1.Clinical Observation of Recombinant Human Endostatin Combined with CT-guided Percutaneous Micro-wave Ablation in the Treatment of NSCLC Complicated with COPD

OBJECTIVE:To observe clinical efficacy and safety of recombinant human endostatin(rh-endostatin)com-bined with CT-guided percutaneous microwave ablation in the treatment of non-small cell lung cancer(NSCLC)complicat-ed with chronic obstructive pulmonary disease (COPD). METHODS:A total of 80 cases of NSCLC complicated with COPD were selected from our hospital during Feb. 2014-Feb. 2016,and then divided into control group and observation group according to random number table,with 40 cases in each group. Control group was treated by CT-guided percutane-ous microwave ablation. Observation group was additionally given rh-endostatin injection 7.5 mg/m2,once a day,d1-14, added into 500 mL 0.9% sodium chloride injection,ivgtt lasting for 4 h,for consecutive 14 d,on the basis of control group;7 d later,next course was performed. A treatment course lasted for 21 d,and they received 4 courses of treat-ment. Survival time,clinical efficacy as well as KPS score and lung function indexes before and after treatment,the oc-currence of ADR were compared between 2 groups. RESULTS:Median survival time of observation group(19.8 months) was significantly longer than that of control group(15.2 months),and total response rate of observation group(72.5%) was significantly higher than that of control group (55.0%),with statistical significance (P<0.05). Before treatment, there was no statistical significance in KPS score,FEV1,FEV1/FVC or DLCO between 2 groups(P>0.05). After treat-ment,KPS score and above lung function indexes levels of 2 groups were increased significantly,and those of observa-tion group were significantly higher than those of control group,with statistical significance (P<0.05). No grade Ⅳwas found in 2 groups. There was no statistical significance in the cases of ADR at different levels between 2 groups (P>0.05). CONCLUSIONS:rh-endostatin combined with CT-guided percutaneous microwave ablation in the treatment of NSCLC complicated with COPD show good clinical efficacy with less ADR,and can significantly improve lung function and quality of life.
2.Effect of Modified Bazhentang on Nutritional Status and Immune Function of Gastric Cancer Patients with Neoadjuvant Chemotherapy

Objective:To observe the effect of modified Bazhentang on the nutritional status and immune function of patients with Qi and blood deficiency syndrome in neoadjuvant chemotherapy (NAC) for gastric cancer. Method:One hundred and ten patients were randomly divided into observation group and control group with 55 cases each. Both groups accepted FOLFOX6 protocol. Patients in control group took Jianpi Shengxue tablets orally, 3 tablets/time, 3 times/day. Patients in observation group received modified Bazhentang, 1 dose/day. The course of treatment was six weeks in both groups. Before and after treatment, scores were graded according to patient generated-subjective global assessment (PG-SGA), Qi and blood deficiency syndrome, and the Revised Piper Fatigue Scale (PFS-R). Levels of serum total protein (TB), albumin (ALB), prealbumin (PAB), CD4+, CD8+, helper T lymphocyte 17 (Th17), regulatory T cell (Treg), immunoglobulin G (IgG), IgM, and IgA were detected before and after therapy. Objective:
To investigate the effect of transcatheter arterial chemoembolization(TACE) combined with three dimensional conformal radiotherapy(3-DCRT) and hyperthermia on inoperable primary hepatic carcinoma.
Methods:
This study retrospectively analyzed 96 patients with hepatic carcinoma in the Third Hospital of Chengde between 2014 and 2017 who received TACE or combined therapy (TACE combined with 3-DCRT and hyperthermia). Kaplan-Meier curve was used to compare the overall survival(OS) of the two groups.
Results:
There were 50 cases in the combined therapy group, 46 cases in the TACE group.The effective rate of the combined therapy group was 76.1%, and that of TACE group was 45.6%, the difference was statistically significant(
4.Correlation between the trigiuceride-glucose index and early neurological deterioration in patients with mild ischemic stroke

