1.Clinical features and prognostic factors analysis of children with acute idiopathic myelitis

Objective To study the clinical features and prognostic factors of children with acute idiopathic myelitis.Methods This study retrospectively collected and summarized 75 patients' clinical data who were diagnosed with acute idiopathic myelitis for the first time in Children's Hospital of Chongqing Medical University from Jan.2003 to Dec.2012.The relationship between clinical features and prognosis was analyzed by using SPSS 19.0 software.Results In this study,37 boys and 38 girls were inclued.The distribution of ages was bimodal.The incidence of the disease seemed to have two peaks.One was broader ranging between age 1 and age 4 and the second was between age 9 and age 13.The main symptoms of acute idiopathic myelitis were weakness,sensory loss and autonomic dysfunction.Among the 75 cases,all patients had different degrees of paralysis,59.0% (44 cases) of which had sensory abnormity and 80.0% (60 cases) had autonomic dysfunction.In addition,root pain (50.7 %,38/75 cases),bulbar paralysis (9.3 %,7/75 cases) and fever(66.7%,50/75 cases) also appeared in the process.Seventy cases had spinal magnetic resonance imaging(MRI) exams,in which 61 showed abnormal signal and T2-weighted hyperintensities could be found.Forty children were followed up successfully and 32 of which had a good outcome.Factors associated with poor outcome included spinal shock(P =0.001),the duration of spinal shock beyond 2 weeks (P =0.015),low score of maximal neurologic defect (9.25 ± 0.89 vs 14.03 ± 2.87,P =0.000),plateau period ≥ 12 days (P =0.004),the time to maximal defect ≤ 24 hours(P=0.029),fever in initial stage(P =0.029),and being female(P =0.021).Conclusions Acute idiopathic myelitis was mainly manifested by weakness.Autonomic dysfunction happened more commonly than sensory abnormity.Some patients suffered bulbar paralysis during the couse of the disease.Spinal MRI signal usually changed on T2-weight sequences.Meanwhile,identifying the factors associated with poor outcome as early as possible is beneficial to preliminary evaluation and active treatment.
2.CTA signs for blunt thoracic aortic injury: a review of 37 cases

Objective To review computed tomographic angiography (CTA) signs and its potential use in the diagnose of blunt thoracic aortic injury Methods We reviewed 37 cases with blunt thoracic aortic injury (TAI) confirmed by surgery or aortography in Chinese PLA General Hospital emergency department from June 2004 to June 2014.CTA were performed on these patients,and CTA images were analyzed retrospectively to obtain characteristic CTA signs of blunt thoracic aortic injury.Results Of the 37 cases,the blunt injury in aortic isthmus were 83.8% (31/37),aortic arch 5.4% (2/37),descending aorta 8.1% (3/37),and ascending aorta 2.7% (1/37),respectively.CTA signs of thoracic aortic injury manifested as intimal flap in 34 patients (91.8%),abnormal aortic contour or cavity in 37 cases (100%),arterial contraction in 17 cases (45.9%),aortic dissection in 10 cases (27.0%),pseudoaneurysm in 21 cases (56.7%),thrombosis in 1 case (2.7%).Conclusions CTA signs could be used to facilitate the diagnosis of blunt thoracic aortic injury with a qualitative diagnostic value.
3.Clindamycin-loaded Calcium Phosphate Cement as Local Antibiotic Delivery System

OBJECTIVE To determine the feasibility of clindamycin-loaded calcium phosphate cement(CLCPC) as a local antibiotic delivery system.METHODS The initial setting time(tI) and the final setting time(tF) were measured for 0%,2% and 5% CLCPC according to ASTM C266-89 method.Clindamycin concentrations eluting from the samples of 2% and 5% CLCPC in PBS were analyzed by HPLC at different times.The bacteriostasis tests were done by plate diffusion method for 2% and 5% CLCPC and 2% Palacos R-40 bone cement(PMMP) samples,and the diameters of the bacteriostasis ring and bacteriostasis duration were observed.The setting product and crystal size of 0%,2% and 5% CLCPC were analyzed and observed by X-ray diffraction(XRD) and scanning electron microscopy(SEM).RESULTS The setting time could be shortened by adding clindamycin(tI,tF) of 2% and 5% CLCPC.Clindamycin was with burst-release from CLCPC within the intial 6-hour period and the release rate slowed down on 4th day.Clindamycin could still release until to the 42th day.The ring of 2% Palacos R-40 bone cement(PMMP) bacteriostasis was smaller than that of 2% CLCPC,and the ring of 2% CLCPC bacteriostasis was smaller than that of 5%CLCPC.The bacteriostasis still existed to the 42th day of test for 2% and 5%CLCPC and 2% Clindamycin-loaded Palacos R-40 bone cement(CLPMMP).From the 30th day,many bacterial colonies were seen in the culture media laying 2%CLPMMP sample.On the contrary,bacterial colony was not found in the media putting 2% and 5%CLCPC.XRD and SEM showed that clindamycin did′t have an influence on setting product,crystal size and structure of CPC.CONCLUSIONS Clindamycin-loaded calcium phosphate cement can be used as a local antibiotic delivery system.
4.Animal Model with Implant Infection by Biofilm:Establishment and Evaluation

