1.Tensile mechanical properties of lateral collateral ligaments in animal models of osteoarthritis

BACKGROUND:It is very nscessary for the prevention and treatment of ostaoarthritis(OA)to understand the tensile mechanical properties of lateral collateral ligaments(LCLs)of knee joints with osteoarthritis,while reports on it are still few.OBJECTIVE:To compare the LCL tensile mechanical property indexes of knee joints between normal animals and OA medels established for this purpose,and to provide qualitative and quantitative referential data about effects of OA on the LCL tensile mechanical properties of animal knee joints.DESlGN,TIME AND SETTING:A randomized controlled trial was finished at the Mechanical Expedmental Center of Jilin University from December 20th to 30th in 2008.MATERIALS:A total of 20 male SD rats of 6 months old were divided into a normal control group and a model group,with 10ones in each group.METHOOS:Rats in the model group were copied into OA models.after which test on tensile was performed to each of the ten rats in the two groups at the speed of 5mm/min on Universal Testing Machine,Shimadzu.Data obtained from the test were processed in polynomial though least square method.MAIN OUTCOME MEASURES:Tensile maximum load.maximum displacement,maximum stress,maximum strain and stress-strain curves.RESULTS:The tensile maximum load,maximum stress,maximum displacement and maximum strain in the normal control group and the model group were(12.754±2.795)N,(27.681±5.832)MPa,(2.754±0.707)mm,(11.679±2.373)%and (7.183±1.817)N,(16.162±3.403)MPa,(1.827±0.768)mm,(8.019±2.811)% respectively,from which we could see that each index in the normal control group was greater than that in the model group obviously (P<0.05).CONCLUSION:The tensile maximum load,maximum stress,maximum displacement and maximum strain all decreased after OA,which indicates that osteoarthritis has certain effects on tensile mechanical properties of LCLs of knee joints as well as stress relaxation.
2.An experimental study of the use of autologous periostcum wrapping tendon, grains of cancellous bone and BMP to substitute lunate bone

Objective To explore the feasibility of implantation of a composite of tendon, grains of cancellous bone and bone morphogenic protein (BMP) wrapped in antologous periosteum to substitute lunate bone lunatum in the treatment of Kienbock disease. Methods 42 Japanese big ear rabbits were randomly divided into two groups. In group A periostum wrapping a composite of tendon, grains of cancellous bone and bone morphogenic protein (BMP) was implanted. In group B only tendon wrapped by periostum was used. Both of them were respectively implanted into rabbits' suprapatellar bursa, the physiological environment of which seemed to be similar to that of the carpal joint after extirpating the lunate bone in human being. The composites were removed in 1,2,4,8,12,16 and 24 weeks after the operation. X ray imaging, gross, histological and EM changes at varies intervals were observed. And CT value of the composite was statistically analyzed (Stata 4 0). Results Periosteum wrapped BMP/grains of cancellous bone could induce bone and cartilage formation. In group A, newly formed bone could be found at 2 weeks. Rounded bony body with cortex and bone marrow was found to be formed by the implanted composite at 8 weeks. No degenerative changes were observed up to 24 weeks. In group B, the composite didn't form bony body in 24 weeks. 3.Value of glucocorticoid steroids in the treatment of patients with severe community-acquired pneumonia complicated with septic shock

