Objective To study the effect of clinical nursing pathway on oral and maxillofacial trauma.Methods One hundred inpatients were assigned according to the odd or even number of registration into the experiment group and the control group equally.The experiment group received clinical nursing pathway while the control one routine nursing.Results The rates of awareness of health education and satisfaction in the experiment group were significantly higher than the control group and the average stay was significantly shorter than that of the control group(both P<0.05).Conclusion The clinical nursing pathway used in the care of oral and maxillofacial trauma may standardize the manipulations of nursing care,enhance the nursing staff's sense of responsibility, and promote the patients' satisfaction,as well as the quality of care.

Objective To compare the effects on cardiac function between pacings of right outflow tract ( RV-OT) and right vcntricular apex ( RVA) .Methods According to the digital table ,30 patients with high degree atrio-ventricular block which need for permanent pacemaker were randomly divided into the group RVA and group RVOT , 15 cases in each group ,and the DDD or VVI pacemaker was implanted .The programmable pacemakers was carried out,3,12 months after the operation.By Doppler ultrasound,the left and right ventricular synchronization time ,end diastolic diameter ( LVDd) ,left ventricular ejection fraction ( EF) was checked ,ECG QRS wave width ( QRSd) ,6 mi-nutes walking distance (6MWT) was examined.Results Followed up for 12 months,there was no shift,electrode dislocation and other complications .The left and right ventricular synchronization time of group RVOT [(59 ±10)ms] was significantly better that that in the RVA group [(80 ±11)ms](P<0.05).Following up,the EF[(60 ±4)%], LVDd[(45 ±6)mm],6MWT[440 ±10)m]in RVOT group after operation were better than [(48 ±5)%,(54 ±7) mm,(380 ±13)m](all P<0.05),the QRSd of RVOT group[(134 ±9)ms]was significantly shortened than that of group RVA[(186 ±20)ms](P<0.05).Conclusion RVOT septum pacing can be more effective in cardiac func-tion than RVA pacing .

AIM: To evaluate the efficacy of simvastatin on exercise-induced myocardial ischemia in patients with stable exertional angina pectoris and hypercholesterolemia. METHODS: Ninety patients with stable exertional angina pectoris, positive rate of exercise test and hypercholesterolemia were randomly divided into two groups: the therapy group assigned to a 12-week treatment period either diet plus simvastatin (n=45); the control group with diet alone (n=45). All the patients underwent exercise testing before and after the treatment period. BACKGROUND: There are so many experimental and clinical researches on levels of nitrogen monoxide (NO) in serum and endothelins in plasma of patients with stroke; however, ratio and significance between them are still unclear.OBJECTIVE: To observe dynamic changes of levels of NO in serum and endothelins in plasma of patients with stroke.DESIGN: Case-controlled observation.SETTING: Neurological Department and Clinical Neurological Laboratory of the Second Hospital affiliated to Suzhou University.PARTICIPANTS: A total of 216 patients with acute cerebral infarction including 133 males and 83 females and 112 cases with cerebral hemorrhage including 68 males and 44 females were selected from Neurological Department of the Second Hospital affiliated to Suzhou University from September 1999 to December 2001. Another 106 subjects including 63males and 43 females were regarded as healthy control group.METHODS: Contents of NO in serum and endothelins in plasma were measured on 328 patients with stroke and 106 healthy subjects in the courses of 1-3 days, 1, 2, 4, 8 and 12 weeks with nitrate reductase and radio-immunity methods, respectively.MAIN OUTCOME MEASURES: Contents of NO in serum and endothelins in plasma; ratio between NO in serum and endothelins in plasma (NO/endothelins).RESULTS: ① As compared with those in the control group, content of NO in serum of patients with cerebral infarction and cerebral hemorrhage was decreased and reached the lowest value during acute period (within 1-3 days), and then increased gradually and closed to the normal level at about 4 weeks. In addition, content of endothelins in plasma was increased obviously during acute period, reached the peak at 2 weeks, and then decreased gradually. The level was still high at stage of recovery and closed to normal value within 4-8 weeks. ② As compared with that in the control group, NO/endothelins was decreased in cerebral infarction group at the courses of 1-3 days (P ＜ 0.05), reached the lowest value at 1 week (P ＜ 0.001), and increased to the normal level at 2 weeks. Moreover,NO/endothelins was remarkably decreased in cerebral hemorrhage group at the courses of 1-3 days (P ＜ 0.001), reached the lowest value at 1week (P ＜ 0.001), and increased gradually. The changes of course were great and the level reached above normal value at 8 weeks. There was significant difference of dynamic changes of NO/endothelins between cerebral hemorrhage group and cerebral infarction group (P ＜ 0.05).CONCLUSION: NO and endothelins play an important role in onset and development of ischemic cerebrovascular disease and hemorrhagic cerebrovascular disease, and their contents are related to prognosis.

