1. Determination of three saponins in Compound Shiwei Tablets and study on transfer rate in its preparation process

Objective: High performance liquid chromatography-evaporative light scattering detector (HPLC-ELSD) method was used to establish the determination method for the three kinds of saponins (astragaloside I, II, and IV) in Compound Shiwei Tablets (CST), and investigate the three saponins components’ s transfer rate in the preparation process of CST in order to improve the quality control method of CST. Methods: A HPLC-ELSD method was operated on the column of Agilent 5-HC C18 (2) (250 mm × 4.6 mm, 5 μm), with acetonitrile-water as the mobile phase for gradient elution, at a flow rate of 1.0 mL/min, with column temperature of 30 ℃ and injection volume of 20 μL. The ELSD parameters were as follow: the carrier gas flow rate was 1.5 L/min, the drift tube temperature was 90 ℃. Determinate the content of astragaloside I, II, and IV in products, granules and extracts of CST, and calculate the transfer rate of three saponins in the preparation process of CST. Results: A method for the determination of astragaloside I, II, and IV in CST was established. Under this condition, all three components reached baseline separation with good linear relationship. The average recovery rates were 99.58%, 99.31% and 99.51%, and RSD values were 3.0%, 2.5% and 2.5%, respectively. Astragaloside I had lower transfer rate during the preparation process, and the transfer rate of astragaloside IV was the higher in the preparation process, both of which were greater than 100%. Conclusion: This study established a method for simultaneous determination of three kinds of saponins of astragaloside I, II, and IV in CST. The method has good reproducibility and strong specificity, which is simple and easy,and can be used to inspect the transfer rates of three kinds of saponins in the preparation process and improve the quality control standard of saponins in CST, and provide reference for the quality control of other traditional Chinese medicine preparations containing astragalus.
2. Establishment of HPLC fingerprint of Compound Shiwei Tablets and application of its preparation process

Objective To establish an HPLC fingerprint determination method of Compound Shiwei Tablets (CST), and apply it to the quality control during its preparation. Methods Agilent 5 TC C18 (2) (250 mm × 4.6 mm, 5 μm) column was used for HPLC with a gradient elution of acetonitrile-0.2% phosphoric acid in mobile phase at a flow rate of 1.0 mL/min. The wavelength was 235 nm, the column temperature was 25 ℃, and the injection volume was 10 μL. A total of 14 batches of CST and three batches of CST intermediates (extracts and granules) were measured. Results A common pattern of HPLC fingerprints of CST was established, and 18 common peaks were demarcated and five of them were identified. The relative peak retention time RSD of each batch was < 0.3%, and the similarity between batches was > 0.990. The similarity between the same batch of CST intermediate and its preparation was > 0.990, and the correlation was good. Conclusion The established HPLC fingerprint of CST is simple, effective, and reproducible, and can be applied to the quality control of intermediates in the preparation process of the preparations, which provides a basis for the overall quality control of CST.
3.Influencing factors of anxiety and nursing intervention among college first-time blood donors

【Objective】 To investigate the factors influencing the anxiety of college first-time blood donors and the effect of nursing intervention. 【Methods】 A total of 168 college students who participated in voluntary blood donation for the first time in a university in Chengde from April to May 2020 were scored using Hamilton Anxiety Scale (HAMA) to analyze the influencing factors of anxiety among first-time college donors.They were randomly divided into control group (just routine blood collection) and observation group (nursing intervention was performed on first-time blood donors before, during and after blood collection) to compare the HAMA scores and incidence of adverse reactions to blood donation between the two groups before and after nursing intervention. 【Results】 Among the 168 college students, the incidence of anxiety was 61.9%(104/168), and the influencing factors included gender, knowledge of blood donation and blood donation knowledge received before blood donation.After nursing intervention, HAMA score of the observation group was lower than that of the control group (P<0.05). The incidence of adverse reactions was 15.48% (13/84)in the control group, higher than 8.33%(7/84)in the observation group (P<0.05). 【Conclusion】 For first-time blood donation, college donors female, lack of blood donation knowledge, with few health knowledge before blood donation were prone to anxiety in blood donation.Effective nursing intervention during blood donation can help alleviate anxiety and reduce the incidence of adverse reactions to blood donation.
4.Effects of Gualou Xiebai Banxia decoction on blood lipid content, oxidative stress and ox-LDL/Lox-1 pathway in ApoE-/- mice.

