Objective To evaluate the independent effect of premorbid statins on incident ischemic stroke severity and outcome at discharge.Methods We included consecutive patients with ischemic stroke and recorded demographic data,vascular risk factors,Oxfordshire Classification,the method of treatment National Institutes of Health Stroke Scale(NIHSS)score,previous statins treatment,and outcome discharge.We analyzed the data using univariate methods and a logistic regression with the dependent variable of good outcome(Barthel Index,BI 95 to 100).Results A total of 185 individuals met study criteria:57 patients were on statins agents prior to admission and 128 were on no statins.Pretreatment with statins was associated with lower presenting median NIHSS(6.0 vs 13.0,P
3.Observation of limbal - stem - cell transplantation to treat pterygium combined with conjunctivochalasis

AIM:To observe the efficacy, complications and ocular surface tear dynamics of the treatment for pterygium combined with conjunctivochalasis by inferior limbal-stem-cell transplantation.
METHODS: All 30 patients ( 46 eyes ) diagnosed as pterygium combined with conjunctivochalasis in our hospital from December, 2010 to December, 2012 with a mean age of 61. 2y (SD 7. 2, range 49-75) were subjected to observation. After excision of nasal pterygium, an equivalent size of conjunctival flap linked with corneal limbal stem cell from inferior limbus was transplanted to the exposed sclera. Afterwards, the inferior bulbar conjunctiva was trimmed and fixed onto surface of sclera and inferior corneal limbus. After one-year follow-up, the comparisons regarding vision acuity between pre-operation and post-operation ( 1mo later ) as well as ocular-surface tear dynamics changes including several indexes like altitude and integrity of tear meniscus, break-up time ( BUT ) , corneal fluorescein staining and chloramphenicol gestation test before and 3mo after surgery were made. The operative complications and the recurrence rates 1y after operations were recorded.
RESULTS: Median visual acuity was 0. 2 (0. 04-0. 6) before the operation and median visual acuity was 0. 4 (0-04-1. 0) 1mo after the operation. Of total 46 eyes in 21 eyes ( 45. 7%), visual acuity improved. No vision changes were found in other 25 eyes ( 54. 3%). Significant difference ( P<0. 01 ) of vision acuity before and after surgery was found. In addition, break-up time ( BUT) , corneal fluorescein staining and chloramphenicol gestation test and altitude and integrity of tear meniscus show better results 3mo after surgery than that of pre-operation, which showed statistically significant ( P <0-05). After 1y follow-up, 33 eyes (71. 7%) healed while problems in 11 eyes ( 23%) were relieved. The total efficacy rate was 95. 6%. Nonetheless, invalid surgeries were noticed in two eyes ( 4. 4%) that affected by recurrence of pterygium. The 33 eyes (71. 7%) affected by conjunctivochalasis was cured in terms of subjective evaluation and improved in 9 eyes (19. 6 %), but 4 eyes (8. 7%) exhibited no evident improvement in terms of subjective complains. Total efficacy was 91. 3%. The 41 eyes (89. 1%) were reported to be normal while 5 eyes ( 10. 9%) were revealed to be abnormal in terms of objective therapeutic evaluation. And this condition was so-call conjunctivochalasis recurrence with a recurrence rate of 10. 9%. No proliferation of granulation, narrowing inferior fornix and malfunction of eye movements was found.
CONCLUSION: Inferior - limbal - stem - cell transplantation to treat pterygium combined with conjunctivochalasis obtains better results. It proves to eradicate or relieve symptoms of patients effectively and ameliorate ocular-surface tear dynamics.
4. HPLC-TOFMS in rapid separation and identification of chemical components in Oldenlandia diffusa and its injection preparations

Objective To rapidly separate and identify the chemical components in traditional Chinese herbal medicine of Oldenlandia diffusa and its injection preparations by high performance liquid chromatography-time of flight mass spectrometry (HPLC-TOFMS). Methods An Agilent Zorbax XDB-Q18 column (250 mm×4. 6 mm, 5 μm) was used for separation and identification of chemical components in Oldenlandia diffusa, with a mobile phase of 0. 3% acetic acid (A) and methanol (B) in gradient elution, 0-30 min, 30%-90%B. The flow rate was set at 1. 0 ml/min and the injection volume was 10 μl. The time- of-flight mass spectrometer was equipped with an EIS ion source. The scanning mass range was between m/z 100-1 000. Results The traditional Chinese medicine of Oldenlandia diffusa and its injection preparation were on-line separated and characterized by HPLC-TOFMS, and 11 chemical compounds were identified in Oldenlandia diffusa, 6 compounds in market injection preparation, and 2 compounds in the injection prepared by ourselves. Conclusion Chromatographic demonstration of chemical compounds in Oldenlandia diffusa in one run provides a foundation for the further studying the metabolism and mechanism of Oldenlandia diffusa and its injection preparations.
5.Determination of Flavonoids in Rubus multibracteatus Levl.et Vant.

