1.Diagnostic value of spectral CT with multimodal parameters in evaluation of lymphatic metastasis of gastric adenocarcinoma

Objective To evaluate the value of energy spectrum CT with multimodal quantitative parameters in diagnosis of the metastatic lymph nodes of gastric adenocarcinoma compared with the conventional CT.Methods 37 patients with gastric adenocarcinoma diagnosed by gastroscopy underwent enhanced CT scan with gemstone spectral imaging mode.We positioned and marked the stomach lymph nodes on CT images and compared with postoperative pathologic results one to one.ROC was used to identify the critical quantitative index of energy spectrum CT in distinguishing the metastasis from benign lymph node,and to evaluate the diagnostic effect compared with CT features.Results The 40-80 keV single energy spectrum curve slope (λHU )and normalized iodine concentration (NIC)in metastasis group were lower than those in the benign one in the arterial phase with statistical differences (P <0.05 ),but without statistical differences in venous phase.The combination of arterialλHU and NIC showed significant advantages compared with CT features(P <0.01), with the sensitivity,specificity and accuracy of 84.3% vs 61.4 %,88.4% vs 81.3 % and 86.8% vs 73.6 %.Conclusion The energy spectrum CT can provide quantitative analysis for differential diagnosis of gastric cancer metastasis from benign lymph nodes,and significantly improve the diagnostic accuracy of lymph node metastasis of the cancer.

3. Effects of anesthesia on neurodevelopment assessment in preschool children

Objective To investigate the effects of anesthesia on neurodevelopment of preschool children. Methods A total of 445 children, scheduled to undergo surgery in the Fourth Medical Center of Chinese PLA General Hospital from 1st May 2017 to 1st May 2018, were enrolled and, according to different test purpose, grouped as follows: (1) 120 children (GA group) who underwent surgery before Denver Developmental Screening Test (DDST) were matched to 325 unexposed children (Non-GA group). Meanwhile, 168 children (Naive group) were measured as blank control. (2) According to the number of anesthesia that children had undergone, those in GA group were assigned to three subgroups: single, twice and multiple groups. (3) Subgroup analyses was performed based on the time of cumulative duration of anesthesia exposures (less than 3 and greater than or equal to 3h). Data were collected with a questionnaire to evaluate the children's physical development, DDST results were recorded, and the effects were evaluated of the number of anesthesia and the time of cumulative duration of anesthesia exposures on the DDST results. Results For the children aged 0 to 6 yr, the DDST positive rates in Naive, Non-GA and GA groups were 6.0%, 6.5% and 12.5%, respectively. No significant difference existed in DDST positive rate between Naive group and Non-GA group (P=0.825). Compared with Non-GA group, the DDST positive rate increased in GA group (6.5% vs. 12.5%) with significant difference (P=0.038). Compared among the four domains of DDST separately, statistical difference was found only in terms of personal-social, those in GA group showed poor performance than in Non-GA group (P=0.025). For the children aged less than 3 yr, the DDST positive rates in GA group and Non-GA group were 18.6% and 3.9%, respectively, showing significant differences (P=0.019), but no statistical difference was found on DDST positive rate among the three groups of children aged 3 to 6 yr (P>0.05). In GA group, there was no increase in odds of early developmental vulnerability with increasing frequency of anesthesia exposure (P=0.784). However, the DDST positive rate was significantly higher with longer cumulative duration of anesthesia exposure (≥3h) than that of <3h (18.7% vs. 2.2%, P=0.008). Conclusions Exposure to anesthesia is an increased risk for the later neurodevelopment of preschool children, especially before 3 years old. The time of cumulative duration of anesthesia may be positively correlated to the children's neurodevelopment disabilities.
4.Micro-hardness and elastic modulus of Ti-30Nb-8Zr-2Mo alloy for dental implants

BACKGROUND: As dental implants, pure titanium and Ti-6Al-4V has achieved broad clinical applications, but they also contain toxic vanadium and aluminum element. Moreover, their elastic modulus is so high as to produce stress shield. OBJECTIVE: To examine the micro-hardness and elastic modulus of the self-made Ti-30Nb-8Zr-2Mo titanium alloy. DESIGN, TIME AND SETTING: An observational experiment was performed at the laboratory of College of Material Science and Engineering at Hebei University of Technology between March 2003 and February 2006. MATERIALS: Titanium alloy was prepared using titanium sponge (≥ 99% purify), niobium strip (≥ 99.9% purify), molybdenum powder (≥ 99% purify) and zirconium sponge (≥ 99.4% purify).METHODS: The micro-hardness of the specimens was determined after uniformly annealing, hot-forging and solution. Compression test was conducted on post-aging samples. MAIN OUTCOME MEASURES: Hardness and stress-strain curve.RESULTS: The maximal alloy strength was obtained after solution under 800 ℃ for 0.5 hours. Post-aging alloy's hardness was improved significantly although little change occurred on solution alloy. Compressive strength of alloy samples was 1 054 MPa, while elastic modulus reached 16.5 GPa. CONCLUSION: Both micro-hardness and elastic modulus of the self-made Ti-30Nb-8Zr-2Mo titanium alloy have satisfied performance requirements for dental implant materials.
5.Effect of pulsed electromagnetic fields of different daily treatment durations on biomechanical properties of femur in ovariectomized rats

