5.Interventional occlusion of intracristal ventricular septal defects using domestic occluder:Experience of type selection, size definition and injury avoidance in 46 cases

OBJECTIVE: Interventional occlusion of intracristal ventricular septal defects (IVSD) is a node point. Reports concerning angiography features and successful occlusion of IVSD using domestic occluder are rare. The aim of this paper is to investigate the angiography features of IVSD and the experience of interventional occlusion with domestic occluder. METHODS: Totally 46 cases of IVSD were diagnosed by color Doppler echocardiography that they had interventional indications.According to the short axis view of aortic, they were divided into 4 groups: A (11:30-12:00), B (12:00-12:30), C (12:30-13:00) and D (13:00-13:30). Different groups were performed with adequate angle angiography to show ventricular septal defect, as well as to decide strategy, which to choose occluder and occluded methods.RESULTS: All 46 patients were occluded successful. Technical success rate was 100%, without related complications. A group included 10 cases, B group 13 cases, C group 16 cases and D group 7 cases. The left anterior oblique angle of IVSD left ventricular angiography was greater than other type of VSD, and increased gradually with the changes in the defect location (Group A to D). According to the defect size and different groups, the defects were successfully occluded with symmetric,decentered, zere-decentered type occluder. Modified pigtail catheter was good value in practice occlusion of IVSD.CONCLUSIONS: Left anterior oblique angle in IVSD left ventricular angiography should be bigger. Domestic occluder is safe,effective, with less complication and cost, which can be the first choose for patients with LVSD.
6.Effects of baicalin on the expression of TAFI,NF-κB,and ACE2 in the atherosclerotic rats

Objective To explore the effects of baicalin on the expression of TAFI,NF-κB,and ACE2 in the atherosclerotic rats and discuss the anti-atherosclerosis mechanisms of baicalin.Methods The subjects were healthy male Wistar rats.The rats were divided into sham operation group,model group,baicalin high dose group (1 50 mg·kg-1 ·d-1 )and baicalin low dose group (100 mg·kg-1 ·d-1 ).Plasma TAFI activity were detected with enzymatic assays.The expression of NF-κB and ACE2 were detec-ted with immunohistochemical staining.Results Compared with sham group,the rest groups of TAFI and ACE2 activity were sig-nificantly higher (P <0.05),and the expression of NF-κB,had significant decreased (P <0.05).Compared with the model group, the level of TAFI and ACE2 in baicalin drug group was significantly lower than the model group (P <0.05),and the expression of NF-κB,had significant increased.Conclusion Baicalin can reduce TAFI and ACE2 level,and upregulate the expression of NF-κB to play its role in anti atherosclerosis.
7.Related factors of postpartum depression in mothers of preterm infants in neonatal intensive care unit

Objective To assess the prevalence and related factors of postpartum depression in mothers of preterm infants in neonatal intensive care unit (NICU).Methods A total of 83 mothers who met the inclusion and exclusion criteria were investigated by using Edinburgh Postnatal Depression Scale (EPDS) and the comprehensive questionnaire for mothers of preterm infants in NICU,and the analysis was performed on SPSS 13.0 using the Spearman correlation analysis and Logistic regression analysis.Results Among the 83 mothers,38 got the EPDS score of 10 and above,which accounted for 45.78% (38/83).The disease severity (OR =3.318,P < 0.01),the worry about disease prognosis (OR =4.348,P <0.01),the worry about mental development of the newborn (OR =5.420,P < 0.01) were the risk factors for postpartum depression in mothers of preterm infants in NICU.Conclusions It's different from the correlation to postpartum depression in mothers of full-term newborns,the correlation to postpartum depression in mothers of preterm infants in NICU are closely related to premature and admission in NICU.The mothers of preterm infants in NICU are the high-risk population for postpartum depression.Routine screening mechanism for mothers of preterm infants should be established for early detection and treatment of postpartum depression,and to promote maternal and infant health.
8.A comparative study on safety and efficacy of FOLFOX4 treatment in elderly versus young patients with advanced colorectal cancer

Objective To observe the safety and efficacy of FOLFOX4 regiment in elderly versus young patients with advanced colorectal cancer. Methods There were 61 patients enrolled in this study, with 28 elderly patients aged 70 years and over, 33 young patients aged less than 70 years.They suffered from advanced/recurrent colorectal cancer and received FOLFOX4 regiment (Oxaliplatin +CF+5-FU). Every 14 days were as a cycle, and the therapeutic safety and efficacy were evaluated after three cycles. Adverse events and response to treatment were compared between the elderly and young patients. Results The main adverse effects were myelosuppression, gastrointestinal disturbance and neurotoxicity. The incidence rate of diarrhea was significantly higher in elderly patients than in young patients, but the most of diarrhea were at grade Ⅰ - Ⅱ. The incidence rates of leucocyte decrease and neutrophil decrease were higher in elderly patients than in young patients (92. 8% vs. 78. 8%, 39.3% vs. 36.3%), but there were no statistically significant differences between them. The incidence rate of neurotoxicity was 46.5% in elderly patients and 36.4% in young patients (P>0. 05). The recent efficacy rate was 25%, disease control rate was 71.4% and median time-to-progression (TTP) was 6 months in elderly patients and 24.2%, 84.8% and 7 months in young patients (all P>0.05). Conclusions FOLFOX4 regiment is well-tolerated and effective in both young and elderly patients.
9.Clinical study of late-course hyperfractionated radiotherapy in the treatment of local advanced non-small cell lung cancer

Purpose:To evaluate the clinical effect of late-course hyperfractionated (LCH) radiotherapy and conventionally fractionated (CF) radiotherapy for local advanced non-small cell lung cancer (NSCLC). Methods:From August 1995 to April 1998, 74 patients with inoperable stage Ⅲ NSCLC were randomized into two groups: CF and LCH. 37 patients in CF group received a fraction of 2 Gy daily, 5 days per week, to the total dose of 65~70 Gy /6.5~7 weeks. 37 patients in LCH group were treated with the same fractionation as CF group to the dose of 40 Gy, and then followed by LCH radiotherapy: 1.2Gy, twice daily, with the interval of 6 hours between fractions, to the total dose of 64 Gy/6 weeks. Objective Determining the respective incidence, risk factors and prognosis of hyperuricemia post normothermic cardiac surgery with cardiopulmonary bypass. Methods Clinical data was collected from April 2002 to October 2004. 232 patients undergoing cardiac surgery with cardiopulmonary bypass (CPB). Patients were classified into three groups: nonhyperuricemia group, hyperuricemia group, and acute goutynephropathy group.
