2.THE DIAGNOSIS AND TREATMENT OF MENINGIOMA OF THE SPHENOID RIDGE

Twenty four cases of meningioma of the sphenoid ridge were reported. The common symptoms and signs of this tumor were summarized. For practical use, it may be classified as medial and lateral types. The symptoms and signs of these two types are different. Carotid angiograms are helpful to show the blood flow of the tumor. This method cannot be replaced by CT scan. The tumors were removed in fifteen patients totally, among them two died. The authors considered that the most important things for successful operation are to control bleeding carefully, to treat all the blood vessels at the base of the tumor properly and to protect its surrounding important structures during the operation.

4.Functional verification of vimentin as an alarmin

Objective:To identify the function of extracellular soluble vimentin that promotes proliferation, activation and chemotaxis of inflammatory cells. Methods: The proliferation of rat splenocytes stimulated with vimentin was evaluated in vitro. The lymphocyte counts and vimentin-antibody levels of peripheral blood in mice immunized with vimentin were detected in vivo. Peritoneal macrophages were collected and cultured with different concentrations of vimentin to detect the effect on phagocytosing chicken red blood cells. The chemotaxis of NIH3T3 fibroblast towards vimentin was observed in transwell chamber. Results:In vitro vimentin dose dependently promoted the proliferation of splenocytes. The proliferation indexes of primed and naive splenocytes cultured with 16μg/ml vimentin were reached up to ( 196. 0 ± 9. 7 )% and ( 208. 9 ± 4. 6 )% respectively without significant difference. In vivo vimentin significantly enhanced the lymphocytes number(109 L-1)of peripheral blood(5. 74±0. 51 vs. 1. 69±0. 13)and the levels of vimentin specific antibody( OD value 2. 31 ± 0. 06 vs. 0. 19 ± 0. 08 ) that shown no significant difference from immunization with vimentin plus CFA. In vitro vimentin dose dependently stimulated phagocytic ability of macrophages and performed the chemotactic effects on NIH3T3 fibroblasts. Conclusion:Extracellular soluble vimentin promotes the proliferation,activation and chemotaxis of concerned inflammatory cells and possesses the functions as an alarmin.

6.Differential expressions and clinical significance of labour-associated genes in preterm labour and term labour myometrium

Objective To study the differential expressions of labour-associated genes,including interleukin-8 (IL-8),oxytocin receptor (OTR) and prostaglandin H synthase type-2 (PGHS-2)) between preterm labour and term labour myometrium and their clinical significance.Methods Seventeen cases of preterm labour (PTL) and 31 cases of preterm no labour (PTNL) of pregnant women were selected as study group,while 6 cases of term labour (TL) and 6 cases term no labour (TNL) of pregnant women were chosen as control.Lower myometrial tissue biopsies were collected to detect the expressions of IL-8,OTR and PGHS-2 by real-time fluorescence quantitative polymerase chain reaction (qRT-PCR) as well as Western blot analysis.The expression data were then statistically analyzed in combination with clinical data.Results In the lower myometrial tissues,the mRNA levels of IL-8 and PGHS-2 were significantly lower in PTL group than in TL samples (P0.05),but the PGHS-2 mRNA level was significantly higher in TL group than in TNL group (P0.05).Correlation analysis suggested that the mRNA level of IL-8 was significantly correlated with gestational age in PTNL group (r=0.294,P=0.042).Multiple linear regression analysis showed a mutual control effect of the expressions of IL-8,OTR and PGHS-2 in preterm but not in term labour group.Conclusion Our results show that there are significant differences of the expression of labour-associated genes in lower myometrum between preterm labour and term labour,which might becorrelated with the progression of preterm labour.
7.Serum asymmetric dimethylarginine and endothelial function after renal transplantation

