7.Efficacy and perioperative effect of docetaxel plus oxaliplatin and S-1 in treating advanced gastric cancer

Objective: This study investigates the effect of docetaxel + oxaliplatin + S-1 (DOS program) in treating advanced gastric cancer and surgical safety assessment. Methods: Fifty patients with advanced gastric cancer admitted to the Department of Gastrointestinal Surgery, The Affiliated Tumor Hospital of Xinjiang Medical University between January 2011 and May 2012 were enrolled in this study. These patients were randomized into the observation arm (n=25) and the control group (n=25). The observer group was administered three cycles of chemotherapy using a DOS program before surgical treatment, whereas the control group underwent surgery. Results: Compared with the control group, the clinical response rate (64.0%), D2 lymph node dissection rate (88.0% vs. 64.0%), and R0 resection rate (92.0%vs. 68.0%) in the observation group were significantly higher (P<0.05). Moreover, the number of postoperative lymph node metastasis in the observation group was significantly less than that in the control group (3.2±2.5 vs. 6.3±2.9, P<0.05). The operative time (230.5 min±45.6 min vs. 205.6 min±42.4 min) and intra-operative blood loss (425.5 mL ±115.4 mL vs. 210.6 mL±125.6 mL) of the two groups were sta-tistically significantly different (P<0.05). The incidence of postoperative complications and lymph node sweeping number of the two groups showed no significant difference (19.6 ±2.8 vs. 21.2 ±2.0, P>0.05). The patients exhibited good tolerance to chemotherapy, with bone marrow suppression and gastrointestinal reactions as the main adverse effects. Conclusion:The DOS program is a highly efficient, advanced gastric cancer neoadjuvant chemotherapy. The program can improve patient survival and has good patient tolerance and compliance, good peri-operative safety, high R0 resection rate, and low postoperative lymph node metastasis rate.
8.EFFECT OF HEPATITIS B VIRUS X-GENE HETEROGENEITY ON ITS TRANSACTIVATION

Polymerase chain reaction(PCR) was employed to amplify the whole HBV X region from the serum of patients with chronic hepatitis B virus(HBV) infection, and then the PCR products were subcloned into pGEM Teasy vectors. Clones were randomly selected to be sequenced. Comparison of the cloned sequence was made to look for the differene. The sequencing results showed that each sequence of selected clones was differenct and there were HBV quasispecies groups in patients. There was hot deletion region near the 3' end of X gene. To address whether the mutations were responsible for the transactivating effect,the wild type(wt) and the mutants of HBV X genes were subcloned into the EcoRI site of pcDNA3 1( ) vectors, and the recombinant plasmids were cotransfected into HepG2 cells with reporter plasmid pSV lacZ,respectively. The activity of ? galactosidase controlled by SV40 early promoter/enhancer was detected, which reflected the transactivating function of HBx protein. The cotransfection results indicated that the wt HBV X gene acted as a transactivator on the SV40 early promoter/enhancer, and the mutations which caused premature termination of the X open reading frame reduced their transactivating effects to certain extent.

10.Comparison of concurrent chemo-radiotherapy and sequential chemo-radiotherapy for locally advanced non-small cell lung cancer

Objective Prospective comparison was done on concurrent chemo-radiotherapy and se- quential chemo-radiotherapy for unresectable stageⅢnon-small cell lung cancer(NSCLC) and to evaluate three different regimens of concurrent chemo-radiotherapy.Methods Ninety-six such patients were ran- domized into four groups:1.sequential chemo-radiotherapy group received two cycles of induction chemother- apy with 40 mg/m~2 of cisplatin on D 1-3,29-31 and 100 mg/m~2 of etoposide on D 1-3,29-31 before conven- tional radiotherapy,2.concurrent chemo-radiotherapy group 1 received 100 mg/m~2 etoposide on D 1-3 and DDP 40 mg/m~2 on D 1-3,D 29-31,iv.drip,3.concurrent chemo-radiotherapy group 2 received concurrent chemotherapy with 40 mg/m~2 of paclitaxel every Monday during conventional radiotherapy,4.concurrent chemo-radiotherapy group 3 received concurrent chemotherapy with 40 mg/m~2 of paclitaxel every Monday during three-dimensional conformal radiotherapy.All patients were irradiated with 2.0 Gy/fraction,5 frac- tions/week,to a total dose of 60-64 Gy.They all received two cycles of consolidation themotherapy with 40 mg/m~2 of cisplatin on D 1-3 and 100 mg/m~2 of etoposide on D 1-3.Results The overa/1 response rate was 67%,71%,71% and 79% for sequential ehemo-radiotherapy group,concurrent chemo-radiotherapy group 1,2 and 3,respectively.There was a significant difference between the concurrent chemo-radiotherapy and sequential chemo-radiotherapy(P<0.05).The 1-,3-and 5-year overall survival rate(OS) was 54%,8% and 4%;71%,17% and 8%;79%,17% and 8%;83%,46% and 13%,respectively for the four groups. The difference among all these groups(P=0.017) was significant.It was also significant between the con- current chemo-radiotherapy group 1 and 3 (P=0.046).The difference of distant metastasis rate among all the groups was statistically insignificant (P>0.05) also was the difference of toxicity (P>0.05),but the severe toxicity of concurrent chemo-radiotherapy groups 1 and 2 were higher than the sequential chemo-radio- therapy group and concurrent chemo-radiotherapy group 3.Conclusions Better locoregional progression- free survival and overall survival of unresectable stageⅢnon-small cell lung cancer could be achieved by concurrent chemo-radiotherapy as compared with sequential chemo-radiotherapy though at the expense of in- crease in toxicity.With the combination of concurrent chemo-radiotherapy and conforrnal radiotherapy,the o- verall survival rate could be much improved with miider toxicity.
