Objective To evaluate the importance of clinical screening and follow up by direct colonoscopy for colorectal cancer at an early and curable stage. Methods There were 2 196 elderly people aged between 60 to 89 years. The clinical screening by direct colonoscopy was performed according to the protocol. 1 740 of 2 196(79.2%) patients were followed up every year. Results Fifty two elderly persons were found to be colorectal cancer patients by colonoscopy, with the detectable rate being 2.4%. Nineteen were diagnosed early stage colorectal cancer, accounting for 36.5% of the detected colorectal cancer. Nine among the followed up cases were detected early colorectal cancer, accounting for 45 0% of the detected colorectal cancer. The resectable rate and the 5 year survival rate was 97 7% and 80 9% for colorectal cancer, respectively. 98 9% of the cecum intubation cases was successful. The incidence of complication for colonoscopy was 0 05%. Conclusions By clincal colonscopy screening and follow up study for colorectal cancer and precancerous changes in the elderly, the patients with adenomatoid polyps were early diagnosed and treated, so it raised the detectable rate of early colorectal cancer and the level of grade prevention of colorectal cancer.

OBJECTIVE:Studies have confirmed that basic fibroblast growth factor stimulating periodontal ligament cells can promote the proliferation of human periodontal ligament cells.so as to facilitate the reconstruction of lost periodontal tissues.By use of different concentrations of basic fibroblast growth factor, the in vitro cultured normal human periodontal ligament cells were stimulated to observe the decorin gene expression in the periodontal ligament cells.METHODS:The periodontal ligament cells were isolated and cultured using trypsin digestion method. The third generation of cells at logarithmic growth phase were preserved in DMEM containing 10% DMSO and 20% FBS frozen crvopreservation fluid, then moved into liquid nitrogen preservation at the second day Immunohistochemistry was used to identify anti-vimentin and cytokeratin staining The sixth generation of human periodontal ligament cells were divided into experimental group and control group. The experimental group was cultured in DMEM culture medium containing basic fibroblast growth factor at the concentration of 0.1,1,10 μg/L under standard conditions for 24 hours;control group was cultured with DMEM culture medium under standard conditions for 24 hours. The intracellular decorin gene expression was determined using RT-PCR.RESULTS:By microscopic observation, the cells in the control group had no significant changes, and cells proliferated in the experimental group, before stimulation the cells at the bottom of the bottle became denser than those in sparse regions.Adding basic fibroblast growth factor into periodontal ligament cells, could significantly decrease the mRNA expression of decorin, along with the increase of basic fibroblast growth factor concentration, the inhibition effect gradually weakened and became the strongest in the 0.1 μg/L but the weakest at 10 μg/L.CONCLUSION:The basic fibroblast growth factor stimulation can promote the proliferation of periodontal ligament cells and inhibit the synthesis of decorin in a dose-dependent manner,0 1 μg/L induces the strongest inhibitory effect.

Objective To evaluate the clinical effect of dual-plate for treatment of comminuted distal humerus fractures in adult. Meth-ods 28 patients with distal humeral fractures were treated with condylar anatomical plate interal fixation. The supracondylar fracture was fixed with condylar anatomical plating, and the two plates were kept in 90°with each other. Results The 28 cases were followed up average-ly 26 months (12 to 34 months). All fractures healed and there was no delayed union or nonunion cases. According to Cassebaum scoring system, there were 9 cases of excellent,14 cases of good, 4 cases of moderate and 1 case of bad. According to Morrey-Chao scoring system, there were 16 cases of cases,11 cases of good. The average time of bone healing was 10 weeks. Conclusion In treatment of comminuted distal humerus fractures in adult, open reduction and dual-plate internal fixation can get good clinical results. The placement of the dual-plate may depend on surgeon experience and preference and the fracture pattern present.

Chinese Acupuncture & Moxibustion 2015;35(4):397-399
The relationship between acupuncture and stress is discussed from three aspects, including is it possible for acupuncture stimulation to be a stressor, whether acupuncture will start stress reaction, and whether acupuncture effects contain some stress factors. It is believed that correct acupuncture manipulation will not cause stress response, however, under some circumstances, such as inaccurate manipulation, improper treatment or patients who are very nervous but do not receive effective intervention, acupuncture is likely to cause stress response. Acupuncture-induced stress response is totally different from acupuncture anti-stress. The possible stress factors in acupuncture effect are explored, which can provide a new angle for the research on action mechanism of acupuncture. From the view of stress to review acupuncture treatment, there are three enlightenments: emphasizing on communication between doctors and patients, avoiding over-pursuit of deqi and focusing on analysis of the body constitution.

Chinese Journal of General Practitioners 2013;(5):370-373
Objective To explore the effectiveness of dietary treatment in reducing macrosomia risks for pregnancies with borderline gestational glucose intolerance (BGGI).Methods From July 2009 to June 2011,a total of 1046 pregnant women with BGGI were randomized into group A (intervention,n =525) and group B (non-intervention,n =521).Another 521 pregnancies with normal glucose screening were assigned into group C (normal control).Randomization was applied following stratification according to age, body mass index (BMI),prior Cesarean section (C-section) and multiparity, etc.Women in group A underwent the examinations of fasting plasma glucose (FPG),2 h-post prandial glucose and HbA1c once every 2 weeks.Their newborn outcomes were collected for analysis. Results Women of three groups were similar in age, parity, initial BMI and initial FPG.Dietary treatment for group A improved glucose-related indices and women's pregnancy weight gain (P <0.0l).Also,in comparison with group B,the intervention of group A reduced risk of macrosomia (9.14% vs.13.82%,P =0.02),prior C-section rate (43.87% vs.56.07%,P < 0.01) and postpartum hemorrhage (3.81% vs.7.10%,P =0.02).However these indices were no better than group C.Dietary treatment did not increase the risk of fetal growth restriction, neonatal hypoglycemia and hyperbilirubinemia. Conclusion As a simple noninvasive therapeutic measure for improved glucose tolerance,BGGI may reduce the risk of risk of macrosomia and prior C-section rate.

Objective To observe the levels of Foxp3+ CD+ CD25+ regulatory T cells in the peripheral blood of condyloma acuminatum (CA) patients and investigate their roles in the pathogenesis of CA. Methods The peripheral blood was collected from 30 CA patients (including 15 with relapsing and 15 with first onset) and 20 healthy controls. Peripheral blood mononuclear ceils (PBMC) were isolated and stained with anti-human CD4-PE-Cy5 and anti-human CD25-fluorescein isothiocyanate (FITC) monoclonal antibodies on cell membrane, followed by intraeellular staining with anti-human Foxp3-PE. The percentage of Foxp3+ CD4+-CD25+ regulatory T cells was detected by three-color flow cytometry. Comparison between groups was done by ANOVA test. Results The percentages of Foxp3+ CD4+ CD25+ regulatory T cells among total CD4 + T cells in CA patients and relapsing CA group were (3.4 ± 1.0) % and (4.7 ±+ 1.2) %, respectively, which were both significantly higher than that in healthy control group [(1.2±0. 5)%, P<0.01]. Furthermore, that in first onset CA group was (2. 1 ± 1.0) %, which was higher than that in healthy control group, but without statistical significance; but that in relapsing CA group was significantly higher than that in first onset group (P<0.05). Conclusions The number of Foxp3+ CD4+ CD25+ regulatory T cells increases in the peripheral blood of CA patients. The disorder of cellular immunity may be involved in the immunopathogenesis of CA.
