end of the third month after cerebral infarction respectively. Barthel index (BI) was taken in the second week as well as at the end of third month by follow-up in the OPD or by telephone. The correlation analysis and multiple linear stepwise regression analysis were used to find the risk factors. Results The PG level, WBC count and NIHSS score were independently associated with ADL in the second week as well as at the end of the third month after cerebral infarction. Besides, the urinary tract infection during hospital stay was also independently associated with ADL at the end of the third month after cerebral infarction. Conclusions The plasma glucose level, WBC count, NIHSS score and urinary tract infection are the risk factors. Positive measures should be taken to control these risk factors so as to improve the capability of ADL of the patients after cerebral infarction.
3.CT and MRI features of testicular solid lesions

Objective To analyze the imaging features of testicular solid lesions.Methods The CT and MR findings of 18 cases of testicular solid lesions confirmed by pathology were retrospectively reviewed.The location,attenuation/signal,size,shape,edge,and enhancement patterns of lesions were analyzed.Results In all of the 18 cases,5 cases were seminomas,1 of them was huge with punctate calcification,4 of them had low signal separations in T2WI.Lymphomas were in 4 cases,and all of them were primary diffuse large B cell lymphoma;T2WI was priority with low signal.Solitary fibrous tumor was in 1 case,and the attenuation was lower than that of muscle.Embryo in sinus tumor were in 1 case,and the attenuation/signal was unhomogeneous;the continued enhancement liking grid was obviously seen after contrast injection.Nonspecific inflammatory masses were in 4 cases with involving epididymis in 2 cases,and the attenuation/signal was unhomogeneous.Tuberculous mass were in 3 cases,and 1 case with points of calcificationwas on the edge of lesion,2 cases characterized by short T2 signals.Conclusion It is possible to make an accurate diagnosis by the combination of clinical data with imaging features in most of the testicular solid lesions.
4.Application of low doses of digital subtraction angiography three-dimensional reconstruction in joint fracture of the trauma

ObjectiveTo compare X-ray,spiral CT and low does digital subtr,action angiongraphy technology of three dimensional reconstruction in 30 cases of the applications in the joint fractures,to study the application value of the digital subtraction angiography three- dimensional reconstruction in joint fracture.MethodsA retrospective analysis was made on 30 cases with joint fracture in Guangdong Provincial Guangzhou Development District Hospital,who were confirmed by the X-ray and spiral CT three-dimensional reconstruction and digital subtraction angiography three-dimensional reconstruction and the demonstrations three-dimensional reconstruction,spiral CT three-dimensional reconstruction and X-ray were compared.ResultsAll 30 cases fractures showed clear line,line of walk line,of the number of fracture,source and separation shift condition.Digital subtraction angiography could be clearly shows 30 cases in spiral CT could clearly show in 20 cases,10 cases not clearly show in,while X-ray clearly show in 10 cases,not show in 20 cases.Spiral CT showed clearly joint capsule and soft tissue.The SPSS 11.0 statistical soft package was used.Spearman correlation analysis,the single factor analysis of variance and the independent t test and the chi-square test were employed for data analysis,P < 0.05 was considered statistical significance.Conclusions Digital subtraction angiography three-dimensional reconstruction can more clearly display fracture line,fracture pieces,walk line,source and separation shift condition compared with spiral CT and Xray essaminations,which is helpful for treatment options and prognosis estimation.
5.Synthesis and preliminary anti-diabetic activity evaluation of novel PEGylated GLP-1 receptor agonists

In order to obtain glucagon-like peptide-1(GLP-1)analogs which can sustainedly control the levels of glucose, 12 derivatives were designed and synthesized by coupling monomethoxy polyethylene glycol(mPEG, with average molecular weights of 350, 550 and 750)to GLP-1 analogs. Preliminary pharmacological activities showed that all compounds retained GLP-1 receptor agonist activities, and the hypoglycemic activity of compound I-12 was similar to those of Ex-4 and Liraglutide, suggesting I-12 could be a potential long-acting GLP-1 receptor agonist.
6.Analysis of clinical diagnosis and treatment in children with cerebral vascular malformation

