1.ADR Caused by Quinolones:Analysis of 199 Cases in Our Hospital

OBJECTIVE:To investigate the characteristics of adverse drug reactions (ADR) caused by quinolones in our hospital and to promote their rational use in clinic. METHODS: By a retrospective review,a total of 199 quinolones-induced ADR cases collected from May 2004 to December 2007 in our hospital were analyzed statistically in respect of patients' gender and age,drug species involved,past ADR history,route of administration,clinic manifestations,etc. RESULTS: A total of 9 kinds of quinolones involved in the 199 ADR cases,of which,118 cases were induced by levofloxacin,16.1% had past ADR history,93.5% of the ADR cases were induced via intravenous drip,and most common presentations of the ADR were lesions of skin and its appendants as well as digestive system. There were 7 serious ADR cases. CONCLUSION: The indications of quinolones should be controlled strictly,which should be preferably administered orally and monitoring of its administration should be emphasized in clinical practice to decrease and avoid the occurrence of ADR.
2.Therapeutic effects of xylazine and dexmedetomidine in establishing the model of spinal cord inj ury in beagle dogs

ABSTRACT:Objective To observe the anesthetic effects of xylazine and dexmedetomidine in establishing the model of spinal cord injury in beagle dogs.Methods Thirty female beagle dogs were randomly divided into 3 groups with 1 0 in each:xylazine group (group S ), dexmedetomidine group (group D ), and xylazine +dexmedetomidine group (group S+D).Basal anesthesia with ketamine was performed in all the dogs.Then group S was intramuscularly injected with xylazine discontinuously, group D was intravenously injected with dexmedetomidine 2μg/(kg·h)continuously,but group S+D was injected with xylazine and dexmedetomidine in combination.The heart rate (HR),respiratory frequency (RR),Ramsay score and spasm index (SI)were observed at the time of skin incision (T1),after the beginning of the operation 30 min (T2),before the end of the operation 30 min (T3),and the end of the operation(T4);and the usage of anesthetics was recorded.Results Compared with that in groups S and D,the usage of xylazine and dexmedetomidine in group S+D was decreased by 1/2 and 1/4,respectively (P<0.01),but HR and RR did not change significantly;the sedative effect was superior to that in S.No obvious increase in muscular tensility in the foreleg was observed before operation.Conclusion Xylazine and dexmedetomidine used in combination to establish the model of spinal cord injury in beagle dogs have advantages of better sedative effect,less respiratory depression and lower dosage of anesthetic drugs.
3.ENDOSCOPIC TREATMANT ANASTOMOTIC CICTRICIAL STENOSIS AFTER RESECTION OF ESOPHAGEAL CANCER BY ENDOTHERN KNIFE.

Anastomotic cicatricial stenosis after resection of esophageal cancer were treated by self- making electric knife through endoscipe.Fifty two cases were located in upper segment while 37 cases in middle-lowere part.Anastomotic diameter of 40 cases were less than 2.0mm,17 cases less than 3.0mm. Sixteen cases more than 3.0mm. Anastomotic stoma were incarcerated by food in 16 cases.The operatic procedure:First,the cicatricial stenosis was resected and became"*"shape by electric knife,then dilated by baloon catheter and by endoscope.sitytwo cases of stenosis were unobstrated(can swallow solid food)after one time treatment ; 11 cases each needed 2 and 3 times;1 case each needed 4 , 5 , 12 , 43and 81 times ,respec- tively. The last 2 cases were treated repeatly every 1- 3 months. The authors think that this procedure is ef- fective and safe. The technique is mastered easily and could be wildely used.
5.Preliminary observation on the interference effect of Helicobacter hepaticus infection on the immune response in BALB/c mice

Objective To determine the interference effect of H. hepaticus infection on the functional characteris-tics of dendritic cell ( DC) surface molecules and immune response in mice. Methods Male BALB/c mice were inocula-ted with H. hepaticus (ATCC 51450). Murine bone marrow-derived dendritic cells (DC) were isolated and co-cultured which were stimulated by GM-CSF and IL-4 at the fifth month after the last inoculation. Then the DCs were subjected to FACS analysis for surface markers (CD11c, CD40, CD80 and MHCII) detection. On this basis, virus suspension of New-castle disease virus( NDV) ZJ1 strain was inoculated into the mice. Serum was collected for detection of the NDV antibody titer in serum weekly to explore the difference of antibody titer between the two groups. Results The expression rates of CD40 and MHCII on the mouse DCs in experimental group were higher than that in the control group. The NDV antibody ti-ter of experimental group was slightly lower than that in the control group in the first week. During the 2nd to 5th weeks, the titer was higher than that in the control group, with a very significant difference. In the 6th week, the titer of both the two groups tended to fall. Conclusions H. hepaticus infection can promote bone marrow DC maturation in mice, stimulate the expression rates of MHC II and CD40, and enhance the NDV antibody levels.
6.Value of combination of three-vessel views in ultrasound diagnosis of fetal aortic arch and pulmonary arterial branch anomalies

