Abstract?AIM: To analyze the effects of femtosecond laser assisted cataract surgery ( FLACS ) in the treatment of cataract and its effect on prognosis.?METHODS:Forty-two cases (42 eyes) of patients with cataract who were treated in the Department of Ophthalmology in our hospital between January 2012 and December 2014 were selected as the study objects. According to the order of treatment, they were divided into control group and observation group, 21 cases in each. The control group was treated with traditional phacoemulsification cataract surgery ( PCS ) . On the basis, the observation group was treated with femtosecond laser. The effective phacoemulsification time (EPT), cumulative dissipated energy (CDE), fluid flow and monitored pressure of the two groups were recorded. The rate of corneal endothelial loss and the situation of Tyndall phenomenon were statistically analyzed. The two groups were followed up for 1a.The long-term visual acuity recovery was observed. The best corrected visual acuity ( BCVA ) was recorded, and the long-term complications were statistically analyzed.?RESULTS: 1 ) The total response rate in observation group was 95% while in control group was 90% ( P>0.05);2) the surgery time of the observation group was longer than that of the control group ( P<0.05) but EPT was shorter than that of the control group.CDM and liquid flow were less than those of the control group ( P<0.05 ); 3 ) at 1d after surgery, there was no significant difference in intraocular pressure between the two groups (P>0.05); the rates of Tyndall phenomenon and corneal endothelial loss in the observation group were lower than those in the control group ( P<0.05);4) BCVA of the two groups at different time after surgery were significantly higher than that before surgery (P<0.05). However, at 1d, 3mo, 6mo and 1a after surgery, BCVA of the observation group was better than that of the control group ( P <0.05 ); 5 ) the incidence of complications in the observation group after surgery (14%) was lower than that in the control group (43%) (P<0.05).?CONCLUSION: The surgical effects of FLACS in the treatment of cataract are good. After surgery, the visual acuity of patients is improved significantly and the incidence of postoperative complications is low. However, the surgery time is long and cost is high, so it is difficult to popularize.

Objective To investigate the transactivating effect of hepatitis C virus(HCV)core protein on inducible nitric oxide synthase(iNOS)gene promoter and the molecular biological mecha- nisms of HCV pathogenesis. Methods Polymerase chain reaction(PCR)technique was employed to amplify the sequence of iNOS promoter by using HepG2 genomic DNA as template, and the product was cloned into pGEM-T vector. The iNOSp gene was cut from T-iNOSp by KpnⅠand XhoⅠ,and then was cloned into pCAT3-Basic,the constructed vector was named as pCAT3-iNOSp,pCAT3-iN- OSp was transfected into the LO_2 cell line.LO_2 cell was also cotransfected with pcDNA3.1(-)-core and pCAT3-iNOSp by FuGENE 6 transfection reagents. The LO_2 cells transfected with pCAT3-Basic was used as negative control. The activity of CAT in LO_2 cells was detected by an ELISA kit after 48 hours, which reflected the transactivating function of HCV core protein to iNOS gene promoter.Re- sults The expressive vector pcDNA3.1(-)-core and report vector pCAT3-iNOSp had been construc- ted and confirmed by restriction enzyme digestion and sequencing. The expression of CAT in LO_2 cells transfected with pCAT3-iNOSp and peDNA3,1(-)-core was 11 times as higher as that of pCAT3-bas- ic,and 6 times as higher as that of pCAT3-iNOSp.Conclusion It is suggested that HCV core protein can transactivate iNOS gene promoter.

Objective To explore the mechanisms of posterior pilon fractures and evaluate the curative effects of different types of fixation on the treatment of posterior pilon fractures. Methods We included in this retrospective study 20 patients with posterior pilon fracture who had been treated from January 2012 to January 2015 at our department. They were 10 men and 10 women, from 23 to 77 years of age (average,50.6 years).According to the classification by Yu Guangrong,5 cases belonged to type Ⅰ,3 to type Ⅱa,4 to type Ⅱb,and 7 to type Ⅲ.One was not indentified because of lacking CT examination. The mechanisms included ground level fall in 2 cases, motor vehicle accident in 7,fall off stairs in 5,sport injury in 2,fall from a bike in one and fall from a height in 3.More than 25% of the articular surface was involved in 13 patients.Syndesmosis injury was identified in 6 patients by Cotton test during operation.Internal fixation varied accordingly. We recorded the mechanism, classification, proportion of the articular surface involved (more or less than 25%),and syndesmosis injury to figure out the characteristics of posterior pilon fractures. We used the Burwell-Charnley radiographic criteria to assess the postoperative reduction of the articular surface, and the Olerud-Molander scoring scale and visual analogue scale (VAS) to assess the ankle function. The curative effects of different types of fixation on the treatment of posterior pilon fractures were compared. Results Of the 20 patients,17 were available for follow-up for 6 to 36 months (average,17.8 months). Two patients received reoperation because of implant failure after cannulated screw fixation from anterior to posterior. The Burwell-Charnley radiographic evaluation revealed 12 anatoinical reducations and 8 fair reductions. The mean Olerud-Molander score for the 17 patients at the final follow-ups was 81.5 (range, from 35 to 100) and the mean walking VAS was 1 (from 0 to 3).Conclusions Posterior pilon fractures are mostly caused by medium to high energy violence, resulting from a combination of rotational and vertical forces.Since there is a high risk of implant failure, the cannulated screw fixation from anterior to posterior is not recommended.Good clinical outcomes are observed in the cannulated screw fixation from posterior to anterior and the plate/cannulated screw fixation for posterior pilon fractures.

AIM: To explore the role of activation and apoptosis of splenic lymphocytes in the development of viral myocarditis (VMC). METHODS: Apotosis and MHC II antigen of splenic lymphocytes were detected in the VMC group (VMC, death, 8 cases) and control group (non-cardiac death, 4 cases) with TUNEL and immunohistochemistry methods.RESULTS: Compared with the control group, the increased expression of MHC II antigen and apoptosis were found in the splenic lymphocytes in the VMC group. CONCLUSION: These results suggest that the abnormality of the apoptosis/activation of splenic lymphocytes may be involved in the pathogenesis of VMC.

BACKGROUND:At present, there stil remain controversies concerning the choice of tibial slope in unicompartmental knee arthroplasty, and the related biomechanical studies are rare. OBJECTIVE:To find the rational tibial slope in mobile-bearing unicompartmental knee arthroplasty through the results of finite element analysis. METHODS:A finite element model of normal knee was established and validated using the techniques of three-dimensional reconstruction and finite element pre-processing. Then the finite element models of unicompartmental knee arthroplasty with different tibial slopes were established based on the normal knee model. The finite element analyses were conducted after the boundary conditions and loads were unified. RESULTS AND CONCLUSION: The finite element models of unicompartmental knee arthroplasty with different tibial slopes were established successfuly. The results of the finite element analyses indicated that the posteromedial tibial cortical and cancelous bone stress increased gradualy as the posterior slope increasing, as wel as the load distribution and the cartilage contact pressures of lateral compartment. The anteromedial tibial cortical stress increased significantly with the anterior slope. A posterior tibial slope of 0°-7° can be recommended when implanting a mobile-bearing unicompartmental knee arthroplasty.
