Objective To investigate the change and its clinical significance of serum erythropoietin(EPO) content in the patients with acute cerebral stroke.Methods The serum EPO content in 60 patients with acute cerebral infarction(ACI),45 patients with cerebral hemorrhage(CH) and 40 patients with other nervous system diseases(control group) were detected by radio-immunity means.The patients with acute cerebral stroke were evaluated by National Institute of Health Stroke Scale(NIHSS).Results The serum EPO content in ACI group and CH group [(1.64?0.41)ng/ml,(1.59?0.54)ng/ml] were significant higher than that in the control group [(1.17?0.86)ng/ml](all P
2.Pathogenetic mechanism of microcirculatory disturbance and treatment advances in acute pancreatitis

In recent years,there has been increasing evidence that microcirculatory disturbance,including vasoconstriction,shunting,inad-equate perfusion,increased blood viscosity,and coagulation,is closely associated with the pathogenesis of acute pancreatitis (AP).These processes may be exacerbated by ischaemia-reperfusion injury and the generation of oxygen radicals.The anatomical features of pancreatic microcirculation,the pathophysiological mechanism of pancreatic microcirculation disturbance and related inflammatory mediators,and pro-gress in the treatment of microcirculatory disturbance in AP are reviewed.It is suggested that the pancreatic and systemic microcirculation may play a key role in the development and progression of AP.

Acute pancreatitis happens rapidly and leads to patient's condition changing swiftly.Acute pancreatitis may be complicated by acute lung injury (ALI) and acute respiratory distress syndrome (ARDS),or even multiple organ dysfunction syndrome,and the mortality rate has been high.The mechanism of acute pancreatitis with complications of ALI and ARDS is intricate.It involves the uncontrolled inflammatory response,the damage and apoptosis of cell,the role of trypsin,the imbalance of coagulation and fibrinolysis,etc.These respects interrelate with each other,forming a complex network.Further study of mechanism of acute pancreatitis complicated with ALI and ARDS will supply more new target for clinical diagnosis and treatment.

Transforming growth factor-β (TGF-β) is a multifunctional protein and regulates a wide variety of cellular bio-effects,such as proliferation,differentiation,migration and apoptosis.Studies have proven that TGF-β is one of the important cytokines that promote fibrosis,and it is confirmed to be closely related to the progression of tumor.Smad signaling is the major pathway in which TGF-β fulfills its functions.These years,it has been found that E3 ubiquitin ligases Arkadia can enhance the biological effect of TGF-β signal transduction pathway through Smad signaling pathway.Therefore,it is increasingly attracting public attention.This study will summarize the effects of Arkadia on TGF-β/Smad signal transduction pathway.
5.The clinical characteristics of adult patients with community acquired pneumonia caused by acute Mycoplasma ;pneumoniae infection:a multicentre cross-sectional study

Objective To investigate the clinical characteristics of adult patients with community acquired pneumonia (CAP) caused by acute Mycoplasma pneumoniae (MP) infection, and provide evidence for early identification of MP infection. Methods A prospective, multicenter and cross-sectional study was conducted. 452 adult patients with CAP admitted to Beijing Friendship Hospital, Beijing Guangwai Hospital and Air Force General Hospital from August 2011 to October 2015 were enrolled. The diagnosis of adult MP infection was confirmed by the combined application of double serum antibody titer and MP-DNA nested polymerase chain reaction (PCR) through testing serum and throat swab samples from patients to identify acute infections, past infections, pathogen carrying, and non-MP infection. The clinical characteristics of patients with acute MP infection were summarized by analyzing the baseline data, clinical parameters and chest imaging findings in patients with non-MP infection and acute MP infection. Results Of 452 enrolling patients with CAP, 288 patients (63.7%) suffered from MP infection, and 164 patients (36.3%) with non-MP infection. There were 56 patients (12.4%) with acute infection, 10 patients (2.2%) with past infections, 222 patients (49.1%) with pathogen carriers in MP infective patients indicating susceptible to MP in adult patients. There were no significant differences in gender, age, fever extent, duration of fever, sputum production, shortness of breath, rales, underlying diseases, etc. between non-MP infection and acute MP infection patients, which suggested that the baseline data of the two groups were equilibrium. The acute infection rates of MP in summer and autumn (43.9% and 43.5% respectively) were more than those in spring and winter (13.3% and 12.3% respectively). It was shown by laboratory examination results that serum cardiac troponin T (cTnT) increased significantly in acute MP infectious patients more than that in non-MP infection patients (30.4% vs. 9.8%, P < 0.01), which indicated that patients with acute MP infection were more likely to have myocardial injury. While there were no significant differences in blood routine, blood electrolytes, blood glucose, as well as heart, liver and kidney function between the two groups. It was shown by chest imaging that the diffuse lesions (57.1% vs. 37.2%), mediastinal lymphadenopathy (60.7% vs. 37.8%) were less founded in the middle lobe of the right lung (12.5% vs. 32.9%), which were the main manifestations in patients with acute MP infection as compared with non-MP infection patients with statistical difference (all P < 0.01). There were no significant differences in the chest imaging performances of pulmonary ground glass shadow, lobar and segmental consolidation, patch shadow, a shadow, acinar nodules, grinding glass density nodules, the photic zone, hilar lymphadenopathy and pleural effusion occurrence between the two groups. Conclusion Adult CAP patients are easy to carry MP, myocardial damage is a common complication in acute MP infectious patients which are characteristic of image findings of diffuse lung disease, mediastinal lymphadenopathy and less founded in the middle lobe of the right lung.
6.Effects of Buccal Acupuncture on Expression of Plasma Metabolites Spectrum of Rabbits with Rheumatoid Arthritis

