1.Simultaneous determination of seven active ingredients in Xiongju Shangqing pills by HPLC

Objective To establish a method for simultaneous determination of seven active ingredients(Jasminoidin,Forsythoside A,Caffeotannic acid,3,5-Di-O-caffeoylquinic acid,Jatrorrhizine Hydrochloride,Baicalin,Berberine Hydrochloride).Methods The determination was carried out with Agilent Poroshell 120 SB-C18(100 mm ×3.0 mm,2.7 μm);The mobile phase consisted of acetonitrile(A)-0.1% phosphoric acid solution(B)with gradient elution at a flow rate of 1.0 mL/min;The detection wavelengths were 230 nm in 0~4.5 min(determination of Jasminoidin),348 nm in 4.5 ~9.0 min(determination of Forsythoside A,Caffeotannic acid,3,5-Di-O-caffeoylquinic acid),and 278 nm in 9.0 ~15.0 min (determination of Jatrorrhizine Hydrochloride;Baicalin;Berberine Hydrochloride).Results The calibration curves were linear in the ranges of 4.024~80.48μg/mL for Jasminoidin(r=0.9991),2.042~40.84μg/mL for Forsythoside A(r=0.9993),3.956 ~79.12 μg/mL for Caffeotannic acid(r=0.9996),6.090 ~121.80μg/mL for 3,5-Di-O-caffeoylquinic acid(r=0.9990),4.026 ~80.52 μg/mL for Jatrorrhizine Hydrochloride(r=0.9995),3.924~78.48 μg/mL for Baicalin(r=0.9998),6.030~120.6μg/mL for Berberine Hydrochloride(r=0.9998).The average recoveries(n=6)were 96.5%,97.5%, 95.9%,98.6%,99.8%,98.4% and 97.3% ;RSDs were 1.5%,1.3%,1.2%,1.4%,1.2%,1.1% and 0.9% respectively.Conclusion The method is simple and repeatable,which can be applied to the quality control of Xiongju Shangqing pills.
2.Investigation and relationship analysis of glucolipid metabolism and insulin resistance in patients with first-episode non-medicated schizophrenia