Objective To investigate the relation between serum asymmetric dimethylarginine (ADMA) level and endothelial function before and after living donor kidney transplantation in uremic patients.Methods A total of 38 renal transplant patients (21males and 17 females) and 36 healthy controls (20 males and 16 females) were enrolled. Plasma ADMA, symmetric dimethylarginine (SDMA), malondialdehyde (MDA), glutathione peroxidase (SeGSHPx), C-reactive protein (CRP) were mea-sured before transplantation and on Days 1, 3, 7, 14, and 28 posttransplantation. The brachial artery flow mediated dilatation (FMD) was studied before transplantation and on the 28th day after the transplantation. Results Serum levels of ADMA,SDMA,MDA and CRP were significantly increased, and the activities of nitric oxide (NO) and SeGSHPx were decreased in uremic patients compared with age matched healthy subjects (all P<0.01). Serum levels of ADMA,SDMA,MDA and CRP decreased while the activities of NO and SeGSHPx increased significantly at the first day after the transplantation (all P<0.01). The decrement of plasma SDMA normalized on the 28th day (P>0.05). The FMD was lower in the patients than the control group (P<0.01) and improved significantly on the 28th day of posttransplantation (P<0.05). Serum levels of ADMA were positively correlated with MDA (r=0.412, P<0.01;r=0.342,P<0.01) and negatively correlated with the values of SeGSHPx (r=-0.345, P<0.01;r=-0.315, P<0.01) and FMD (r=-0.452,P<0.01;r=-0.416,P<0.01) both before and after kidney transplantation. Conclusion The level of serum ADMA is associated with endothelial function improvement in uremic patients both before and after kidney transplantation.
8.Relation between antibody titer in pregnant women with maternal-fetal ABO blood incompatibility and hemolytic disease of fetuses and newborns

Objective To investigate the relationship between IgG antibody titer in pregnant women with maternal-fetal ABO blood incompatibility and hemolytic disease of fetuses and newborns.Methods From January 31 2009 to January 31 2010,1269 singleton pregnant women who were suspected to have maternal fetal ABO blood incompatibility in Department of Obstetrics and Gynecology,Southwest Hospital,Third Military University were collected.Anti-A or anti-B IgG titers of them were detected at 28-30 gestational age,and umbilical cord blood were taken when delivery and hemolytic disease of the newborn serological test were done to diagnose hemolytic disease of the newborn (HDN).The relationship between the titers and incidence of fetal or neonatal hemolytic disease was retrospectively analyzed by Kendall tau rank correlation.Results No IgG of anti-A or anti-B in serum were found in 58.4% (741/1269) pregnant women,while the antibody titer of 5.1% (65/1269) pregnant women were more than or equal to 1 ∶ 128.When they were tested again at 36 gestational week,the titer of 17 cases increased twice but lower than 1 ∶ 512.No signs of intrauterine hemolysis,such as edema,ascites and pleural effusion,were found.Three hundred and eighty neonates (29.9%,380/1269) were diagnosed as HDN.Among which,12 cases (3.2%,12/380) showed mild anemia and (or) jaundice within 24 hours after delivery.There was positive correlation between incidence of neonatal hemolysis and antibody titer(Tb=-0.293,P<0.01).The incidence of HDN increased from 85.4% (35/41) in women with antibody titer of 1 ∶ 128 to 5/5 inwomen with antibody titer at 1 ∶ 512 (x2=108.906,P<0.01).Among 380 HDN neonates,322 cases were transferred to neonatal intensive care unit for phototherapy based comprehensive therapy,and two underwent exchange transfusion.All patients were cured.Conclusions The intrauterine hemolysis incidence of patients with suspected maternal-fetal ABO blood incompatibility is very low,and no special care is required during pregnancy.Anti-A or anti-B tests during pregnancy is helpful in early diagnosis and management of HDN.
9.Effects of different type of parathyroid damage to the postoperative functional recovery of parathyroid during thyroid operation