Objective To supply scientific basis for the reasonable development and utilization of Rubus multibracteatus Levl.et Vant.resources.Method The total flavonoids was extracted from Rubus multibracteatus Levl.et Vant.with 60% methanol and its content was determined by spectrophotometry.The reliability and repeatability of the method were tested through stability,repeatability,accuracy and recovery experiments.Result Both standard and tested solutions had the maximum absorbance at 510 nm.The RSD values of repeatability,accuracy and recovery experiments were 2.11%,0.97% and 2.4% respectively.The results of content determination showed that the total flavonoids in Rubus multibracteatus Levl.et Vant.was 3.8%.Conclusion Extracted with 60% methanol,colored with aluminiumnitrate and determined by spectrophotometry at 510 nm were suitable for the determination of total flavonoids in Rubus multibracteatus Levl.et Vant..
6.Microanatomical study of the endoscopy-assisted extreme lateral supracerebellar infratentorial keyhole approach to the dorsolateral mesencephalon region

Objective To explore the microanatomy technique of the endoscopy-assisted extreme lateral supracerebellar infratentorial keyhole approach and clinical significance.Methods The extreme lateral supracerebellar infratentorial keyhole approach was imitated in 10 vascular perfusion adult cadaveric heads fixed in formalin,the dorsolateral mesencephalon region was dissected and observed by microscopy and neuroendoscopy from December 2012 to March 2013.Results The extreme lateral supracerebellar infratentorial keyhole approach could be fully exposed the ambient cisterns,the posteromedial tentorial incisura,the dorsolateral mesencephalon region,the trochlear nerve,the root of the trigeminal nerve,the superior cerebellar artery,the posterior cerebral artery and other important anatomical structures.The temporal lobe and parahippocampal gyrus could be exposed when the tentorial was incised.The application of neuroendoscopy could clearly reveal the trigeminal nerve and surrounding structures.Conclusion The extreme lateral supracerebellar infratentorial keyhole approach is suitable for the surgical resection in dorsolateral mesencephalon region.The application of neuroendoscopy can make up for the lack of the trigeminal nerve observed under microscopy and expand the operative field.
7.Minimally invasive pie-crusting technique combined with arthrolysis for the treatment of the stiff knee.

OBJECTIVETo study the effectiveness of pie-crusting technique in improving the stiff knee.
METHODSFrom February 2012 to December 2013, 13 patients with stiff knee were reviewed retrospectively. There were 6 males and 7 females, ranging in age from 39 to 70 years old (averaged, 55.6 years old). Of the 13 cases, 8 patients had stiffness following fracture (comminuted tibial plateau fracture in 4, femoral supracondylar fracture in 3 and patellar fracture in 1), 5 patients had TKA-related stiffness.
RESULTSA follow-up lasted 8 to 12 months (mean 10 months)in 13 cases. The mean maximum flexion increased from (37 ± 6)° preoperatively to (52 ± 7)° after arthrolysis, and (108 ± 7)° after pie-crusting. At the final follow-up, mean maximum flexion was (105 ± 6)°. According to Judet evaluation system, 10 patients got an excellent result and 3 good. No major complications, such as extensor lag, skin necrosis, deep infection, dislocation of the patella or recurrent stiffness were found.
CONCLUSIONThe percutaneous technique of pie-crusting is a simple, minimally invasive and effective treatment for knee stiffness.

