1.Clinical research of emergency early mechanical ventilation and bronchoalveolar lavage and drug intervention to prognosis of severe asthma

Objective To study the clinical efficacy of emergency early mechanical ventilation and bron -choalveolar lavage and drug intervention to severe asthma .Methods 140 cases with severe asthma were selected ,the patients were divided into the study group and the control group according to a random number table ,70 cases in each group,the control group were received early mechanical ventilation ,the study group were given bronchoalveolar lavage and interventional therapy on the basis of the control group ,clinical efficacy ,alveolar function ,her condition improved and hospital stay of the two groups were compared .Results The total efficiency of the study group were 92.9%(65/70) significantly higher than 81.4%(57/70) in the control group,the difference was statistically significant (χ2 =12.382,P<0.05);Forced expiratory volume in one second (FEV1),forced vital capacity (FVC) and FEV1/FVC of the study group were significantly better than that of the the control group ,the difference was statistically significant ( t=9.098,10.382,11.204,all P<0.05);Condition improved and hospital stay of the study group were significantly shorter than that of the control group,the difference was statistically significant (t=9.898,12.393,all P<0.05). Conclusion Emergency early mechanical ventilation and bronchoalveolar lavage and drug intervention to severe asthma had better clinical efficacy ,which can significantly improve the clinical symptoms of the patient .
2.Percutaneous cervical diskectomy in the management of radicular type cervical spondylosis

Objective To discuss the clinical significance of percutaneous cervical diskectomy (PCD) in the management of radicular type cervical spondylosis. Methods A retrospective analysis was carried out on clinical records of 106 radicular type cervical spondylosis cases treated by PCD from July 1993 to June 2002. Results Follow-up observations for 6 months~8 years(mean 3.6 years) in 91 cases showed the outcomes were excellent in 36 cases, good in 40, and unchanged in 15, the rate of excellent-or-good results being 83.5%. Conclusions PCD has an excellent therapeutic effect for nerve-root symptoms resulted from cervical disc herniation, such as numbness and pain of the upper extremities or shoulders.
3.STUDY ON THE HISTOCHEMISTRY OF ENZYMES FROM CHORIONIC VILLI IN EARLY PREGNANCY AND ITS POSSIBLE USE IN PRENATAL DIAGNOSIS

To try to find out the possible use of histochemistry in the prenatal diagnosis in first trimester of pregnancy, 20 kinds of enzymes in chorionic villi from 6th to 9th week of gestation which are related to inborn errors of metabolism were investigated and graded according to their histochemical reactive intensity; the enzymes listed below: (1). ?-glucuronidase, (2). ?-galactosidase, (3). arylsulfatase, (4). ?-N-acetylglucosaminidase, (5). 3?-hydroxysteroid dehydrogenase, (6). NADH dehydrogenase, (7). adenosine triphosphatase, (8). alkaline phosphatase,(9). acid phosphatase, (10). creatine phosphokinase, (11). glutamic dehydrogenase, (12). aldolase, (13). xylitol dehydrogenase, (14). alcohol dehydrogenase, (15). xanthine oxidase, (16). catalase, (17). ornithine carbamoyl transferase, (18). lipase, (19). cholinesterase, (20). ?-glutamyl transpeptidase. The experimental results show that the last 6 kinds of enzymes present negative reactions. Since the his- tochemical reactions of the first 14 kinds of enzymes are positive, it is possible that they can be used for the prenatal diagnosis of inborn errors of metabolism caused by dificiency of corresponding enzymes. The results also show that compared with biochemical method, histochemical method has the advantages of requiring smaller amount of chorionic villi, being without contamination by maternal cells, simplicity and rapidity.
4.Activity of attentional and executive cortex and its relationship with cognitive impairment in subcortical ischemic vascular dementia

Objective To investigate the activity of attentional and executive cortex and its relationship with cognitive impairment in subcortical ischemic vascular dementia (SIVD).MethodsTwenty patients with SIVD and twenty normal control subjects who were matched by age,sex and education were enrolled.All of them underwent fMRI using SEMENTS 3.0T MR during Stroop task performance. The correlation between cognitive impairment and cortex activation in fMRI was analyzed.Results Cortical activation of bilateral dorsolateral prefrontal cortex,Nentrolateral prefrontal cortex,inferior parietal lobe have closed correlation with total score,visual trabecular spaces and execution,attention,verbalization,delayed memory and orientation score in MoCA test ( r =0.447-0.837,P < 0.05 ).ConclusionsCortex activation in fMRI can reflex the cognitive impairment of SIVD.

