1.Dynamic changes and clinical significance of pulmonary function in segmental mycoplasma pneumonia children

Objective To study the dynamic changes and clinical significance of pulmonary function (PF) in segmental mycoplasma pneumonia (SMPP) children and make the dynamic analysis of X-ray features.Methods Eighty-three SMPP children treated from June 2013 to December 2014 in Jinshan Institute of the Sixth People's Hospital of Shanghai were selected and the PF changes at acute phase, recovery phase and rehabilitative period was monitored and all the patients received the imageological diagnosis.The Pulmonary function includes forced vital capacity (FVC), forced expiratory volume in the first second (FEV1), forced expiratory volume ratio(FEV1/FVC), peak expiratory flow(PEF), and maximum expiratory flow (FEF25-75).Pulmonary function and imageological diagnosis of two groups was compared.Results Compare the acute phase and recovery phase and rehabilitative period of 83 SMPP Children, at acute phase, FVC ((1.58±0.49), (1.76 ±0.62),(2.08±0.84) L),FEV1((1.27±0.46),(1.58±0.53),(1.83±0.66) L),FEV1/FVC((79.64±6.61)%,(85.25±7.38)%,(87.24±8.61)%),PEF((2.61±0.84),(3.15±0.92),(3.52±1.06) L/s), FEF25 ((2.29±0.83), (2.86± 0.95), (3.26± 0.98) L/s), FEF50 ((1.51 ± 0.52), (2.12 ± 0.64), (2.26±0.63) L/s),FEF75((0.58±0.42),(0.76±0.46),(1.02±0.42) L/s) and FEF25-75 ((0.61±0.33),(0.87±0.36), (1.01 ±0.41) L/s) of two groups were reduced, especially FEF25, FEF50, FEF75 and FEF25-75.At recovery phase and rehabilitative period ,the indicators were significantly better than those of acute phase (P< 0.05);for SMPP children at acute phase, FVC ((1.51 ± 0.44), (1.31 ± 0.36) L) and FEV 1 ((1.46±0.56), (1.21±0.48) L) in the lesions of multiple parts was significantly lower than that of lesions of single part(P<0.05).At acute phase, X-ray majorly showed the unilateral lesion with increased density and blurring edges in lung lobe or segment.After 14 d, the condition was obvious improved;a few cases still had the blurring textures and thickening conditions after 2 weeks.Conclusion For SMPP children at acute phase,the major and micro airway function is damaged in different extent.It is majorly expressed as restrictive ventilation dysfunction,especially micro airway function.At recovery phase, PF is obviously improved.The damage of major airway function in the multiple parts of lung lobe is more serious than that of lesions in the single part.
2.Application of CD-4 theory in determining the width of implant in breast augmentation

Objective To introduce a new method (CD-4theory) to determine the width (W) of the implant and to explain the reasons why to do so in details.Methods From January 2006 to August 2015,the authors have found and applied CD-4 theory to determine the width (W) of breast implant in dual plane Ⅰ or Ⅱ breast augmentation cases through transaxillary or periareolar incision for 630 patients.CD was defined as the curved distance on skin from the midline of the sternal bone to the anterior axillary line (AAL) on the lateral chest wall through the horizontal level on inferior mammary fold (IMF).W =CD-4.0 cm or 3.5 cm.Results The 630 patients were treated by using both round and anatomic implants with width from 10.5 cm to 12.5 cm.Their CDs were from 14.5 cm to 17 cm.78% of the patients were followed up from 1 month to 5 years.Except for 5 patients were unilateral capsular contractions,all the other patients were satisfied with their nature new breast shapes and volumes.Their new intermammary cleavages without bras were between 1-2.5 cm and lateral borders of breast were on the area of the anterior axillary line (AAL).Conclusions Width of the implant is equal to CD-4.0 cm when doing dual plan Ⅰ or Ⅱ breast augmentation.For the very thin patient,the width should be 3.5 cm.
3.Children's motor nerve conduction: the normal value and developmental regular rule