Objective:To investigate the status of glucolipid metabolism and insulin resistance in patients with first-episode non-medicated schizophrenia, and to explore their relationship with psychiatric symptoms and cognitive function.Methods:One hundred and seventeen patients with first-episode non-medicated schizophrenia admitted to Wenzhou Seventh People′s Hospital from January 2018 to August 2020 were included in case group, and 61 healthy subjects with physical examination during the same period were used as control group. The glucose metabolism, including serum fasting blood glucose (FBG), 2 h postprandial blood glucose (2 h PBG), fasting insulin (FINS), fasting C peptide; lipid metabolism, including triglyceride (TG), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C); apolipoprotein A1 (Apo-A1) and homeostatic model assessment insulin resistance index (HOMA-IR) level were compared between 2 groups. The abnormal glucolipid metabolism and incidence rate of insulin resistance were counted in the two groups. According to the condition of abnormal glucolipid metabolism or insulin resistance in case group, the patients were divided into abnormal glucolipid metabolism group and non-abnormal group, and insulin resistance group and non-insulin resistance group. The psychiatric symptoms (positive and negative symptom scale, PANSS) and cognitive function (MATRICS consensus cognitive battery, MCCB) were compared between 2 groups. Pearson correlation analysis was used to explore the correlation between glucolipid metabolism and insulin resistance and psychiatric symptoms and cognitive function in case group.Results:The levels of glucolipid metabolism indexes of 2 h PBG, FINS, fasting C peptide, TG and HOMA-IR in case group were significantly higher than those in control group: (7.06 ± 1.88) mmol/L vs. (6.19 ± 1.53) mmol/L, (8.61 ± 2.46) mU/L vs. (6.25 ± 1.71) mU/L, (0.49 ± 0.16) nmol/L vs. (0.32 ± 0.09) nmol/L, (1.33 ± 0.47) mmol/L vs. (1.02 ± 0.24) mmol/L, 2.01 ± 0.71 vs. 1.51 ± 0.45 ( P<0.05); while the levels of HDL-C and Apo-A1 were significantly lower than those in control group: (1.19 ± 0.38) mmol/L vs. (1.57 ± 0.32) mmol/L, (1.21 ± 0.25) g/L vs. (1.43 ± 0.17) g/L ( P<0.05). The total incidence rate of abnormal glucolipid metabolism or insulin resistance in case group was significantly higher than that in control group: 62.39%(73/117) vs. 13.11%(8/61) ( P<0.05). The scores of dimensions of positive symptoms, negative symptoms and general psychopathology and total score of PANSS in combined group were significantly higher than those in non-abnormal group: (25.14 ± 5.09) scores vs. (22.95 ± 4.72) scores, (24.68 ± 5.25) scores vs. (22.05 ± 4.59) scores, (41.52 ± 5.85) scores vs. (38.12 ± 4.18) scores, (94.68 ± 11.64) scores vs. (85.43 ± 8.51) scores ( P<0.05). The above scores points in insulin resistance group were higher than points in non-insulin resistance group: (26.62 ± 4.18) scores vs. (23.62 ± 4.98) scores, (25.92 ± 5.07) scores vs. (23.02 ± 4.96) scores, (42.94 ± 5.26) scores vs. (39.43 ± 4.47) scores, (97.35 ± 10.07) scores vs. (89.37 ± 10.25) scores ( P<0.05). The scores of continuous performance test-identical pairs (CPT-IP), working memory (WM), brief visuospatial memory test-revised (BVMT-R) and Mayer-Salovey-Caruso emotional intelligence test (MSCEIT) of MCCB scale in abnormal glucolipid metabolism group were significantly lower than those in non-abnormal group: (23.82 ± 5.21) scores vs. (27.15 ± 4.69) scores, (21.72 ± 5.95) scores vs. (25.35 ± 5.14) scores, (19.56 ± 5.28) scores vs. (22.34 ± 5.43) scores, (22.62 ± 5.13) scores vs. (26.47 ± 4.96) scores ( P<0.05), and the scores in insulin resistance group were significantly lower: (22.26 ± 4.84) scores vs. (25.42 ± 5.12) scores, (20.35 ± 4.87) scores vs. (23.46 ± 5.08) scores, (18.05 ± 4.27) scores vs. (20.98 ± 5.71) scores, (21.15 ± 4.67) scores vs. (24.48 ± 5.02) scores ( P<0.05). Pearson correlation analysis showed that 2 h PBG in case group was positively correlated with PANSS positive symptoms ( P<0.05), and was negatively correlated with CPT-IP and MSCEIT in MCCB scale ( P<0.05). FINS and HOMA-IR were positively correlated with positive symptoms, negative symptoms and PANSS total score ( P<0.05), and were negatively correlated with CPT-IP, WM, BVMT-R and MSCEIT ( P<0.05). HDL-C was negatively correlated with positive symptoms ( P<0.05), and was positively correlated with CPT-IP, WM and MSCEIT ( P<0.05). Apo-A1 was negatively correlated with positive symptoms and negative symptoms ( P<0.05), and was positively correlated with CPT-IP and WM ( P<0.05). Conclusions:Abnormal glucolipid metabolism and insulin resistance have a higher detection rate in first-episode non-medicated schizophrenia, and have a certain relationship with the psychiatric symptoms and cognitive impairment of patients.
3.The effects of venlafaxine on the extinction of fear memory in rat

ObjectiveTo identify the effects of venlafaxiue on the acquisition and consolidation of fear extinction memory in conditioning fear memory extinction model.MethodsMale Sprague-Dawley rats were treated with fear conditioning training in the A environment.Before the extinction training,all the experimental rats were given different doses of venlafaxine intraperitoneal injection.After 24 hours,all the rats test in the B environment.ResultsRepeated-measures ANOVA were conducted on the percent of freezing time for between-session extinction,test condition (F2,44 =458.958,P<0.001 ) and VEN dose(F2,22 =43.026,P<0.001 ) and a Test condition * Treatment interaction (F4,44 =31.363,P < 0.001 ).For the within-session,post hoc comparisons indicated that the three groups that received different dose of VEN (0,20 and 40 mg/kg) did not differ from each other (P > 0.05 ),indicating similar extinction following the post-conditioning.The rats injected with high-dose venlafaxine (40 mg/kg) intraperitoneally before extinction training showed pro,motion of between-subjects extinction of fear memory,but does not affect the within-subjects extinction.There was no significant catabolism in the rats injected with middle-dose (20 mg/kg).ConclusionThe available date indicate that venlafaxine could promote the extinction of fear memory and there is a dose-dependent relationship of venlafaxine in the facilitation of fear memory.Our results could provide some clinical guidance for the treatment of post-traumatic stress disorder and panic disorder.
4.Pathologic and etiologic studies of membrane obstruction of the inferior vena cava in Budd-Chiari syndrome