Objective To discuss the value of glucocorticoid steroids (GCs) in the treatment of patients with severe community-acquired pneumonia (SCAP) complicated with septic shock.Methods A prospectively controlled randomized trial was conducted.Fifty-eight SCAP patients complicated with septic shock admitted to emergency intensive care unit (ICU) of China-Japan Friendship Hospital from May 2014 to February 2016 were enrolled.The patients were randomly divided into conventional treatment group (n =29) and GCs group (n =29).Fluid resuscitation,vasopressors,mechanical ventilation if needed,antibiotics and other general treatment including symptomatic treatment and eliminating phlegm were given to patients in both groups.Beside the treatment mentioned above,80 mg methylprednisolone once a day for 7 days was added to patients in GCs group.The changes in oxygenation index (PaO2/FiO2) and C-reactive protein (CRP) at 1,4,8 days after treatment as well as the imaging improvement in both groups were observed.Discharge or death was set as a cut-off point,the average time of temperature controlling,duration of mechanical ventilation,time of vasopressors usage and 28-day mortality were observed.The incidence of hyperglycemia,infection and hemorrhage of digestive tract were observed.Results There were no statistically differences in gender,age and body mass between the two groups,indicating that baseline data for the two groups were balanced.PaO2/FiO2 after treatment in the two groups was gradually increased,and it was significantly higher at 8 days after treatment in GCs group than that of conventional treatment group [mmHg (1 mmHg =0.133 kPa):426.46 ± 86.97 vs.363.00 ± 83.96,P < 0.05].CRP after treatment in the two groups was gradually decreased,and it was significantly lower at 4 days and 8 days after treatment in GCs group than that of conventional treatment group (mg/L:95.78 ± 47.38 vs.124.72 ± 51.01,57.60 ± 47.44 vs.88.85 ± 48.18,both P < 0.05).Radiographic imaging improved rate at 4 days and 8 days after treatment in GCs group was significantly higher than that of conventional treatment group (55.2% vs.27.6%,75.9% vs.51.7%,both P < 0.05),and average time of temperature controlling (days:3.94 ± 2.39 vs.7.22 ± 3.11),time of vasopressors usage (hours:13.64 ± 6.47 vs.28.34 ± 12.56),and the average hospitalization days (days:28.50 ± 8.61 vs.36.21 ± 15.26)in GCs group were significantly shorter than those of conventional treatment group (all P < 0.01).There was no significant difference in duration of mechanical ventilation between GCs group and conventional treatment group (days:13.39 ± 2.62 vs.16.16 ± 5.85,P > 0.05).28-day mortality of the two groups was 10.3% equally (P > 0.05).No significant differences in the incidences of hyperglycemia (10.3% vs.6.9%),infection (51.7% vs.55.2%) and gastrointestinal bleeding (3.4% vs.0) were found between GCs group and conventional treatment group (all P > 0.05),indicating that glucocorticoid steroids could not increase the common side effects.Conclusion GCs is an important adjuvant treatment of patients with SCAP complicated with septic shock.
4.Biomechanical analysis of two fixation methods in treating spine fracture and dislocation

BACKGROUND:Thoracic or lumbar spine fracture and dislocation mainly treated by internal/external fixation devices with surgical methods.Fixation methods in treating spine fracture and dislocation is an important subject for scholars.OBJECTIVE:To quantitative analyze the treatment of spinal fracture and dislocation using pedicle screw and plate fixation,and to provide mechanical parameters for clinical application.METHODS:Shimadzu electronic universal testing machine was used to simulate L_1 lumbar fracture and dislocation treated by plate fixation and pedicle screw fixations,specimens were underwent flexion,extension,compression,as well as torsion tests,with speed of 5 mm/min.The torsion test was performed on the torsion machine with speed of 0.05 (°) / s.All the experimental data were analyzed by statistical analysis and paired t test.RESULTS AND CONCLUSION:The results demonstrated that the outcomes of compression test had no significant differences between 2 groups (P>0.05).The left and right torsion angle of the pedicle screw fixation group was smaller than that of the plate fixation group (P<0.05),and the flexion and extension displacements was smaller in the pedicle screw fixation group than in the plate fixation group (P<0.05).It suggested that plate fixation is worse than pedicle screw fixation,while pedicle screw fixation is conductive to bone healing,thus,pedicle screw fixation is a better internal fixation device.
5.The relevance between single nucleotide polymorphism in the downstream of Toll-like receptor signal transduction pathway and rheumatoid arthritis susceptible genes

Objective To study the correlation between single nucleotide polymorphism (SNP) of downstream mononucleotide in signal transduction of Toll-like receptors and predisposing genes of rheumatoid arthritis (RA).Methods One hundred and sixty-two RA patients were selected from northern part of Han people in China,and 188 healthy subjects were enrolled as normal controls.Rs7744 of Myd88,rs5030445 of TRAF6,rs11265618 and rs4845626 of IL-6R were analyzed.The data of genotypic frequency and allele genotypic frequency were statistical analyzed by x2 test between the two groups.Results The difference of allele frequency of TRAF6 rs5030445 between the two groups were remarkable (x2=5.716,P<0.05),and so did the IL-6R rs11265618 (x2=5.704,P<0.05).But the difference of genotypic frequency was not statistically significant for locus of Myd88 rs7744,TRAF6 rs5030445 and IL-6R rs11265618 (P>0.05).And the differenceof allele frequency and genotypic frequency between the two groups had no statistical significance in locus of Myd88 rs7744 and IL-6R rs4845626 (P>0.05).Conclusion There is significant correlation between single nucleotide polymorphism and predisposing genes of RA.The G allele of TRAF6 rs5030445 gene locus is a possible predisposing gene for RA.The T allele of IL-6R rs11265618 is a possible predisposing gene of RA.
6.Discussion on ethical issues in experimental rats of fentanyl combined with midazolam in anesthesia

