1.Biomechanical study of neotype shape memory alloy cervical hook for atlantoaxial instability

BAcKGROUND:In recent years,the pedicle screws fixation technique,which is used in fixation for atlantoaxial instability associated with trauma,severe degeneration and tumorectomy,has been developed.However,this kind of technique easily causes several complications,including malpositional screws,vascular injuries,and even vertebral artery injury.Based on the biomechanical characteristics of memory alloy and determination of atlantoaxial data,a neotype shape memory alloy cervical hook was designed to treat atlantoaxial instability.OBJECTIVE:To investigate the biomechanieal characteristics of the neotype shape memory alloy cervical hook for atlantoaxial instability.DESIGN,TIME AND SETTING:Repeated measurement analysis of variance test was performed in the Laboratory of Clinical Anatomy and Medical Biomechanics,Southern Medical University between March and April 2008.MATERIALS:Eight fresh adult craniocervical specimens(C0-C4)were provided by Department of Clinical Anatomy,Southern Medical University.Atlantoaxial neotype shape memory alloy cervical hook(50.8%-51.8%nickel and the remaining part was titanium)was fabricated by Shanghai Xinchang Memory Alloy Co.,Ltd.METHODS:The included eight C0-C4 specimens were used to test three-dimension ranges of motion(ROM)by fixation and neotype shape memory alloy cervical hook fixation.Then,the positions of spine varying from no loading to the maximum loading status were scanned and analyzed using image processing software to determine the three-dimensional ROM under different statuses.MAIN OUTCOME MEASURES:Three-dimensional ROM of tested specimens.RESULTS:Neotype shape memory alloy cervical hook fixation and Germany AESCULAP SSE hanger fixation had similar flexion-extension range of motion(P=0.595).Lateral bending three-dimensional ROM was greater in the neotype shape memory alloy cervical hook fixation group than in the Germany AESCULAP AAE hanger fixation(P< 0.05).The rotatory three-dimensional ROM was smaller in the neotype shape memory alloy cervical hook fixation group than in the Germany AESCULAP AAE hanger fixation(P<0.05).CONCLUSION:Neotype shape memory alloy cervical hook fixation had comparative post-surgery immediate stability with the Germany AESCULAP AAE hanger fixation.Neotype shape memory alloy cervical hook fixation produced a little worse biomechanical lateral bending stability and a little better biomechanical rotatory stability than Germany AESCULAP AAE hanger fixation.

3.Effect of 17AAG-cypate polymer micelle on radio-sensitivityof A549 cells

Objective To investigate the radiosensitizing effect of 17AAG-cypate micelles on human non-small cell lung cancer A549 cells and its possible mechanism.Methods (1) A single-hit multi-target model formula was used to analyze the radiosensitizing effects of 17AAG-M and 17AAG-cypate-M.(2) The effects of 17AAG-cypate-M on the viability of A549 cells under laser and X-ray irradiation were analyzed by MTT assay.(3) The effect of the drugs on the cell senescence was observed by β-galactosidase staining assay.(4) The effects of different treatment conditions on DNA damage repair were analyzed by γ-H2AX immunofluorescence staining assay.(5) The expression of p-Erk1/2 and p-Akt was measured by Western blot.The paired t test was used for analyzing the differences between groups.Results Compared with the X-ray irradiation group,the X-ray+17AAG-cypate-M group had a lower mean lethal dose and a sensitization enhancement ratio greater than 1,indicating that 17AAG-cypate-M had a radiosensitizing effect.Compared with the 17AAG-M group,the 17AAG-cypate-M group showed significantly lower cell viability (P<0.01),a significantly higher percentage of aging cells (P<0.01),and significantly further delayed DNA damage repair (P<0.01).And the 17AAG-cypate-M group had lower expression of p-Erk1/2 and p-Akt than the 17AAG-M group.Conclusions Compared with 17AAG-M,17AAG-cypate-M has a higher radiosensitizing effect on A549 cells.The mechanism might be inducing the cell senescence,delaying DNA damage repair,and inhibiting the expression of p-Erk1/2 and p-Akt.
4.Radiographic features and clinical value of persistent trigeminal artery with vascular disease

Objective To explore CTA and MRA characteristics of persistent trigeminal artery (PTA)with vascular disease. Methods 54 patients diagnosed as PTA by CTA and MRA were collected retrospectively,whose classification,variation and associated vascular disease were analyzed.Results There were more females than males in 54 patients,whose average age was 59.72±16.32,and the incidence of left ones were higher than that of the right ones.Type Ⅰ of Weon classification was 16 cases (29.63%),type Ⅱ 7 cases (12.96%),typeⅢ 16 cases (29.63%),type Ⅳ 1 1 cases (20.37%),type Ⅴ 4 cases (7.41%).The lateral type of Salas classification was significantly higher than that in the middle type.There were 16 cases (29.63%)with aneurysm,in which 3 cases (18.75%)were rupture.3 cases were with moyamoya disease (5.56%).39 cases (72.22%)were with basilar artery dysplasia.There were 33 cases (61.11%)with the postembryonic cerebral artery.There were 29 cases with cerebral infarction (53.7%),4 cases with cerebral hemorrhage (7.4%),3 cases with subarachnoid hemorrhage (5.5 6%).Cerebral arteriosclerosis with different degrees of stenosis was showed in 1 7 cases (31.48%). 2 cases were accompanied with trigeminal neuralgia (3.7%),and only 1 case was with ocular nerve palsy (1.85%).Conclusion CTA and MRA could be used to display the anatomical features and radiographic classification of PTA ,and also clearly display and evaluate PTA associated with vascular disease.