Objective To observe the effect of pulsed electromagnetic fields(PEMFs)of different daily treatment durations on biomechanical properties of femur in ovariectomized rats,so as to find out the optimal daily treatment time.Methods Fifty female Sprague-Dawley rats were randomly divided into five groups:(1)SHAM control(no PEMFs treatment),(2)OVX control(no PEMFs treatment),(3)OVX I(PEMFs treatment at 8Hz fre- quency with 3.8 mT intensity,20 min daily for 30 days),(4)OVX 11(PEMFs treatment at 8 Hz frequency with 3.8 mT intensity,40 min daily for 30 days),and(5)OVXⅢ(PEMFs treatment at 8Hz frequency with 3.8 mT in- tensity,60 rain daily for 30 days).All the rats were subject to bilateral overiectomy except those in the SHAM control group.The biomechanical properties of the femur were assessed after 30 days of PEMFs treatment.Results The values of the parameters of the biomechanical properties obtained with the OVX control group were significantly lower than those of the other 4 groups(P0.05).Conclusion PEMFs of the three different daily treatment durations can maintain the biomechanical properties of the femur in ovariectomized rats.Under certain in- tensity(3.8 mT)and frequency(8 Hz),PEMFs of the three different treatment durations can significantly maintain biomechanical properties of femur in ovariectomized rats approximately to the nomal level,but among the three groups,the difference is not significant.It was shown that exposure to PEMFs for 20 to 60 minutes daily had similar effect of maintaining biomechanical properties of the femur in ovariectomized rats.
6.Urinary endometriosis.

OBJECTIVEAimed at illustrating the means of early diagnosis for prevention of the impairment of kidney function, the results of conservative medical therapy and the surgical treatment in patients with endometriosis of urinary tract.
METHODSSurgical interventions were done in 5 cases with pathological of endometriosis. Ureters were involved by endometriotic lesion in 3 patients, ureters and bladder in one and urethra in another patient. All of the cases were shown at least partial obstruction of the urinary tract by clinical symptoms, diagnostic examinations and the outcomes of various kinds of treatment.
RESULTSHydropnephrosis and hydroureter were shown by ultrasonography, IVP or retrograde pyelography in one case of ureteral obstruction. A small lump of endometriotic foci along the urethra was detected by manual examination and ultra-sonography in one patient. The relieving of the symptoms and signs of the obstruction of the urinary tract in two cases by GnRH-a or progest in was the evidence of the endometriotic lesion. There were two patients underwent surgical resection of the endometriotic foci surrounding the ureter after medical therapy.
CONCLUSIONSUrinary tract endometriosis are rarely seen and usually escape the attention of the physicians, while they usually cause obstruction of the urinary tract and finally impairment of kidney function if not treated in time. This kind of disease can be easily diagnosed by imaging detection with ultra-sonography, IVP or retrograde pyelography. Conservative treatment with GnRH-a and long acting progesterone as well as surgical treatments are effective for this kind of disease.

7.Protection of local intraspinal infusion With danshen injection against acute spinal cord injury

BACKGROUND: A large amount of cell dies of apoptosis in secondary injury stage after spinal cord injury.OBJECTIVE: To probe into the influence of local intraspinal infusion with danshen injection on necrosis and apoptosis of spinal cell after acute spinal cord injury.DESIGN: Randomized controlled experiment was designed.SETTING: Clinical Medical Trial Center of People' s Hospital Affiliated to Yunyang Medical College.MATERIALS: Forty-four Chinese white rabbits of first grade were employed, aged varied from 4 to 5 months, of either sex, mass weighted varied from 2.0 to 2.5 kg.INTERVENTIONS: The experiment was performed in Clinical Medical Trial Center of People' s Hospital Affiliated to Yunyang Medical College from June 2002 to July 2003. Two groups were randomized, named danshen group and the control group, 22 rabbits for each. In both groups, modified Allen method was used to prepare the model of incomplete spinal cord injury. In danshen group, danshen injection was infused from subdural catheter for 4times at 0. 3 mL/kg per day of the total amount (once every 6 hours). In the control group, physiological saline of same dose was injected. The animals were sacrificed at the 8th, 24th and 72nd hours successively after injection for pathological and histomorphologic observation, peroxide dismutase and malondialdehyde determination and determination of positive cell count of B-cell lymphoma-2 (Bcl-2), the apoptosis-inhibition gene.MAIN OUTCOME MEASURES: Cell apoptotic index and cell apoptotic rate in the region of spinal cord injury.RESULTS: Forty-four rabbits entered result analysis for all. ① Results of cell apoptosis: Apoptotic index in danshen group was less remarkably than that in the control group(13.10 ± 1. 38, 20.39±2.96, 4.101, P <0.01); cell apoptotic rate was lower remarkably than the control group[ (9.67 ± 1.09)%,(14.68±2.81)%, t=4.072, P <0.01] and Bcl-2 expression was more than that in the control group[ (19. 12 ± 4.74) /mm2, ( 13.37 ± 3.68) /mm2,t = 2. 347, P < 0.01 ]. ② POD content: The result in danshen group was higher thanthe control group[ (136.20 ± 13.64) NU/mL, (101.70 ± 15.24) NU/mL,t = 4. 132, P < 0.01 ]. ③ Malondialdehyde content: The result in danshen group was lower than the control group[ (1.27 ± 0. 22) nmol/mL,(2.54±0.69) nmol/mL, t=4.309, P <0.01] . ④Degeneration and necrosis of neuron and neural fiber: The result in danshen group was milder than the control group.CONCLUSION: After local infusion with danshen(Radix Salviae Miltiorrhizae), cell apoptosis was decreased in local spinal cord injury and cell necrosis was inhibited and alleviated after acute spinal cord injury.
8.Analysis of curative effect of chondroitin sulfate and glucnsamine sulfate on Kaschin-Beck disease

