1.Therapeutic efficacy and health economic evaluation of early rehabilitation intervention in stroke unit applied to patients with cerebral hemorrhage

Objective To observe the therapeutic efficacy and evaluate the health economics of early rehabilitation intervention applied to patients with cerebral hemorrhage in stroke unit. Methods A total of 131 cerebral hemorrhage patients were randomly assigned to the stroke unit group (n=62) and the control group (n=69). The patients in the stroke unit group received rehabilitation training at a very early stage, while there were no rehabilitation training program and no professional treatment team for patients in the control group. Results of the national institute health stroke scale (NIHSS), activities of daily living (ADL) Barthel index were compared between the two groups. Infection rate, mortality, cost of hospitalization and length of stay were also compared. Results There were better therapeutic effects as revealed by NIHSS (4.3±3.5 vs. 7. 9±5.0, t=-3. 211, P<0.05), ADL Barthel index score (85.9±29.6 vs. 67.1±37.1, t=3.194, P<0.05), lower incidence of infection in lung (8.06% vs. 15.94%, χ~2 =3.901, P<0.05) and in urinary tract (6.45% vs. 11.59%, χ~2 =4. 138, P<0. 05), lower mortality (4.84% vs. 7.25%, χ~2=4. 351, P< 0.05), lower cost of hospitalization[¥ (17506. 90±954.10 ) vs. ¥(21096.49±923.46), t=-20. 786, P<0.01)] and shorter length of stay[(20. 47±7. 03)d vs. (31. 42±8.14)d, t=-8.196, P< 0.01)] in the stroke unit group compared to the control group. Conclusions Early rehabilitation intervention by stroke unit is advantageous to patients with cerebral hemorrhage.

Aim Through interfering the expression of survivin with short hairpin RNA(shRNA) technology,to investigate the effect of downregulation of survivin on cell apoptosis and chemosensitivity to docetaxel in breast cancer MCF-7 cells.Methods(1) Using MCF-7 cells as a model system,three groups were set up transfected with lipofectamine,RNAi control plasmid and survivin RNAi plasmid,respectively.The expression of survivin in MCF-7 cells was measured at transcriptional and translational level by using RT-PCR and Western blot methods.(2) The effect on the cell cycle and apoptosis was analyzed with flow cytometry.(3) The viability of cells applied with different doses of ducetaxel was determined by using the method of 3-[4,5-Dimethylthiazol-2-yl]-2,5-diphenyltetrazolium bromide reduction(MTT method).Results(1) RT-PCR and Western blot demonstrated that survivin expression was significantly decreased by transfection with RNAi targeting plasmid;the expression proportion was reduced by nearly 75.4% and 79.8%(P

5.Determination of nutrient constituents in the gonad of Hemicentrotus pulcherrimus

Objective To analyze the nutrient constituents of the gonad from Hemicentrotus pulcherrimus.The results could provide a theoretical basis for the development of the H. pulcherrimus.Methods The contents of the water,ash and protein were determined by the national standard methods,total sugar by phenol-sulfuric acid method,rude fat by soxhlet's method,fatty acids by gas chromatography-mass chromatography,and inorganic elements by atomic absorption spectroscopy method.Results The contents of water,ash,rude fat, protein and total sugar were 64.20%,12.70%,2.34%,12.25%and 5.59%,respectively. The contents of arachidonic acid and EPA were higher in fatty acids.Inorganic elements, such as Ca,Mg,Fe were also in higher level.Conclusion The gonad of H.pulcherrimus was useful in the exploitation of nutrient food.
6.Medical teaching reform:perspectives from translational medicine

Translational medicine emerging as a new concept in medical field, concentrates on the integration of theory and practice , and the conjunction of basic medical science and clinical medicine. Barriers between the basic medicine and clinical medicine existing in medical education in China make students possess limited intelligence, narrow perspectives and low creativity etc., causing the medical teaching unable to conform to the demand of translational medicine. The article introduces the basic conception of translational medicine and the necessity of medical teaching reform, the meth-ods of promoting teaching reform by integrating the curriculum systems, bettering experimental teach-ing systems, strengthening humanities education, improving faculty construction and optimizing eval-uation system for students.
7.The clinical significance of determining serum paraquat by spectrometry