Objective To evaluate the role of combination of three-vessel views in ultrasound diagnosis of fetal aortic arch and pulmonary arterial branch anomalies.Methods The data of 66 cases of fetal aortic arch and pulmonary arterial branch anomalies were retrospectively analysed,including echocardiographic data,autopsy and operation records and postnatal follow-up results.Echocardiogaphic features and display frequencies on three vessels and trachea view(3VT),three vessels view(3VV)and three vessels and pulmonary arterial branches view(3VP)were summarized.Results There were 52 cases of aortic arch abnormalities,including 32 cases of right aortic arch with left ductus arteriosus,4 cases of double aortic arch,7 cases of aberrant right subclavian artery,6 cases of coarctation of aorta and 3 cases of interruption of aortic arch.Fourteen cases were diagnosed pulmonary aterial branch abnormalities,including 1 0 cases of crossed pulmonary arteries,2 cases of anomalous origin of left pulmonary artery from aortic arch and 2 cases of pulmonary artery sling.The display frequencies of fetal aortic arch and pulmonary arterial branch anomalies on 3VT,3VV and 3VP were 80.3%,19.7% and 39.4%.Conclusions The combination of three-vessel views is of great value in prenatal diagnosis of fetal aortic arch and pulmonary arterial branch anomalies.
7.Help-seeking behavior among individuals with overactive bladder in type 2 diabetes mellitus and its influencing factors

Objective To estimate help-seeking behavior among individual with overactive bladder (OAB) in type 2 diabetes mellitus and its influencing factors.Methods This was a cross-sectional survey with a representative sample of 133 patients with OAB in type 2 diabetes mellitus responded the questionnaires,including a self-designed Demographic Data Questionnaire,12-item Short-Form health survey questionnaire,Overactive Bladder Symptom Score,Patient Perception of Bladder Condition,Help-seeking Behavior Questionnaire.Results Among cases,21.8%(29/133) had seen a doctor for voiding problems.In the Logistic model,educational status,Charlson comorbidity index,glycosylated hemoglobin,and OAB symptom bother were found to be associated with help-seeking behavior,OR value(95% confidence interval) were 3.158(1.511-6.599),2.537(1.327-4.849),0.631(0.430-0.926),1.655(1.126-2.434),P <0.01 or 0.05.Conclusions The help-seeking rate of OAB individuals in type 2 diabetes mellitus patients is low.Health care providers should understand the predictors of help-seeking behavior and take actions to improve patient's health care utilization and quality of life.
8.Comparison of culture and PCR assays for detection of bacteria in laboratory rats and mice

Objective To compare the efficiency of bacteria culture and PCR assays for detection of Staphylococcus aureus ( S.aureus) , Pseudomonas aeruginosa ( P.aeruginosa) and Klebsiella pneumoniae ( K.pneumoniae) in laboratory rats and mice.Methods Bacteria culture combined with biochemical identification and PCR assay were used to detect 78 SPF rats and 422 SPF mice and the results of the two methods were compared .Results All the 78 rats were negative .Of the 422 mice, the positive rate by culture was 7.11%(30/422), of which, 10 were S.aureus, 22 were P.aeruginosa, and 2 were K.pneumoniae.The positive rate by PCR was 7.58%(32/422), of which, 10 were S.aureus, 25 were P. aeruginosa, and 2 were K.pneumoniae.Conclusions The high sensitivity , rapid procedure and easy to operate of PCR assay makes it valuable for rapid bacteria diagnosis and large-scale screening in laboratory animals .
9.Development and Application of Electronic Medication Record Web Page and Its Remote Data Management

OBJECTIVE:To develop a new electronic medication record web page and facilitate its remote data management.METHODS:With the aid of the Windows 2000 platform,the new electronic medication record web page was set up by using Visual Studio.Net,and its remote data management was achieved by using SQL Server.RESULTS:With the new mode of electronic medication record web page,detailed and accurate clinical data become available for clinical pharmacists and a remote data management has come off,furthermore,it is easy to operate.CONCLUSION:As a new mode of electronic medication record,it deserves to be popularized in clinical pharmacists.
10.Changes on blood glucose of different operation scheme after gastrectomy for gastric cancer in patients complicated with diabetes mellitus

Objective To observe the effect of gastrectomy for gastric cancer on blood glucose in patients complicated with type 2 diabetes mellitus by using different operation strategies.Methods The retrospective analysis was carried out of 68 cases who undergone radical surgery for gastric cancer and complicated with type 2 diabetes mellitus.These patients were divided into 3 groups by different kinds of digestive tract reconstruction,Billroth Ⅰ (n =26),Billroth Ⅱ (n =22) and Roux-en-Y (n =20).The levels of fasting blood glucose (FBG) were detected before and after operation one week,one month and six months in three groups.Results Comparing to the level before operation,the level of FBG after operation showed a descent in group B and group C((10.03 ±5.12) vs.(13.56 ±3.36) mmol/L; (8.44 ±5.21) vs.(13.56 ±3.36) mmol/L;(10.61 ±3.16) vs.(12.84 ±3.36) and(7.68 ±4.88) vs.(12.84 ±3.36) mmol/L) which was better than group A((10.03 ±5.12)vs.(11.12 ±6.32),(8.44 ±5.21) vs.(11.12 ±6.32) mmol/L;(10.61 ±3.16) vs.(11.12 ±6.32) mmol/L),(7.68 ±4.88) vs.(11.12 ±6.32) mmol/L;P<0.05,but there was no significantly difference between B and group C (P > 0.05).The 3 groups of patients,with B,C groups of patients with diabetes improved obviously,of which nearly 50% of patients cured,normal blood sugar control,the other part with the exception of a few effective,degradation of diabetes drug treatment,and operation effect was significant.Conclusion It is effective to control the blood glucose of gastric cancer patients complicated with type 2 diabetes with Billroth II or Roux-en-Y gastrectomy.There is a correlation between postoperative blood glucose change and different gastrojejunal anastomosis.
