1.Assessment of low quantitative ultrasound values of calcaneus in Chinese southern healthy females

Objective To study the pattern of ultrasonic bone mineral density of calcaneus with age, height and body weight, and to establish a normal reference value of stiffness index (SI) of females in Southern China for providing scientific foundation for osteoporotic diagnosis and prevention. Methods SI for calcaneus in 2498 healthy people 10~87 years old was measured with Achilles Express ultrasound apparatus made in USA. They were divided into groups according to sex and age. One group for 10 years each and the records beyond 69 years were classified into one group, total 7 groups. Results The SI showed a characteristic mild rise-then-fall pattern with increasing age. And the peak value of SI presented in 20~29 age group. Pearson correlation analysis showed negative correlation between SI and age and a positive correlation between SI/ height and weight. The prevalence of osteoporosis gradually increase with age. Conclusion There are significant correlations in SI with age, height and weight; the values of SI will provide an important data reference to establish the normal values and diagnostic]standard of osteoporosis.
2.Experimental study of cryoablation on rabbit's VX2 brain tumor under MRI guidance and real-time monitoring

Objective To verify the feasibility of eryoablation on rabbit brain tumor under interventional MRI guidance.Methods The procedures were performed under general anaesthesia.Twenty-two New Zealand white rabbit'S brains were inoculated with VX,tumor tissue through cranial holes. MR-guided cryoablation were conducted as preliminary experiment when diameter of brain tumor grew up to 0.8 cm in size.Cryoablation lasted two cycles of 5 minutes freezing, in 2 rabbits.and of 10 minutes freezing in another 2 rabbits.After procedure and MR scanning.rabbits were executed.Specimens of rabbit'S brain showed complete necrosis pathologically after cryoablation of cycles of 10 minutes freezing.The optimal parameters of eryoablation were defined.Except for the two rabbits that died of infection,the other 16 rabbits were divided into control group and therapeutic group.According to the optimal parameters of cryoablation,cryoablation of 10 minutes freezing were performed for rabbit brain tumors in therapeutic group.Venous injeetion of 20%mannitol and subcutaneous injection of dexamethasone were used for dehydration.None of rabbit brain tumors in the control group were treated.After cryoablation,MRI scanning Was performed immediately,7 days later,14 days later and 60 days later,to observe the ablative area and the pathologic changes.The survival time wag recorded.Resuits Brain tumors could be found in all New Zealand white rabbits after inoculation except two died of inflammation.The maximum diameter of brain tumors Was from 0.7 to 1.0 elEL The tumors showed slight low signal on T1WI and slight high signal on T2WI as well as comrast enhancement after injection of Gd-DTPA.Pathologically coagulative necrosis were observed after cryoablation immediately,7 days and 14 days later,and liquefactive necrosis was found 60 days later.The correlation between MRI and pathologic appearance of tmnors are consistent.The therapeutic area ranged from 1.0 to 1.5 em.,The maximium diameter of ice ball ranged from 1.5 to 2.0 cm Conclusion CiToablation for brain tumor iS a safe and feasible therapy.
3.Creatinine concentration of 2630 healthy students in primary and middle schools in Inner Mongolia

ObjectiveTo analyze the serum creatinine level among apparently healthy primary and secondary school students in Inner Mongolia and explore the distribution of serum creatinine by ethnic,regional,gender and age,and establish the reference interval of serum creatinine in different gender and different age groups of primary and secondary school students.MethodsLargesample clinical epidemiological investigation was applied by two-stage clustering sampling method.Random sample of 2630 primary and secondary school students from 9 to 18-year-old was selected from four district in Inner Mongolia including Hohhot,Wulanchabu,Xilin Gol and Bayan Drow from July 2009 to June 2010.After screening outlier individual,the total of 2614 subjects were enrolled,involving 1288 male and 1326 female subjects,1584 Han and1030 Mongolian.The venous blood was collected and serum was separated.The serum creatinine concentration was measured as soon as possible.Furthermore, creatinine levels of different regions,ethnic,gender and age group were compared by analysis of variance or t-test and that of different group were compared by SNK method.Percentile was used to describe the distribution of serum Cr level of different age groups.The reference interval of serum Cr for primary and secondary school students were established by gender and age (P2.5 -P97.5 ).The curve was smoothed using age-specific percentile ( LMS )curve smoothing method.ResultsThe differences of Cr levels were statistically significant between different regions,ethnic,gender and age groups.The reference intervals of creatinine for 9 - 11,12,13 - 14,15,and 16 - 18 year-old males were 35 - 66,37 - 73,39 - 78,47 - 87 and 49 - 91 μmol/L,respectively.The intervals for 9 - 10,11 - 12,13 - 15,and 16 - 18 year-old females were 32 - 60,34 - 63,38 -73 and 40 -74 μmol/L,respectively.Conclusion The reference intervals of serum creatinine for health primary and secondary school students in the Inner Mongolia is established,which is useful for clinicians,especially pediatricians to judge and assess renal function for 9 to 18 year-old patients.( Chin J Lab Med,2012,35:805-809 )
4.Suitable treatment of post operative anastomotic recurrence after anterior resection of rectal cancer and relationship between different kinds of therapy with prognosis