Objective To search for the normal value and developmental regular rule of motor norve conduction in children.Methods One hundred and fourty-nine children aged from 0 to 14 years old were divided into 7 groups in accordance with age. There were 19 pecrsons aged from 20 to 35 years old in adult group. The motor nerve conduction function of median nerve, ulnar perve, tibial nerve and peroneal nerve of every suhject was determined by nerve evoked potential meter.Results The terminal latency of action potential of every nerve is decreased along with growing up of age before 6 years old and increased after 6 years old. The conduction velocity of ulnar nerve is the fastest,then the peroneal nerve and median nerve, that of tibial nerve is the slowest. Median nerve development quicken after 3 months of birth and approch ulnar nerve at the time of 3 years old. Tibial nerye get into fast development period from 3 months to 1 years old and catch up peroneal nerve. The conduction velocity of every nerve extend to adult level in th period of 3~6 years old.Conclusion The every parameler of nervous conduction has a great difference of age in the period of child,particularly in that of infancy. therefore, the normal values should be set up in accordance with proper age groups. This study shows that it's suitable to divide groups in accordance with new-born, 3 and 6 months, 1, 3 and 6 years old. Adult criterion should be used in the children dbove 6 years old.
4.Changes on blood glucose of different operation scheme after gastrectomy for gastric cancer in patients complicated with diabetes mellitus

Objective To observe the effect of gastrectomy for gastric cancer on blood glucose in patients complicated with type 2 diabetes mellitus by using different operation strategies.Methods The retrospective analysis was carried out of 68 cases who undergone radical surgery for gastric cancer and complicated with type 2 diabetes mellitus.These patients were divided into 3 groups by different kinds of digestive tract reconstruction,Billroth Ⅰ (n =26),Billroth Ⅱ (n =22) and Roux-en-Y (n =20).The levels of fasting blood glucose (FBG) were detected before and after operation one week,one month and six months in three groups.Results Comparing to the level before operation,the level of FBG after operation showed a descent in group B and group C((10.03 ±5.12) vs.(13.56 ±3.36) mmol/L; (8.44 ±5.21) vs.(13.56 ±3.36) mmol/L;(10.61 ±3.16) vs.(12.84 ±3.36) and(7.68 ±4.88) vs.(12.84 ±3.36) mmol/L) which was better than group A((10.03 ±5.12)vs.(11.12 ±6.32),(8.44 ±5.21) vs.(11.12 ±6.32) mmol/L;(10.61 ±3.16) vs.(11.12 ±6.32) mmol/L),(7.68 ±4.88) vs.(11.12 ±6.32) mmol/L;P<0.05,but there was no significantly difference between B and group C (P > 0.05).The 3 groups of patients,with B,C groups of patients with diabetes improved obviously,of which nearly 50% of patients cured,normal blood sugar control,the other part with the exception of a few effective,degradation of diabetes drug treatment,and operation effect was significant.Conclusion It is effective to control the blood glucose of gastric cancer patients complicated with type 2 diabetes with Billroth II or Roux-en-Y gastrectomy.There is a correlation between postoperative blood glucose change and different gastrojejunal anastomosis.