To investigate the correlation between the trigiuceride-glucose index and early neurological deterioration(END) in patients with mild ischemic stroke(MIS). Methods This study was retrospective. Patients with acute MIS who were admitted to the Department of Neurology of the First Central Hospital of Baoding from January 2019 to December 2020 for the first time and within 72 h of onset were consecutively included. The National Institute of Health Stroke Scale(NIHSS) scores were recorded at the time of admission,and fasting venous blood was drawn in the early morning of the following day to determine routine blood,liver function,renal function,lipids and other relevant blood indices,and the trigiuceride-glucose index was calculated ln[TG(mg/dl)×FBG(mg/dl)]/2.The patients were divided into END and non-END groups according to their neurological deterioration. Multi-factor logistic regression was used to analyze the factors associated with the occurrence of END in acute MIS patients,and the subject operating characteristic curve(ROC) was used to evaluate the predictive effect of the trigiuceride-glucose index on END in acute MIS patients. Results A final total of 149 patients with acute MIS were included,39 in the END group and 110 in the non-END group. Multi-factor logistic regression analysis showed that neutrophil ratio(OR 1.054,95%CI 1.009~1.101,P=0.017)、TBIL(OR 0.718,95%CI 0.611~0.845,P<0.001)and trigiuceride-glucose index(OR 5.072,95%CI 1.645~15.640,P=0.005) were independent risk factors for the development of END in patients with acute MIS. The ROC curve analysis showed that the diagnostic cut-off value of trigiuceride-glucose index for END was 1.74 with a sensitivity of 53.8% and specificity of 81.8%. Conclusion Elevated trigiuceride-glucose index is one of the risk factors for the development of END in patients with acute MIS and has a predictive role for the development of END in patients with acute MIS.
5.Relationship between opposite acupoints and acupoint effect from acupoint effect of Sanyinjiao (SP 6)-Xuanzhong (GB 39).

The acupoint effect of Sanyinjiao (SP 6) and Xuanzhong (GB 39) as well as the internal relation of Sanyinjiao (SP 6)-Xuanzhong (GB 39) were analyzed in this paper to explore the relationship between opposite acupoints and acupoint effect. It was found both Sanyinjiao (SP 6) and Xuanzhong (GB 39) had acupoint effects, and the two acupoints had close relationship in acupoint effects (specificity along meridian and specificity of acupoints), acupoint-meridian relationship (anatomical locations, meridians and organs, indications). It is indicated the opposite acupoint of Sanyinjiao (SP 6)-Xuanzhong (GB 39) had related aspects in acupoint effects, which is related with acupoint effect through specificity and relationship of acupoint-meridian.
6. Effect of TGF-β1 on epithelial mesenchymal transformation and invasion of choriocarcinoma cells

Objective To explore the role of TGF-β1 on epithelial mesenchymal transformation and invasion by promoting cancer stem cell marker CD133 expression in choriocarcinoma cells. Method Choriocarcinoma cells JEG-3 were cultured in vitro and incubated with TGF-β1 at different time and concentration, and the expression of CD133 protein and mRNA of EMT markers were detected by Western blotting and PCR respectively. The effect of TGF-β1 on invasive ability of JEG-3 cells were assessed with transwell method. Results TGF-β1 promoted the expression of cancer stem cell marker CD133, downregulated the epithelial marker E-cadherin, upregulated mesenchymal marker N-cadherin, and promoted invasion ability in choriocarcinoma cells. Conclusion TGF-β1 could promote stem cell property of cancer, EMT property, and invasive property of choriocarcinoma cells.
7.Establishment of cisplatin-resistant melanoma cell line with stable expression of T-cadherin and its biological characteristics.

