Chinese Journal of Geriatrics 2009;28(10):820-823
Objective To evaluate the ventricular systolic and diastolic synchronicity in elderly patients with the complete right bundle branch block (CRBBB) by Doppler tissue velocity imaging (TVI) technology. Methods The 35 elderly CRBBB patients were selected as CRBBB group, while 31 healthy people were chosen as control group to evaluate myocardial function. The time to peak velocity at systolic and early diastolic phase (Ts and Te) in the base and interlude 14 segments of left and right ventricles was measured. The average time of Ts in right ventricular 2 segments(Ts-2-RV), Ts and Te in left ventricular 12 segments (Ts-12-LV and Te-12-LV), and the standard deviation in left ventricular 12 segments (Ts-12 SD and Te-12-SD), the maximum Ts and Te difference (Ts-diff and To-dill) were calculated. The left ventricular end-systolic and end-diastolic diameter (LVEDs and LVEDd) and volume (LVEVs and LVEVd) were measured. Results (1)The time to peak velocity at systolic phase in 14 segments of left and right ventricles was longer in CRBBB group than that in control group(P<0.05 or P<0.01). Compared with the control group, the Ts-12-LV,Ts-12-SD and Ts-diff showed significant prolongation (P<0.01). The Ts-2-RV was longer than Ts-12-LV in CRBBB group[(226.3±37.4 ) ms vs. (195.5±69.5) ms, P < 0.05]. (2) There was no significant difference in the time to peak velocity at early diastolic phase in 14 segments between CRBBB group and control group(P>0.05), but the Te-12-SD and Te-diff was longer in CRBBB group than that in control group (P< 0.01). (3) The LVEDs and LVEDd were obviously increased in CRBBB group compared with controls (P<0.05 and P<0.01), but no significant difference was showed in LVEVs and LVEVd(P>0.05). Conclusions Compared with normal people, the Ts at systolic phase is longer, especially in the right ventricle, the LVEDs and LVEDd are significantly increased, and the ventricular systolic and diastolic synchronicity is not well in CRBBB patients.

A cross-sectional study was conducted on 1 052 patients with coronary heart disease (CHD) who visited 10 community health service centers in Chaoyang and Haidian districts of Beijing from March 2019 to September 2019. Basic information, medical history, cardiovascular risk factors, β-blocker use, height, weight, blood pressure level and resting heart rate were collected by face-to-face interview;biochemical tests and on-site physical examination were performed;and the influencing factors of heart rate control were analyzed by logistic regression. There were 563 males (53.5%) and 489 females(46.5%),with a mean age of (66.7±9.5) years. The rate of β-blockers use was 43.6% (459/1 052). The average resting heart rate was (70±8)/min, and only 14.5% (153/ 1 052) patients had the heart rate under control. The resting heart rate control rate in patients with β-blockers use was 17.9% (82/459), higher than that of those without β-blockers use [12.0% (71/593), χ 2=7.23, P=0.007]. Moderate leisure activities were beneficial to the control of resting heart rate ( OR=2.14, 95% CI: 1.26-3.62). The study shows that β-blockers use and resting heart rate control in CHD patients in Beijing community health institutions were both at low levels, and the resting heart rate was not well controlled even in patients taking beta-blockers.

Objective:To assess the prevalence of metabolic syndrome(MS) and its relationship with hyperuricemia(HUA) in perimenopausal women in Anning city, Yunnan province. Methods:This is a cross-sectional survey. In May 2021, a multi-stage stratified sampling method was used to collect demographics and clinical data [ethnicity, living community, height, weight, waist circumference, blood pressure, fasting plasma glucose, triglycerides(TG), serum uric acid, high density lipoprotein-cholesterol(HDL-C), alanine transaminase(ALT), etc] in a total of 6 721 perimenopausal women aged 45-60 years. Results:A total of 6 721 perimenopausal women were included in this study. The prevalences of MS and HUA were 14.05%(95% CI 13.22%-14.88%) and 6.46%(95% CI 5.88%-7.07%), respectively. The average age, HDL-C, urea, direct bilirubin, and albumin levels in the perimenstrual HUA population were lower than those in the non-HUA population while the levels of TG, ALT, heart rate, body mass index(BMI), and creatinine were higher(all P<0.05). The prevalence of HUA in perimenopausal women with ethnic minorities and family history of chronic diseases was higher than that in Han nationality and without family history of chronic diseases. The prevalence of MS in perimenopausal women was increased with the increase of serum uric acid( Z=-15.313 8, P<0.001). Multivariate logistic regression model showed that HUA was positively correlated with MS( OR=1.526, 95% CI 1.192-1.954) after adjusting for covariates such as BMI and ethnicity, and the incidence of MS in perimenopausal women in HUA group was 1.526 folds higher than that in non-hyperuricemia group. Conclusion:HUA is highly positively correlated with MS in perimenopausal women. The management of uric acid level in perimenopausal women should be strengthened.
