1.Effect of aspirin resistance on cardiovascular events in non-insulin-dependent diabetes mellitus patients

Objective To explore the effect of aspirin resistance on cardiovascular events in non-insulin-dependent diabetes melli-tus patients .Methods 496 patients with non-insulin-dependent diabetes mellitus received daily aspirin therapy (100mg/d) 4 weeks were recruited from Jun .2006 to Sept .2010 .Platelet aggregation was measured by light transmission aggregometry .The patients were divided into three groups :aspirin resistance group ,aspirin semi-resistance group and aspirin-sensitive group .The clinical bio-chemistry indications and the adverse cardiovascular events were collected .Results 11 .69% of patients were found to be resistant to aspirin therapy ,and 37 .90% of patients to be aspirin semi-responders .There were 67 .48% female in aspirin resistance group and aspirin semi-responders .The preprandial glucose of group AR and ASR was higher than that of group AS (P<0 .05) .The HbA1C and TG of group AR were higher than those of group AS(P<0 .05) .There was statistically significant difference of the rate of car-diovascular events .Conclusion The measure of platelet aggregation should be taken in the diabetic mellitus patients and pay atten-tion on the dose of aspirin recommended .Particular attention is should also devote to the incidence of adverse cardiovascular events .
2.THE QUANTITATIVE ANALYSIS OF FRACTIONS OF GLUCOSE OLIGOSACCHARIDES BY HIGH PERFORMANCE LIQUID CHROMATOGRAPHY

The contents of the individual fractions of glucose oligosaccharides were determined simultaneously by means of reversed-phase HPLC. The sample was dissolved in water and filtered and the filtrate was used directly for the analysis. The sugar was separated on a column of u-Bondapak C18 using water as the mobile phase and determined refractometrically.The method was applicable to the analysis of glucose oligosaccharides from monomeric glucose through polymeric maltooctose. Quantitative results were obtained for glucose, maltose, maltotriose, maltotetrose, maltopentose and maltohexose in the samples prepared in our laboratory. Recovery tests revealed this method reliable.
3.Effects of sufentanil combined with bupivacaine on postoperative analgesia and immunity function in elderly patients under lower extremity orthopaedic surgery

Objective To investigate effects of intrathecal injection of sufentanil combined with bupivacaine on postoperative analgesia and immunity function in elderly patients under lower extremity orthopaedic surgery.Methods Totally 60 elderly patients with ASA Ⅰ or Ⅲ under lower extremity orthopaedic surgery were randomly divided into two groups:bupivacaine and sufentanil group (each n =30).3 ml bupivacaine (2 ml 0.75% bupivacaine plus 1 ml 0.9% normal saline) were given in bupivacaine after successful spinal anesthesia and 3 ml sufentanil (1 ml 0.75 % bupivacaine plus 1 ml 0.9% normal saline and 1 ml with 5 μg sufentanil) were used in sufentanil group respectively.The anesthetic feeling and block level before operation recovery time were recorded postoperation.Visual analogue scale (VAS) and adverse reaction were recorded at 1,6,12,24 and 48 h after surgery.Cellular and humoral immunity function were recorded at the following time points:T0,the day before surgery; T1,the day after surgery; T2,the first 5 postoperative days; T3,the first 10 postoperative days; T4,the first 15 postoperative days.Results The sensory and motor block levels in sufentanil group were T9~~T10 and T11~T12 respectively,which were higher than in bupivacaine (T7~T8 and T8~T10).The recovery time in sufentanil group was shorter than in the bupivacaine (P<0.05).There were no significant differences in VAS scale between the two groups at any postoperative time points (P<0.05).The incidence of skin pruritus was higher in sufentanil group (6 cases) than in bupivacaine (P<0.05).There were significant differences in the indexes for the immune condition including CD3+,CD4+/CD3+,CD4/HLADR,CD8/HLADR,B-cell,NK-cell between time points of T2[(48.7±2.3)%,(33.4±1.8)%,(10.2±1.8)%,(0.75±0.07)%,(1.02±0.16)%,(3.12 ±0.19) %,(2.53±0.14)%,respectively] and T0 in bupivacaine (P<0.05).There were significant differences in the indexes for the immune condition including CD3+,CD4+/CD3+,CD4/HLADR,CD8/HLADR,B-cell,NK-cell at T2 between sufentanil group [(54.5 ± 2.4) %,(36.3 ± 1.7) %,(15.1±1.6)%,(1.32±0.11)%,(1.25±0.14)%,(4.96±0.25)%,(4.17±0.10)%,respectively] and bupivacaine (P<0.05).Conclusions Intrathecal injection of 5 μg sufentanil combined with low concentration bupivacaine can be safely and effectively applied in elderly patients under lower extremity orthopaedic surgery,and can reduce local anesthetic concentration,shorten recovery time of anesthesia and accelerate the recovery of postoperative immunity function.
4.Charlson index combined with agestratification used to assess the tolerance of elderly patients with esophageal carcinoma to chemoradiotherapy