This paper is aimed to study the effects of nitrogen form on the growth and quality of Chrysanthemums morifolium at the same nitrogen level. In order to provide references for nutrition regulation of Ch. morifolium in field production, pot experiments were carried out in the greenhouse at experimental station of Nanjing Agricultural University. Five proportions of ammonium and nitrate nitrogen were set up and a randomized block design was applied four times repeatedly. The results showed that the growth and quality of Ch. morifolium were significantly influenced by the nitrogen form. The content of chlorophyll and photosynthesis rate were the highest at the NH4(+) -N /NO3(-) -N ratio of 25:75; The activities of NR in different parts of Ch. -morifolium reached the highest at the NH4(+) - N/NO3(-) -N ratio of 0: 100. The contents of nitrate nitrogen in the root and leaves reached the highest at the NH4(+) -N/NO3(-) -N ratio of 50:50. The activities of GS, GOGAT and the content of amylum increased with the ratio of NO3(-) -N decreasing and reached it's maximum at the NH4 + -N/NO3 - -N ratio of 100: 0. The content of ammonium nitrogen were the highest at the NH4 + -N /NO3 --N ratio of 75: 25, while the content of soluble sugar reached the highest at the NH4(+)-N/NO3(-) -N ratio of 25: 75. The content of flavones, chlorogenic acid and 3,5-O-dicoffeoylqunic acid were 57.2 mg x g(-1), 0.673% and 1.838% respectively, reaching the maximum at the NH4(+) -N /NO3(-) -N ratio of 25:75; The content of luteoloside increased with the ratio of NO3(-) -N increasing and reached it's maximum at the NH4(+) -N/NO3(-) -N ratio of 0: 100. The yield of Ch. morifolium reached it's maximum at the NH4(+) -N /NO3(-) -N ratio of 25:75. Nitrogen form has some remarkable influence on the nitrogen metabolism, photosynthesis and growth, Nitrogen form conducive to the growth and quality of Ch. morifolium at the NH4(+) -N /NO3(-) -N ratio of 25: 75.

8.Non-osteogenic malocclusion after anatomic reduction and miniplate rigid fixation in condylar neck and subcondylar fractures.

OBJECTIVETo analysis the cause of malocclusion that was found in patients suffering from condylar neck and subcondylar fractures after perfectly anatomic reduction and rigid fixation.
METHODSThere were 23 cases of malocclusion in the patients of condylar neck and subcondylar fractures after anatomic reduction and rigid fixation during 1994 to 2001. The possible reasons were studied, and the treatment methods were proposed.
RESULTSBoth distortion and(or) fracture of miniplates and displacement anain of the condylar process after operation were not found in all 23 cases. There were 6 cases of open bite at the molar teeth of the fractured side, while the normal occlusions were showed in the nonfractured side. 11 cases showed that the whole mandibles were moved slightly to the nonfractured side. 4 cases showed that the mandible of fractured side moved slightly to the nonfractured side and the occlusion of nonfractured side were normal. 2 cases showed premature contact at the fractured molar teeth and the anterior teeth showed open bite. There were 5 cases of facial nerve damages, 2 cases wound infection, 2 cases TMJ chronic pain and 1 cases TMJ click and(or) murmur.
CONCLUSIONSAlthough the anatomical reduction and miniplate rigid fixation were operated on the condylar neck and subcondylar fractures, there were probably having the chance of showing malocclusion. The malocclusion was not caused by the fracture itself, but in the TMJ and/or muscular injury. Therefore, the malocclusion can be cured with intermaxillary elastic traction and functional exercise. Sometimes, the occlusal adjustment may be performed to resume the normal occlusion. In any case, operation is not needed.

9.Spectrum-effect relationship of Moutan cortex against lipopolysaccharide-induced acute lung injury.

This research is to study the relationship between HPLC fingerprints of Moutan Cortex, Paeoniae Radix Rubra and Paeoniae Radix Alba and their activity on lipopolysaccharide-induced acute lung injury. HPLC fingerprints of each extract of Moutan Cortex,Paeoniae Radix Rubra and Paeoniae Radix Alba were established by an optimized HPLC-MS method. The activities of all samples against protein and tumor necrosis a factor were tested by the model of lipopolysaccharide-induced acute lung injury. The possible relationship between HPLC-MS fingerprints and the activitieswere deduced by the Partial least squares regression analysis method. Samples were analyzed by HPLC-MS/MS to identify the major peaks. The results showed that each sample had some effect on acute lung injury. Four components with a lager contribution rate of efficacy were calculated by the research of spectrum-effect relationship. Moutan Cortex exhibited good activity on acute lung injury, and gallic acid, paeoniflorin, galloylpaeoniflorin and paeonol were the main effective components.