Radiation-induced lung injury is a common complication of thoracic cancer radiotherapy.A lot of animal experiments and clinical studies have shown that ionizing radiation-induced lung injury is a complex process regulated by a variety of cytokines.The transcription factors including nuclear factor κB,MAPK and activator protein-1 are involved in radiation-induced lung injury by regulating the synthesis of a variety ofinflammatory proteins.The flavonoids play a role in radiological protection by blocking the pathway of nuclear factor kB and activator protein-1.
6.Effect of Cilostazol on the Ion Channel of Right Ventricular Cells in Experimental Rats

Objective: To observe the effect of cilostazol on the ion channel of right ventricular cells in experimental rats, and to explore the ion channel mechanism of ciolstazol for preventing the ventricular arrhythmia in Brugada syndrome.
Methods: Our research was composed of 2 groups: ①Perfusion group, the cells were treated in 4 sub-groups by cilostazole at 1, 2, 5, 50μmol/L respectively, and there were 9, 5, 3, 7 cells were recorded at each sub-group to observe the differences of current density Ito at before and after treatment. ②Oral group, which included 4 sub-groups:Control 1 with 7 rats, Experiment 1 with 5 rats, and Control 2 with 8 rats, Experiment 2 with 6 rats respectively. The differences of current density Ito and ICa,L were studied between each Control and Experiment sub-groups.
Results: In Perfusion group,①with cilostazole 1, 2, 5, 50μmol/L treatment, the current density Ito decreased in all sub-groups, when the self-command voltage at+60mV, the Ito was signiifcantly different in each sub-group at before and after treatment, all P<0.05.②When each command voltage decreasing, the reduction rates of Ito were similar among 4 sub-groups, all P>0.05. In Oral group,①When the self-command voltage from-50mV reached the maximum of+60mV, the Ito was similar between Control 1 and Experiment 1 sub-groups, P>0.05.②When the self-command voltage at+10mV, the current density of ICa,L was slightly higher in Control 2 sub-group than that in Experiment 2 sub-group, P>0.05.
Conclusion: Direct perfusion of cilostazole in right ventricular cells may inhibit Ito in experimental rats, such effect was similar with cilostazole treatment at (1-50)μmol/L. Cilostazole might prevent the ventricular arrhythmia in Brugada syndrome in experimental rats.
7.Effect of epigallocatechin gallate against exercise-induced fatigue in mice.

OBJECTIVETo investigate the effects of epigallocatechin gallate (EGCG)against exercise-induced fatigue in mice.
METHODSTotal 120 mice were randomly divided into three groups and tested separately. For each test, there were 30 mice subdivided into high dose (50 mg/kg . d EGCG) and low dose (10 mg/kg . d EGCG) groups as well as saline control group(1 ml/kg . d) with 10 in each. Burden swimming, running wheel endurance, stick climbing and hypoxia tolerance exercise were used to establish fatigue mice training model in three groups. And intraperitoneal injection with different doses of EGCG per day for consecutively 28 days and the mice in the control group were treated with normal saline. After the last each test, the blood lactic acid (BLA), blood urea nitrogen (BUN), blood lactate dehydrogenase (LDH), muscle glycogen (MG) and liver glycogen (LG) of each group of mice were determined.
RESULTSEGCG treatment groups(B and C)revealed a prolonged the mice survival time of burden swimming test, hypoxia tolerance, running wheel time and the ability of stick climbing(P < 0.05 or P <0.01), and increased LDH activity and MG and LG contents, reduced contents of BLA and BUN. High dose group had an obviously increase effect than lower dose group(P <0.05).
CONCLUSIONEGCG has significant effects against exercise-induced fatigue in mice.

9.Synthesis and biological evaluation of H2S donor ADT-OH derivatives

5-(4-Hydroxyphenyl)-3H-1,2-dithiole-3-thione (ADT-OH) is a slowly releasing H2S donor with some neuroprotective effect.In order to study the structure-activity relationships,seventeen compounds (Y1-Y17) were synthesized by modification of ADT-OH at the aromatic ring position,and their structures were confirmed by 1H NMR,13C NMR and HR-MS.Among them,6 compounds (Y4,Y13-Y17) were novel compounds.Their effects had been evaluated on HT-22 hippocampal neuron cells damaged by glutamate with MTF method.The pharmacological results demonstrated that all the Y compounds had potent neuroprotection at most of the tested concentrations (1-100 μmol/L).Compounds Y1-Y9 and Y11 significantly improved the survival rates of the damaged cells at 1-100 μmol/L (P <0.01).Specially,compounds Y1,Y4,Y6-Y9,Yu are more potent than their parent compound ADT-OH at concentration of 1-10 μmol/L,which is worthy of further study.
10.Treatment for lumbar disc herniation with percutaneous transforaminal endoscopic discectomy and discussing the relative problems

Objective To investigate the efficiency,decision of intra-operative puncture route,treatment of perioperative complications and discuss the other relative problems of the treament for lumbar disc herniation with percutaneous transforaminal endoscopic discectomy.Methods To excise the nucleus pulpesus under percutaneous transforaminal endoscopic discectomy,use the Macnab standard,visual analogue scale and infrared thermal imaging to estimate the efficiency.Results Among followed-up of 208 patients,182 patients were excellent and good outcome,23 patients favorable,2 patients fair,0 patient poor.The leg and back VAS was a significant improvement 1 week post-operation compared with pre-operation (P < 0.05),but no statistical difference among 3 months,6 months,12 months and 1 week post-operation (P > 0.05).The infrared thermal imaging point out that the legs skin temperature of D-value was a significant improvement post-operation compared with pre-operation (P < 0.05).Conclusions The method excised the nucleus pulpesus,and provided the spine maximum protection about the stability and flexibility.Intra-operative puncture route of individuation design can reduce the complications of intervertebral foramen perioperative,and the key to improve the effectiveness.