To establish cisplatin (CDDP)-resistant melanoma B16F10 (CDDP-R B16F10) cell line with stable expression of T-cadherin, and to study its biological characteristics.
Methods: CDDP-R B16F10 cell line was exposure to high and gradually increased dose of CDDP. 3-(4.5-Dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide (MTT) was used to test the proliferation of CDDP-R B16F10 cell line, and the sensitivity of CDDP-R B16F10 cell line to CDDP and paclitaxel was examined. The pEGFP-N1-T-cadherin, a plasmid vector encoding human T-cadherin, was generated by inserting T-cadherin cDNA into a pEGFP-N1 vector. The pEGFP-N1-T-cadherin was transfected into CDDP-R B16F10 cell line. The expression of T-cadherin mRNA and protein were measured by reverse transcription polymerase chain reaction (RT-PCR), Western blotting and immunohistochemistry SP method, respectively. The effect of T-cadherin combined with CDDP on proliferation of CDDP-R B16F10 cell line was determined by MTT assay. The sensitivity of CDDP-R B16F10 cell line with stably transfected T-cadherin to paclitaxel was examined by MTT assay.
Results: The CDDP-R B16F10 cell line was established successfully. There was no difference in proliferation between the CDDP-R B16F10 cell line and B16F10 cell line (P>0.05). The IC50 of CDDP-R B16F10 cell line and B16F10 cell line to CDDP were 268.706 and 19.748 mg/L, respectively, and the resistance index was 13.61. The IC50 of CDDP-R B16F10 cell line and B16F10 cells to paclitaxel were 11.415 and 7.799 mg/L, respectively, and the resistance index was 1.46. The expression vector pEGFP-N1-T-cadherin was constructed successfully. RT-PCR, Western blotting and immunohistochemistry SP method showed that T-cadherin could be transcribed and expressed. MTT assay showed that T-cadherin combined with CDDP could inhibit the proliferation of CDDP-R B16F10 cell line (P<0.05). Factorial analysis showed that there was interaction between T-cadherin and CDDP in inhibiting the proliferation of CDDP-R B16F10 cell line (P<0.05). T-cadherin combined with paclitaxel could inhibit the proliferation of CDDP-R B16F10 cell line (P<0.05). Factorial analysis showed that there was no interaction between T-cadherin and paclitaxel in inhibiting the proliferation of CDDP-R B16F10 cell line (P>0.05).
Conclusion: The CDDP-R B16F10 cell line with stable expression of T-cadherin is established successfully. T-cadherin can reverse the CDDP resistance to CDDP-R B16F10 cell line, and promote its sensitivity to paclitaxel.

8.Sufentanil promotes autophagy and improves ischemia -reperfusion -induced acute kidney injury via up -regulating microRNA -145.

OBJECTIVES:
Sufentanil has a good protective effect on myocardial and liver injury caused by ischemia reperfusion (IR), but its protective effect on kidney is still unclear. This study aims to investigate whether sufentanil can prevent IR-induced acute kidney injury (AKI) and to determine whether its efficacy is related to miR-145-mediated autophagy.
METHODS:
A total of 40 rats were randomly divided into 5 groups (n=8 in each group): A sham group, an IR group, a sufentanil group, a sufentanil+miR-145 inhibitor control group (an anti-NC group) and a sufentanil+miR-145 inhibitor group (an anti-miR-145 group). Except for the sham group, the other groups established a rat AKI model induced by IR. The sufentanil group, the sufentanil+anti-NC group, and the sufentanil+anti-miR-145 were injected with sufentanil (1 μg/kg) through femoral vein 30 min before ischemia. The sufentanil+anti-NC group and the sufentanil+anti-miR-145 group were injected with miR-145 inhibitor control or anti-miR-145 (80 mg/kg) through the tail vein before sufentanil pretreatment. The structure and function of kidneys harvested from the rats were evaluated, and the protein levels of autophagy-related proteins, oxidative stress levels, and apoptosis levels were measured.
RESULTS:
Compared with the IR group, the renal structure and function were improved in the sufentanil group. The levels of blood urea nitrogen (BUN), creatinine (Cr), urinary kidney injury molecule 1 (KIM-1), neutrophil gelatinase related lipid transporter (NGAL), tumor necrosis factor-α (TNF-α), interleukin (IL)-1β, IL-6 and ROS were significantly decreased (all P<0.05). In addition, compared with the IR group, the levels of Beclin-1 and LC3 in renal tissues in the sufentanil group were significantly increased (both P<0.05), and the apoptosis in renal tissues was significantly reduced (P<0.05). Compared with the sufentanil+anti-NC group, the levels of BUN, Cr, KIM-1, NGAL, TNF-α, IL-1β, IL-6 and ROS in the sufentanil+anti-miR-145 group were significantly increased (all P<0.05), the levels of Beclin-1 and LC3 in renal tissues were significantly decreased (both P<0.05), and the apoptosis in renal tissues was significantly increased (P<0.05).
CONCLUSIONS
Sufentanil can prevent the AKI induced by IR, which is related to the up-regulation of miR-145-mediated autophagy.

