Objective To develop a portable endoscopic thermal device which possesses the merits of compact structure,convenient operation,security and good compatibility.Methods Rapid heating elements and several materials are used,which are excellent insulation,heat conduction and can withstand high temperatures.The production process of miniaturization,anti-conglutination and constant temperatureare adopted.Results: The size of Heat probe is about 2.4mm in outer diameter and 8 mm in length.The maximum temperature will reach 250℃,heating time within 10s,output 4.5～6.5W.Conclusion Heat Probe can be compatible with various types gastrointestinal endoscopic channel.Besides hemostasis of upper gastrointestinal,it can also be applied to small polyps,erosive gastritis,etc.

Objective:To investigate the protective effect of adenosine on cochlea in acute acoustic trauma. Methods: Guinea pigs were exposed to 118 dB SPL white noise for 30 min. During the process of exposure, artificial perilymph or artificial perilymph containing adenosine (0.1 mmol/L,1 mmol/L or 5 mmol/L) was infused into the cochlea through perilymphatic perfusion. Concentration of glutamate in perilymph and thresholds of compound action potentials (CAP) were measured before and after noise exposure. Results: After exposure, the concentration of glutamate and the mean threshold shifts evoked by click were significantly higher than those of before exposure. Concentration of glutamate and thresholds of CAP in adenosine (1 mmol/L and 5 mmol/L) treated groups were significantly lower than those of the control group. Conclusion: Adenosine can inhibit the elevation of glutamate and decrease the threshold shift of CAP after exposure to noise,which can protect cochlea in acute acoustic trauma in guinea pigs.

ObjectiveTo investigate the alteration of Fc gamma receptors expressed on monocytes in childhood acute immune thrombocytopenia (aITP).Methods27 patients with aITP and 25 health controls were enrolled in this study.The expression of FcγR Ⅰ and FcγRⅢ on monocytes surface was determined by flow cytometry.Real-time PCR was performed to detect the levels of FcγR Ⅱ a and FcγR.Ⅱ b mRNA.The plasma concentration of IL-4 and IFN-γwas analyzed by ELISA.ResultsThe expression of FcγR I and FcγRⅢ on monocytes was significantly highet in ITP pationts than thatinhealth control group.Transcription levels of FcγR Ⅱ a mRNA and the ratio of FcγR Ⅱ a/Ⅱ b mRNA on monocytes were significantly elevated,while the inhibitory FcγR Ⅱ b mRNA was significant lower in aITP patients (P＜0.05) ; in comparison with healthy controls. After dexamethasone treatment,the stimulatory FcγR Ⅰ,FcγRⅢ and FcγR Ⅱ a mRNA were significantly decreased while inhibitory FcγR Ⅱ b mRNA was increased.(2)The higher levels of plasma IFN-y and lower IL-4 were to be found in aITP patients compared with healthy controls.Correlation analysis showed that the level of plasma IFN-γwas positively correlated with the elevated expression of FcγR Ⅱ a mRNA on monocytes,while the level of IL-4 was positively correlated with FcγRⅡ b mRNA.Conclusion The over expression of inhibitory FcγRs and the down-regulation of inhibitory FcγR Ⅱb mRNA on monocytes,and the imbalance of stimulatory and inhibitory FcγR might be involve in immunological pathogenesis of the acute ITP.The alteration levels of plasma pro-inflammatory cytokine IFN-γand anti-inflammatory cytokine IL-4 might be involved in imbalance expression of FcγR in acute ITP.Dexamethasone therapy could shift the balance between stimulatory and inhibitory FcγR toward inhibitory FcγR Ⅱ b in aITP patients.

Objective To explore the feasibility and superiority of the simplified partial pressurized tourniquet used in bleeding or injured limbs in battlefield.Methods The tourniquet was composed of the disposable infusion bag,bulb for blood pressure apparatus,pressure gauge and heparin cap.The disposable infusion bag wrapped around the bottom of the cravat took the places of the balloon and the harness.The pressure on the upper and lower limbs were 180mmHg and 200mmHg respectively.Results The low-cost tourniquet,with easily adjustable pressure,was easy to operate and carry.It was efficient in hemostasis and had a low rate for complications.Conclusion The tourniquet can be applied to self and buddy aid,and thus the incidence of shock can be decreased.

