1.Clinical Observation of Dexmedetomidine Combined with Propofol for Anesthesia in Elderly Patients with Painless Gastroscopy

Objective:To observe the clinical effect of the combined use of dexmedetomidine and propofol for anesthesia in the elderly patients with painless gastroscopy. Methods:Totally 70 elderly patients with painless gastroscopy were selected and randomly divided into the observation group (35 cases) and the control group (35 cases). The observation group was given dexmedetomidine and propofol for anesthesia, while the control group was given propofol for anesthesia. The anesthesia induction time, the vital signs and recovery time of the patients, intraoperative complications and postoperative adverse reactions were observed and compared between the two groups. Results:The anesthesia induction time of the observation group was shorter than that of the control group (P<0. 01). The mean arterial pressure (MAP) in T2, T3 and T4 stage in the two groups were significantly lower than that in T1 stage (P<0. 05, P<0. 01). The HR in T3 stage and RR in T2 stage in the control group were lower than those in T1 stage (P<0. 05). The RR in T2 stage in the observation group was significantly lower than that in T1 stage (P<0. 05). The MAP in T2 and T3 stage in the observation group was significantly higher than that in the control group (P<0. 01), while that in T4 stage in the observation group was significant-ly lower than that in the control group (P<0. 05). The HR in T3 stage in the observation group was significantly higher than that in the control group (P<0. 05). The incidence of intraoperative complications in the observation group was 5. 7%, while that in the con-trol group was 22. 9%, and there was significant difference between the two groups (P<0. 05). The recovery time in the observation group was shorter than that in the control group (P<0. 01). Conclusion:Dexmedetomidine combined with propofol has better anes-thesia effect and higher safety than propofol alone, which is worthy of clinical promotion.
2.One-stage anterior debridement,allografts and internal fixation for leaping tuberculosis of lumbar vertebrae

[Objective]To assess the efficacy of one-stage allograft fusion and anterior spinal stabilization as an alternative treatment of lumbar leaping tuberculosis.[Methods]Eight patients with lumbar leaping tuberculosis underwent anterior route decompression and fusion.Combined chemotherapy was delivered to each patient at least three weeks before operation.There were 5 men and 3 women ranging in age from 21 to 62 years(average,37.2 years).The involved area included L1 and L3 in 2 patient,L1、2and L4 in 3 patient,L2、3and L5 in 1 patient,L2 and L5 in 2 patients.There were 1 patient with Frankel Grade B,2 with Grade C,1 with Grade D and 4 with Grade E.The kyphosis angle ranged from 5?~40?(average 21.5?).The patients were followed up for 12~24 months.[Results]The patients folerated operation well.The operation time were 120~180 min and the bleeding during operation were 400~900 ml.After surgery,pain reliefed in all patients.One patient in Grade B improved to Grade C,1 patient in Grade C improved to Grade D,1 patient in Grade C improved to Grade E,1 patients in Grade D improved to Grade E.The mean angle of kyphosis correction were 5?~20?(average angle:12.5?).There was no postoperative complication.During the follow-up period,all cases healed without any recurrence.There was no breakage of nails or fall of the internal fixation.Spinal fusion occurred after 4~7 months(average 5.3 months) after operation.[Conclusion]Lumbar leaping tuberculosis treated with this surgical technique can achieve a high satisfactory rate with advantages of restoring the spinal stability,providing early fusion,preventing and correcting progression of the kyphosis.
3.Custom-made Artificial Total Knee Prosthesis Replacement for Pathological Fracture of Distal Femur Caused by Giant Cell Tumor of Bone