Objective To investigate the suitable treatment of post operative anastomotic recurrence after anterior resection of rectal cancer and analyze the relationship between different kinds of therapy with prognosis. Methods The clinical data of 41 cases of local recurrent rectal cancer after anterior resection admitted in our hospital from 1999 to 2009 were analyzed retrospectively. The median survival time and survival rate were calculated by Life Tables method. The influence of different kinds of treatment to prognosis was evaluated by Kaplan-Meier method and the variability was analyzed by Log-rank method. P <0.01 means statistical significance. Results Thirty-three (80.5 %) of 41 patients were recurrent in the first 3 years and the median survival time was 23 months. Seventeen (41.5 %) of 41 patients underwent radical R0 resection. The median survival time of radical resection patients and non-radical resection ones were 49 months and 18 months, respectively, and the difference was significant (χ2=12.245, P=0.000). Thirty-one patients with radiotherapy and/or chemotherapy showed a statistically longer median survival time than the other 10 patients without these adjuvant treatment (39 months and 9 months, respectively) (χ2=17.533, P =0.000). Conclusion Most post operative anastomotic recurrent of rectal cancer cases occurs in the first 3 years after primary surgery. Radical resection, radiotherapy and chemotherapy can improve the prognosis.
5.Clinical characteristics and prognosis of combined hepatocellular-cholangiocarcinoma

Objective To investigate the clinical characteristics and analyze prognostic risk factors of combined hepatocellular-cholangiocarcinoma. Methods The clinical data of 19 cases of combined hepatocellular-cholangiocarcinoma admitted in our hospital from January 1999 to December 2009 were analyzed retrospectively. The survival function was analyzed by Kaplan-Meier. The possible prognostic risk factors were tested by χ2-test. Results Hepatocellular-cholangiocarcinoma was diagnosed by pathology in the 19 patients, among which hepatic tunic was infiltrated in 13 cases, peritoneum involved in 1 case, intravascular cancer embolus in 1 case. At that time lymphocyte nodes metastasis in 2 cases were found by regional lymphadenectomy in 7cases. The 1-year and 3-year survival rates were 61% and 42%,respectively. Prognosis of patients with tumor size > 5 cm ( χ2 = 4. 392, P = 0. 036 ), history of heavy drinking ( χ2 = 11.010, P = 0.001 ) or intraoperative blood transfusion ( χ2 = 4. 645,P = 0. 031 ) were worse than others. Conclusion It was difficult to get correct preoperative diagnosis of combined hepatocellularcholangiocarcinoma. Tumor size, history of heavy drinking and blood transfusion were all prognostic related risk factors.
6.Value of combined detection of CRP,PCT,IFN-γ,IL-6 and IFN-γ/IL-10 in early clinical diagnosis of HIV/TB co-infection

Objective To evaluate the expression differences of C-reactive protein(CRP),procalcitonin(PCT)and cytokines[interferon-γ(IFN-γ),tumor necrosis factor-α(TNF-α),interleukin(IL)-10,IL-6,IL-4,IL-2,IL-17 A,and IFN-γ/IL-10]in human immunodeficiency virus(HIV)infection,tuberculosis(TB)infec-tion and HIV/TB co-infection in order to provide evidence for early clinical diagnosis of HIV/TB co-infection.Methods Medical records and clinical information of 103 patients with HIV(HIV group),42 patients with TB(TB group)and 67 patients with HIV/TB co-infection(HIV/TB group)were collected from January 2021 to September 2022.CRP,PCT and cytokines were detected in all patients,and their expression differ-ences among the three groups were analyzed.Receiver operating characteristic(ROC)curves were drawn and screening ability was evaluated.Results Compared with HIV group and TB group,CRP,PCT,IFN-γ,IL-10,IL-6 levels and IFN-γ/IL-10 in HIV/TB group were increased,with statistical significance(P<0.001).Bon-ferroni correction results showed that CRP,PCT,IFN-γ,IL-6 levels and IFN-γ/IL-10 were significantly differ-ent between HIV and HIV/TB groups(all P<0.001).ROC curve analysis showed that the area under the curve of CRP,PCT,IFN-γ,IL-6 and IFN-γ/IL-10 combined detection was 0.826,which was higher than that of single detection.Conclusion CRP,PCT,IFN-γ,IL-6,IFN-γ/IL-10 combined detection can help the early screening of HIV infection and HIV/TB co-infection.
7.Clinical observation of the combined use of Yulu Shengjing Decoction and Levocarnitine in the treatment of oligoasthenozoospermia resulting from spleen and kidney deficiency