Objective: To study the influence of negative electrets on apoptosis of fibroblast cells and to probe its mechanism. Methods: Fibroblast cell were treated with -300, -500 and -1 000V PTFE electrets for 24, 48 and 72 h, respectively, and the influence of negative electrets on cell apoptosis was studied by means of flow cytometry and transmission electron microscope. Results: Compared with control group, apoptosis cells increased from 0.5% to 10% (some even to 15%) after 24，48 and 72 h action of -300, -500 and -1 000 V electrets. After action of -500 V PTFE electrets for 48-72 h, fibroblast cells showed characteristic morphological features of apoptosis. These features included chromatin aggregation, nuclear and cytoplasmic condensation and partition of cytoplasm and nucleus into membrane bound-vesicles (apoptotic bodies). The effect of negative electrets on apoptosis was in proportion to the time and electric field intensity. Conclusion: Negative electrets can enhance apoptosis of fibroblast cells.

Objective: To study the influence of electret on surface charge of fibroblast cells (3T3 cells) and to probe the relationship between cell growth, apoptosis and cell surface charge. Methods: Electrets Teflon PTFE, ±300 V,±1 000 V were used to treat 3T3 cells for 24, 48 and 72 h. Then the influences of electrets on cell cycle and surface charge of 3T3 cells were studied by flow cytometry and electrophoresis, respectively. Results: (1) After 24 h action of negative electrets, electrophoretic mobility (or surface charge) and cell number in S phase of 3T3 cells were significantly increased compared with those in control group. (2) Effect of negative electrets enhancing cell growth and increasing cell surface charge was in proportional to the surface potential of electret. (3) Surface charge density of apoptotic cell was reduced by electret. (4) After 24 h action of positive electret, the cell number in S and G2 phase were decreased and cell surface charge was also reduced. Conclusion: Negative electret can improve cell growth and increase cell surface charge density. Positive electret can restrain cell growth and reduce cell surface charge density. Surface charge of apoptotic cell is less than that of normal cell.

Objective To explore the application value of CEUS in diagnosis of renal occupying lesions.Methods Totally 67 patients with renal occupying lesions underwent conventional ultrasound and CEUS before surgical operation.CEUS enhancement characteristics were observed and compared between 40 patients with clear cell renal cell carcinoma (CCRCC)and 14 patients with angiomyolipoma (AML).Meanwhile the time-intensity curves were analyzed,including peak intensity (IMAX),rise time (RT),time-to-peak time (TTP) and mean transit time (mTT).Results There were significant differences in enhancement patterns,degrees,homogeneity of enhancement and pseudocapsule sign between CCRCC and AML (all P＜0.01).Compared with AML,IMAX of CCRCC was higher,RT and mTT were both earlier (P＜0.05).The sensitivity,specificity and accuracy of CEUS in diagnosis of malignant renal occupying lesions was 91.49 ％ (43/47),75.00 ％(15/20) and 86.57％ (58/67),respectively.Conclusion CEUS combined with quantitative analysis software is helpful to diagnosis and identification of CCRCC and AML.

Objective To explore the causes of persistent abnormal muscle response (AMR) after microvascular decompression (MVD) for hemifacial spasm (HFS) and the clinical outcomes of these patients.Methods MVD was performed under intraoperative electrophysiological monitoring of AMR in 372 HFS patients in 2014.Before MVD,the characteristic AMR of HFS was recorded in 326 patients.The patients were divided into two groups based on whether AMR disappeared or persisted following MVD;21 patients showed persistent AMR after successful MVD while AMR disappeared after decompression in the other 305 patients.The clinical features,treatment efficacy and postoperative complications were compared between these two groups.Results Gender,side of depression and mean age between the two groups showed no significant differences (P＞0.05).The immediate postoperative cure rate of the AMR disappeared group (88.9％) was significantly higher than that in the AMR persisted group (28.6％,P＜0.05).The follow-up cure rate showed no significant difference between the two groups (P＞0.05),and the postoperative and follow-up complications showed no significant differences (P＞0.05).Conclusion The long duration of HFS patients may be responsible for persistent AMR after successful decompression,and it is more likely for these patients to get delayed cured;their long-term outcomes showed no difference as compared with those in patients with disappeared AMR after MVD.