Objective To investigate the diagnosis and treatment characteristics in children with cerebral vascular malformation. Methods From September 2007 to December 2016,25 consecutive children with cerebral vascular malformation admitted to the Department of Neurosurgery,the third people′s Hospital of Hainan Province were enrolled retrospectively. The diagnosis was confirmed by CT angiography (CTA) or DSA,including 10 males and 15 females,aged 3-15 years,with a median age of 8 (5,10) years. There were 2 children without hemorrhage and 23 had cerebral hemorrhage. The sites of hemorrhage included frontal lobe in 8 cases,temporal lobe in 5 cases,occipital lobe in 4 cases,and frontal,parietal and temporal lobes in 3 cases,thalamus in basal ganglia in 2 cases,and ventricle in 1 case. The patients were treated with hematoma removal and vascular malformation resection,endovascular embolization,and stereotactic radiotherapy. In the acute phase,15 patients were treated with emergency hematoma evacuation and vascular malformation resection due to intracranial hypertension,and 1 patient was treated with embolization, including 1 craniotomy and 1 external ventricular drainage after embolization. After the subacute phase or when the condition was stable,3 patients underwent surgical resection of vascular malformations and clearance of hematoma,3 received embolization,1 underwent simple hematoma clearance (cerebral venous malformations), and 2 underwent stereotactic radiotherapy. They were followed up at 6 months and 1 year after surgery. The clinical efficacy was evaluated by Glasgow outcome scale ( GOS) score,and CTA and DSA were used to conduct imaging evaluation. Results (1) Twenty-four patients were diagnosed as intracranial arteriovenous malformation,including 9 patients with Sperzler-Martin gradeⅠ,11 with gradeⅡ,and 4 with gradeⅢ. There were 18 patients mainly with anterior circulation blood supply and 6 mainly with posterior circulation blood supply;1 with venous malformation. ( 2 ) No rebleeding occurred during the follow-up, 8 children had seizures,and 1 died at 8 months after operation. The first clinical follow-up was performed in 25 patients at 6 months after treatment. The GOS score was 5 in 5 cases,4 in 12 cases,3 in 4 cases,2 in 3 cases,and 1 in 1 case;they were followed up for more than 1 year. CTA or DSA reexamination showed that 3 children had residual cerebrovascular malformations at 6 months after operation,and the remaining 22 children showed no intracranial vascular malformation. The follow-up time of all the surviving children was >1 year,and DSA was completed at 12 to 38 months after treatment. There was no progress in 3 children with residual vascular malformation during the first follow-up at 6 months after operation and no cerebral hemorrhage occurred. No recurrence was found in 21 children with negative DSA. Conclusions Cerebral vascular malformations in children are mainly arteriovenous malformations,and most them go to see a doctor after spontaneous bleeding. Early diagnosis and reasonable treatment can leave nerve dysfunction as little as possible.
7.Expression of SPNS2 in colorectal cancer tissues and its clinical significance

Objective: To investigate the expression and clinical significance of sphingosine-1-phosphate transporter 2 (spinster homolog 2, SPNS2) in colorectal cancer. Methods: Colorectal cancer and adjacent normal tissues were collected from 133 patients undergoing colorectal cancer resection in Anhui Provincial Hospital from January 2010 to June 2013. The expression of SPNS2 protein was detected by immunohistochemistry. RT-qPCR was performed on the cancer tissues and adjacent normal tissues of 29 patients with colorectal cancer from February 2018 to June 2018. The relationship between the expression of SPNS2 in cancer tissues and clinicopathological features of colorectal cancer patients was analyzed using χ2test. The relationship between the expression of SPNS2 and prognosis of patients with colorectal cancer was analyzed based on univariate and multivariate analysis. Results: Immunohistochemistry results indicated that the expression of SPNS2 in colorectal cancer tissues was higher than that in adjacent normal tissues (81.20% vs. 22.60%, χ2=69.136, P<0.001). The results of RT-qPCR indicated that the expression of SPNS2 mRNA in colorectal cancer tissues was significantly higher than that in the corresponding adjacent normal tissues (t=3.974, P<0.001). The overexpression of SPNS2 protein was closely related to tumor differentiation, lymph node metastasis (pN), tumor size, and depth of invasion (Tx); this difference was statistically significant (P<0.001). The survival time of patients with negative SPNS2 expression was significantly longer than that of patients with positive expression (χ2=13.080, P<0.001). Univariate and Cox multivariate analyses showed that abnormal expression of SPNS2 affects the overall survival of colorectal cancer patients and is an independent influencing factor for patient prognosis. Conclusions: The positive expression of SPNS2 in colorectal cancer is related to the occurrence and development of colorectal cancer. SPNS2 can be used as a new colorectal tumor marker to evaluate the prognosis of patients, monitor the invasion and metastasis of postoperative tumors, and serve as a new target for diagnosis and treatment.
8.Analysis of clinical prognosis and influencing factors of optimal medical therapy for patients with coronary heart disease after percutaneous coronary intervention