ObjectiveTo evaluate the reliability and clinical value for detecting paraquat (PQ)concentration in serum by spectrometry. MethodsThe determinations of wave length for detecting serum PQ concentration by ordinary spectrometry and second-derivative spectrometry were carried out. When the second-derivative spectrometry was used for detecting PQ in serum, the linear range and precision for PQ concentration were well defined. The results of serum PQ concentration determined by second-derivative spectrometry and by HPLC (high performance liquid chromatography) were compared in 8 patient with PQ poisoning. A total of 21 patients with acute poisoning after PQ ingestion over 4 hours admitted from October 2008 through September 2010 were retrospectively studied. Patients were divided into two groups as per the serum concentrations more than 1.8 μg/mL or less than that by second-derivative spectrometry on the day of admission. The severity of clinical manifestations between two groups was analyzed with t-test or Fisher's exact probabilities analysis. Results ( 1 ) The absorption peak of 257 nm could not be found by using ordinary spectrometry to detect the PQ concentration in serum. (2) The calibration curve in the 0. 4 ~ 8.0μg/mL range for detecting PQ concentration by second-derivative spectrometry observed the Beer's law (r =0. 996) . The average retrieval rate of PQ was within the range of 95.0%~ 99. 5% with relative standard deviation (RSD) within 1.35%~ 5.41% ( n = 6), and the lowest detection limit was 0. 05μg/mL. (3) The results of PQ concentrations from 8 patients with PQ poisoning detected by second-derivative spectrometry were consistent with those of the quantitative determinations by HPLC ( r = 0. 995,P<0. 01 ) . (4) The survival rate of patients with serum PQ concentration more than 1.8 μg/mL was 22. 2% ,and the incidences of acidosis, oligouria and pneumomediastium in these patients were 55.6%,55. 6% and 77.8%, respectively. These clinical manifestations were significantly different from those in patients with serum PQ concentration less than 1.8 μg/mL ( P < 0. 05 ) . Conclusions ( 1 ) It was inappropriate to take 257 nm as the determination wave length for detecting serum PQ concentration by ordinary spectrometry. (2) The method of second-derivative spectrometry was reliable for detecting serum PQ concentration. (3) Serum PQ concentration detected by second derivative spectrometry could be used to predict the severity of clinical manifestations of patients with PQ poisoning and was an important predictive factor for poor prognosis if the serum PQ concentration more than 1.8 μg/mL after PQ ingestion over 4 hours.
8.Efficacy of propofol-fentanyl-sevoflurane anesthesia under spontaneous respiration with nasopharyngeal airway-face mask for pediatric minor surgery: a comparison with laryngeal mask airway

Objective To compare the efficacy of combined intravenous-inhalational anesthesia with propofol-fentanyl-sevoflurane in children undergoing minor surgery under spontaneous breathing with nasopharyngeal airway-face mask versus laryngeal mask airway (LMA).Methods Seventy-two children,scheduled for elective high ligation of indirect inguinal hernia,were randomly divided into 2 groups (n =36 each):nasopharyngeal airway-face mask group (group M) and LMA group (group L).Propofol 1.0 mg/kg and fentanyl 2.0μg/kg were injected intravenously.The spontaneous breathing was kept.After lass of consciousness,a face mask was placed for inhalation of pure oxygen at 2 L/min and 3 % sevoflurane.When BIS value reached 50-60,a size 2 LMA was inserted in group L and a nasopharyngeal airway was placed via the left nasal cavity and the face mask was held tightly and connected to the anesthesia machine in group M.Anesthesia was maintained with inhalation of 2%-4% sevoflurane.Fentanyl 1 μg/kg was injected intravenously 5 min before skin incision.The concentration of sevoflurane was adjusted to maintain BIS value at 40-60 during surgery.Mean arterial pressure,heart rate,partial pressure of end-tidal carbon dioxide and pulse oxygen saturation were recorded before induction,at 3 min during induction,immediately after successful insertion of the airway,at skin incision,3 min after skin incision,and immediately after removal of the airway.The airway insertion time,success rate of insertion at first attempt,emergence time,and adverse reactions were recorded.Results Compared with group L,mean arterial pressure and heart rate were significantly decreased during airway insertion or removal,nasopharyngeal airway insertion time was shortened and the success rate of nasopharyngeal airway insertion at first attempt (100%) was increased,and the agitation score and incidence of obstruction of the upper respiratory tract were decreased in group M (P < 0.05).Conclusion Combined inravenous-inhalational anesthesia with propofol-fentanyl-sevoflurane is easy to establish the airway,can maintain spontaneous breathing,has little influence on circulatory function and reduce complications during recovery from anesthesia in children undergoing minor surgery under spontaneous breathing with nasopharyngeal airway-face mask,and the efficacy is better than that with LMA.
10.Analysis of the correlation between the occurrence of low birth weight full-term infants and the level of blood lipids in the third trimester of pregnancy

Objective:To investigate the relationship between blood lipid level and low birth weight infants in late pregnancy.Methods:Data of 303 low birth weight newborns delivered in ShengJing Hospital affiliated to China Medical University were collected from January 2018 to February 2020, and 303 cases of full-term normal birth weight infants delivered in our hospital during the same period (birth weight 2500 ~ 3999 g) for retrospective study.The general situation of puerpera was analyzed, including age, gestational age, times of pregnancy, times of labor, weight growth during pregnancy, pre pregnancy body mass index, and blood lipid indexes of puerpera within one week before termination of pregnancy, including triglyceride, total cholesterol, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, apolipoprotein A, apolipoprotein B.Results:The normal birth weight group was (31.60±3.90) years old, the weight gain during pregnancy was (16.29±5.31) kg, and the triglyceride levels was(3.21±1.33) mmol/L.The low birth weight group was (30.30±4.65) years old, the weight gain during pregnancy was (14.45±8.08) kg, and the triglyceride level was(2.88±1.37) mmol/L, and there were differences between groups( P<0.01). In the low term birth weight group, birth weight was negatively correlated with maternal age ( r=-0.24, P=0.01), positively correlated with gestational age ( r=0.24, P=0.02), and negatively correlated with pregnancy weight gain ( r=-0.26, P<0.01), and was positively correlated with triglyceride ( r=0.15, P=0.01) and total cholesterol ( r=0.20, P<0.01). Conclusion:The incidence of full-term low birth weight infants is closely related to the blood lipid level, age, gestational age at delivery and the growth of body weight during pregnancy.