Objective To assess the tolerance of elderly patients with esophageal carcinoma to chemoradiotherapy through.Methods A retrospective analysis of 596 elderly patients with esophageal carcinoma receiving chemoradiotherapy was conducted.Patients were divided into radiachemotherapy group and radiotherapy group,and according to age,patients were divided into four age groups (aged 70-74,75-79,80 80 and 85years and over).The tolerance to treatment was assessed by Charlson index.Results 185 patients completed chemoradiotherapy,113 patients reduced the dose of chemotherapy (reduction group),81 patients unfinished chemoradiotherapy (unfinished CRT group),160 patients completed radical radiotherapy alone (unfinished RT group),and 57 patients incompleted radiotherapy.Further analysis found that the tolerance to treatment in reduction group had significant differences between patients aged ≥ 75 years and < 75 years (x2 =6.815,P=0.009),and between WIC ≥ 1 and < 1(x2 =10.636,P=0.001); the tolerance to treatment in unfinished CRT group had significantly differences between aged ≥ 80 years and <80 years (x2 =63.842,P=0.000),and between WIC =0,1 and ≥ 2 (x2 =21.153,P=0.000).Conclusions Further age re-grouping and WIC assessment is necessary before treating esophageal cancer patients.Reduction of therapy dosage is more beneficial for patients aged ≥ 75 years and WIC≥1,and greater caution is required in chemotherapy for patients aged ≥ 80 years and WIC>1.
5.Risk factors of periventricular-intraventricular hemorrhage in extremely low birth weight infants

Objective To study the risk factors of periventricular-intraventricular hemorrhage (PIVH) in extremely low birth weight infants(ELBWI). Methods A retrospective study was performed in 41 ELBWI hospitalized between January 2001 and August 2008. Univariate analysis and Logistic regression analysis were performed to detect the risk factors of PIVH. Results Of 41ELBWI, twenty-three suffered from PIVH with the incidence of 56.1%. Univariate analysis revealed that,in PIVH group,gestational age,mean blood pressure and the minimum values of blood pressure were lower than non-PIVH group[ ( 27.1 ± 1.9 ) weeks vs ( 28. 7 ± 1.6) weeks, t = 2. 834, P < 0. 05 ;(28.9±4.8) mm Hg vs (33.1±4.9) mm Hg, t=-2.747,P<0. 05; (24.4±4.3) mm Hg vs (31.4 ± 6.6) mm Hg,t= -3. 863, P<0. 05], while blood pressure fluctuation and the highest values of PaCO2 during the first week of life were higher[(19.0 ± 5.2) mm Hg vs (13.7 ± 4. 8) mm Hg;(60. 2± 19. 4) mm Hg vs (49.5±12.1) mm Hg] (t= 3. 310 and 2. 166, P<0. 05), the incidence of administration of pulmonary surfactant, neonatal respiratory distress syndrome, shock, hypotension before 4 days of age, hyperglycemia and mechanical ventilation therapy were higher[73. 9 % (17/23) vs 27. 8%(5/18), 60. 9%(14/23) vs 27.8%(5/18),52. 2%(12/23) vs 5.5%(1/18),73. 9%(17/23) vs 33.3%(6/18) ,78. 3%(18/23) vs 44. 5%(8/18),87. 0% (20/23) vs 44. 5% (8/18)]( all P<0.05).Multivariate Logistic analysis revealed that blood pressure fluctuation (OR = 1. 260, 95% CI: 1. 009-1. 572, P = 0. 041 ) and lowest mean blood pressure(OR = 0. 805,95 % CI: 0. 672-0. 965, P = 0. 019)were risk factors of PIVH. Among twenty-eight ELBWI received mechanical ventilation, only peakinspiratory pressure(OR=- 2. 086,95% CI: 1. 140-3. 819, P= 0. 017) was the risk factor of PIVH by Logistic analysis. Conclusions Low blood pressure and blood pressure fluctuation may be risk factors of PIVH in ELBWI. The high values of peak inspiratory pressure is a risk factor of ELBWI with mechanical ventilation.
6.Gene Cloning and Expression of PACAP and Study of Its Biological Activity