Objective To research the curative effect of chondroitin sulfate and glucosamine on Kashin-Beck disease(KBD). Methods According to Diagnosis for Kashin-Beck disease,80 patients of adult KBD were detected from Guanghui village Shangzhi city Heilongjiang province in July,2007,and they divided into treatment group and control group according to their condition,age and sex,40 person in each group. Treatment group was given chondroitin sulfate and glucosamine sulfate,and control group was given placebo(equivalent amount of starch). Bilateral knees X-ray films were shot before and after treatment (8th month),scale division magnifying glass was used to measure the width of joint space on X-ray films. Results The width of joint space respectively was (4.30±2.14) and (4.10±2.07)mm in control group before and after treatment,and treatment group respectively was (4.17±2.15),(4.16±2.11)mm. Medicine had no obviously role on joint space (F = 0.50,P > 0.05),Time and both of time and medicine had obvious role on joint space(F= 67.66,46.74,all P< 0.05). Joint space of treat group was thinner than control group(P < 0.05) before treatment,but thicker after treatment(P < 0.05). To compare with the width of before treatment,joint space width of control group became obviously narrow(P < 0.05). Conclusions Experimental group taking chondroitin sulfate and glucosamine sulfate alleviated knee joint space narrowing process of adults KBD patients compared with control group. It plays a protection role in articular cartilage and provides evidences for choosing drug and evaluating effect in the treatment of adults KBD.
9.Effect of qianlie huichun capsule on microstructure and ultranstructure of prostate glandular tissues in rats.

OBJECTIVETo investigate the effect of Qianlie Huichun capsule on the microstructure and ultranstructure of prostate glandular tissue in the model rat.
METHODHynertophy of prostate model rat was established by injecting testosterone to gelding male rats. After having been fed with Qianlie Huichun capsule for 30 days, the rats were killed and prostate tissues were resected for pathomorphological studies with microscope and electromicroscope, and the diameter of glandular lumer and the height of glandular epithelial cells were measured under the microspcope for different groups of rats.
RESULTIn the model groups, the glandular epithelial cells mutiplycated notably, showing stratified and pseudostratified cells that made the glandular lumer cramped. Under the electromicroscope, the glandular epithelial cells became high columnor and the rough endoreticulum extremely expanded. But in treatment groups, the change of the diameter of the glandular lumer and the height of the glandular epithelial cells were less remarkable than those in model groups. So the differerence between the model group and the treatment groups was remarkable (P < 0.01).
CONCLUSIONQianlie Huichun capsule can depress the glandular epithelialceu multiplication of prostate gland in model rats.

10.Effect of premenopausal hysterectomy on ovarian function.

OBJECTIVETo investigate the influence of premenopausal hysterectomy on the function of the conserved ovaries.
METHODSA retrospective survey was conducted by sending questionnaire to 521 cases of hysterectomized women (with the conservation one or two ovaries) for benign gynecological diseases and 1,600 cases of normal controls (with contact uterus and bilateral ovaries). Age, type of operation, diagnosis and the time of the onset of the menopausal syndrome were asked in the questionnaire.
RESULTSThe mean age of the onset of the hot flush (293 cases) in the hysterectomized group was (44.64 +/- 4.31) years, which was significantly lower than that of the normal group [(46.87 +/- 4.22) years, 197 cases] (P < 0.01). The mean age of the above 293 women undergoing hysterectomized was (42.84 +/- 4.37) years. The range of the operation was positively correlated with the time of the onset of the menopause syndrome. So there was only (1.99 +/- 2.40)-year interval between the hysterectomy and the onset of the hot flush.
CONCLUSIONHysterectomy with the conservation of bilateral/unilateral ovaries may have some certain influence on ovarian function.
