Pancreatic cancer is one of the most lethal human malignant tumors, and its incidence is rising. Now, its treatment includes surgical resection,chemo - radiotherapy and supplementary treatment. The prognosis of pan-creatic cancer is still dismal. And its one-year survival is low. This article talks about the effect of clinical treatment on quality of life in patients with pancreatic cancer.
4.Effects of compositive cognitive training for healthy community elders in reasoning ability: a controlled trial

Objective To evaluate the effects of compositive cognitive training on reasoning ability for healthy community elderly.Method All the participants were selected from one district in Shanghai by every 50 samples.151 community healthy elderly who accorded with the standard were collected at last.They were divided into cognitive intervention group ( n =90) and control group ( n =61 ) by sequence.The interventions ( includes reasoning,memory training,et al) were conducted in 24 sessions over 12 weeks.All individuals were assessed by Neuropsychological Test Battery for Elderly (NTBE) at baseline,follow-up,half a year follow-up and one year follow-up phase,and Raven' s Standard Progressive Matrices on reasoning classes.Results The scores of Raven' s Standard Progressive Matrices were significantly higher after reasoning training in intervention group( (15.54 ±5.70),(10.35 ± 5.10),t =3.595,P < 0.01 ).Compared with baseline,the reasoning test scores of NTBE were significantly higher after cognitive intervention in treatment group at follow-up phase,and were also significantly higher than control group (P < 0.05 or P < 0.01 ).Conclusion Results support the effectiveness of compositive cognitive training in improving reasoning ability are positive,and can last for one year.
5.Significance for the expression of transforming growth factor beta in human nasal polyp and mucosa tissue

Objectives:To explore the pathogenesis of nasal polyposis by examining the expression of transforming growth factor beta (TGF-β) in human inflammatory nasal polyps. Methods: Twenty five polyposis patients were selected, tissue TGF-β1~3 from nasal polyps and inferior turbinate mucosa were determined with immunohistochemistry APAAP method .The inferior turbinate mucosa from eight healthy volunteers were selected as control. Six polyp tissues were estimated with western-blot analyses to compare the characteristics of TGF-beta isoforms expression. Results: ①The expression of TGF-β1~3 in nasal polyps was significantly higher than that in nasal mucosa, it was undetectable in nasal mucosa from healthy volunteers;②TGF-β1 was the main isoform detected in nasal polyps. Conclusions: TGF-β, especially TGF-β1 is strongly expressed in inflammatory nasal polyps and its mucosa. TGF-β could induce modification of epithelium and connective tissue and therefore be involved in the pathogenesis of nasal polyposis.
6.Experimental study on relationship between Smad4 and angiogenesis related factors in human gastric cancer cell line

Objective To study the effect of Smad4 gene on angiogenesis related factors in human gastric cancer cell line.Methods Recombinant eukaryotie expressing plasmid pcDNA3.1 (-)-Smad4 containing Smad4 gene and empty vector pcDNA3.1 (-) were introduced into human gastric cancer cell line MKN28 using lipofectam and selected by G418,respectively.Two cell lines were obtained as follows:Smad4+-MKN28 cell line which was MKN28 transfected with a stable hybrid containing Smad4 gene and Smad4--MKN28 cell line with empty plasmid as control.The transcription and expression of VEGF and TSP1 were investigated by RT-PCR and Western blot.Results The mRNA expression of TSP1 in Smad4+-MKN28 cells was higher (P<0.05) than that in control cells,while VEGF was lower(P<0.05).Western blot showed the consistent results as measurement by RT-PCR.Conclusion Smad4 restoration in gastric cancer cells reduced angiogenesis rates through down-regulation of angiogenesis activitor and up-regulation of angiogenesis inhibitor.
7.Regulation of Moxibustion on the Expression of NF-κBp65 and PPARγ mRNA in Colon of Rats with Ulcerative Colitis