Objective:To study the clinical prognosis and related factors affecting optimal medical therapy (OMT) compliance of patients with coronary artery disease (CAD) undergoing percutaneous coronary intervention (PCI).Methods:A prospective study was conducted to select 3 818 patients who were diagnosed with CAD and successfully underwent PCI in TEDA International Cardiovascular Hospital from October 2016 to September 2017. The clinical information and application of OMT during hospitalization and 1 year later were collected for research.The patients were divided into OMT group and non OMT group according to whether they adhered to OMT during follow-up one year after discharge. After comparing the imbalance baseline data of hypertension,diabetes and hyperlipidemia with propensity score,demographic characteristics, coronary revascularization history, CAD, laboratory related laboratory examinations,and the use of OMT drugs were compared between the two groups. Cox regression model was used to analyze the relationship between long-term OMT and clinical prognosis in patients with CAD.Multivariate binary logistic regression was used to analyze the related factors affecting long-term OMT compliance.Results:A total of 3 818 cases of CAD patients were matched by propensity score and 2 596 patients were included in the study. There were 1 609 males and 987 females. The age was (62.51±9.56) years old.One year later,1298 patients (50%) insisted on OMT,including dual antiplatelet therapy(DAPT), statins, β-blockers and ACEI/ARB were 97.0% (2 517/2 596),94.5%(2 454/2 596),69.6% (1 806/2 596) and 64.2% (1 666/2 596), especially angiotensin converting enzyme inhibitors / angiotensin receptor blockers and β Receptor blockers decreased the most.Cox regression analysis showed that after adjusting for other factors, compared with non-adherence to OMT group,OMT after PCI was associated with better prognosis ( HR=0.416,95% CI 0.270-0.641, P<0.001). The prognosis of CAD patients with history of old myocardial infarction ( HR=1.804,95% CI 1.070-3.041, P=0.027),cardiac insufficiency ( HR=2.074,95% CI 1.161-3.702, P=0.014),multivessel coronary disease ( HR=2.211,95% CI 1.228-3.983, P=0.008) and BMI>24 ( HR=1.570,95% CI 1.037-2.377, P=0.033) were related to worse clinical outcomes. Multi-factor binary Logistic regression showed that OMT at hospitalization was a strong influencing factor of long-term adherence to OMT ( OR=41.278,95% CI 29.961-56.871, P<0.001). Patients with higher education,employee medical insurance and with history of PCI tend to persist in OMT. Conclusion:The medication compliance of patients with long-term OMT after PCI is still poor,while the high compliance of OMT is related to the lower incidence of adverse cardiovascular events,including death, nonfatal myocardial infarction and stroke. If there is no obvious contraindication,all patients after PCI should adhere to OMT.

Pyroptosis,an inflammatory caspase-dependent programmed cell death,plays a vital role in maintaining tissue homeostasis and activating inflammatory responses.Orthodontic tooth movement(OTM)is an aseptic force-induced inflammatory bone remodeling process mediated by the activation of periodontal ligament(PDL)progenitor cells.However,whether and how force induces PDL progenitor cell pyroptosis,thereby influencing OTM and alveolar bone remodeling remains unknown.In this study,we found that mechanical force induced the expression of pyroptosis-related markers in rat OTM and alveolar bone remodeling process.Blocking or enhancing pyroptosis level could suppress or promote OTM and alveolar bone remodeling respectively.Using Caspase-1-/-mice,we further demonstrated that the functional role of the force-induced pyroptosis in PDL progenitor cells depended on Caspase-1.Moreover,mechanical force could also induce pyroptosis in human ex-vivo force-treated PDL progenitor cells and in compressive force-loaded PDL progenitor cells in vitro,which influenced osteoclastogenesis.Mechanistically,transient receptor potential subfamily V member 4 signaling was involved in force-induced Caspase-1-dependent pyroptosis in PDL progenitor cells.Overall,this study suggested a novel mechanism contributing to the modulation of osteoclastogenesis and alveolar bone remodeling under mechanical stimuli,indicating a promising approach to accelerate OTM by targeting Caspase-1.