Objective To study the anatomical location and pathology of membranous obstruction of the inferior vena cava(IVC)in Budd-Chiari syndrome(BCS)with research on the etiologic mechanism and pathology.Methods Analysis of 100 normal adults was performed including the gross anatomy of IVC segment from the level of diaphragm up to right atrium.The conventional,microscopic pathologic examination of the biopsy sampling IVC obstruetice mambrane tissue in 70 cases toghther with the complete resected membrane from the radical therapy for 20 cases of BCS,were collected and under investigation.Results The macroscopic examination revealed the obstructive membrane in one case(1%)localizing at the diaphragmatic level,approximately 28 mm,away from the IVC entrance into the right atrium and a newly found valvula was seen on the left lateral wall of the upper part of the hepatic vein orifice.Simultaneously,47% adults showed Eustachiun valve existing in IVC near the entrance to right atrium.Microscopy confirmed all the forementioned membranes consisting of vascular valvular structures.Among them(21/70),30% showed additional organized thrombus formations,and 9%(6/70)with a few amount of inflammatory cellular infiltrations.The total intact resection membrane was continuous with the vascular wall under microscopic examination.Conclusions The first newly report of the existence of a special valvula at the diaphragmatic level of IVC reveals the possibility of high correlation with the occurance of IVC membranous obstruction type in BCS.
5.Effects of custom made fiber posts on the fracture resistance of young permanent teeth treated with calcium hydroxide

Objective · To observe the effects of custom made fiber posts on the fracture resistance of young permanent teeth treated with calcium hydroxide for different time. Methods · Forty-five extracted human young permanent teeth (premolar) were selected and randomly assigned to 9 groups.One group was the control group and the rest 8 groups were treated with conventional root canal preparation. Of these 8 groups, 4 groups (F Ⅰ , F Ⅲ , F Ⅵ ,FⅨ ) were restored by custom made fiber posts after being treated with calcium hydroxide for 1, 3, 6, and 9 months respectively, and other 4 groups (C Ⅰ ,CⅢ , C Ⅵ , C Ⅸ ) were not restored by custom made fiber posts. All samples underwent fracture strength tests and their fracture modes were analyzed for the possibility of second repair. Results · The fracture load was decreased with the treatment time after being treated with calcium hydroxide for 1 month (P<0.01). The fracture loads in groups restored by custom made fiber posts were increased after being treated with calcium hydroxide for 6 and 9 months (P<0.05, P<0.01). After being treated with calcium hydroxide for 1 month, groups restored by custom made fiber posts were likely to have fracture modes that facilitate the second repair as compared with groups not restored by custom made fiber posts (P<0.05). Conclusion · The restoration by custom made fiber posts can increase the fracture resistance of young permanent teeth treated with calcium hydroxide for medium and long term ( ≥ 1 month).

Objective To access the development of thoracic volume of pectus excavatum use of Nuss operation used CT-basedpulmonary volumetric evaluation.Methods 98 patients with pectus excavatum in Beijing Children's Hospital under Nuss operation were documented.All patients had CT scan pre-and postopration.The CT-scan's data were calculated as lung's volume by handdraw-layers summation method.The lung's volume result was compared,and enquiry the position in CT lung volume developmental scale.Results The lung's volume was elevated 28.3% after operation.The development of thoracic volume was keep with age,70% was maintained and 15% was elevated.Conclusion Nuss operation can prevent the decrease of thoracic volume in pectus excavatum.
9.Alteration of leukocyte iNOS-mRNA expression in rat with diabetic mellitus induced by streptozocin

AIM: To clarify the relationship between expression of leukocyte iNOS-mRNA and pancreatic islet function in the diabetic rats induced by streptozocin(STZ). METHODS: Wistar rats were randomly divided into the control group ( n =10) and the diabetic group ( n =15). Expression of iNOS-mRNA in the peripheral blood leukocyte, liver and lung were detected with in situ hybridization and the blood sugar and insulin were also measured. RESULTS: It showed that the blood glucose content increased from (8 95?1 80) to (22 84?4 90) mmol?L -1 ,however, the plasma insulin content decreased from (81 76?20 12) to (58 92?18 20) mU?L -1 at the third day after the ? cell was disfunctioned by STZ injection. No expression of leukocyte iNOS-mRNA in normal rat was detected. The percent rate of positive cells were significantly increased in the rats with diabetes. CONCLUSION: The expression of leukocyte iNOS-mRNA is positively related to the damage of ? cells caused by STZ.
10.Advances in clinical assessment and decision-making of intraductal papillary mucinous neoplasm in pancreas

Intraductal papillary mucinous neoplasm (IPMN) is one of the precancerous lesions of the pancreas. Currently there is controversial over the management and follow-up strategy of IPMN, including the timing of surgery. The core problem lies in the accurate preoperative assessment of the nature of the lesions and the risk of malignant transformation. Cumulation of high-quality evidence and development of efficient evaluation methods are vital for the establishment of standardized decision-making system and the improvement of clinical benefits to patients. This review aims to summarize the consensus and controversies on surgical evaluation standards in the latest guidelines and representative literatures, and to look forward to the development direction of IPMN diagnosis and treatment decisions in combination with the progress of related evaluation techniques.