In order to study the expression and the feasibility of scaled production of neuropeptide in the routine expression system such as E.coli with the pituitary adenylate cyclase activating polypep tide(PACAP) as an example, the following experiments were carried out. First, on the basis of the reported amino acid sequence of PACAP, DNA sequence of PACAP w as deduced and six partially complementary oligonucleotide fragments were design ed. The coding region of PACAP was obtained by renaturing the DNA fragments and ligation and identified by DNA sequencing. The coding region of PACAP was cloned into plasmid pGEX-4T-3 and transformed into E.coli BL21(DE3 ). An expression strain BLPACAP was selected. SDS-PAGE analysis revealed that t he GST-PACAP fusion protein was highly expressed and accumulated to about 30% o f the total bacterial proteins. By affinity chromatography, up to 90% GST-PACAP was purified by one step from bacterial lysate. The purified protein could prom ote neurite outgrowth of PC12 cells and the survival of spinal cord neurons.

9.Clinical observation of acute skin and oral mucous membrane reactions in patients with nasopharyngeal carcinoma treated with concurrent radiochemotherapy and correlated factors

Objective To observe the acute skin and mucous membrane reactions in patients treated with concurrent radiochemotherapy for nasopharyngeal carcinoma,and to analyze the influencing factors.Methods A total of 85 nasopharyngeal carcinoma cases treated with concurrent radiochemotherapy were enrolled in the study.Fifteen clinical and laboratory indexes,including BMI,radiation dose,degree of acute oral mucous and skin reactions and blood routine test were observed weekly.Univariate and multivariate regression analysis were performed to assess the factors,and screen the independent factors.Results Multiple-factor analysis showed that the risk factors cloesly related with acute radioactive oral mucosa reactions were smoking history(OR =3.467,P < 0.05),single-dose of gross tumor volume (GTV) >2.15 Gy(OR =3.393,P < 0.05),while those with acute radiation skin reactions were diabetes history(OR =87.859,P < 0.05) and hemoglobin values 1 week before radiotherapy > 130 g/L (OR =21.404,P < 0.05).Conclusions In the patients treated with concurrent radiochemotherapy for nasopharyngeal carcinoma,smoking history and single-dose of GTVnx is the independent risk factors of acute radiation oral mucosa reactions,while diabetes history and hemoglobin values I week before radiotherapy are the independent factors of acute skin reactions.
10.Optimization of the Formulation and Technology of Compound Bovis Calculus Sativus Gel by Orthogonal Test

OBJECTIVE:To optimize the preparation technology of Compound bovis calculus sativus gel. METHODS:The ul-trasonic emulsifying technology was optimized by orthogonal test using ultrasonic power,ratio of ultrasonic time to interval time, total ultrasonic time as factors,using centrifugal stability constant(KE)as index.Ultrasonic emulsifying method was applied to pre-pare O/W emulsions using paeonol,berberine hydrochloride and eucalyptus oil;then calculus bovis sativus powder was added into O/W emulsions,and then mixed with carbomer(940)gel matrix to prepare gel. The formulation of gel was optimized by orthogo-nal test with the amount of carbomer (940),glycerool and triethanolamine as factors,using compactibility score,comprehensive score of release rate in vitro as index. Validation test,stability test and content determination of bilirubin were conducted for gel pre-pared by optimized technology. RESULTS:The optimal ultrasonic emulsifying technology was as follows as ultrasonic power 450 W,ratio of ultrasonic time to interval time 2:1,and total ultrasonic time 5 min. The optimal formulation of gel was as follows as carbomer(940)0.5%,glycerool 15%,triethanolamine 0.20%(g/100 g). The average of KE of validation test and average compre-hensive score were 0.175 and 98.67(RSD<2%,n=3);the appearance of the preparation had no obvious change in stability test, and average percentage of bilirubin in labeled content was 100.8%. CONCLUSIONS:The optimal formulation and preparation tech-nology of gel is feasible,and the prepared gel is stable and controllable in quality.
