1.Medical cost analysis of the diabetic in-patients

A total of 2794 diabetic patients admitted in the PLA Hospital N. 306 from 2000 to 2004 were analyzed for their in-patients cost, based on database according to the ICD-9 code and the standard forms for the investigation of medical expenditure. The average total medical cost of these in-patients Was 6557,6887,8235,9633 and ll785 RMB Yuan (1 US Dollar=8.1 RMB Yuan) from 2000 to 2004, respectively. Comparing with that in 2000, the average medical cost for these in. patients increased 5%,26%,47% and 80% from 2001 to 2004, respectively; of which 17%,19%,47% and 96% for drugs; 5%,29%,56% and 92% for the examinations. The total medical cost, drug and examination cost increased 1.01,1.14 and 1.10 times for the diabetic patients with cerebral infarction, 1.16,1.37 and 1.12 times with hypertension, and 1.124,1.11 and 1.18 times with cholecystitis and/or gallstones, as compared with their counterparts without diabetes mellitus.
2.The clinical effect of emergency endoscopic therapy on the patients with severe acute pancreatitis

Objective To study the clinical effect of endoscopic therapy in the patients with severe acute pancreatitis(SAP).Methods Totally 289 patients with SAP were divided into endoscopic therapy and control groups in random.The patients in control group received conventional medications.The patients in endoscopic group not only received conventional medications but also took endoscopic sphincterotomy (EST),removing stones,endoscopic nasobile or nasopancreatic drainage within 72 h after onset.The remission time of abdominal pain and distention,the duration of serum amylase back to normal,APACHEⅡscores,hospitalization days and expenditure were compared between two groups.Results The remission time [(10.5?3.0) day] of abdominal pain and distention in endoscopic group was significantly lower than that [(12.4?6.8) day] of control group,P
3.Narrow-Band Imaging in diagnosis of colorectal neoplasm

Objective To evaluate the efficacy of narrow-band imaging (NBI) in diagnosis of color-ectal neoplasm. Methods A total of 96 colorectal neoplasm from 78 patients were observed under NBI, and the findings were compared with those from magnifying chromo-endoscopy and pathologic examinations. Re-suits The sensitivity of conventional colonoscopy in diagnosis of polyps was 78. 7%, which was increased to 99% (P < 0. 05) under NBI, with the clear identification of the shape and boundary of the lesions. Magnif-ying NBI colonoscopy provided images of pit patterns similar to those from chromo-endoscopy in diagnosis of type Ⅱ , ⅢL, Ⅳ and ⅤN, which showed better recognition than conventional colonoscopy, but less sensitiv-ity than chromo-endoscopy. Superficial vascular morphologic features could be better classified with NBI, with sensitivity of 100% and specificity at 87. 8%, which was superior to conventional colonoscopy but infe-rior to chromo-endoscopy. Conclusion Both chromo-endoscopy and NBI colonoscopy have better sensitivity and specificity in detection of colon polyps, while chromo-endoscopy reveals clear superficial structure of le-sion and pit pattern, and NBI demenstrates capillary morphology, which can distinguish neoplasm from non-neoplasm colorectal lesions. NBI, easy and convenient to switch, is an effective technique to make early di-agnosis of colorectal neoplasm.
4.Clinical observation on acupuncture plus chiropractic treatment for cervical spondylosis

Objective:To observe the therapeutic effect of Governor Vessel-unblocking and yang-regulating acupuncture plus chiropractic treatment on upper cervical spondylosis. Methods:A total of 64 eligible cases were randomly allocated into an observation group (n=32) and a control group (n=32) according to the random digital table. Cases in the observation group were treated with Governor Vessel-unblocking and Yang-regulating needling method, fine adjustment of the upper cervical spine and lumbosacral Ban-pulling manipulation; cases in the control group were treated with routine acupuncture and same spinal adjustment as the observation group. The treatment was done once a day in both groups. The therapeutic efficacies were observed after 10 times of treatment. Results:The total effective rate was 78.1% in the observation group and 67.5% in the control group. The difference in the total effective rate between the two groups was statistically significant (P<0.05). Conclusion:Governor Vessel-unblocking and yang-regulating acupuncture and regulate yang plus chiropractic therapy is better than routine acupuncture plus chiropractic therapy in the therapeutic effect in the treatment of upper cervical spondylosis.

7.The inhibition of antisense RNA retroviral vector of mTOR on proliferation of vascular smooth muscle cells

Objective To study the effect of antisense RNA retroviral vector of mTOR on the proliferation of vascular smooth muscle cells(VSMC).Methods The conservative region of mTOR gene was inserted into pLXIN reversedly,then the vector was packaged in PT67 cells by transfection with lipofectamine and(transfected) to VSMC.The efficiency of antisense inhibition was verified,and the changes of p70S6k and(4E-BP1) were also determined at the same time.The proliferation activity was determined by flowcytometry and MTT.Results After the vector was successfully transfected to the cells.pLXIN-A-mTOR vector could(efficiently) decrease mRNA and protein expression of mTOR and p70S6k of the cell,while the expression of 4E-BP1 increased significantly.The cell cycle and proliferation activity of VSMC was also stunned in phase G_1.Conclusions The mTOR antisense retroviral vector,pLXIN-A-mTOR,has been constructed(successfully),which can significantly inhibit the proliferation of VSMC.

Objective To study the expression of mammalian target of rapamycin (mTOR) in autogenous vein graft. Methods Autogenous vein graft model was established in 64 Wistar rats. Graft vein was harvested 6 hours, 1 day, 3 days, 1 week, 2 weeks, 4 weeks, 6 weeks and 8 weeks after transplantation. mTOR mRNA was measured by RT-PCR and in situ hybridization. Western blotting and immunohistochemistry methods were used to detect the expression of mTOR. PCNA expression was detected. The efficiency of cold storage red blood cells (CSRBC) or whole blood at -80 degrees C used in 27 Rh (D) negative patients during surgical operation was reported. The Rh (D) negative patients received the transfusion of CSRBC or whole blood stored at -80 degrees C for 180 to 360 days. The changes in the indexes, such as blood TB, DB, K+, Na+, BUN, Cr, urine protein (URPO), UOB, Hb, HCT, serum total protein, relative to hemolytic reaction and blood volume before and after transfusion were observed. The results showed that after transfusion of CSRBC or whole blood 27 cases were negative for urine protein and UOB, and the levels of BUN and Cr were normal (P > 0.05). Blood TB, DB, Hb, and HCT were increased, while pH, blood K+ and blood Na+ was normal with the difference being not significant before and after operation (P > 0.05). Plasma protein was decreased, but there was no significant difference before and after operation (P > 0.05). It was suggested that CSRBC or whole blood at -80 degrees C could be safely infused to the Rh(D) negative patients without side effects during the surgical operation.