To observe the functions of Gualou Xiebai Banxia decoction(GXBD) on regulating lipid metabolism, anti-oxidation, and interposing ox-LDL/Lox-1 pathway, and to explore its anti-atherosclerosis (AS) mechanisms. AS models were established by using 42 Apo-E-/- male mice with high fat diet. AS model mice were randomly divided into the model group, simvastatin group, and GXBD high and low dose groups. C57BL/6J male mice were used as the normal control group, n=10 and the treatment lasted for 8 weeks. The levels of TC, TG, LDL-C, HDL-C, SOD, MDA, GSH-px, and ox-LDL in blood serum were tested 24 h after the last administration. The changes of aortic tissues structure were observed by HE staining; the expression levels of Lox-1 protein and the expression levels of mRNA were detected by Western blot and PCR respectively.Results showed that the blood lipid levels and MDA, ox-LDL levels in blood serum of model group were significantly higher than those in the normal control group, but SOD, GSH-px levels were significantly lower than those in the normal control group, and the Lox-1 protein and mRNA expression levels were also significantly higher than those in the control group(P<0.05), namely aortic atherosclerosis lesions were obvious in model group.The levels of blood lipid and MDA, ox-LDL of GXBD high and low dose groups and simvastatin group were significantly lower than those in model group, while SOD, GSH-px levels were significantly higher than those in model group, and Lox-1 protein and mRNA expression levels were significantly lower than those in model group(P<0.05), namely the aortic atherosclerosis lesions were significantly relieved. The above results indicated that GXBD was capable of modulating blood lipid, anti-oxidation, and inhibiting the expression of Lox-1, and interposing ox-LDL/Lox-1 pathway in the AS model Apo-E-/- mice, which may be one of the mechanisms of anti-atherosclerosis.
5.Effect of Modified Bazhentang on Nutritional Status and Immune Function of Gastric Cancer Patients with Neoadjuvant Chemotherapy

Objective:To observe the effect of modified Bazhentang on the nutritional status and immune function of patients with Qi and blood deficiency syndrome in neoadjuvant chemotherapy (NAC) for gastric cancer. Method:One hundred and ten patients were randomly divided into observation group and control group with 55 cases each. Both groups accepted FOLFOX6 protocol. Patients in control group took Jianpi Shengxue tablets orally, 3 tablets/time, 3 times/day. Patients in observation group received modified Bazhentang, 1 dose/day. The course of treatment was six weeks in both groups. Before and after treatment, scores were graded according to patient generated-subjective global assessment (PG-SGA), Qi and blood deficiency syndrome, and the Revised Piper Fatigue Scale (PFS-R). Levels of serum total protein (TB), albumin (ALB), prealbumin (PAB), CD4+, CD8+, helper T lymphocyte 17 (Th17), regulatory T cell (Treg), immunoglobulin G (IgG), IgM, and IgA were detected before and after therapy. Body mass index (BMI) and fat free mass index (FFMI) were measured before and after treatment. Weight loss was recorded, and the acute or subacute toxicity of anticancer drugs was evaluated. 6.Effects of Antihypertensive Protein from Pheretima on Angiotensin Ⅱ and Angiotensin Ⅱ AT_1 Receptor in Spontaneously Hypertensive Rats

OBJECTIVE:To investigate the effects of the antihypertensive protein from Pheretima on blood pressure, angiotensinⅡ and angiotensin Ⅱ AT1 receptor in spontaneously hypertensive rats (SHR).METHODS: The dynamic change of blood pressure in SHR after singel intravenous injection of antihypertensive protein from Pheretima was observed. After intervention with antihypertensive protein from earthworm for 28 d,the levels of blood pressure, angiotensinⅡ and expression of angiotensin Ⅱ AT1 receptor in SHR were detected. RESULTS: Either single intravenous injecion or multiple dosing of the antihypertensive protein from Pheretima could significantly reduce the blood pressure in SHR (P0.05).The expression of angiotensin Ⅱ AT1 receptor in kidney of SHR model increased significanlty compared the normal control (wistar rats), but decreased after the intervention of the antihypertensive protein from Pheretima.CONCLUSION: The antihypertensive protein from Pheretima has antihypertensive effect on SHR. The mechanism may be related to the reduction of angiotensinⅡ level and lowering of the expression of a angiotensin Ⅱ AT1 receptor in kidney.
7. Determination of atractylodin, atractylon, and β-eudesmol and study on characteristic spectrum of Atractylodis Rhizoma