Objective: TO evaluate the effect of custom-made artificial total knee prosthesis replacement on pathological fracture of distal femur caused by giant cell tumor of bone. Methods." A total of 12 knees of 12 patients with pathological fracture of distal femur caused by giant cell tumor of bone were treated with cus-tom-made artificial total knee prosthesis replacement. There were 5 males and 7 females aged from 23 to 40 years (34.2 years on average). Results: All patients tolerated surgery very well. The surgical duration was 120~ 180 rain and the intraoperative bleeding was 500 ~ 900mL. The drainage tube was taken out in 5 days after surgery. The postoperative drainage volume was 300 ~ 700mL. There were no postoperative complica-tions. The postoperative X-ray examination showed that all of the prosthesises were in the right position. Lo-cal pain disappeared in all patients after surgery. The pathological examination identified all the patients with giant cell tumor of bone. All patients were followed up for 18 ~ 30 months (24 months on average). No recur-rence was observed after surgery. No infection or implant loosening occurred during the follow up. According to the Enneking assessment system of function after limb salvage surgery, the mean Enneking scale was 23.8 points (ranged from 15 to 28 points) at the last follow-up visit. Conclusion: Custom-made artificial total knee prosthesis replacement can achieve a high satisfactory rate in clearance of lesion and restoring function of the knee in patients with pathological fracture of distal femur caused by giant cell tumor of bone, improving patients' quality of life.
4.Identification of a new shrimp allergen enolase from Litopenaeus vannamei

Objective:To identify enolase,47 kD allergen,from Litopenaeus vannamei by Mass spectrometry. Methods: The proteins were extracted from Litopenaeus vannamei tissue with acetone precipitation method. The protein components were analyzed by SDS-PAGE and Western blot. By using Matrix-Assisted Laser Desorption/Ionization time of flight mass spectrometry ( MALDI-TOF/TOF-MS) ,the 47 kD allergen from Litopenaeus vannamei was identified as enolase. Results:By SDS-PAGE,we proved that the native protein components from Litopenaeus vannamei were completely. According to the Western blot result more than 14 components could react with the positive serum. MALDI-TOF/TOF analysis results showed that the suspected proteins were enolase. A sensitization frequency was 55%. Conclusion:Enolase was identified as a new allergen of Litopenaeus vannamei.
5.Multiplicity of prognosis of pancreatic carcinoma patients

Objective To explore the factors affecting the prognosis of pancreatic cancer patients,using COX risk model to carry on the statistical analysis of 154 cases of pancreatic carcinoma.Methods Retrospectively analyzed 154 pancreatic cancer cases' history date treated in April 2001-April 2008 of Fujian Medical University Union Hospital.Survival rate was caculated by Kaplan-Meier estimation while survival cruve was analysed by Log-rank test.Prognostic factors were studied by multivariate analysis with COX proportional hazards survival model.Results In this group,survival time was from 1 to 106 months and the median survival time was 8 months.Overall survival rates at 6 months,12 months,18 months,36 months were 58.4%,29.2%,16.3% and 3.7% respectively.Multivariate revealed that independent prognostic factors were follows:TNM tumor staging,operation mode and synthetic therapy.Conclusions The prognosis of patients with pancreatic carcinoma is determined by multiple factors.Patients of advanced TNM staging have a poorer prognosis.Radical resection and comprehensive treatment of patients prognosis is better.
6.REPAIRING EFFECTS OF NERVE GROWTH GRANULE ON RAT COMMON PERONEAL NERVE TRANSECTION INJURY

Objective To study the repairing effects of nerve growth granule(NGG) on rat common peroneal nerve transection injury.Methods After 50 Sprague-Dawley rats were subjected to nerve suture after transaction,they were randomly divided into 5 groups for daily intragastric administration of drugs:NGG high-dose(5.2g/kg),medium-dose(2.6g/kg),low-dose(1.3g/kg) groups,mecobalamin group(positive control) at 625 ?g/kg,control group(control group control).The drug administration lasted for 4 weeks.Footprint test was performed 2-,3-and 4-weeks after surgery to evaluate toe spread function(TSF).Electrophysiology was performed 4 weeks after operation to determine the compound muscle action potential(CMAP) and nerve action potential(NAP).The number of regenerated myelinated nerve fibers,thickness of myelin sheath and cross sectional area of tibial muscle were measured by histomorphology.Results TSF,amplitude and recovery rate of CMAP and NAP,the number of regenerated myelinated nerve fibers,thickness of myelin sheath and section area of tibial muscle were all increased significantly in a dose-dependent manner compared with the control group.Conclusion NGG contributes to axon growth and myelination,and thus promotes peripheral nerve regeneration in rats with functional recovery.
7.Increased ICAM-1 expression on bronchial epithelial cells induced by eosinophils