Objective:To analysis the effects of alprostadil and yishen huashi particles on blood glucose,blood lipid,renal function and urinary podocyte proteins of patients with diabetic nephropathy.Methods:98 patients with diabetic nephropathy were selected and randomly divided into the control group and the experimental group with 49 cases in each group.The patients in the control group were treated with alprostadil,while the patients in the experimental group were treated with yishenhuashui particles on the basis of the control group.Then the curative effect,the levels of glycosylated hemoglobin (HbAlc),blood glucose (FPG),2 h postprandial blood glucose (2h PG),triglycerides (TG),total cholesterol (TC),high-density lipoprotein (HDL-C),low density lipoprotein (LDL-C),blood urea nitrogen (BUN),creatinine (Cr),β2 microglobulin (β2-MG),bladder inhibition (Cys-C) and urinary podocyte proteins (PCX) in the two groups were observed and compared between the two groups before and after the treatment.Results:The total effective rate of experimental group was higher than control group (P＜0.05).After treatment,there was no statistically significant difference about the HbA1c,FPG and 2 HPG between the two groups (P＞0.05).After the treatment,the levels ofTG,TC,LDL-C,BUN,Cr,β2 MG,Cys C,PCX and urinary nephrin/urine Cr of the experimental group were lower than those of the control group (P＜0.05).The HDL-C of experimental group was higher than that of the control group (P＜0.05).Conclusion:The curative is effect of alprostadil and yishen huashi particles in treatment diabetic nephropathy patients,can conducive to the improvement of blood glucose,blood lipid,renal function,reduce the concentration of urinary podocyte related proteins.

Objective To compare the ameliorating effect of collagen peptide chelated calcium (CPCC) and estrogen on the bone quality in ovariectomized rats in order to serve the development of safe drugs for prevention of osteoporosis (OP).Methods Bilateral ovariectomized rats were divided into ovariectomized group (OVX),sham group,17β-estradiol injection group (OVX+E2) and CPCC gavage group (OVX+CCCP).Bone and serum indices of these groups were assessed and compared at 9 weeks after treatment.Results Bone density of the OVX group was significantly lower than the sham group (P<0.01),indicating that the rat OP model was successfully established.Like estrogen,CPCC inhibited the abnormal changes of all indices and maintain similar levels with those of the sham group (P>0.05),while the body weight gain of the E2 group at weeks 8 and 9 was significantly lower than those of the sham group (P<0.01).As regarding the prevention of bone loss,the Mg and Ca levels of the E2 group were significantly lower than those of the moderate and high dose CPCC groups.The Cu level was not significantly different compared with the sham group,while those in the moderate and low dose CPCC groups were significantly higher than the sham group.The Mn,Zn and hydroxyproline levels of the E2 group were significantly lower than those of the sham group,while the CPCC group maintained levels similar to that of the sham group.In regarding to the inhibiting effect on the increased blood BGP and StrACP,the E2 group was still maintained at levels similar to that of the OVX group,while those of the CPCC group were significantly lower than the OVX group.As regarding the decreased blood Ca,the E2 group was not significantly different with that of the OVX group,while that of the CPCC group was significantly higher than the OVX group.Conclusions CPCC is more effective than estrogen in ameliorating the bone quality of ovariectomized rats.

Objective To explore the correlation between the sperm indicators and early spontaneous abortion after in vitro ferti ‐lization(IVF) .Methods 251 couples receiving IVF ,were retrospectively analyzed and divided into the research group (98 couples) and control group (153 couples) according to whether spontaneous abortion occurring .The male′s indicators of sperm DNA frag‐mentation index (DFI) ,sperm nucleoprotein immaturity rate ,percentage of normally morphologic sperms ,etc .in the two groups were analyzed .Results DFI ,percentage of normally morphologic sperms ,sperm concentration ,motility and forward motile sperm had no statistical differences between the research group and the control group (P> 0 .05) ;while the sperm nucleoprotein immaturi‐ty rate of the research group was significantly higher than that of control group ,the difference was statistically significant (P <0 .05) ;the sperm nucleoprotein immaturity rate had weak correlation with spontaneous abortion in early pregnancy (P < 0 .05) . Conclusion The sperm nucleoprotein dysmaturity is correlated with the spontaneous abortion in early pregnancy .The sperm nucle‐oprotein immaturity rate could be used to evaluate the risk of spontaneous abortion in early pregnancy after IVF .

OBJECTIVE:To optimize the formulation of azithromycin sustained-release vaginal suppository(ASVS).METHODS:The ASVS was prepared using S-40 as suppository base,HPMC as sustained-release material and glycerine as humectant.The formulation of ASVS was optimized by orthogonal design with accumulated release percentage and hardness as indexes and the amount of 8% HPMC and glycerol as factors;meanwhile,a verification test and the fitting of in vitro drug release model were performed.RESULTS:The optimized formulation ASVS was as follows:azithromycin 6 g,8% HPMC 23.52 g,glycerol 29.40 g,ethanol absolute 3 g,ethylparaben 0.59 g,and S-40 294 g.Three batches of suppositories prepared under the optimized formulation reached a mean content of 99.5%,with hardness up to the standard,showing good reproducibility and homogenicity in drug release in vitro.The accumulative release rates of all samples were greater than 98% at 180 min,and the release dynamics in vitro were in line with Higuchi equations.CONCLUSION:The optimized ASVS is feasible in formulation,stable and reproducible in preparation technology and up to the standard for sustained-release preparation.