Objective: To study the content determination of atractylodin, atractylon, and β-eudesmol in Atractylodis Rhizoma by GC and characteristic spectrum in order to provide a scientific basis for the quality control. Methods: Using GC and Agilent HP-5 capillary column, taking nitrogen as carrier gas, FID as detector, temperature programming, split ratio, injection port temperature: 250℃, detector temperature: 250℃, column temperature: 130℃; The contents of atractylodin, atractylon and β-eudesmol of 25 samples between Chengde and purchased from other markets were determined by external standard method. The characteristic spectrum was set up and the similarity was analyzed by Estimating System of Similarity on the Chinese Medicine Fingerprint Chromatogram. Results: The determination method and characteristic spectrum by GC for atractylodin, atractylon, and β-eudesmol in Atractylodis Rhizoma were established. Nine characteristic peaks were identified; The linear range of β-eudesmol was 20.00 - 406.10 μg/mL (r = 0.999 9), and the average recovery was 100.75%, RSD = 1.17% (n = 6), and the limit of detection was 0.12 ng. The linear range of atractylon was 35.00 - 348.70 μg/mL (r = 0.999 5), and the average recovery was 99.84%, RSD = 1.29% (n = 6), and the limit of detection was 0.04 ng; The linear range of atractylodin was 16.46 - 329.30 μg/mL (r = 0.999 6), and the average recovery was 100.12%, RSD = 0.88% (n = 6), and the limit of detection was 0.06 ng. Conclusion: The concent determination and characteristic spectrum method of atractylodin, atractylon, and β-eudesmol established by this study are sensitive, simple, stability, which could make the determination result accurate and reliable.
8. Effect of Jiawei Changfengyin with Retention Enema Therapy on Th1/Th2 in Patients with Acute Radiation Proctitis

Objective: To discuss the efficacy of Jiawei Changfengyin with retention enema therapy for acute radiation proctitis and investigate its influence on regulating levels of helper T cell(Th)1/Th2. Method: One hundred and twenty-eight patients were randomly divided into control group and observation group by random number table. Patients in control group (64 cases) got montmorillonite powder (3.0 g), dexamethasone (10 mg) and normal saline (100 mL), with retention enema therapy. Patients in observation group (64 cases) got Jiawei Changfengyin(Changfengyin+Xileisan+pearl powder), with retention enema therapy. Both groups of patients received enema once every night, and the treatment course was 4 weeks in both groups. Before and after treatment, scores of main symptoms, proctoscopy, routine examination of stool+occult blood, and KPS scores of quality of life were graded. Levels of Th1 cell factors[interleukin(IL)-1β, IL-8 and interferon-γ(IFN-γ)], Th2 cell factors (IL-4 and IL-10) were detected. Result: The total effective rate for comprehensive clinical efficacy was 95.31% in observation group, higher than 82.81% in control group (χ2=5.132, P<0.05). Scores of each symptom and the total scores in observation group were lower than those in control group (P<0.01). Degree of rectal mucosa injury in observation group was lighter than that in control group (Z=2.764, P<0.05). Score of rectal mucosal injury was lower than that in control group (P<0.01), but score of KSP was higher than that in control group(P<0.05). Levels of IL-1β, IL-8 and IFN-γ in observation group were lower than those in control group, while levels of IL-4 and IL-10 were higher than those in control group (P<0.01). Conclusion: Jiawei Changfengyin can relieve symptoms of acute radiation proctitis, promote healing of rectal mucosa, improve quality of life, and regulate Th1/Th2 cytokines, with good repairing effect for intestinal mucosa.
9.Effect of moxibustion at "oppositely-located points" on neurogenic bladder after spinal cord injury and endoplasmic reticulum stress pathway in rats.