Objective To study the effects of human bronchial epithelial cells (BEAS-2B cells ) coculture together with human eosinophils on intercellular adhesion molecule (ICAM-1) expression on BEAS-2B cell surface.Methods BEAS-2B cells cocultured contact with eosinophils for 4 h and 12 h,total RNA was extracted and ICAM-1 gene in BEAS-2B cells were analyzed by reverse transcription polymerase chain reaction (RT-PCR); ICAM-1 protein on BEAS-2B cell surface was assayed by fluorescent antibody in flow cytometry.Results ICAM-1 gene expression in BEAS-2B cells was upregulated and ICAM-1 protein on BEAS-2B cell surface increased by coculture with eosinophils.Conclusion Coculture of BEAS-2B cells and eosinophils might induce BEAS-2B cells to express ICAM-1.

9.Relationship between parental control and aggressive behavior of middle school students

Objective:
To explore the relationship between parental control and aggressive behavior of middle school students.
Methods:
Random cluster sampling was used to select 1 046 students.Information was collected through the Chinese version of Parental Control Questionnaire and Buss & Perry Aggression Questionnaire.
Results:
The score on anger were high among female students and the total score were high among male students(t=6.28,2.56,P<0.01).Multiple comparison showed. the grade two junior middle school students’aggression level is significantly higher than grade three junior middle school students and grade one, two high school students(t=3.22,4.07,3.01,P<0.05). There were significant gender differences in parental control,father’s behavior control and father’s psychological control(P<0.05).Parental control shows a significant grade difference,multiple comparison showed, parental control among junior middle school students is significantly higher than that among high school students(t=3.22, 4.07, 3.01, P<0.05).Parental control and aggressiveness of the students were significantly positively correlated(r=0.16,0.29).There was a significant positive correlation between the psychological control of the parents and the aggressiveness of the boys(r=0.26,0.23).The behavior control and psychological control of the parents were positively correlated with the aggressiveness of the girls(r=0.09,0.13,0.29,0.35)(P<0.05).The Logistic regression analysis showed,father’s psychological control(OR=0.50,95%CI=0.21-0.79) and mother’s psychological control(OR=0.32,95%CI=0.02-0.62) showed significant positive association with aggression of the boys.Mother’s psychological control(OR=0.55,95%CI=0.23-0.87)and parents’ control(OR=0.17,95%CI=0.01-0.33) were positively associated with aggression of the girls(P<0.05).
Conclusion
Effective intervention of parental control might help prevent and control aggressive behavior of middle school students.
10.Establishment of early prediction model for patients with hyperlipidemic severe acute pancreatitis

Objective:To establish an early prediction model with multiple indicators to predict the risk of severe acute pancreatitis (SAP) in hyperlipidemic acute pancreatitis (HLAP).Methods:The clinical data of 92 patients with HLAP admitted to the Emergency Department of our hospital from March 2018 to February 2020 were analyzed retrospectively. Among them, 29 cases deteriorated to SAP and 63 cases did not. Univariate analysis was used to screen predictive indicators related to hyperlipidemic severe acute pancreatitis (HL-SAP), and logistic regression analysis was used to screen independent predictive indicators related to HL-SAP. Then a prediction model was established. The area under (AUC) the receiver operating curve (ROC) was used to evaluate the predictive ability of each predictive indicator and the model for HL-SAP. Bootstrap resampling technology was used to validate the predictive ability of the model.Results:Univariate analysis showed that procalcitonin, D-dimer, C-reactive protein, albumin, cholesterol and CT grade had influence on the progression of HLAP to SAP ( P<0.05). Logistic regression analysis showed that D-dimer ( OR=2.112, 95% CI: 1.022-4.366; P<0.05), CT grade ( OR=5.818, 95% CI: 2.481-13.643; P<0.01) and cholesterol ( OR=1.146, 95% CI: 1.004-1.308; P<0.05) were independent risk factor of HL-SAP. The AUC of D-dimer, CT grade, cholesterol and the model were 0.802, 0.875, 0.665 and 0.927, respectively. Internal validation of the predictive ability of the model showed that the C-index was 0.927. Conclusions:In the early phase, application of the prediction model that composes D-dimer, CT grade and cholesterol has a good predictive effect on HL-SAP.
