1.Spectrum CT with Isotonic Mannitol Gastrointestinal Hypotonic Radiography in the Staging of Rectal Carcinoma

Purpose To evaluate the diagnostic efficiency of spectral CT imaging combined with oral isotonic mannitol gastrointestinal hypotonic radiography in the diagnosis of preoperative T and N staging of rectal carcinoma. Materials and Methods Forty-seven patients with rectal cancer confirmed by surgery and pathology were included in this study, all the subjects underwent oral isotonic mannitol gastrointestinal hypotonic radiography prior to CT scan, then abdominal and pelvic plain and double-phase contrast-enhanced CT scan in the spectral imaging mode was performed, the optimal contrast-to-noise ratio imaging were got by the spectrum analysis software, then T-stage was evaluated and compared with pathological T-stage; N-stage was evaluated using both traditional method and the spectral curve method, and compared with pathological N-stage. Results ① All the 47 patients were able to successfully complete the examination and the rectal filling and lesion display were satisfactory; ② the best diagnostic accuracy of T-stage by the monochromatic images was 83.0% (39/47), which had a general correlation with pathologic results (Kappa=0.713, P<0.001); ③ the accuracy, sensitivity, specificity, positive predictive value and negative predictive value of N-stage by the traditional method were 78.72%, 80.95%, 76.92%, 73.91% and 83.33%, respectively, which had a general correlation with pathologic results (Kappa=0.574, P<0.001); the accuracy, sensitivity, specificity, positive predictive value and negative predictive value of N-stage by the Spectral curve method were 89.13%, 75.00%, 100.00%, 100.00% and 83.87%, respectively, which had a fair correlation with pathologic results (Kappa=0.772, P<0.001). Spectral curve method had higher Youden index for N-stage evaluation than traditional method (0.64 vs. 0.58). Conclusion Oral isotonic mannitol gastrointestinal hypotonic radiography can provide a good rectal background, and it is a safe and reliable method for rectal examination, when combined with spectrum CT imaging, it is able to display the detail of lesions and identity the homology of lymph nodes, which might help to make more accurate preoperative T, N staging of rectal carcinoma.

Obesity has a harmful impact on health and is imposed by both environmental and genetic factors. Recently, over 60 obesity-related susceptible loci have been identified through genome-wide association study. However, it still remained unknown whether these loci were specific or generalized to the ethnic groups for genetic distinction. This review summarized the obesity-related susceptible loci in Asian populations, which were recently identified among Europeans or Asians.
4.Urethral realignment for treating early urethral injury under the guidance of ureteroscope

Objective To summarize the clinical experience of urethral realignment for treating early urethral injury under the guidance of ureteroscope,and evaluate its curative effect.Methods Twenty-nine male patients with urethral injury were selected,and 12 patients of posterior urethral injury,17 patients of former urethral injury.All the patients were treated with urethral realignment under the guidance of ureteroscope,postoperative indwelling catheter 3-8 weeks,every 7-16 days changed diameter increase 2 F the catheter 1 time.Results The 29 patients with urethral injury were a indwelling catheter success,all patients were no incontinence after operation 3 months.In the 29 patients,27 patients were urination unobstructed after catheter removal,2 patients were appeared urine line slim after 2 weeks,the 2 patients were normal urination after short urethral expansion.Conclusions The urethral realignment for treating early urethral injury under the guidance of ureteroscope has simple,lower complication,rapid recovery,better effect.The continuous flexible progressive urethral expansion and the strict nursing,which can effectively reduce the occurrence of urethral stricture.
5.Expression of 5-HT2A receptor in brainstem affects the occurrence of central sleep apnea in Sprague-Dawley rat

Objective To investigate the correlationship of 5-HT2A receptor expression in the brainstem and sleep apnea in Sprague-Dawley (SD)rat. Method PSG monitoring for sleep and sleep apneas scoring was performed in freely moving SD rats. The level of 5-HT2A protein in rat brainstem was detected by Western blot and the relationship of 5-HT2A level with sleep apneas was analyzed. Results Two types sleep apnea model were obtained in rats, one was post-sigh sleep apnea (PS) and the other was spontaneous apnea (SP). The sleep apnea index was negatively correlated with the amount of 5-HT2A receptor level in brainstem (r=0.672,P <0.001). The post-sigh sleep apnea index was negatively correlated with the amount of 5-HT2A level in brainstem (r=0.686, P<0.001). The NREM-PS index was also negatively correlated with the amount of 5-HT2A level in brainstem (r =0.663, P < 0.001),but the spontaneous apnea index was not correlated with the amount of 5-HT2A level in brainstem (P>0.05). Conclusion The expression of 5-HT2A receptor in brainstem was negatively correlated with the severity of post-sigh sleep apnea. This association implies that 5-HT 2A receptor plays a critical role in the respiratory network and is closely correlated with the occurrence of central sleep apneas.
6.A preliminary study on mechanism of experimental epilepsy induced by icv quinolinic acid in mice

The effect of GABA ergic system in the brain in mediating convulsions induced by quinolinic acid (QA) was studied. Muscimol,an agonist of GABAA-receptor, and aminooxyacetic acid (AOAA),an inhibitor of GABA transami-nase(GABA-T) ,and alprazolam which increased the affinity of GABA-receptor as it binds with benzodiazepine-receptor, were used to increase GABAergic function in the brain. Results showed that the above substances antagonized QA-induced convulsions. In contrast, bicu-culline,an antagonist of GABAA-receptor, and 3-mercaptopropionic acid (3-MP), an inhibitor of both GABA synthesis and its release, were used to decrease GABA ergic function in the CNS. They were found to potentiate QA-induced seizures. All these results suggest that GABA ergic system in brain plays an important role in modulating QA-induced convulsions.

Purpose:To study the influence of chemotherapy on radiotherapy in locally advanced nasopharyngeal carcinoma. Methods:96 patients with histologically proven nasopharyngeal carcinoma in stages Ⅲ,Ⅳ were divided randomly into two groups. Radiotherapy group (48 cases) only received radiotherapy with DT66—76Gy in nasopharynx and DT50—70Gy in the neck. Chemoradiotherapy group (48 cases) received chemotherapy with cisplatin and 5 fluorouracil before the same radiotherapy as the radiotherapy only group. 9.The effect of salmon calcitonin and physical therapy on lumbar spinal stenosis

Objective To study the effect of salmon calcitonin and physical therapy on lumbar spinal ste- nosis.Methods Eighty cases of lumbar spinal stenosis were divided into treatment and control groups.Physical therapy alone was given to the control group,but salmon calcitonin was injected intramuscularly in addition to phys- ical therapy for the patients in the treatment group.Visual analogue scale,range of motion (ROM),pain-free walking distance,tendon reflexes and functional independence measures (FIM) were observed to assess the re- sults.Results The VAS,ROM and walking distance of the treatment group improved more than those of the con- trol group,but the groups' tendon reflexes and FIM were similar.Conclusions Salmon caleitonin can reduce the symptoms of lumbar spinal stenosis,and it has special effects in relieving pain.
10.Application and advance of finite element method in spinal biomechanics

As a novel research tool in the field of the spinal biomechanics, the finite element method (FEM) complements and verifies the traditional research methods, which has been attached increasingly greater importance. In this paper, we briefly introduce the achievement made in the field of spinal biomechanics by FEM in the past decade, summarize the physiological and pathological behaviors of various spinal anatomical parts described by FEM, and present the basic view of using FEM to optimize the surgery instrument design.
