1.Construction of behavioral observation scale for appraising the performance of charge nurses of class three hospital

Objective Based on a job analysis known as the Critical Incident Technique, this study is to develop a behavioral observation scale (BOS) for appraising the performance of charge nurses of class three hospital in Sichuan province. Methods First, 56 charge nurses and supervisors were interviewed by using the Critical Incident Technique and 498 incidents were reported.Then the 498 incidents were cate-gorized into15 criteria and 169 behavior items which formed one original BOS. 48 supervisors preexamined,on a five-point Likert scale as to the frequency, 190 charge nurses who were engaged in the behavior de-scribed by each incident. According to the item analysis above, 12 performance criteria and 54 behavioral items came out to form a formal behavioral observation scale for appraising performance of charge nurses Fi-nally,to test the reliability and validity of behavioral observation scale, 192 charge nurses were put under re-al test. Results The Cronbach's alpha coefficient ranged between 0.65 and 0.93. The retest reliability ranged from 0.50 to 0.76. Factor Analysis: 9 factors with eigenvalue greater than one were selected and ac-cumulative prepertion was 68%. Conclusions Behavioral observation scale developed on a job analysis known as the Critical Technique to appraise the performance of charge nurses has moderately high reliabili-ty and validity.
2.Diagnostic value of colposcopy in cervical lesions

Objective To assess the diaghosis vaIne of colposcopy in cervical disease.Methods 169 with patients colposcopyaere admitted in endoscopic biopsy.Results Colposeopy to be diagnosed as chronic cervicifis 121 cases(71.6%),cervical intraepithelial neoplasia 27 cases(16.0%),6 cases of cervical cancer(3.6%),8 cases of cervical polyps(4.7%),incisive condyloma 7 cases(4.1%).Biopsy pathology report to the chronic cervicitis 123 cases(72.8%),cervical intraepithehal neoplasia 26 cases(15.4%),6 cases of cervical cancer(3.6%),8 cases of cervical polyps(4.7%),incisive condybma 6 cases(3.6%).The sensitivity and specificity of colposcopy higher.Conclusion Colposcopy for cervical disease is similar to the result of the pathology results,Which has a very imporrant diagnostic value.Combining colposcopy and pathological diagnosis of a certain relevance,the organic integration of the two can significantly improve the accuracy of diagnosis of cervical lesions.

4.Wnt5a involves the inflammation process and bone destruction in chronic periodontitis

Objective:To study the expression of Wnt5a in gingival tissues of the patients with chronic periodontitis.Methods:Wnt5a expression was tested with RT-PCR and Western-Blot in gingival tissues in 20 cases of chronic periodontitis and 20 correspond-ing normal tissues.Results:In the gingival tissues of chronic periodontitis cases Wnt5a overexpression was found comparing with nor-mal tissues(P <0.05).The higher clinical attachment loss was correlated with the higher expression of Wnt5a.Conclusion:Wnt5a may affect the course of the inflammatory process and alveolar bone destruction in patients with chronic periodontitis.
5.Effect of low molecular weight heparin combined with low dose aspirin on hormone level and immune function in patients with recurrent spontaneous abortion

Objective To study effect of low molecular weight heparin combined with aspirin on hormone level and immune function in treatment of recurrent spontaneous abortion.Methods 118 cases patients with recurrent spontaneous abortion from January 2013 to December 2013 were divided into observation group and control group, control group was given low-dose aspirin therapy, observation group was given low molecular weight heparin combined with low-dose aspirin.hormone levels, immune function, therapy effect were compared between two groups.Result Clinical curative effect:observation group prevent miscarriage success rate 86.44% was significantly higher than control group 55.93%(χ2 =13.387, P<0.05); Hormone levels: observation group serum HCG, P, E2 levels were significantly higher than the control group(P<0.05); Immune function: observation group IFN-γ, IFN-γ/IL-4 were significantly lower than control group, IL-4 were significantly higher than the control group(P<0.05);Adverse reaction:there were no statistical differences of adverse reaction between two groups(22.03% vs 15.25%)(χ2 =0.894, P>0.05).Conclusion Low molecular weight heparin combined with aspirin therapy help to improve recurrent hormone levels and immune function, then improve prevent miscarriage success rate.
6.Expression of Wnt3a in chronic periodontitis and its clinical significance

Objective To study the expression of Wnt3a in chronic periodontitis patients and its clinical significance. Methods Wnt3a expression was examined by RT-PCR and Western-Blot in 15 cases of chronic periodontitis gingival tis?sues and 15 normal tissues. Probe depth and attachment loss were also compared between these two groups. Correlation be?tween Wnt3a expression and attachment lose were also analyzed. Results Probe depth was deeper in chronic periodonititis groups than that in control group(mm:7.93±1.56 vs 1.83±0.37). Attachment loss was(7.76±1.34)mm in chronic periodonti?tis group while it was not seen in control gorup. Wnt3a was lower both in transcription level(0.49±0.03 vs 0.36±0.04)and expression level(0.57±0.14 vs 0.43±0.17, P<0.05) in chronic periodonitis tissue compared with that in normal tissues. The higher clinical attachment loss was negatively correlated with expression of Wnt3a (r=-0.564,P=0.028). Conclusion Wnt3a is expressed less in chronic periodontitis than that in control group. Wnt3a plays an important role in attachment loss and bone absorption.