Objective: To explore the mechanism of herb-partitioned moxibustion for ulcerative colitis (UC) through observing the colonic mucosal histopathological changes and the expression of nuclear factor kappaB (NF-kB) and peroxisome proliferators-activated receptorγ (PPARγ) mRNA of UC rats. <br> Methods: Male SD rats were randomly divided into a normal group and a model group. UC model was established by general immunological plus local irritation method. After model identification, rats in the model group were randomly divided into a model group, a herb-partitioned moxibustion (HPM) group and a Western medicine (Salicylazosulfapyridine, SASP) group. Rats in the HPM group received treatment at bilateral Tianshu (ST 25) and Dachangshu (BL 25), two cones for each point, once a day for 7 d. SASP group rats were gavaged with SASP. The pathological scores were evaluated according to hematoxylin-eosin (HE) staining of colonic tissues. We used light microscopy to observe degree of colonic mucosal damage and the quantitative polymerase chain reaction (QPCR) to detect the expression of NF-κBp65 and PPARγ in colorectal mucosa. <br> Results: Compared with the normal group, histopathological scores were significantly higher in the model group (P<0.05); compared with the model group, the scores were decreased significantly in the HPM group and the SASP group. Compared with the normal group, NF-κBp65 mRNA expression was increased with statistical significance in the model group (P<0.05); compared with the model group, NF-κBp65 mRNA expressions were decreased significantly in the HPM group and the SASP group. Compared with the normal group, PPARγ mRNA expression was increased significantly in the model group (P<0.05); compared with the model group, PPARγ mRNA expressions were decreased significantly in the HPM group and the SASP group. <br> Conclusion: HPM could improve the mucosa damage of UC rats, which is possibly through down-regulating NF-κBp65 to achieve anti-inflammatory effect. Whether decreasing the PPARγ mRNA is possibly involved in preventing precancerosis will need further study.
8.Management of the femoral neck fractures in younger patients and prevent the necrosis of femoral head by microsurgical techniques

Objective To explore the clinical application of microsurgical techniques to therapy the femoral neck fracture in younger patients and prevent the necrosis of femoral head.Methods Seventy-four younger adults,age from 23 to 50,with femoral neck fractures were treated by open veducition,internal fixa- tion and pedicled bone transplantation from Jan.1995 to Dec.2004.All of the 74 patients were reviewed clin- ically and radiologically after an average of 3.2 years.Results In this group,19/74 cases(25.68%)had avaseular necrosis of the femoral head,which were diagnosised after an average of 28.5 months following inju- ry.Despite these results,these patients assessment with Harris system had been very good or good in 55/74 patients(74.32%).Conclusion It's an effective method to decrease the incidence of necrosis of femoral head after management the femoral neck fracture in younger patients by microsurgical techniques.
9.Air Quality Between Two Types of Operating-rooms:A Comparative Analysis

OBJECTIVE To learn about the factors affecting the quality of the air in operating-rooms and discuss the methods of improving their air quality.METHODS The dynamic and static air bacteriology between laminar air flow operating-room and normal operating-room of a fertiary care level hospital of grade Ⅰ in Changchun were observed.RESULTS The passing rate of samples for air static bacteriology between two types of operating rooms at the begining of selective operations(T0) was without difference.At the begining of emergency operations,there was a disparity.The total number of bacteria in normal operating-room increased with the operating-time,but in the laminar flow operating-room,the total number of bacteria increased in fluctuations during the(T2) time(60 ming since operation begain),and descended dramatically during the(T3) time(90 min since operation).CONCLUSIONS The emergency and operation-time are the main factors affecting the air quality in operating-room and the cleaning air-condition could improve the air quality.
10.Effect of integrative cognitive training on mild cognitive impairments: a one-year follow-up study

Objective To evaluate the middle and short-term effects of integrative cognitive training on mild cognitive impairments(MCI).Methods All the participants with MCI aged over 70 years were selected from one sub-district of Putuo District,Shanghai and divided into cognitive intervention group (n=46) and control group (n=35).The integrative cognitive training was conducted in 24 sessions for 12 weeks.All subjects were assessed by Neuropsychological Test Battery for Elderly (NTBE) and by a questionnaire of Shanghai Health Survey for the Elderly (VER2006)at baseline,follow-up and one-year follow up phases.Results At baseline phase,semantic relations test in NTBE was better (t=2.13,P=0.037),verbal fluency test (vegetables) in NTBE was worse(t =-2.25,P=0.038) in intervention group than in control group,while there were no significant differences in scores of other neuropsychological tests between intervention group and control group(all P>0.05).At follow up phase,16 subscales in reasoning test of NTBE were improved (t=-2.70,P=0.010)and 2 subscales were declined in cognitive intervention group(all P<0.05).9 subscales in semantic relations test of NTBE (t=-2.27,P=0.013) were improved and 5 subscales were declined in control group (all P< 0.05).Comparison between groups at one year follow-up showed that 5 subscales in reasoning test of NTBE (F=16.80,P=0.000) were better in intervention group than in control group(all P<0.05).General linear model (GLM) with repeated measures at three time points showed that 4 subscales in reasoning test of NTBE had a time-by group interaction (F=5.16,P=0.01).Conclusions Integrative cognitive training can improve cognitive function in patients with mild cognitive impairments,and the validity of reasoning ability can sustain one year.