OBJECTIVE:
To observe the effect of moxibustion at oppositely-located points "Mingmen" (GV 4) and "Shenque" (CV 8) on the motor function of the hind limbs and bladder function in rats with neurogenic bladder after suprasacral spinal cord injury (SCI), so as to explore the effect of this therapy on bladder tissue apoptosis mediated by endoplasmic reticulum stress pathway.
METHODS:
Twenty-eight female Wistar rats were randomly divided into a sham-operation group (8 rats) and a model establishment group (20 rats). Using the modified Allen's method, the spinal cord of T10 segment was injured to establish a neurogenic bladder model in the model establishment group. Sixteen rats were modeled successfully and then divided into a model group (8 rats) and a moxibustion group (8 rats). In the moxibustion group, 2 h after consciousness regaining from modeling anesthesia, moxibustion was exerted at "Shenque" (CV 8) and "Mingmen" (GV 4), 2 cones at each acupoint in one intervention. The intervention was administered once every two days and 5-time intervention was required totally. After intervention, Basso, Beattie and Bresnahan locomotor rating scale (BBB) score for the motor function of the hind limbs, and the urodynamics indexes (maximum bladder capacity, urine leakage pressure and bladder compliance) were compared among groups. HE staining method was adopted to observe the morphological changes of bladder tissue. With Western blot method and real-time PCR assay, the protein and mRNA expressions of the endoplasmic reticulum stress-related genes (glucose- regulated protein 78 [GRP78], activating transcription factor 4 [ATF4] and cysteinyl aspartate specific proteinase-12 [Caspase-12]) were determined.
RESULTS:
The transitional epithelial cells were arranged irregularly, the bladder wall was getting thinner, and the cellular vacuolar degeneration and neutrophil infiltration were found in the model group. Whereas, compared with the model group, in the moxibustion group, the arrangement of transitional epithelial cells was clear and continuous in layers, the cellular vacuolar degeneration was mild and the infiltration presented in a small amount of neutrophil granulocytes. Compared with the sham-operation group, in the model group, the BBB score was reduced (P<0.01), the maximum bladder capacity and bladder compliance were increased (P<0.01), and the protein expression levels of GRP78, ATF4 and Caspase-12, as well as mRNA expressions were all increased (P<0.01). In comparison with the model group, in the moxibustion group, BBB score was increased (P<0.01), the maximum bladder capacity and bladder compliance were decreased (P<0.01), and the protein and mRNA expression levels of GRP78, ATF4 and Caspase-12 were all decreased (P<0.01).
CONCLUSION
Moxibustion at the "oppositely-located points" improves the urination function, alleviate urine retention in neurogenic bladder rats after spinal cord injury. The underlying mechanism may be related to the down-regulation of the expressions of GRP78, ATF4 and Caspase-12 in the endoplasmic reticulum stress pathway of the bladder tissues, and thus to alleviate the apoptosis of bladder tissue.

10.Efficacy of two kinds of posterior chamber intraocular lens fixation in the treatment of lens dislocation

AIM: To compare the efficacy of 25-gauge(25G)vitrectomy combined with intraocular lens transciliary sulcus scleral suture fixation and sutureless scleral tunnel interlamellar fixation in the treatment of complete lens dislocation.METHODS: Retrospective case-control study. A total of 40 patients(40 eyes)diagnosed with complete lens luxation in the vitreous cavity in our hospital from May 2015 to September 2021 were selected, among which 21 eyes(suture group)underwent 25G vitrectomy combined with intraocular lens fixation via ciliary sulci scleral suture, and 19 eyes(sutureless group)underwent 25G vitrectomy combined with interlamellar scleral tunnel fixation of intraocular lens. The patients in both groups were followed up until 3mo after surgery to observe the operative time, best corrected visual acuity(BCVA), corneal endothelial cell count(CECC), central corneal thickness(CCT)and postoperative complications.RESULTS: The operation time was significantly shorter in the sutureless group than in the suture group(31.79±6.01min vs. 45.38±8.04min, P<0.001). BCVA in both groups was significantly improved after surgery(all P<0.05), and the BCVA(LogMAR)at 1wk after operation in the sutureless group was significantly better than that in the suture group(0.32±0.14 vs. 0.57±0.25, P<0.001). At 3mo after surgery, CECC in both groups was lower than that before surgery(all P<0.01). The CCT at 1wk after operation in the suture group was greater than that before operation and at 3mo after operation(all P<0.01), and there was no significant change in CCT before and after surgery in the sutureless group. During follow-up period, the total complication rate in the sutureless group was lower than that in the suture group(26% vs. 38%, P>0.05).CONCLUSION: 25G vitrectomy combined with intraocular lens sutureless scleral tunnel interlamellar fixation in the treatment of complete lens luxation has shorter operation time, faster postoperative visual acuity improvement and fewer postoperative complications.