OBJECTIVE To establish the implanted biofilm model of rabbit implant infection.METHODS SEP was cultured,purified,identified,and tested for the drug sensitivity and the biofilm production.Healthy adult rabbits were randomly divided into 4 groups,8 in each group.The stainless steel screws and washer UHMWPEs were implanted into the femoral condyle of the non-articular surface of the rabbit right stifle knee.The knee joints were inoculated with 1ml sterile saline,102,103 and 104 CFU SEP,respectively.Wounds were observed postoperatively.After the 14th days postoperation,the knee synovium of 32 rabbits sacrificed were taken out by aseptic technique and were cultured to determine whether the knees were infected and which concentration of SEP was the appropriate for causing knee joint infection.UHMWPEs from the appropriate ID group were observed to find whether there were biofilms on the surface by SEM and LCSM.RESULTS The bacterial strain was identified as SEP and could produce biofilm.Among the knee joints inoculated with 1ml saline,102,103 and 104 CFU SEP,the infective rate was 0,37.5%,100.0% and 100.0% and poor wound healing was in 0,1,2 and 4 rabbits,respectively.It showed 103 CFU of SEP was the appropriate ID.Biofilms were found on all UHMWPE surfaces from 103 CFU SEP group by SEM and LCSM.SEM showed SEP in the biofilms on the surface of UHMWPE was agglomerated and wrapped in the matrix.The structure of biofilms in which SEP radiated red fluorescence was inlaid in mucopolysaccharide stained green fluorescence was observed by LCSM.CONCLUSIONS The model provides an effective method to investigate the biofilm.
5.The changes of myocardial enzymes and troponin I in patients with hepatocellular carcinoma closed to heart after high intensity focused ultrasound ablation

Objective To investigate the influences of heart in patients with hepatocellular carcinoma closed to heart after high intensity focused ultrasound ablation(HIFU). Methods Thirty four patients with hepatocellular carcinoma closed to heart received detecting the values of periphery blood aspartate transarninase(AST), creatine kinase (CK), CK-MB, α-hydroxybutyrate dehydrogenase (α-HBDH), lactate dehydrogenase(LDH), myoglobin(Myo) and troponin I (cTnI) before and 1,3, 7 days after HIFU respectively. ECG was monitored in all patients simultaneously. Results The values of AST,CK,CK-MB,α-HBDH, LDH and Myo increased 1 day after HIFU significantly (P<0.05), which decreased greatly 3 days after HIFU and nearly recovered well (P>0.05) 7 days after HIFU. However, the value of cTnI and ECG were normal before and after HIFU. Conclusions No obvious myocardium injury was found after HIFU in patients with hepatocellular carcinoma dosed to heart, cTnI may be an important marker to evaluate myocardium injury after HIFU.
6.High intensity focused ultrasound for treating residual liver cancer after radiotherapy: primary clinical observation

Objective To investigate the safety and effect of high intensity focused ultrasound ablation(HIFU) on residual liver cancer in patients after radiotherapy. Methods Twenty liver cancer patients with twenty five residual tumors after three-dimensional conformal radiotherapy or stereotactic radiotherapy received HIFU ablation. Liver function and periphery blood cell counts were performed before HIFU and at 1 week after HIFU in all patients. Enhanced MRI and α-fetoprotein (AFP) level were performed before HIFU and at 2 weeks after HIFU to evaluate the effect of HIFU ablation. The survival of all patients was assessed by the Kaplan-Meier method. Results The mean follow-up time was (12.6 ± 8.0)months. The median survival time and 1-year survival rate were 22 months and 87.5% respectively. No skin burns were observed in all patients. As compared with before HIFU,there were no significant differences in the levels of albumin,alanine transarninase, aspartate transarninase, total bilirubin, direct bilirubin, blood red cell counts,blood white cell counts and blood platelet counts at 1 week after HIFU (paired t test, P =0. 156,0. 356,0. 203,0.659,0. 531,0. 519,0. 310,0. 346, respectively). Significant difference in AFP level of 9 patients with AFP>20 μg/L was observed before HIFU and 2 weeks after HIFU (paired t test, P =0.030). Among 25 residual liver tumors,sixteen with complete ablation and six with ablation volume of ≥80% were observed by enhanced MRI at 2 weeks after HIFU. Conclusions HIFU may be a safe and effective new method to ablate residual liver cancer after three-dimensional conformal radiotherapy or stereotactic radiotherapy.
7.In vitro Effect of Four Kinds of Implant Materials on Biofilm Formation of Staphylococcus epidermidis