8.Association between fibrinogen and carotid atheromatous plaque in patients with ischemic stroke

BACKGROUND: Increase of fibrinogen is an independent risk factor for cardio-cerebrovascular disease and peripheral angiopathy. Fibrinogen and its degradation product exist in carotid atheromatous plaque and it can stimulate proliferation and migration of smooth muscle cells so as to play a key role in early atherosclerosis.OBJECTIVE: To analyze the relationship between plasma fibrinogen and carotid atheromatous plaque.DESIGN: Descriptive observation.SETTING: Department of Neurology, Xinhua Hospital, Medical Collegeof Shanghai Jiaotong University.PARTICIPANTS: A total of 81 patients with ischemic stroke were selected from Neurological Department of Xinhua Hospital affiliated to Medical College of Shanghai Jiaotong University from September to December 2001.There were 53 males and 28 females aged 41-85 years with the mean age of (65±11) years.METHODS: On the basis of carotid ultrasonography, 81 patients with ischemic stroke were divided into present plaque group (n=45) and absent plaque group (n=36) to measure plasma fibrinogen, serum total cholesterol (TC), low density lipoprotein cholesterol (LDL-C) and other risk factors of atherosclerosis so as to observe the correlation between risk indexes and carotid atheromatous plaque.MAIN OUTCOME MEASURES: Risk indexes of carotid atherosclerosis and carotid atheromatous plaque.RESULTS: All 81 patients were involved in the final analysis. ① Values of fibrinogen, TC and LDL-C in present plaque group were (4.38±1.33) g/L,(5.19±1.27) mmol/L and (3.15±0.37) mmol/L, respectively, which were higher than those in absent plaque group [(3.20±1.30) g/L, (4.56±1.30) mmol/L,(2.49±0.92) mmol/L, P < 0.05]. ② Ratios of carotid atheromatous plaque were 11% (5 cases), 16% (7 cases) and 73% (33 cases) in fibrinogen groups at Iow, moderate and high dosages, respectively. There was significant difference (P =0.002). ③ Multiple factor analyses of carotid atherosclerosis showed that the risk factors which promoted the formation of carotid atheromatous plaque were diabetes mellitus, low density lipoprotein (LDL), fibrinogen and age growing.CONCLUSION: Fibrinogen plays an active role in development and progression of carotid atheromatous plaque.
9.Treatment of type 2 diabetes mellitus patients of qi-yin deficiency phlegm-stasis inter-obstruction syndrome by jiangtang xiaozhi capsule and pioglitazone tablet: a non-inferiority randomized controlled trial.

UNLABELLEDOBJECTIVE; To evaluate the efficacy and safety of Jiangtang Xiaozhi Capsule (JTXZC) in treating type 2 diabetes mellitus (T2DM) of qi-yin deficiency phlegm-stasis inter-obstruction syndrome (QYDPSIOS), and to observe its effect on inflammatory factors and fibrinolytic factors.
METHODSBy adopting a randomization grouping, parallel control, and prospective study, 73 T2DM patients of QYDPSIOS were assigned to two groups by random digit table, the Pioglitazone Tablet group (36 cases, as the control) and the JTXZC group (37 cases). All patients maintained their basic therapies and lifestyle as previous after recruitment. Patients in the JTXZC group took JTXZC, 4 pills each time, three times per day, while those in control group took Pioglitazone Tablet, 15 mg each time, once daily. The therapeutic course for all was 8 weeks. The body weight (BW), the height, body mass index (BMI), waist circumference (WC), hip circumference, waist-to-hip ratio (WHR), and scoring of Chinese medicine (TCM) symptoms were observed. Levels of fasting blood glucose (FBG), 2 h postprandial blood glucose (2 h PBG), glycated hemoglobin (HbA1c), tumor necrosis factor alpha (TNF-alpha), nuclear factor kappaB (NF-kappaB), and plasminogen activator inhibitor 1 (PAL-1) were detected. The safety indices such as liver and renal functions and adverse reactions were also observed.
RESULTSCompared with before treatment, BW, BMI, HbA1c, and PBG were obviously lower after 8-week treatment than before treatment in the JTXZC group (P < 0.05). There was no statistical difference in post-treatment BW, BMI, HbA1c, or 2 h PBG between the two groups (P < 0.05). Compared with before treatment, levels of TNF-alpha and PAI-1 were lowered after 8 weeks of treatment in both groups (P < 0.01). The level of NF-kappaB was obviously lowered after 8 weeks of treatment in the control groups (P < 0.05), but it also decreased in the JTXZC group with no statistical difference. The scorings of CM symptoms were somewhat improved after treatment in the two groups (P < 0.01). Besides, better effects were obtained in the JTXZC group (P < 0.05). No severe adverse event occurred in either group during the whole therapeutic course.
CONCLUSIONSJTXZC showed similar therapeutic effect to pioglitazone. Both of them could effectively improve patients' clinical symptoms, the inflammation and fibrinolytic activities in different pathways, with no severe adverse reaction.
