1.Clinical efficacy and safety of TAPP surgery in inguinal hernia repair for the elderly

Objective To investigate the clinical efficacy and safety of TAPP surgery in inguinal hernia repair for the elderly by different mesh materials.Methods 24 elderly patients (32 cases) with inguinal hernia were randomly divided into the two groups.The control group was repaired by the braid mesh,while the other 16 cases in the observation group were repaired by the three dimension mesh.The operative indicators,the VAS pain score,the incidence rate of complications and recrudesce rate were compared between the two groups.Results The operative time,time length of staying and costs of the two groups were close,no statistical differences were found (all P > 0.05) ; VAS scores of postoperative 24h,48h,72h of the observation group were (0.97 ± 0.76) points,(0.62 ± 0.55) points,(0.49 ± 0.37) points respectively,which were significantly lower than those of the control group (t =6.331,6.031,5.882,all P <0.05).The incidence rate of complication in control group was 16.67% (2/12),no statistical difference was found between two groups (x2 =0.000,P > 0.05).No recurrence was found in two groups.Conclusion The three dimension mesh and the braid mesh in TAPP are safe and effective,the three dimension mesh has better performance in pain.
2.Applying psychological intervention of husband and wife for the center to patients with in vitro fertilization and embryo transfer

Objective To evaluate the application effect of psychological intervention of husband and wife for the center to patients with in vitro fertilization and embryo transfer. Methods Eighty infertile couples with in vitro fertilization and embryo transfer were selected and divided into intervention group and control group with 40 cases in each group according to the document order numbers, the two groups were given conventional health education of in vitro fertilization and embryo transfer, in addition,the patients in the intervention group received psychological intervention of husband and wife for the center, the intervention time was 5 weeks. Questionnaire investigation about Self- rating Depression Scale (SDS), Self- rating Anxiety Scale (SAS),Coping Style Scale and Subjective Feeling of Marriage Scale were conducted before and after the intervention. Results The scores of SDS and SAS were (30.43±8.92), (33.48±9.92) points after the intervention in the intervention group, and (40.26±9.54), (41.51±7.24) points in the control group,and there were significant differences (t=7.611, 6.245, P<0.01).The scores of help seeking, self- accusation, fantasy, wince and rationalization in Coping Style Scale were (0.63±0.19), (0.25±0.21), (0.41±0.22), (0.43±0.24), (0.42±0.21) points after the intervention in the intervention group, and (0.51±0.21), (0.38±0.22), (0.51±0.19), (0.55±0.23), (0.55±0.22) points in the control group, and there were significant differences(t=3.008-3.668, P < 0.05). The scores of subjective feelings of marriage and family relationship were (64.79±9.75), (34.37±5.12) points after inntervention in the intervention group, and (58.09±11.42), (29.23±4.97) points in the control group, and there were significant differences (t=7.212, 3.463, 15.234, P < 0.05). Conclusions Applying psychological intervention of husband and wife for the center to patients with in vitro fertilization and embryo transfer can improve the psychological state, reduce depression and anxiety, adopt positive coping style, increase the quality of marriage.
3.Evaluation on the antiemetic effect of azasetron hydrochloride in patients with liver neoplasm after transcatheter arterial chemoembolization

Purpose: To evaluate the antiemetic effect of azasetron hydrochloride in patients with liver neoplasm after transcatheter arterial chemoembolization. Methods: 62 patients (33 with primary hepatocellular carcinoma and 29 with hepatic metastasis), who underwent transcatheter arterial chemoembolization( TACE) therapy 78 times in all, were allocated into the azasetron group( administration of azasetron hydrochloride) and the control group( administration of metoclopramide) randomly. The inhibitory effects on nausea and vomiting were observed in the two groups respectively. Results: The antiemetic CR ratio and the response rate( CR + PR) in the azasetron group was 81.0% and 99. 7%, respectively. These results were statistically higher than those in the control group( P
4.A study of protein expression of MAGE-A3, MAGE-A4 and MAGE-A10 genes in colorectal carcinoma and its clinical significance