From December 2019, Coronavirus disease 2019 (COVID-19) pneumonia (formerly known as the 2019 novel Coronavirus [2019-nCoV]) broke out in Wuhan, China. In this study, we present serial CT findings in a 40-year-old female patient with COVID-19 pneumonia who presented with the symptoms of fever, chest tightness, and fatigue. She was diagnosed with COVID-19 infection confirmed by real-time reverse-transcriptase-polymerase chain reaction. CT showed rapidly progressing peripheral consolidations and ground-glass opacities in both lungs. After treatment, the lesions were shown to be almost absorbed leaving the fibrous lesions.

7.Effect of down-regulation NEK-2 level on radio-sensitibity of A549 cells

Objective To investigate the influence of down-regulation NEK-2 level on the radiosensitivity of A549 cells.Methods NEK-2 siRNA was transfected to A549 cells with liposome and NEK-2 expression level was inspected by Western blot.The radiosensitivity was detected by clone formation experiment.Cell cycle and cell apoptosis were analyzed by flow cytometry.Immunofluorescence experiment was used to detect the DNA double strand break and repair.Results NEK-2 siRNA successfully suppressed NEK-2 expression in A549 cells and resuced the cell proliferation ability after irradiation compared to the blank control group and the negative control group.It can improve the radiosensitivity of A549 cells (The radiosensitivity of A549 cells enhanced significantly.).The D0 values declined form 1.80 Gy to 1.40 Gy,and the sensitizing enhancement ratio was 1.32.After irradiation,compared to negative control group,the apoptosis rate was significantly improved (7.85% to 17.17%),cells in G2/M phase were obviously increased (9.23% to 30.16%),the DNA double strand break rate was increased (100% to 165%) and the DNA damage repair rate was reduced (100% to 48%) in NEK-2 siRNA group.The comparisons among the groups wer statistically significant (P<0.05).Conclusions NEK-2 siRNA reduced the proliferation and increased the radiosensitivity of A549 cell line,probably by affecting the cell cycle,promoting cell apoptosis and suppressing DNA damage repair.
8. Inhibitory effect of 17AAG-cypate polymer micelles on A549 cell xenografts in nude mice in vivo

Objective:
To investigate the inhibitory effect of 17AAG-Cypate micelles on the non-small cell lung cancer A549 cells in nude mice and to explore its possible mechanism.
Methods:
A549 lung adenocarcinoma tumor-bearing nude mice were established. The nude mice were treated with saline ( saline group), X-ray (X-ray group), 17AAG micelles+ X-ray (17AAG-M/X group) and 17AAG-Cypate micelles+ laser/X-ray (17AAG-Cypate-M/L+ X group), respectively. The growth of xenograft tumors in different groups was measured on a regular basis to delineate the growth curve. The expression of proliferating cell nuclear antigen (PCNA) was measured by immunohistochemistry. The microvascular density was detected. The apoptosis of xenograft tissues was observed by TUNEL staining. The expression levels of p-ERK1/2 and p-AKT were quantitatively measured by Western blot.
Results:

9.Comparison of percutaneous versus surgical approach in femoro-femoral veno-arterial ECMO cannulation: a propensity score matched study

Objective:To investigate the safety and the efficacy of percutaneous and surgical approach in femoro-femoral veno-arterial extracorporeal membrane oxygenation (VA-ECMO) cannulation.Methods:All consecutive patients implanted with femoro-femoral VA-ECMO between January 2018 and December 2020 in Beijing Anzhen Hospital, Capital Medical University. Propensity score matching was used to compare outcomes of percutaneous and surgical groups while controlling for confounders.Results:Among the 276 patients who received femoro-femoral VA-ECMO (62 surgical and 214 percutaneous), propensity-score matching selected 52 pairs of patients with similar characteristics with mean age of(59.6±13.0)years old, in which 26 patients were female. There were a lower ECMO cannulation-associated complication (28.8% vs. 48.1%, P=0.044) and a lower hospital mortality (42.3% vs. 67.3%, P=0.010) in the percutaneous group. The circuit blood flow after ECMO initiation was similar in both groups[(3.3±0.8)L·min -1·kg -1 in percutaneous group vs. (3.2±0.7)L·min -1·kg -1 in surgical group, P=0.738]. The serum lactate was declined in both group after ECMO initiation[(5.4±5.8)mmol/L vs. (9.2±6.9)mmol/L, P<0.001 in percutaneous group; (6.3±6.2)mmol/L vs. (10.5±7.0)mmol/L, P=0.003 in surgical group]. Conclusion:Percutaneous approach is a safe and efficient technique in emoro-femoral VA-ECMO cannulation. Compared with surgical cannulation, percutaneous approach is associated with lower ECMO cannulation-associated complication and lower hospital mortality.
10.Anti-mGluR5 encephalitis with mental disorders as the initial symptom: a case report

This article reported the clinical features of a rare patient with anti-metabotropic glutamate receptor 5 (mGluR5) encephalitis with mental disorders as the initial symptom, so as to provide references for clinical diagnosis and treatment. The patient was a 38-year-old male, developed pharyngeal pain as prodromal symptoms, and the main clinical manifestations included rapidly progressive memory loss, anxiety and depression, and psychomotor excitement symptoms including irritability and impulsive behaviors. The disease had a progressive deterioration. In the most severe state, the patient became unconscious in a shallow coma, with further cognitive decline, hallucinations and delusions, and lack of self-awareness. Both cerebrospinal fluid and serum anti-mGluR5 antibody were strongly positive (1∶100). After two sessions of hormone shock therapy, the patient showed significantly improvement in consciousness, cognitive, emotional and psychiatric dimensions.