Objective To observe the effects of buccal acupuncture therapy and body acupuncture therapy on plasma metabolomites of rheumatoid arthritis rabbits model; To search for the metabolites related to the mechanism of buccal acupuncture and body acupuncture. Methods Sixteen rabbits were made into rheumatoid arthritis models by complete freund's adjuvant before the treatment. The buccal acupuncture group was treated by buccal acupuncture on the knee acupoint; 13 mm × 0.32 mm needle straight 0.2–0.5 mm, twist the main points of stimulation 15 s, once a day. The body acupuncture group was treated by body acupuncture on the Xiyan and Zusanli acupoints; with 13 mm × 0.32 mm needle straight 2–3 mm, every 5 min each time, twisting needle method, 100 times/min for 15 s, once a day. After the continuous treatment for 5 days, UPLC-QTOF/MS was applied to test metabolite from the plasma sample of all groups. Similarity analysis, hierarchical cluster analysis (HCA), principal component analysis (PCA), orthogonal partial least squares-discriminant analysis (OPLS-DA) were applied in this research; meanwhile load analysis method was used to identify the meaningful metabolites. Results The similarity analysis, HCA and PCA all showed the significant difference of endogenous metabolites in plasma samples among the buccal acupuncture group, the body acupuncture, the model group and the blank group. The 4 groups of different plasma sample metabolite were completely separated in three dimensions by OPLS-DA and all the samples were gathered respectively. The load analysis indicated that there were 19 kinds of endogenous metabolites which showed the significant differences between buccal acupuncture group and model group, and also happened between the body acupuncture group and the model group; N-cetyl-L-phenylalanine and 2-phenyl propionate in metabolism of phenylalanine and tyrosine decreased significantly in the buccal acupuncture group while significantly increased in the body acupuncture group. Conclusion There are relative similarity on the mechanism of treating rheumatoid arthritis between buccal acupuncture and body acupuncture. However, N-acetyl-L-phenylalanine and 2-phenyl propionate, as the main potential biomarkers of the difference, indicate that the dissimilarity can be significant.

AIM: To evaluate the blood compatibility of a new bioartificial reactor membranous material (propylene-acidamide grafted poly propylene membrane, PP-g-AAm) in vitro. 8.Efficacy of pancreaticoduodenectomy by da Vinci robotic surgical system

Objective To investigate the efficacy of pancreaticoduodenectomy by using the da Vinci robotic system.Methods The clinical data of 10 patients who received pancreaticoduodenectomy (PD) by the da Vinci robotic surgical system at the Ruijin Hospital from March 2010 to March 2011 were retrospectively analyzed.The perioperative condition of the patients,incidence of complications and postoperative survival were observed.Results Operations were successfully performed on all the patients,and there was no conversion to open surgery.Nine patients received pancreaticojejunostomy,and 1 received pancreaticogastrostomy.The mean operation time,operative blood loss and postoperative duration of hospital stay were 449.5 minutes (range,405-510 minutes),614 ml (range,340-1100 ml) and 22.7 days (range,14-39 days),respectively.One patient developed pancreatic obstruction which was alleviated by operation.Two patients developed pancreatic fistula and were cured by conservative treatment.One patient developed anastomotic bleeding and was cured by conservative treatment.The results of pathological examination confirmed that 3 patients were diagnosed as with pancreatic ductal adenocarcinoma,2 with duodenal papillary adenocarcinoma,1 with ampullary adenocarcinoma,2 with serous cystadenocarcinoma,1 with carcinoid tumor and 1 with pancreatic duct stone and inflammatory mass.All patients were followed up for 6-12 months.The prognosis of the 9 patients was good,except for 1 patient who had tumor recurrence at the sixth month after operation.No mortality was observed.Conclusion PD performed with the da Vinci robotic surgical system is safe,and its efficacy is comparable to that of open surgery.

A scientific and feasible development plan can only be compiled based on conditions of the hospital, scientific rules and framework.

BACKGROUND: Microcarrier culture technology has become a new and large scale cell culture technology. It has been mainly used in the amplification research of tissue engineering seed cells. Microcarder possesses the advantage of larger surface area and plays an essential role in microcarrier culture technology.OBJECTIVE: To sum up the biomaterials and methods of microcarrier preparation, and provide theoretical foundation for the study of microcarrier culture technology and tissue engineering.METHODS: Articles were retrieved from PubMed, Wanfang, and VIP databases with the key words of "micrecarrier, biomaterials cell culture, tissue englneering" in both English and Chinese between 1967/2009 and 1990/2009, respectively. Inclusion criteria:study addressing microcarrier materials, preparation, and performance; study of microcerrier cell culture; animal experiments and clinical applications. A total of 34 articles were originally retrieved based on their titles and abstracts.RESULTS AND CONCLUSION: Although a lot of studies have reported research and preparation of microcarrier, clinical application remains still difficult. Recently, varying materials will be made into novel compound materials by new technology,which can adjust mechanics and biodegredation of microcarder via surface modification.
