2.Alteration and clinical significance of NK cell and T lymphocyte subpopulation in patients with rectal carcinoma

Objective To investigate the alteration and clinical significance of NK cell and T lymphocyte subpopulatien in patients with rectal carcinoma. Methods The numbers of NK cell and T lymphocyte in 26 healthy persons and 35 rectal carcinoma patients were detected by 3H-releasing method and FCM. Results NK cell and CD3, CD4, CD4/CD8 were decreased significantly in patients with rectal carcinoma (P<0.05), which was positively correlated with Dukes staging (P<0.01), and was inereased after redical or palliative operation (P<0.05), but NK cell and CD3, CD4, CD4/CD8 were further decreased in patients with unresectable tumor after operation (P<0.05). Conclusions The immunity of rectal patients was decreased with the development of the disease, and it will be improved after radical of palliative operation.

At present, diagnostic standards and treatment guidelines for mental stress-induced myocardial ischemia have not been established.Clinical symptoms are often seen in patients, but clinical evidence of myocardial ischemia is lacking.With increasing research closely related to mental stress-induced ischemia and adverse cardiovascular outcomes, more and more attention has been paid to coronary microcirculation in stenosis or coronary microvasospasm.With mental stress as a starting point, this article analyzes the incidence and characteristics of mental stress-induced myocardial ischemia and reviews its pathological mechanisms from the perspectives of myocardial inflammatory reaction, vasoconstriction, endothelial dysfunction, sympathetic nerve conduction and genetic factors.

5.Clinical analysis and management of retained common bile duct stones

Objective To investigate the risk factors and management of retained common bile duct stones.Method The clinical data of 14 patients with retained common bile duct stones between 1994 and 2000 was statistically analyzed in comparison with 191 patients of the negative postoperative T tube cholangiography. Results Common bile duct stones were present in 20% of patients undergoing cholecystectomy. In 7% of patients, stones were left in the bile ducts after common bile duct exploration. Statistically significant difference was observed in the age of the patient population, the percentage of associated acute cholangitis, the number of the stones present in common bile duct and the frequercy of the intraoperative choledochoscopy when compared retained bile duct stones with negative postoperative T tube cholangiography. Conclusions Retained common bile duct stones is related to age of the patient, the association of acute cholangitis, the number of the common bile duct stones and the operative choledochoscopy. Postoperative flexible choledochoscopy or endoscopic papillotomy is an effective method for managing patients with retained common bile duct stones.

Objective:To study the effect of extraneous sugar on rea ct ive oxygen species metabolism of Streptococcus mutans. Methods: Electron spin resonance (ESR) method was used to measure reactive oxygen spe cies of Streptococcus mutans incubated in the different sugar incubation con ditions. Results:With the increase of glucose at 0.0062~0.05 mo l/L, the quantities of reactive oxygen species produced by Streptococcus mutan s increased and increased to the maximum when Streptococcus mutans were in cubated in different sugar culture fluids within 2 h. All kinds of sugar promote d reactive oxygen species metabolism of Streptococcus mutans. Conclu sion:Extraneous sugar promotes reactive oxygen species metabolism of Streptococcus mutans.

Paroxysmal supraventricular tachycardia includes atrioventricular node reentry tachycardia, atrioventricular reentry tachycardia and atrial tachycardia.At present, the commonly used methods in clinical diagnosis include ventricular pacing in tachycardia, pre ventricular stimulation in refractory/non refractory period of his bundle, pacing in different parts of the atrium, parahisian pacing in sinus rhythm and stimulation in different parts of the ventricle.Ventricular pacing is one of the most commonly used methods in clinical diagnosis.However, it may lead to " false V-A-A-V" sequence after pacing.The ventricular pre phase stimulation of his bundle refractory period may appear false negative or lead to termination of tachycardia, which is not condutive to differential diagnosis.The results may be affected by pacing position, bypass position and refractory period when parahisian pacing and ventricular stimulation at different sites/frequencies are performed in sinus rhythm.Because of their respective advantages and limitations, it is necessary to integrate a variety of identification methods to improve the accuracy of diagnosis and the success rate of operation.

Objective: To study the effect of methylene blue on the metabolism of reactive oxygen and acid production of Streptococcus mutans . Methods: Streptococcus of cricetusa, rattusb, mutansc, sobrinsd and mutans e,f and g were exposed to methylene blue at 2.5?10 -4 g/L for 1 h. The metabolism of reactive oxygen and acid production were detected by electron spin trapping method and gas chromatography respectively. Results: Electron spin resonance (ESR) spectrums of DMPO-O - ? 2 and DMPO-OH ? were observed in Streptococcus mutans cultured in glucose fluid, but were not found in those exposed to methylene. The total of acid production by Streptococcus mutans incubated with glucose + methylene blue were significantly lower than that with single glucose. Conclusion: Methylene blue may inhibit oxygen species metabolism and acid production of Streptococcus mutans , it may be used as a kind of dental caries prevention agent.

HM(hibernating myocardium)is an adaptive phenome-non of myocardium against sustained ischemia,which maintains its tissue vitality through balancing energy supply and demand.It widely exists in patients suffering from coronary heart disease. HMhas its own metabolic pattern,instead of regular FAO(fatty acid β-oxidation)-based metabolism,glycolysis became main pro-cedure.Reduction of FAO,TCA (tricarboxylic cacidcycle),ETC (electron transport chain)enzyme has been observed,ROS(reac-tive oxygen species)and UCP2(uncoupling protein 2)have been up-regulated.UCP2,which promotes proton leak across innermembrane of mitochondrial and leads to ATP reduction,has e-merged as an important regulator of the energy production.It is regulated by up-stream proteins such as AMPK,PPARs,PGC-1α,and other factors like FFA(free fat acid),ROS and purine nucleotide.HM has potential function of ischemic myocardium, which can improve cardiac function through reasonable interven-tion.Modulation of UCP2 can optimize energy production,and is essential to HM metabolism.