OBJECTIVE To investigate the in vitro effect of 4 kinds of implant materials on the biofilm formation of Staphylococcus epidermidis.METHODS S.epidermidis was cultured,purified and identified.Susceptibility test was done for S.epidermidis and the ability to produce biofilm was proven.The test samples were made into wafer shape for titanium alloy,Co-Cr-Mo alloy,ultrahigh molecular weight polyethylene(UHMWPE)and home-made polymethyl methacrylate(PMMA).S.epidermidis was cultured with 4 kinds of test samples for 5 days respectively.Bacteria adhering on surfaces of 4 kinds of test samples were dissolved with trypsin,and then diluted into bacterial suspensions.Each bacterial suspension was inoculated quantitatively and CFU were counted.Biofilm on surfaces of 4 kinds of test samples prepared by vacuum drying method was observed with SEM.RESULTS The strain proven to be S.epidermidis,was resistant to semisynthetic penicillins,and could produce biofilm.CFU count showed that CFU were the most on the UHMWPE surface and the number of CFU were(24.96?1.459)?105.CFU were(17.44?1.883)?105 on the PMMP surface.(0.424?0.065)?105 CFU were discovered on the surfaces of titanium alloy and(0.382?0.075)?105 CFU on Co-Cr-Mo alloy.When each group compared with UHMWPE and domestic PMMA respectively,all P value was
8.Phagocytic function of macrophage after partial splenectomy of rabbits using microwave tissue coagulation.

The immune function of spleen after partial splenectomy (PSM) of rabbits was stuided using microwave tissue coagulation (MTC). Streptococcus pneumoniae suspension was injected into the ear veins of rabbits 4 weeks after they were subjected to partial splenectomy using microwave tissue coagulation (PSM) group and to sham operation (SO groups), respectively. India ink was given via portal vein. Residual spleen were resected and microscopic examinations were performed on spleen slices stained using HE to compare phagocytic functions of macrophage. Gradings of phagocytic function in macrophage were of no significant difference between PSM and SO groups (P > 0.05). Weights of spleens showed no significant difference between the two groups (P > 0.05). The phagocytic function of macrophage after PSM can be preserved well. The result of this experiment implies that splenic salvage using MTC is a clinically applicable method.

9.Effect of individualized intervention on social function rehabilitation in patients with schizophrenia

Objective To investigate the promoting effect of individualized intervention on social function rehabilitation in patients with schizophrenia. Methods Total of 200 hospitalized patients with schizophrenia in a tertiary psychiatrichospital were selected as the research object and were randomly assigned to the study group ( n=100) and the control group ( n=100) . Patients in control group were treated with routine treatment and nursing, whilepatients in study group were given individualized intervention combined with routine treatment and nursing. Brief psychiatric rating scale ( BPRS) was used to assess the severity of the psychotic symptoms.Insight and treatment attitude questionnaire ( ITAQ) was used to assess the treatment compliance of patients. Activity of daily living scale (ADL) and personal and social performance scale(PSP) were used to assess their social function. Then, we compared the results of two groups that before and after the intervention. Results Before the intervention, there was no significant difference in BPRS, ITAQ and ADL scores between two groups ( P>0.05) . The PSP scores of the study group were lower than those of the control group ( P<0.05) . After the intervention, the ITAQ and PSP scores of two groups were significantly higher than those before the intervention, and the BPRS and ADL scores were significantly lower than those before the intervention ( P<0.05) . After the intervention, there was no significant difference in BPRS and ITAQ scores between two groups ( P>0. 05) . The ADL scores of the study group were significantly lower than those of the control group, and the PSP scores were significantly higher than those of the control group (P<0.05).Conclusions Personalized intervention enhances the patient′s self-care ability, daily activities ability and social competence, and promotes the recovery of social function.
