1.Study on the association between vitamin D receptor gene polymorphism of ApaI and BsmI and vitamin D deficiency rickets

ObjectiveTo study the distribution characters and linkage disequilibrium of vitamin D receptor(VDR) gene ApaI and BsmI polymorphism,and explore the genetic susceptibility of VDR and vitamin D deficiency rickets.MethodsVDR gene ApaI and BsmI polymorphisms were determined by PCRRFLP technology in 56 cases of rickets and 76 cases of normal children.The frequencies of the VDR genotype and allele were compared between the two groups.The software SHEsis was used to make linkage disequilibrium analysis.Results No significant difference was found in either the frequency distribution of VDR gene ApaI and BsmI polymorphism or allele of them between two groups; Two polymorphisms didn't show linkage disequilibrium and D' and r2 were 0.23 and 0.01,respectively.ConclusionsThe ApaI and BsmI polymorphism of VDR gene might be not associated with vitamin D deficiency rickets.Two polymorphisms didn't show linkage disequilibrium.
2.Short-term efficacy and influencing factor analysis of modified transanal Soave surgery on hirschsprung′s disease infants in initial radial resection

Objective To observe the short-term clinical efficacy of modified transanal Soave surgery on infants with hirschsprung′s disease (HD) in initial radial resection, and to analyze the influencing factors. Methods 132 HD infants were selected. After initial radial resection with modified transanal , Soave surgery was conducted under general or sacral anesthesia. Postoperative antibiotics were routinely given. Results 19-56 cm intestinal canal samples were excised, with average length of (35.07 ± 3.15) cm. The average surgical duration and intra-operative hemorrhagic volume were (120.48 ± 18.34) min and verage was (45.74 ± 8.14) mL. All infants′ gastrointestinal function was recovered in 24 h postoperatively. The total excellent and effective rate of anal function in 6 months was 90.90% postoperatively. Cox modal multivariate analysis suggested that anastomotic orifice stenosis and enteritis were the two factors that greatly influenced the excellent and effective rate of anal function 6 months after the surgery (P < 0.05). Conclusion The modified transanal Soave surgery has small trauma and low complication rate on HD infants in initial radial resection , and the postoperative enteritis and anastomotic orifice stenosis are independent influencing factors for the short-term efficacy.
3.Clinical study of Beling Weitong granule combined with proton pump inhibitors in senile gastroesophageal reflux disease

Objective:To explore the clinical effect of Beling Weitong granule combined with proton pump inhibitors in senile gastroesophageal reflux disease(GERD).Methods:A total of 186 patients with GERD treated in the Second People′s Hospital of Dujiangyan from January 2020 to December 2021 were retrospectively selected as the study subjects, of which 93 patients received proton pump inhibitor (prilosec) orally (control group) and 93 patients received Beling Weitong granule combined with prilosec orally (combination group). Both groups were treated for 8 weeks. Symptom score, reflux symptom-based questionnaire (GERDQ) score, clinical efficacy, serum levels of gastrin-17 (G-17), motilin (MTL), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α) before and after treatment and safety were compared between the two groups.Results:After treatment, the symptom score and GERDQ score in the combination group were lower than those in the control group: (8.28 ± 2.57) scores vs. (15.68 ± 3.69) scores, (8.15 ± 2.23) scores vs. (10.50 ± 2.45) scores, there were statistical differences ( P<0.05). The total effective rate in the combination group was higher than that in the control group: 96.77% (90/93) vs. 87.10% (81/93), there was statistical difference ( χ2 = 5.87, P<0.05). After treatment, the levels of serum G-17 and MTL in the combination group were higher than those in the control group, the levels of IL-6 and TNF-α were lower than those in the control group: (7.48 ± 0.96) pmol/L vs. (6.55 ± 0.83) pmol/L, (206.96 ± 37.83) pg/L vs. (184.75 ± 35.69) pg/L, (9.38 ± 1.92) ng/L vs. (14.13 ± 2.04) ng/L, (9.41 ± 1.85) ng/L vs. (12.01 ± 2.11) ng/L, there were statistical differences ( P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups ( P>0.05). Conclusions:Beling Weitong granule combined with proton pump inhibitor in the treatment of senile gastroesophageal reflux disease is effective, and it is safe.
4.Application of highly selective arterial indocyanine green angiography in the design of anterolateral thigh free flap

Objective:To introduce the application of highly selective arterial indocyanine green angiography (hereinafter referred to as highly selective arterial angiography) in the design of anterolateral thigh free flap.Methods:This study was a retrospective observational study. From November 2023 to April 2024, 29 patients with wounds in extremities which were repaired by anterolateral thigh free flaps designed under the assistance of highly selective arterial angiography and met the inclusion criteria were admitted to the Department of Hand Surgery and Department of Wound Repair Surgery of Suzhou Ruihua Orthopedic Hospital, including 26 males and 3 females, aged 16 to 71 years. The wound area after debridement ranged from 8.0 cm×4.5 cm to 27.0 cm×16.0 cm. During the surgery, highly selective arterial angiography was used to assist in flap design. The fluorescence development range of the source arteries or perforators of flaps was observed. The blood supply range of the source arteries or perforators of flaps was determined based on the fluorescence development of the skin, and the excision position of the flap was adjusted. The flap incision area ranged from 9.0 cm×6.0 cm to 29.0 cm×16.0 cm. During the surgery, the number of highly selective arterial angiography, the type of source artery of perforators for puncture, and changes in the excision position of flaps were observed and recorded. After surgery, the blood supply and survival of flaps, the healing of wounds and the survival of skin grafts in the flap donor sites, and the angiography-related complications were observed.Results:All the 32 flaps of 29 patients were successfully excised. The highly selective arterial angiography was performed 37 times, including 13 cases of puncture of the oblique branch of the lateral circumflex femoral artery, 6 cases of puncture of the descending branch, 8 cases of double puncture of the oblique and descending branches, and 2 cases of puncture of arteries from other branches. During the surgery, the excision position of 28 flaps did not change, the excision position of 3 flaps moved towards proximal extremity of the thigh, and the excision position of 1 flap moved towards distal extremity of the thigh. All the flaps survived successfully after the surgery, and there was no partial necrosis of the flaps at the proximal or distal ends. The wounds in the flap donor sites healed, and all skin grafts survived. No angiography-related complications occurred.Conclusions:Highly selective arterial angiography can be used to determine the blood supply range of the source artery and perforators of the anterolateral thigh free flaps during the surgery. It can evaluate the blood supply of flaps more intuitively and objectively. Its application in assisting flap design can avoid partial flap necrosis caused by unreasonable preoperative design to a certain extent, and it is safe and reliable.