Objective To explore protein expression and significance of MAGE genes in colorectal carcinoma(CRC)tissues.Methods The expression of MAGE genes were studied by using tissue chip and immunochemistry methods in primary CRC tissue and paired adjacent tissue samples in 97 cases.Data were analyzed with x2-test by SPSS 16.0 software.Results The protein expression of MAGE-A3,MAGE-A4 and MAGE-A10 genes were 57%(56/97),63%(61/97)and 28%(27/97)respectively in 97 cases of primary adenocarcinoma.The protein expression frequency of MAGE-A3 in poor colorectal adenocarcinomas was significantly higher than in well-and moderately disfferentiated adenocarcinomas(x2 =9.133,P =0.010).MAGE-A10 in poor colorectal primary adenocarcinomas was significantly higher than in well and moderately adenocarcinomas(x2 =15.280,P =0.000); MAGE-A10 protein expression was significantly higher in stage TNM Ⅲ + Ⅳ than in stage TNM Ⅰ + Ⅱ(x2 =4.227,P=0.040); MAGE-A10 gene expression was higher in metastasis lymphoid node than in no metastasis lymphoid node(x2 =5.557,P =0.018),and the expression level was higher in primary lesion with the increasing of the numbers of lymphoid node metastasis(x2 =7.296,P =0.026).Conclusions The protein expression of MAGE genes is associated with the tumor differentiation,TNM stage and lymphoid node metastasis.MAGE-A3 and MAGE-A10 genes are the possible prognosis marker and potential target of immunotherapy of CRC.
5.Experimental observation of human adipose mesenchymal stem cells transplantation in the treatment of acute necrotic pancreatitis in rats

Objective To observe the effect of human adipose mesenchymal stem cells (hADMSCs) on pancreatic tissue repair and inflammatory reaction of acute necrotic pancreatitis (ANP) in rat, and explore the possible mechanism.Methods Isolation and purification of hADMSCs and flow cytometry to detect the the surface markers including CD90, CD29, CD34 and CD45 were performed.Eighty SD male rats with the body weight of 170~210 g were randomly divided into 4 groups.There were 8 rats in the control group, 24 rats in other group.Control group underwent no treatment;sham operation group underwent intestinal wall stirring and then abdominal closure;ANP model group was established by open abdominal retrograde injection of sodium taurocholate into bile duct;and in hADMSCs group, DAPI labeled hADMSCs were injected by tail vein into the rat at 12 h after sodium taurocholate injection.The survival of the rats, and gross morphological and pathological changes of the pancreas was observed at 12, 24, and 48 h, and the serum TNF-α, IL-6, IL-10 and amylase were detected.The distribution of hADMSCs in the pancreas, liver and lung was examined in hADMSCs group.Results Rats in control group and sham operation group were all alive.In ANP group, 5 and 11 rats were dead at 24 and 48 h, respectively, and in hADMSCs group 12 rats were dead at 48 h.Compared with ANP group, the difference was not statistically significant (P>0.05).The pathological changes of the pancreas were significantly less severe in hADMSCs group than in ANP group.In hADMSCs group, the amylase at 12, 24 and 48 h was(999±110 )、(1 831±110)、(3 991±130 )U/L;TNF-α level was (62.40±2.35), (80.51±4.51) and (93.46±6.60)ng /L;IL-6 was (60.46±7.34), (80.61±8.40) and(100.58±9.49)ng /L;and these were all significantly lower than those in ANP model group [amylase (2 402±146), (3 292±137) and (5 632±112)U/L;TNF-α(87.13±3.39), (105.41±10.06), (114.57±3.06)ng/L;IL-6 (70.67±10.90)、(107.61±10.53)、(145.34±10.48)U/L], and the differences were all statistically significant (all P<0.05).IL-10 in hADMSCs group was (56.63±6.35), (81.32±5.96), (100.26±6.51)ng/L, which were increased compared with those in ANP model group [(45.26±8.04), (68.25±8.42), (80.38±5.71)ng/L], and the difference was statistically significant (all P<0.05).hADMSCs can migrate to the pancreas, liver, lungs and other damaged tissue, with most in pancreatic tissue, less in lung tissue, and least in liver tissue.Conclusions The mechanism of hADMSCs in repairing pancreatic tissue injury was associated with inhibiting TNF-α and IL-6 secreting and increasing IL-10, thus reducing inflammatory reaction.
6.Clinical effect of nanofat on improving superficial rhytides of face and neck and dark lower eyelids

Objective To evaluate the clinical effect of nanofat on superficial rhytides of face and neck and dark lower eyelids.Methods From September 2014 to January 2017,a total of 86 cases were collected in our hospital for voluntary nanofat transplantation on superficial rhytides and dark lower eyelids.This was a retrospective study,which included 18 cases of eye wrinkles,14 cases of forehead wrinkles,neck wrinkles in 14 cases,and 22 cases of dark lower eyelids.First of all,we harvested mircofat through liposuction,and then transformed mircofat to nanofat.At last,nanofat was grafted into intradermal layer of the skin with sharp needles.We took the standard photographs of the patients.After six months follow-up,doctors and patients evaluated the short term and long-term postoperative effect.Results No serious complications occurred in all patients.Postoperative evaluation of facial and neck superficial wrinkles showed that the satisfactory rate of doctors and patients after one month,was 85.9%and 84.3%,and after 6 months 87.5% and 84.3%,respectively.Postoperative evaluation of dark lower eyelids showed that the satisfactory rate of doctors and patients after one month was 45.5% and 36.4%,and after 6 months 81.8% and 86.4%,respectively.Conclusions Nanofat can rectify the superficial rhytides of face and neck and dark lower eyelids in some patients.However,for some patients the effect is not satisfied.
7.Effect of botulinum toxin A on fat breastaugmentation: a preliminary randomized controlled clinical study

