12 months postoperatively, and excellent results were obtained in 16.0% of the patients((4/25)),good in 40.0%(10/25),and poor in 44.0%(11/25). Conclusions For patients with locally advanced breast cancer previously treated with downstaging neoadjuvant chemotherapy,breast-conserving therapy offers satisfactory results.Strict adherence to technique and use of postoperative irradiation and systemic therapy are crucial to breast-conserving therapy.

Objective To investigate the therapeutic procedures, indications and efficacy of breast-conserving therapy (BCT). Methods BCT was carried out in 148 patients with breast carcinoma in this hospital from January 1996 to October 2003.There were 5 patients in stage 0,85 patients in stage Ⅰ,55 patients in stage Ⅱ,and 3 patients in stage Ⅲ.The surgical treatment was quadrantectomy or local wide excision of tumor, combined with axillary lymph node dissection.Routine adjuvant irradiation, chemotherapy and hormone therapy were given postoperatively. Results Pathological examinations of breast specimens found no positive margins.Follow-up observations for a median of 54 months (range,2 ~ 84 months) revealed a local recurrence rate of 2 7% ( 4/148 ;3 patients in breast and 1 patient in axillary fossa).Distant metastasis occurred in 5 patients (3 4%) and 3 of them died.Sites of metastasis included lung, liver, meninges and bone. The duration from surgery to distant metastasis was 6 ~ 43 months.Evaluation of cosmetic results in 108 patients who had received BCT for at least 12 months showed that good outcomes were achieved in 32 4% of the patients (35/108),fair outcomes,in 49 1% of the patients (53/108),and poor,in 18 5% (20/108). Conclusions For patients with early stage breast carcinoma, and part of those with local advanced breast cancer who have received neoadjuvant chemotherapy and gotten downstaging effects, the effects of BCT is satisfactory.Standard excision, postoperative irradiation and systemic combination treatment are crucial to BCT.Most patients receiving BCT have good cosmetic results.

Objective To assess the osteogenic ability after co-culture BMSC and ADSC in vivo and in vitro. Methods ADSC and BMSC were obtained by adherent screening method and enzymatic digestion method.Flow cytometry was used to confirm the phenotypes of ADSC and BMSC.Oil red O was used to induce MSC to fat.Alkaline phosphatase (ALP) and alizarin red staining were used in osteogenic group. This sample was divided into four groups,no-induced stem cells group;BMSC osteogenic induction group;ADSC osteogenic induction group;co-culture of BMSC and ADSC osteogenic induction group.ALP activities and Calcium absorbance were determined during different periods of osteogenic introduction.OCN and Runx2 expression level were tested via RT-PCR and western blot methods after osteogenic induction for 2 weeks.Furthermore, cells in each group were seeded on HA/CS/PLLA composite scaffolds, and the scaffolds with cells were planted into bone defects in rat models. The rats were sacrificed by overdose anesthesia at 8 weeks after surgery and the scaffolds were removed for further analysis. Results Oil red O staining demonstrated red after adipogenic induction.Alkaline phosphatase and Alizarin red staining showed flaky red under condition of osteogenic induction. There had no statistical change among each group after osteogenic induction for 3 days, and ALP activity significantly increased after osteogenic induction for 5 days.Meanwhile, the ALP activity in co-culture of BMSC and ADSC group was markedly higher than the other three groups. However, there had no significant change in A value of calcium absorbance among each group after osteogenic induction for 7 days, while it increased at 14th day and ALP activity in co-culture of BMSC and ADSC group was significantly higher than the other three groups. After osteogenic induction for 2 weeks, the mRNA expression of OCN and Runx2 in co-culture of BMSC and ADSC group was 78.24±8.11 and 1 180.13±121.16 respectively, and the protein expression of OCN and Runx2 was 6.54±0.59 and 4.43±0.51.These mRNA and protein expression level in co-culture of BMSC and ADSC group enhanced significant compared with the other 3 groups.Histological assay demonstrated that the new bone tissues formed in co-culture of BMSC and ADSC group were 497.75±7.44 μm2,which was larger than that in the other 3 groups at 8 weeks after implantation. Conclusion Co-culture BMSC and ADSC may up-regulated the osteogenic ability in vivo and in vitro.

Chinese Health Economics 2014;(7):33-36
To investigate different effects of fiscal health expenditure, household spending on health and social health expenditure on narrowing the gap between urban and rural health resource allocation. Methods: With the relevant data of China’ s medical and health through 1985-2011 years, taking methodology of the state space model to estimate the varying-time elasticity of different types of expenditures on urban and rural health resource allocation gap. Results: For narrowing the gap, household health expenditure played the leading role, fiscal health expenditure played smaller role and the social health expenditure played the supplementary role; the elastic of different health expenditure proportion was fluctuated before 2002, which became stable after 2002; it is easy to improve the “hard conditions” rather than the “soft conditions” . Conclusion: To accelerate the process of urban and rural medical security system integration, it is inevitable to establish an efficient configuration mechanism for urban and rural health expense, balanced develop urban and rural medical insurance system and scientifically guide social health investment.

Journal of Central South University(Medical Sciences) 2017;42(5):588-590
Partial deletion of the long arm of chromosome 7 is a rare disease and is prone to missing the diagnosis or being misdiagnosed. Here we present a case of a 13-year-old boy that showed symptoms such as growth-retardation,moderate intellectual disability, hypotelorism, microcephaly, epicanthal folds, genu varum and lumbar vertebral cleft, but it did not show serious symptoms like cleft lip, urogenital malformation and hypotonia. He was eventually diagnosed as partial deletion of the long arm of chromosome 7 syndrome through gene analysis.Considering the rare incidence of this disease and more rarely for being hospitalized to endocrine ward due to growth retardation, this case report can provide more information for clinic diagnosis and differential diagnosis for growth retardation.

Totally, 125 elderly inpatients with aspiration pneumonia (AP) were enrolled from departments of respiratory medicine and neurology at Zhongnan Hospital, Wuhan during June 2005 to October 2009 for analysis. Results showed that listlessness was manifested in 70 cases (56%), primary neurological illness in 80 cases (64%), 57 of them (46%) complicated with respiratory failure and 26 died with case-fatality of 21%. Main pathogen for them was Gram-negative bacillus, accounting for 57% of the total. Two kinds or more of antibiotics had ever been administered in 98 cases (78%), with an average length of antibiotics use for 28 days, ranging from one day to 128 days. Nasal feeding was instituted for those with choking over their feeding and coughing, and their inspired feeding was aspirated from the trachea or bronchi and lavished with bronchoscopy, with 38 cases by tracheal intubation, 26 by tracheotomy and 42 with artificial ventilation. Their average duration of hospital stay spanned 28 days with an average cost of 25 000 RMB yuan per capita, significantly higher than that for those with common pneumonia. In conclusion, clinical features of these elderly patients with AP were not so typical, most of them manifest severe, with lots of medical cost and bad prognosis.