Objective:To investigate the clinical efficacy of combined use of Yulu Shengjing Decoction and Levocarnitine in the treatment of oligoasthenozoospermia resulting from spleen and kidney deficiency and its effect on traditional Chinese medicine syndrome score and sperm motility. Methods:A randomized controlled study was conducted on 64 patients with oligoasthenozoospermia resulting from spleen and kidney deficiency who received treatment at the Huaibei Hospital of Traditional Chinese Medicine,Anhui University of Chinese Medicine between March 2022 and December 2023. These patients were randomly divided into an observation group ( n = 33) and a control group ( n = 31) using a random number table method. The control group received treatment with Levocarnitine, while the observation group received treatment with a combination of Yulu Shengjing Decoction and Levocarnitine. After 3 months of treatment, the clinical efficacy of the two groups was compared. The relative changes in traditional Chinese medicine syndrome scores and semen indicators (including semen volume, sperm concentration, progressive motility, and progressive motility + non-progressive motility) were evaluated after treatment compared with their respective pre-treatment levels. Results:The total response rate in the observation group was 84.85% (28/33), which was significantly higher than that in the control group [61.29% (19/31), χ2 = 4.55, P < 0.05]. After treatment, the main symptom score and secondary symptom score of the observation group were (1.64 ± 1.20) points and (1.27 ± 0.84) points, respectively, which were significantly lower than (1.82 ± 1.04) points and (2.19 ± 0.87) points in the control group ( t = -2.94, -4.30, both P < 0.05). The semen volume, sperm concentration, progressive motility, and progressive motility + non-progressive motility of the observation group were (4.07 ± 0.86) mL, (28.81 ± 6.48) × 10 6/mL, (30.82 ± 11.43)%, and (70.69 ± 13.60)%, respectively. These values were significantly higher than those in the control group [(3.62 ± 0.73) mL, (23.87 ± 11.38) × 10 6/mL, (24.31 ± 8.27)%, (63.11 ± 14.28)%, t = 2.14, 2.02, 2.47, 2.05, all P < 0.05). Conclusion:The combination of Yulu Shengjing Decoction and Levocarnitine is effective in treating oligoasthenozoospermia with spleen and kidney deficiency. It can significantly improve patients' symptoms in traditional Chinese medicine, enhance semen quality, and increase sperm motility.
8.Risk factors of pancreatitis after percutaneous transhepatic biliary drainage in patients with pancreatic cancer and obstructive jaundice

Objective:To explore the risk factors and preventive strategies of pancreatitis after percutaneous transhepatic biliary drainage (PTBD) in patients with pancreatic cancer and obstructive jaundice.Methods:A total of 241 patients were retrospectively analyzed from May 2001 to October 2014 in Tianjin Medical University Cancer Institute and Hospital. The possibly correlated 9 factors were analyzed, including gender, age, hemoglobin level, total bilirubin level, degree of pancreatic duct dilatation, degree of pancreatic atrophy, degree of biliary stenosis, the pancreatic duct visualization, and drainage mode.Results:Univariate analysis suggested that pancreatic duct dilatation, pancreatic atrophy, visualized pancreatic duct and drainage mode were associated with the incidence of pancreatitis after PTBD ( P<0.05). Logistic regression analysis showed that visualization of pancreatic duct ( OR=6.33) was a risk factor for pancreatitis, while pancreatic duct dilatation ( OR=0.14), pancreatic atrophy ( OR=0.12) and external drainage ( OR=0.11) were protective factors for pancreatitis. Conclusion:In pateints with pancreatic cancer and obstructive jaundice, pancreatic duct dilatation and pancreatic atrophy predict low risk of pancreatitis after PTBD,while intraoperative pancreatic duct visualization and internal or external drainage may increase the incidence of postoperative pancreatitis.