Objective:To explore whether botulinum toxin A (BTX-A) can improve the retention rate of fat transplantation in fat breast augmentation.Methods:Each patient was divided into control side and experimental side according to the random number table in 14 patients studied. The experimental group received autologous fat and BTX-A combined transplantation on both sides of the breast, while the control side only received autologous fat transplantation. The fat was added with the same volume of normal saline as BTX-A in the control group. All patients were followed up and the effects of BTX-A were evaluated objectively via the comparison of the remained bilateral fat graft volumes that were obtained through a digital three-dimensional reconstructions technique. Moreover, the improvement of each breast appearance and complication were assessed by the physician and patients who were blinded to the recipient treatment assignment.Results:The outcome of the fat breast augmentation was evident for both groups at the follow-up with no evidence of fat embolism, vascular/nervous injury, infection and prolonged bruising. In one of the 14 patients (control group), fat liquefaction necrosis occurred in one side of the breast; after active treatment, it returned to normal, and three patients had different degrees of mass. The analysis on the three-dimensional reconstruction data and the assessments from both the physicians and patients showed significant differences in the fat graft retention volume between the BTX-A group (51.10±20.56)% and the control group (33.06±14.77)%. Nevertheless, there was no significant difference in the incidence of complications between the two sides.Conclusions:Autogenous fat breast augmentation is safe and effective. This study result has shown that BTX-A can significantly improve the retention rate of fat transplantation but cannot reduce the incidence of complications.
8.Seroepidemiological analysis of hepatitis B among children aged 1-14 in 3 counties of Guangdong province in 2013

Objective To investigate the epidemic status of Hepatitis B in children aged 1-14 in 3 counties of Guangdong province in 2013,and to evaluate the effect of hepatitis control in children aged 1-14 after hepatitis B vaccine was integrated into the national immunization program in 2002 and catch-up vaccination was conducted from 2009 to 2011. Methods A multi-stage stratified random sampling was designed to survey 1 621 children aged 1-14 in rural area of Nanxiong county, Haifeng county and Xinxing county by questionnaires including general information, medical history and risk factors .The samples were tested with chemiluminescence method to detect hepatitis B virus (HBV) surface antigen ( HBsAg), antibody to HbsAg (anti-HBs) and antibody to HBV core antigen (anti-HBc).Chi-square test was used to compare the positive rate of HBV serum markers in different age groups, vaccine histories, birth weight and HBV infection status of mother. Results Among the children aged 1-14 in 3 counties rural regions of Guangdong province, the positive rate of HBsAg,anti-HBs,and anti-HBc was 1.11% (18/1 621), 60.69%(982/1 618) and 1.92% (31/1 617), respectively. The HBsAg positive rate of vaccinated children (0.84%, 13/1 547) was lower than that of unvaccinated children (1/13) or children with unknown vaccination status (6.56%,4/61) (χ2=22.64,P<0.001). The HBsAg positive rate (0.45%,5/1 118) of the children with
birth-dose given within 24 hours was lower than those that of children given beyond 24 hours (2.63%, 61/190) (χ2=10.21 ,P<0.001). The HBsAg positive rate(5/18) of children with birth weight under 2 kilogram was higher than that of children with birth weight above 2 kilogram(0.78%,12/1 548)(χ2=120.8,P<0.001).The HBsAg positive rate of children born to HBsAg-positive mothers (2.80%,3/107) was higher than that of children born to HBsAg-negative mothers (0.21%,1/470)(χ2=8.50,P=0.004). With the age increasing, the coverage and timely birth-dose coverage of Hepatitis B vaccine(HepB) decreased,and the positive rate of anti-HBs gradually decreased . Conclusion After the catch-up vaccination was conducted in unvaccinated children aged 1-14 years from 2009 to 2011, the HBsAg and anti-HBc positive rate decreased, while the anti-HBs positive rate increased significantly.

10.Research and Design of Calibration Device for Medical Test Lung.

In order to solve the problems of quality control and traceability of medical test lung for meeting the calibration conditions of JJF 1234-2018 Calibration Specification for Ventilators, the calibration device and method are researched for compliance and airway resistance of medical test lung in this paper. A calibration device for medical test lung is designed using constant volume active piston technology to simulate human breathing. Through comparison experiment, the deviation between this device and the similar foreign device can be found. The deviation is lower than 0.4% for lung compliance and lower than 0.7% for airway resistance. The calibration of lung compliance and airway resistance can be completed by this device. This device has a clear and complete traceability path to ensure quality control from the source. The calibration of ventilator is improved. This paper provides a reference for related metrology departments and medical institutions to study on quality inspection of respiratory medical instruments.