Objective To analyze the mechanism of fentanyl combines midazolam,by comparison with some of the commonly used anesthetic method to determine in the ethical advantages of anesthesia.Methods 45 rats were randomly divided into three groups,Group A:1% anesthetized by intraperitoneal injection of sodium pentobarbital 40 mg/kg; Group B:5 % ketamine 120 mg/kg intraperitoneal injection; Group C:midazolam 5 mg/kg + fentanyl 0.05 mg/kg Last intravenous anesthesia.Observe the effects of anesthesia.Results The rats which are used of fentanyl conbines midazolam anesthesia,the onset of anesthesia time:0.02 ± 0.03 min,maintenance of anesthesia time:89.73 ± 22.59 min,the the narcotic operation time:1.51 ± 0.30 min,the success of anesthesia cases:15 cases.Conclusions Fentanyl combines midazolam compared with the currently used method of anesthesia,the advantages of the more prominent,it has significant improvement and protection about the security,utilization and recovery ethics in experimental rats.
7.Effect of Xuebijing injection(血必净注射液) on level of serum endotoxin and specific immunological function of spleen in rats with sepsis

8.Clinical value of serum procalcitonin in diagnosis of sepsis caused by gram negative bacterial infection

Objective To investigate the correlation between serum procalcitonin (PCT) levels and infection sites,as well as between PCT and bacterial species in gram negative (G-) bacteria induced sepsis,so as to provide rationale for therapeutic strategy of using antibiotic in sepsis.Methods The data of patients with sepsis admitted in Emergency Department and ICU from January 2014 to June 2015 were retrospectively analyzed.The blood culture of G-bacteria and PCT detection were carried out simultaneously within 24 hours after admission.The clinical data was analyzed to find out the correlation between PCT levels and infection sites,as well as between PCT levels and pathogenic bacterial species.Results A total of 187 specimens (came from 162 patients) were enrolled in the study with a median age of 70 years old and a median sequential organ failure assessment (SOFA) score of 4.PCT levels were found to be associated with bacterial species.PCT level caused by Escherichia coli bacteremia infection was higher than that caused by Acinetobacter baumannii bacteremia and Burkholderia cepacia bacteremia infection (4.62 ng/mL vs.2.44 ng/mL;4.62 ng/mL vs.0.81 ng/mL;P < 0.05).Receiver operating characteristic (ROC) analysis showed an area under the curve (AUC) for PCT was 0.61 to discriminate Escherichia coli infection from Acinetobacter baumannii infection and an AUC was 0.66 to discriminate Escherichia coli infection from Burkholderia cepacia infection.When the cutoff point of PCT was 30.32 ng/mL,it could predict Escherichia coli infection rather than Acinetobacter baumannii infection with 94.10% specificity,90.00% positive predictive value and positive likelihood ratio for 4.24.When the cutoff point of PCT was 8.01 ng/mL,it could predict Escherichia coli infection rather than Burkholderia cepacia infection with 85.70% specificity,93.94% positive predictive value,and positive likelihood ratio for 3.01.When PCT cutoff value reached 47.31 ng/mL,the specificity and positive predictive value were both 100.00%.PCT level caused by urinary tract infection was higher than that caused by pulmonary infection (11.58 ng/mL vs.2.07 ng/mL,P < 0.05),and the AUC was 0.69.When the cutoff point of PCT was 32.11 ng/mL,it could predict Escherichia coli infection rather than Acinetobacter baumannii infection with 90.60% specificity,86.18% negative predictive value and positive likelihood ratio for 3.68.Conclusions PCT elevation in G-bacteria induced sepsis might be associated with infection sites and bacterial species.
9.Review of prevention and treatment on influenza A (H1N1) with traditional Chinese medicine.

The influenza A (H1N1) broke out in Mexico and the USA in March 2009, which soon spread over the world in coming months. This paper comprehensively analyzed the work of prevention and treatment to Influenza A (H1N1) by using traditional Chinese medicine in recent years, and briefly introduced the research of Influenza A (H1N1), specially summarized the possibility of using theory of TCM for predicting epidemic disease and the relationship between traditional Chinese medicine and influenza A (H1N1). Meanwhile some suggestions are made on the future work of prevention and treatment to Influenza A (H1N1) by using traditional Chinese medicine.

Clinical trial is the most direct and the most effective way to test the performance of medical equipments.The Belmont Report is the statement of the basic moral principles and guidelines,used to help solve the moral problems produced in scientific researches involving human subjects.This apaper examines the ethical issues in clinical trials for medical equipments from the perspective of the Belmont report.It first introduces the background and basic concepts of Belmont report,background concept and the concept and the basic process of medical equipment clinical trials.Then the three principles of the Belmont Report is presented and how to apply the Report to solve ethical problems arising in medical equipment trials.Application of informed consent risk and benefits assessment,and selection of subjects participants and the distribution of research results to address the ethical issues arising in clinical trials.