8.Clinical application of ultrasound-guided transversus abdominis plane block for postoperative analgesia in patients received lower abdominal surgery

Objective To observe the postoperative analgesia effect of ultrasound-guided trans-versus abdominis plane block (TAP)in patients received lower abdominal surgery.Methods Ninety patients undergoing lower abdominal surgery were randomly divided into group A (received TAP), group B (received patient controlled intravenous analgesia with sulfentanil),and group C (received patient controlled epidural analgesia with sulfentanil and ropivacaine)for postoperative analgeda.All patients received combined spinal and epidural anesthesia.VAS score at postoperative 2,4,8,16 and 24 h,as well as the side effects related to postoperative analgesia,were observed and recorded for each patient.Results Comparing with group A,VAS scores of group B at postoperative 2,4,8,16 and 24 h were obviously higher (P< 0.05);Comparing with group A,VAS score of group C at post-operative 24 h was obviously lower (P< 0.05).Comparing with group A,nausea and vomiting,pru-ritus,and somnolence were more obvious in group B (P < 0.05);nausea and vomiting,sensory and motor abnormality of lower limbs and urinary retention occurred more frequently in group C (P <0.05).Conclusion Ultrasound-guided TAP can effectively relieve postoperative pain 24 hours,and has no obvious side effect in patients received lower abdominal surgery.
9.Inhibition effect of bisoprolol combined with trimetazidine on myocardial remodeling in hypertensive myocardial hypertrophy

Objective To investigate the myocardial remodeling inhibition effect of bisoprolol combined with trimetazidine in the treatment of hypertensive myocardial hypertrophy.Methods 86 cases with hypertension and cardiac hypertrophy in Lingwu People’s Hospital of Ningxia from June 2015 to June 2016 were selected and randomly divided into two groups,43 cases in the control group received routine clinical treatment,43 cases in the experiment group received more with bisoprolol combined with trimetazidine treatment.Blood pressure,ultrasonic cardiogram,levels of serum TNF-α, MMP3 and CRP were compared before and after the treatment.Results Compared with before treatment,levels of blood pressure,IVST,LVPWT,LVMI, MMP3,TNF-αand serum CRP significantly decreased after treatment,and compared with the control group,levels of blood pressure,IVST,LVPWT, LVMI,MMP3,TNF-αand serum CRP in the experiment group were lower,the differences were statistically significant (P<0.05),and the effective rate in the experiment group was higher than the control group ( P<0.05 ) .Conclusion Bisoprolol combined with trimetazidine can effectively inhibit the remodeling of hypertension in patients with myocardial hypertrophy,has good clinical efficacy.
10.Role of ICOS on in-vitro cultured human PBMC Treg

Objective:To investigate the role of mTOR in regulation of ICOS expression in human blood regulatory T cells. Methods:Isolation of Treg cells from human PBMC using MACS beads. We detected the ICOS expression on purified Treg cells and Treg cells viability using flow cytometry in anti-CD3 plus anti-CD28 ( antibody or beads) or anti-CD3 plus ICOSL-Fc for 3 days and 7 days. CFSE labeling human PBMC cells and in vitro cultured Treg mixed, Treg contact inhibition activity was detected by flow analysis. Results:After in vitro stimulation of Treg cells in the presence of anti-CD3+anti-CD28 for 3 days, there was no significant statistic difference in viability between ICOS+(92. 00±2. 69)% and ICOS-(90. 30±3. 53)% Treg-cells. After cultured for 7 days,the decreased ICOS+ Treg cells percentage within total Treg cells from ( 40. 20 ± 1. 83 )% to ( 11. 60 ± 1. 10 )% compared with that of 3 days. Further more,the ICOS expression level between stimulated with anti-CD28 or ICOSL-Fc condition group,compared with the ICOS MFI in the condition of anti-CD3 plus anti-CD28 treatment for 3 days was (2410. 0±746. 4) obviously higher than (403. 30±74. 42), that of the group treated with anti-CD3 plus ICOSL-FC. Rapamycin could partially suppress Treg cells ICOS expression,but unaffected the Treg suppression ability. Conclusion:ICOS expression level may not important for in vitro cultured human PBMC Treg cells survival although mTOR signling is important for regulation ICOS expression on in-vitro cultured Treg cells,but the ICOS expression on Treg regulated by multiply signaling pathways. CD28 signaling is the key stimulation factor for ICOS upregulation on in-vitro cultured Treg cells compared to ICOSL signaling.