Objective To study the effects of different type of parathyroid damage to the postoperative functional recovery of parathyroid,through establish an animal model by simulating total thyroidectomy and parathyroid damage during surgical operation.Methods Experimental rabbits for the study were randomly divided into A,B,C,D four groups (n =8),Group A (control group):simple exposure,exploration thyroid and parathyroid;group B (vascular injury group):total thyroidectomy and ligation bilateral parathyroid blood supply but keep the surrounding membrane;Group C (membrane damage group):total thyroidectomy and damage membrane but reservations blood supply.Group D (composite damage group):total thyroidectomy plus membrane and blood both damage;All animals were monitored of serum calcium and PTH,preoperative 1 days and postoperative 1 st day,3rd day,5th day,7t day;cut the parathyroid HE staining to observed survival of parathyroid tissue and pathology damage when 7th day after operation.Results (1) Animals in each group preoperative serum calcium and PTH were no significant difference (P >0.05);(2)Group A postoperative serum calcium decreased,but at 5th day returned to preoperative level (P > 0.05);Group B and C postoperative 1st day,3rd day,5th day serum calcium decreased significantly(P < 0.05)and to the lowest at 1 d and then gradually recovered,but group C faster recovered than group B (P < 0.05);Group D postoperative 1 st day,3rd day serum calcium continued to decline significantly (P < 0.05);(3) Group A postoperative serum PTH decreased,but at 7th days returned to preoperative level (P > 0.05).Group B and C postoperative 1st day,3rd day,5th days serum PTH decreased significantly(P <0.05)and to the lowest at 1 d and then gradually recovered,but from postoperative 3rd day group C faster recovered than group B(P < 0.05);Group D postoperative 1 st day,3rd day serum PTH continued to decline significantly (P < O.05);(4) Pathology results:Group A parathyroid filled with chief cells and a small amount of vacuolar changes (5% to 10%);Group B parathyroid hemorrhage,necrosis (40% to 50%),part of the cell degeneration (30% to 40%),center with fibrosis,seen granuloma and hyperplasia of parathyroid tissue in surrounding;Group C parathyroid bleeding (10% to 20%),part of the cell degeneration (10% to 20%);Group D parathyroid severe necrosis,almost no normal parathyroid tissue,significant fibrosis,less residual parathyroid tissue was scattered.Conclusions (l) The recover of Parathyroid function is influenced by the type of parathyroid in situ injury during thyroidectomy,composite damage of blood supply and membrane of parathyroid is the most serious,parathyroid ischemia necrosis,the function can not be restored,pure blood supply damaged,some can restore function,and the parathyroid gland with vascular pedicle can be recovered quickly.(2) Severe blood supply and membrane damaged,and even free parathyroid should be transplanted immediately during operation.
10.The influence of preventive calcium supplementation on the function of parathyroid glands after total thyroidectomy

Objective To discuss the influence of preventive calcium supplementation on the recovery of parathyroid glands function after total thyroidectomy.Methods 232 patients meeting the selected criteria were randomly assigned to group A and B,and then divided into group A1 (87 cases,PTH >8 pg/ml)and A2 (30 cases,PTH <8 pg/ml),group B1(83 cases,PTH>8 pg/ml)and B2(32 cases,PTH <8 pg/ml) based on the lowest parathyroid hormone( PTH )value within 3 days after surgery.All patients in group A were immediately supplemented 10% calcium gluconate intravenously 6 g/d after operation.For group B,post operative calcium supplementation was not given,however,anyone whose PTH < 8 pg/ml was supplemented 10% calcium gluconate intravenously 6 g/d no matter hypocalcemia occurred or not.The level of serum calcium and PTH of all patients were assayed before operation and at the 1st,2nd,3rd day,1st week and 1st month after operation.In additon,patients with hypocalcemia received serum calcium and PTH detection at the 2nd and 3rd week.Whether hypocalcemia and hypoparathyroidism occurred or not was recorded.Results ( 1 )The serum PTH was obviously higher in group A1 than in group B1 at the 1st week after operation( P <0.05 ).The serum calcium was obviously higher in group A1 than in group B1 at the 1st,2nd,3rd day and 1st week after operation(P >0.05).The hypocalcaemia and symptomatic hypocalcaemia incidence were obviously lower in group A1 than in group B1 (P < 0.05 ).(2)① Group A2 had obviously higher level of serum PTH than group B2 at the 1 st,2nd,and 3rd week after operation ( P <0.05 ) and returned to normal level of serum PTH earlier than group B2.② Group A2had obviously higher level of serum calcium than group B2 from the 1 st day to the 3rd week after operation ( P < 0.05 ) and returned to normal level of serum calcium earlier than group B2.③ The hypocalcaemia and symptomaic hypocalcaemia incidence ware obviously lower in group A2 than in group B2 (P < 0.05 ).Conclusion The preventive calcium supplementation is beneficial for the recovery of the function of parathyroid glands after total thyroidectomy.