9.Hemorheology and oxidative stress in rats with asymptomatic hyperuricemia

Objective To launch systematic research on long-term asymptomatic hyperuricemia (HUA) from hemorheological viewpoint, so as to provide references for clinical treatment of asymptomatic HUA. Methods Twenty rats were randomly and evenly divided into normal control group and model group. The rats were intraperitoneally injected with 250 mg/(kg•d) oxonate for 8 weeks to induce the model of asymptomatic HUA. The blood samples were obtained to measure the serum uric acid, hemorheological parameters, oxidative and anti-oxidative indices. Results The aggregation index, haemolysis rate, serum xanthine oxidase (XOD), plasma fibrinogen and blood viscosity significantly increased, while the orientation index, electrophoresis rate, serum superoxide dismutase (SOD), activated partial thromboplastin time (APTT) and prothrombin time (PT) significantly induced. Conclusions The asymptomatic HUA can lead to more serious oxidative stress, deteriorate the hemorheological parameters of red blood cells in rats, and induce higher blood viscosity and coagulation status. The research findings indicate that asymptomatic HUA should be correctly understood and timely intervened in clinical diagnosis.
10.Changes in macular retinal thickness detected by spectral-domain optical coherence tomography after Nd:YAG laser peripheral iridotomy

AIM: To investigate the effect of Nd:YAG laser peripheral iridotomy(LPI)on macular retinal thickness in patients with primary angle-closure glaucoma(PACG).METHODS: Prospective study. A total of 30 patients(30 eyes)with PACG in our hospital from October 2019 to October 2021 were selected as the PACG group, and 20 healthy people(20 eyes)were selected as the control group(randomly selected one eye)during the same period. The PACG group received LPI treatment and was followed up for 1mo after surgery. Best corrected visual acuity(BCVA), intraocular pressure, corneal endothelial cells, anterior chamber depth(ACD)and axial length(AL)measured by IOL Master 500 optical measuring instrument and macular retinal thickness measured by spectral-domain optical coherence tomography(SD-OCT)in both groups were collected. The ACD, intraocular pressure, AL and macular retinal thickness were compared between the two groups and the PACG group before and after LPI, and the correlation was analyzed.RESULTS: The ACD in the PACG group at 1wk and 1mo after surgery was deeper than that before surgery(all P<0.05). There was no significant difference in intraocular pressure and AL in the PACG group before and after surgery(all P>0.05). The retinal thickness at central fovea of macula, the superior side, temporal side, inferior side, and nasal side of the inner and outer central macular rings were 243.50±13.24, 324.50±13.46, 308.83±15.94, 310.00±14.24, 314.50±16.29, 300.67±19.95, 290.17±12.58, 302.40±16.37 and 307.33±14.84μm in the PACG group, respectively, and were 266.14±16.16, 342.67±15.86, 327.95±16.41, 337.85±13.03, 341.24±15.58, 313.76±17.59, 290.24±16.29, 303.81±13.91, 323.01±14.80μm in the control group, respectively. The differences at central fovea of macula, the superior side, temporal side, inferior side, and nasal side of the inner central macular rings were statistically significant between the two groups(all P<0.05). In the PACG group, the retinal thickness at 9 areas of macular in 1wk after operation were higher than those before operation, and they were close to the preoperative level at 1mo after operation, but the differences of each area in the overall comparison were not statistically significant(all P>0.05).CONCLUSIONS: The macular retinal thickness of patients with PACG is thinner than that of normal people, and it can become thicker in the early stage after LPI.
